지역사회 거주 여성노인들의 근감소증 실태와 일상생활능력, 영양상태, 및 우울과의 관련성 연구 (Prevalence of sarcopenia in association with ADL, nutritional status and depression among community dwelling elderly women)

  • 본 단면 연구의 조사 목적은 지역사회거주 여성 노인들을 대상으로 근감소증 실태와 일상생활능력, 영양상태, 및 우울과의 관련성을 조사하기 위함이었다. 연구대상자는 2014년 5월~7월 동안 B와 D광역시에 거주하는 65세 이상 여자 노인 90명을 대상으로 하였다. 연구도구는 신체계측(anthropometric measures), Mini-nutritional assessment instrument (MNA). ADL, IADL, MMSE, SGDS-K를 사용하였다. 연구결과. 노인들의 평균 연령은 74.2세이었고, 37.8%가 근감소증이 있었으며, 대부분 도움없이 독립적인 일상생활 능력을 가지고 있었고, 15.6%가 영양부실 위험이 있었으며, 12.2%가 우울증상을 나타내었다. 근감소증이 있는 노인들은 없는 노인들보다 수입과 교육수준이 낮았고, 혼자 사는 노인이 더 많았으며, 동반질환이 더 많은 것으로 나타났다. 근감소증이 있는 노인은 없는 노인들보다 영양상태가 불량하고, 우울점수가 높았으나 일상생활능력에는 차이가 없었다. 근감소증 예측요인은 종아리와 허벅지 둘레, 인지기능이었다. 결론적으로 65세 이상 지역사회거주 여성노인들은 종아리둘레, 허벅지둘레, 및 인지기능이 낮을 때 근감소증 위험이 높을 것이며, 이들 세 요인은 앞으로 근감소증의 조기발견을 위한 간이 지표로서 활용할 수 있을 것이다.

환자평가표에 의한 요양병원 입원 노인들의 일상생활사 의사결정 인식기술 상태와 일상생활수행능력 간의 관계 (Relationship between the State of Decision Making Recognition Technology for Daily Living and Activities of Daily Living(ADL) of Inpatients in Geriatric Hospital on the Patient Core Card)

  • 본 연구는 2013년 12월 현재 요양병원에 입원생활 중인 노인들 중 환자평가표의 평가구분에서 계속입원중인 환자 평가를 받은 3개월 이상 입원중인 노인들을 대상으로 실질적으로 부양 및 요양간병 시 집중적으로 관심을 가져야 하는 일상생활활동이 무엇인지를 파악해 보고자 하였다. 연구결과, 일상생활사에 관한 의사결정을 할 수 있는 인식기술 상태와 일상생활활동수행능력들 간의 인과관계를 로지스틱 함수를 이용하여 추정한 결과, 일상생활사 의사결정 인식기술 상태가 심하게 손상된 경우에는 노인들에게 실질적으로 부양 및 요양간병 시 '식사하기' 활동과 '옮겨 앉기' 활동에 집중적으로 관심을 가져야 한다는 것을 확인하였다. 또한 '식사하기' 활동과 '옮겨 앉기' 활동 외의 활동들은 일상생활사에 관한 의사결정을 할 수 있는 인식기술 상태보다는 노화나 질병으로 인한 신체적 기능이 떨어져서 나타난 결과로 유추해 볼 필요성이 있을 것이다. 따라서 이 경우에는 부양이나 요양간병 시 도움을 받는 노인들의 인격이나 수치심 등 자존감을 고려하여 수발자들은 행동과 언어에 각별히 신경을 써야 할 것이다.

노인들이 지각하고 있는 건강수준에 관한 연구 (A Study on the Health as Perceived by the Aged)

  • The purpose of this study was to develop a nursing model for the aged. This study was conducted to measure the physical, psychological and mental health status of the aged and to identify relationships between these health levels and various variables. The data were collected from 172 aged(over 60 years old) by interviews. The tools used for this study was a structured questionnaire which was developed and revised by the researchers. The tool consisted of 22 items on physical health assessment, 7 items on psychological health assessment, 9 items on mental health assessment and 10 items on an ADL evaluation. The major results of the study were as follows ; 1. In physical health, the aged complained of visual disturbance(60.5%), incontinence of urine (55.2%), back or muscle pain(73.3%), dizziness(70.3%) and diarrhea or constipation(44.2%). In psychological health, most aged people felt anger when they got some order from an other person(80.2%). Also, they had depression (69.8%) and felt like dying(64.0%). The ADL level was mostly normal for daily life. 2. The aged man was more healthy than the aged woman in physical and mental health especially the 60-64 year old group. The physical, mental health and ADL level had a positive correlation with age(p>.004), but psychological health had a negative correlation with age. 3. The physical, psychological, mental health and ADL level was positively interrelated with each other. 4. The physical, mental health, & ADL level lowered with inceasing age. Physical & psychological health worsened rapidly from 65-69 years, but for 10-15 years, their health level was preserved. Psychological health level was high in 80-84. 5. The physical health was affected by edu cational level, sex and pocket money (R=.4029, 16.24%). The psychological health was affected by the supportive style and pocket money (R=.5128, 26.30%). And the mental health was affected by education level, age, support ive style, sex and job(R=.4377, 19.16%). As seen above, we suggest the intervention of the young for the old to cope with their life and to maintain their healthy late adulthood. Also, if they have received psychological support in the institution, they will maintain healthy life condition. For further studies should be a search for variables that affect aged health, and should contribut to a nursing program better suited for the aged.

