Development of Telephone Consultation Algorithm for Patient Discharged with Ophthalmic Disease (눈질환자의 퇴원 후 증상관리를 위한 전화상담 알고리즘 개발)

  • Purpose: This methodological study was done to develop a telephone consultation algorithms and practice guidelines for patient discharged with ophthalmic diseases. Methods: The ophthalmic problems of the patients were identified and expert knowledge on managing the problems was acquired. Algorithms and practice guidelines were developed based on the expert knowledge. The content validity of algorithms and practice guidelines was evaluated by the experts. Results: The preliminary algorithms and practice guidelines were developed from 60 detailed signs and symptoms and 45 nursing interventions. The experts agreed that 57 detailed signs and symptoms linked with nursing interventions were valid, with the content validity index over 80%. Meeting with nurse experts and ophthalmologists was convened to review the rest of the 3 detailed signs and symptoms linked with nursing interventions. Finally, 60 detailed signs and symptoms and 46 nursing interventions were confirmed. Conclusion: This study suggests that the algorithms and practice guidelines are effective decision-making tools and utilization of these algorithms and practice guidelines is expected to improve the quality of clinical nursing and patient satisfaction.

The Study of Serum lipid Profile and Food Behaviors in Healthy Offsprings of Korean NIDDM Patients (인슐린 비의존형 당뇨병의 가족력을 가진 정상 자녀의 혈청 지질 및 식습관에 관한 연구)

  • This study was to investigate the risk factors for the development of diabetes mellitus on healthy offsprings of Korea non-insulin dependent diabetes mellitus(NIDDM) patients. Forty four offsprings who have at least 1 parent with NIDDM and age, sex and body mass index(BMI) matched forty four healthy control subjects were collected for this study. BMI, skinfold thickness, waist/hip circumference ratio(WHR), fasting blood glucose, serum insulin, serum lipid profile, nutrient intakes and food behaviors were measured. There were no significant difference in BMI, WHR, fasting blood glucose, insulin, total cholesterol, triglyceride and daily nutrients intake between offsprings and controls. But skinfold thickness, LDL-cholesterol level and sweety food eating frequency were significantly higher in male offsprings than in male controls. And HDL-cholesterol was significantly lower in male offsprings than in male controls. Although there were no significant difference, offsprings had a overeating habit and ate more confectionery and greasy food than controls.

Development of Detailed Clinical Models for Pain Assessment (통증 간호사정을 위한 임상내용모델 개발)

  • Purpose: The aim of this study was to develop and validate Detailed Clinical Models (DCMs) for pain assessment in nursing. Methods: First, we identified the entities of pain assessment from ICNP. We identified the attributes and values of the attributes to describe the entities in more detail by reviewing the literature. Data types and optionalities of the attributes were defined. Second, we modeled the DCMs by linking an entity and its corresponding attributes with values and by specifying the data types and optionalities of the attributes. Finally, the DCMs were validated by a group of domain experts using a content validity index. Results: In total, 19 DCMs with 11 attributes for pain assessment were developed. The experts' evaluations showed that the DCMs were valid enough to represent pain related information of nursing assessment. Conclusion: The DCMs developed in this study can be used in electronic nursing records. The DCMs for pain can ensure the semantic interoperability of pain related information in electronic nursing records.

Neovastat(AE-941) inhibits the airway inflammation and hyperresponsiveness in a murine model of asthma

  • Matrix metalloproteinase (MMP)-9 plays an important role in the pathogenesis of bronchial asthma. Neovastat, having significant antitumor and antimetastatic properties, is classified as a naturally occurring multifunctional antiangiogenic agent. We evaluated the therapeutic effect of Neovastat on airway inflammation in a mouse model of asthma. BALB/c mice were immunized subcutaneously with ovalbumin (OVA) on days 0, 7, 14, and 21 and challenged with inhaled OVA on days 26, 29, and 31. Neovastat was administrated by gavage (5 mg/kg body weight) three times with 12 h intervals, beginning 30 min before OVA inhalation. On day 32, mice were challenged with inhaled methacholine, and enhanced pause (Penh) was measured as an index of airway hyperresponsiveness. The severity of airway inflammation was determined by differential cell count of bronchoalveolar lavage (BAL) fluid. The MMP-9 concentration in BAL fluid samples was measured by ELISA, and MMP-9 activity was measured by zymography. The untreated asthma group showed an increased inflammatory cell count in BAL fluid and Penh value compared with the normal control group. Mice treated with Neovastat had significantly reduced Penh values and inflammatory cell counts in BAL fluid compared with untreated asthmatic mice. Furthermore, mice treated with Neovastat showed significantly reduced MMP-9 concentrations and activity in BAL fluid. These results demonstrate that Neovastat might have new therapeutic potential for airway asthmatic inflammation.

Relationship between frailty and oral health among the elderly (일부 노인의 노쇠와 구강건강의 관련성)

  • Objectives: The purpose of the study was to investigate the relationship between frailty and oral health among some elderly community residents. Methods: A self-reported questionnaire was completed by 240 elderly in the Gwangju-Jeonnam area from October 1, 2019, to November 30, 2019, based on convenience sampling. The questionnaire consisted of general characteristics of the subjects, frailty level (Kihon checklist), and Geriatric Oral Health Assessment Index. Results: There were 66 (27.5%) frail elderly, and the GOHAI group with a score less than 45 constituted 187 (77.9%), which was higher than the group with a score of 45 or higher. The distribution of the frail elderly was indicated to be higher in the people with older age (p<0.05), lower educational level (p<0.01), current economic inactivity (p<0.05), living alone (p<0.01), more chronic diseases (p<0.01), and GOHAI score below 45 (p<0.01). Compared to those with a good oral health-related quality of life, those with a poor quality of life showed a 3.03 times higher risk of frailty (95% CI=1.291-7.107)(p<0.05). Conclusions: By recognizing the need for oral health care of the elderly through these results and by identifying the relationship between frailty and oral health, it is possible to consider oral health as a predictor of frailty.

Protective Effect of Silkworm Hemolymph against Gamma Irradiation Induced Damage in the Liver of Mice (방사선 손상 Balb/C 마우스 모델에서 누에 체액(Silkworm Hemolymph)의 간조직 보호 효과)

  • This study was designed to evaluate the protective effect of silkworm hemolymph against ${\gamma}-irradiation$ induced damage in the liver of mice. Female Balb/C mice (6 weeks old) were exposed to ${\gamma}-irradiation$ (6 Gy) and administered orally to silkworm hemolymph ($5ml\;kg^{-1}$ BW) for 7 days post-irradiation. The body weight, spleen index, plasma aspartate transaminase (AST), plasma alanine transaminase (ALT), and liver malondialdehyde (MDA) levels were determined. Compared with irradiated control mice, the activity of plasma AST and the level of MDA were significantly decreased in mice treated silkworm hemolymph. These results show that silkworm hemolymph is found to have a protective effect against ${\gamma}-irradiation$ induced damage in mice.

Development of a Drought Detection Indicator using MODIS Thermal Infrared Data

  • Based on surface energy balance climatology, surface temperatures should respond to drying conditions well before plant response. To test this hypothesis, land surface temperatures (LST) derived from MODIS data were analyzed to determine how the data were correlated with climatic water balance variables and NDVI anomalies during a growing season in Western and Central Kansas. Daily MODIS data were integrated into weekly composites so that each composite data set included the maximum temperature recorded at each pixel during each composite period. Time-integrated, or cumulative values of the LST deviation standardized with mean air temperatures had significantly high correlation coefficients with SM, AE/PE, and MD/PE, ranging from 0.65 to 0.89. The Standardized Thermal Index (STI) is proposed in this study to accomplish the objective. The STI, based on surface temperatures standardized with observed mean air temperatures, had significant temporal relationships with the hydroclimatological factors. STI classes in all the composite periods also had a strong correlation with NDVI declines during a drought episode. Results showed that, based on LST, air temperature observations, and water budget analysis, NDVI declines below normal could be predicted as early as 8 weeks in advance in this study area.