코그팩 프로그램이 알츠하이머 치매노인의 인지 및 일상생활동작에 미치는 영향 (The Effects of CogPack program on LOTCA and ADL in Elderly with Alzheimer's Dementia)

  • Purpose : The purpose of this study was to evaluate the influence of Lowenstein Occupational Therapy Cognitive Assessment(LOTCA) and Activities of Daily Living(ADL) by CogPack program in elderly with Alzheimer's dementia. Method : The subjects of this study, elderly diagnosed with Alzheimer's dementia, 8 patients were picked up, who were agreed with this research. Cognitive training for 6 weeks, which consisted of 3 times per week, 30 min of memory, attention and performance with cogpack program. Cognitive and ADL measured by LOTCA and FIM. The SPSS Ver. 14.0 statistical program was used for data processing. The significance level for statistical inspection was set as 0.05. Result : In comparison of LOTCA was significantly correlated in the pre and post test but FIM was not significantly correlated in the pre and post test. Conclusion : Therefore, the CogPack program is useful to improve the LOTCA in elderly with Alzheimer's dementia.

재가노인의 건강문제와 보건ㆍ복지 서비스 지원 실태에 관한 연구 (Health Problems and Support from the Health & Welfare Service in the Elderly at Home)

  • Purpose: This study was conducted to identify health problems and support received from the health and welfare service using MDS-HC(Minimun Data Set for Home Care) in the aged living at home. Method: Eighty-one elderly persons were selected from those listed in community-welfare service centers in Seoul and Kyunggi Province between December 2002 and January 2003. Result: Eleven health problems per elderly person on average were identified, and the frequent care needs were in order: preventive health measure, health promotion, visual function, depression & anxiety, communication disorders, social function, pain, environmental assessment, oral health, cognition and falls. The number of health problems by the level of ADL was ‘ADL 1(Independence)’ 9.87, ‘ADL 2(Partial independence)’ 12.78, ‘ADL 3(Dependence)’ 13.73. Utilization of formal health & welfare services among the elderly was ‘meals on wheels’ 40.7%, ‘home helper’ 38.2%, ‘visiting of social welfare worker’ 21.0%, ‘physical therapy’ 19.6%, ‘day care center’ 12.3%, ‘volunteer's service’ 9.9%, ‘home visiting care’ 3.7%, ‘occupational therapy’ 3.7%, and ‘speech therapy’ 2.5%. Conclusion: The results suggest that using the MDS-HC 2.0 is applicable to help decide criteria for both health and welfare service supplied to the elderly.

치매환자의 인지기능, 우울, 자아존중감, 일상생활수행능력에 미치는 원예치유 효과 - 체계적 문헌고찰 및 메타분석 - (The Effect of Horticultural Therapy on Cognitive Function, Depression, Self-esteem and Activity Daily Living(ADL) in Dementia Care - A Systematic Review and Meta-Analysis -)

  • The purpose of this study is to systematically review and to objectively identify the effect size in order to understand the effects of horticultural therapy on cognitive function, depression, self-esteem, and activity daily living(ADL) of dementia patients. Related studies published in Korean and English were selected by searching domestic and foreign academic databases. The risk of bias assessment of individual studies was performed on the included 18 literatures. Horticultural therapy was found to be significantly effective in cognitive function(effect size=1.06), depression(effect size=0.64), self-esteem(effect size=0.99), and ADL(effect size=1.98) in dementia care. It is expected that related experimental studies will be repeatedly conducted to produce a more objective effect size calculation.

가상현실을 이용한 조기 로봇보조 보행 훈련이 뇌졸중 환자에 미치는 영향 (Effect of early robot-assisted training using virtual reality program in patient with stroke)

  • Purpose: The purpose of this study was to determine the effect of early Robot-assisted training on gait ability, function and ADL in patients with stroke. Methods: 26 patients with stroke were recruited for this study. The subjects were randomly assigned to either the experimental group (EG) or the control group (CG), with 13 patients in each group. All subjects received a routine physical therapy. The robot-assisted training was for 30 min in the case of the EG subjects. The assessment tools of this study involved the gait ability, balance ability, function and ADL. The measurements were recorded before the intervention and after the intervention. Results: EG subjects and CG subjects, the variables measured after the intervention significantly differed from gait ability, balance ability, function and ADL without the FMA (p<0.05). The FMA was only effective experimental group after intervention. Also, there were significant differences in gait ability, balance ability, function and ADL without the FMA at post-test between the 2 groups (p<0.05). Conclusion: The findings indicate that early robot-assisted training exerts a positive effect on gait ability, balance ability, function and ADL in patients with stroke. This result indicates the possibility of application of the early Robot-assisted training to the management for stroke patients. Further studies are required to generalize the result for this study.

노인의 일상활동수행에 대한 자기효능척도의 신뢰도 및 타당도 평가 (Psychometrics of Task Self-Efficacy Scale for Korean Elderly)

  • The purpose of the study was to test the reliability and validity of the Korean version of Task Self-Efficacy Scale for activities of dally living (ADL). The Task Self-Efficacy Scale was developed by Roberts(1996) for low-intensity exercise study with older people to predict their performance of ADL. The scale was translated and back translated by bilingual persons, and then was modified to resolve variations in the translations. The Korean version of Self-Efficacy Scale for ADL was then administered to 193 elderly people including 95 hospitalized patients and 98 outpatients or healthy people. Face to face interview was used to fill out the structured questionnaire, and each interview took approximately 30 minutes. The subjects for the study were 80 women and 112 men with an age range of 65 to 95 years(M=71 years) of whom 82.6% classified themselves as moderate or quite active Most subjects(80.2%) had an education level of elementary school or less. The Self-Efficacy Scale for ADL is measured on a 0 to 10 VAS, assessing three areas of ADL : self care activities, household tasks, and motor tasks. The higher the score is, the higher person's confidence in performing ADL. Psychometric testing revealed that the scale was found to be internally consistent, showing a Cronbach's alpha of .97 The scale was significantly correlated with subjects' level of activity and subjective assessment of their health status. Moderate correlation with health-related hardiness scale also supported the validity. Factor analysis was performed to confirm whether the scale represents the three sub-areas as suggested in the literature. The results of the factor analysis led to a three factor solution according to Kaiser's criterion, but the items were not strongly and cleanly loaded for the third factor. This can be explained in that, among the three sub-ADL areas of the self-efficacy scale, the areas of self care activities and household tasks seem to have similar levels of difficulty in performance with not enough differences for the self-efficacy scale to distinguish between the two areas. Therefore, one factor solution was suggested since ADL can be seen as a unit of activities at similar level of difficulty in performance. One factor solution explained 68.1% of variance of the 19-item scale and all items were correlated over .6 with the factor, showing that the selected factor solution fits the model. The results indicated that the Korean version of Task Self-Efficacy Scale for ADL was reliable and valid in producing useful information to evaluate the effects of various interventions toward promoting health and quality of life for elderly people.

노인요양시설 입소자의 요양등급별 건강 요구: RAI-MDS-FC를 적용하여 (Health Needs of the Elderly in Long-term Care Facilities: Using RAI-MDS-FC)

  • Purpose: This study was to identify health needs of the elderly at nursing homes by long-term care grade. Methods: The health needs of 116 elders at two nursing homes in Seoul were measured with resident assessment protocols (RAPs), activities of daily living (ADL), and cognitive performance scale (CPS), and pain and depression were measured by resident assessment instrument (RAI). Results: With regard to RAPs, 11 out of 18 items had different distribution in the 3 groups significantly. The 1st-grade elders had a higher percentage of 9 items than the 2nd- and 3rd-grade ones but the 3rd-grade ones had the highest health needs related with activity. The 2nd-grade elders had similar health needs to the 1st-grade ones. In terms of functional level, the 1st-grade elders had the highest percentage of CPS and ADL but the 3rd-grade ones had the highest score of pain and depression out of the 3 groups. The standardized assessment instrument to identify specific health needs by the 3 groups should be developed. Therefore, care plans to meet health needs of the 3 groups will be made. Conclusion: It is suggested that nursing care is required to the elderly of the 1st- and 2nd-grade and safe activity and environment to the 3rd-graded ones.

유방암 수술 환자의 감각 변화에 대한 연구 (Patients' Experiences of Sensations After Breast Cancer Surgery in Korean Women)

  • Purpose: The purpose of this study was to obtain information about the experiences of sensations after breast cancer surgery characterized by prevalence, frequency, and severity, distresses, and disturbances in ADL using the BSAS (Breast Sensation Assessment Scale) and to develop the standardized Korean BSAS. Methods: Thirty two women from 3 to 100 days after breast cancer surgery (BCS) completed Roberta's BSAS. The 18 sensations of BSAS were translated using the references of Lee's Korean Pain Rating Scale and English-English, English-Korean dictionaries and the consultation from two native Americans, one bilingual permanent resident of the States, and one nursing professor. The Korean-translated BSAS has high reliability in test-retest. Likert type 4-point scale and 100 mm VAS were used for assessment. Results: Certain sensations remained prevalent (tender, pull, pain), frequent (numb, nag, throb), severe (throb, shoot, numb), causing distress (throb, penetrate, shoot), and influencing on ADL (throb, penetrate, nag). The most frequently experienced sensation other than BSAS was itching. There was little difference in the prevalence of symptom experiences between sentinel lymph node biopsy and axillary lymph node dissection. Conclusion: The women after BCS are suffering from neuropathic sensations. The Korean- translated BSAS could be used in effectively assessing breast sensations after BCS in Korean women.