Clinical Approach of Chukyu Pharmacopuncture and Brainspotting Through a Traffic Accident Patient Case (교통사고 환자 증례를 통한 척유약침과 Brainspotting의 임상적 접근)

  • The purpose of this study was to introduce effects of Chukyu pharmacopuncture and Brainspotting on knee pain, headaches, and acute stress disorder of a patient with a traffic accident. We treated a patient with Chukyu pharmacopuncture and Brainspotting. Numerical rating scale (NRS), Range of Motion (ROM), Hyperextension Position (HEP), Effusion (Eff), Patella Compression Test (PCT), Frontal Flexion Position (FFP), McMurray (MCM), Lateral Joint Line Tenderness (LJLT), Medial Joint Line Tenderness (MJLT), Anterior Drawer Test (ADT), Lachman Test, Varus/Valgus stress, Beck Anxiety Inventory (BAI), Patient Health Questionnaire-9 (PHQ-9), Pittsburgh Sleep Quality Index (PSQI), Core Seven Emotions Inventory Short Form (CSEI-S), Subjective units of disturbance scale (SUDs), and Heart rate variability (HRV) were used to evaluate the patient. After the patient was treated by Chukyu pharmacopuncture and Brainspotting, the patient showed improvement in NRS, physical examination, and psychological symptom assessment. These results indicate that Chukyu pharmacopuncture and Brainspotting are effective on knee pain, headache, and acute stress disorder after a traffic accident.

Growth of abalone (Haliotis discus hannai) in cages using epibiont control measures

  • In this study, the relationship between the growth of abalone and the presence of epibionts was investigated in abalone cultured in Goheung, Jeollanam-do, where there are severe problems high water temperatures and attachment. The experiment was conducted for eight months (May-December 2020), and 40 abalone were collected every month. Water temperature was at its highest at a range of 13.5℃-26.6℃ and dissolved oxygen levels were at their lowest at a range of 4.0-10.2 ㎍/L in August. The shell height (mm) of abalone grew to 117.7% (81.8 ± 1.9 mm) in cultures where epibionts were removed (ER) and 111% (77.4 ± 3.3 mm) where they were not (non-epibionts, NER). Their total weight (TW) and body weight increased significantly and steadily with ER, whereas the TW increased sharply after August with NER. In the condition index, no significant difference was observed between ER and NER. The monthly proportion of epibionts increased significantly in July, accounting for the value of 69.9% reached in December.

Korean Medicine Treatment for Dyspepsia and Constipation in a Patient with HIV: A Case Report

  • Functional dyspepsia (FD) is a chronic and recurrent upper gastrointestinal symptom that has no organic cause. A 49-year-old male patient positive for human immunodeficiency virus (HIV) visited the clinic because of FD and constipation. He received complex Korean medicine treatment, including acupuncture and herbal medicines, from April 14 to July 18, 2022 (27 times) in the outpatient department. Gastrointestinal Symptom Rating Scale (GSRS), Nepean Dyspepsia Index (NDI), FD-related quality of life (FD-QoL), Euro QoL visual analog scale (EQ-VAS), and Numerical Rating Scale (NRS) were used as evaluation tools in this study. Symptoms were relieved after 3 months of treatment, and his QoL improved (GSRS, 15-3; NDI, 110-21; FD-QoL, 52-20 [eating status, 10-3; liveliness status, 12-8; psychological, 1-0; role-functioning status, 18-9]; EQ-VAS, 40-65; NRS, 8-4). The results revealed that complex Korean medicine treatment could alleviate FD and constipation in patients with HIV.
