Characteristics and Causes of Musculoskeletal Disorders for Employees Aged 50 Years or Older (50세 이상 고령근로자의 근골격계질환 발생특성 및 원인에 관한 연구)

  • This study concerned with the characteristics and causes of musculoskeletal disorders (MSDs) for employees aged 50 years or older. In order to do this, based on 8,011 accident analysis reports, we analyzed the differences of characteristics between groups under 50 years and 50 years or more in terms of gender, occupation, duration of employment, part of body, industry, size of business and work-related factors. Results show that the distributions of the injuries aged 50 years or older are statistically different from those of the injuries under 50 years. Also, older employees have high relationship between repetition, or contact stress/vibration and upper extremity MSDs. These findings can be used to develop more effective MSD prevention programs for older employees.

Simple Radiographic Analysis of Chronic Shoulder Pain in Patients 50 Years and Older (만성 견관절 동통을 가진 50세 이상 환자의 단순 방사선 사진 분석)

  • Purpose: To evaluate a usefulness of the simple radiograph in the patients with chronic shoulder pain 50 years and older. Material and method: 1152 patients with chronic shoulder pain and 100 asymptomatic individuals were involved in this study. All patients were 50 years and older. We excluded patients who had a history of fracture or dislocation. Radiographic interpretation was performed on a shoulder AP view, an axillary view and a supraspinatus outlet view. For statistical analysis, a chi-square test was performed. A p value of <0.05 was considered statistically significant. Results: Abnormal radiologic findings were identified in 369(32%) out of 1152 patients with a shoulder pain: greater tuberosity sclerosis, acromial sclerosis, subacromial osteophytes are common abnormal radiologic findings. A rotator cuff tear or impingement syndrome was identified on a final diagnosis in 61(85.2%) out of the 76 patients with radiologic abnormalities in both greater tuberosity and acromion (p<0.05). Abnormal radiologic findings were identified in 18% of the asymptomatic individuals. Conclusion: Simple radiographic analysis is an important primary diagnostic tool in patients (50 years and old) with chronic shoulder pain.

Calcium Intake and Its Major Food Groups and Dish Groups in Korean Adults Aged 50 Years or Older: Korea National Health and Nutrition Examination Survey 2015-2019 (50세 이상 한국인의 칼슘 섭취에 기여하는 주요 급원 식품군 및 급원 음식 분석: 2015-2019년 국민건강영양조사 자료를 이용하여)

  • Unhealthy dietary behavior such as insufficient calcium intake can be one of risk factors of osteoporosis and chronic diseases in older people. This study evaluated the recent trends in dietary calcium intake and the food source in Korean adults aged 50 years or older using the Korea National Health and Nutrition Examination Survey (KNHANES, 2015-2019) data. This study used 24-hour recall survey data to investigate the calcium intake, the major food groups and main dishes contributing to the calcium intake. The mean calcium intake was 479.55-506.81 mg/day. The major food groups that contribute to calcium intake were vegetables, milks and fishes. Dairy and frozen desserts and kimchi were the major dish groups that contributed to the calcium intake. Calcium intake from milk in dairy and frozen dessert group has been high in last 5 years (50-64 years old: 34.71-47.68 mg, 65-74 years old: 29.72-43.65 mg, over 75 years old: 22.91-42.93 mg). In addition, baechu-kimchi is the most contributed to the calcium intake in kimchi group (50-64 years old: 35.10-41.47 mg, 65-74 years old: 29.62-34.96 mg, over 75 years old: 23.79-29.13 mg). In conclusion, various source of calcium needs to be recommended to increase intake calcium in over 50 years, which may reduce chronic diseases and improve quality of life.

Anthropometric Index and Nutrient Intake in Korean Aged 50 Plus Years Living in Kugoksoondam Longevity-belt Region in Korea (전라도 구곡순담 장수벨트지역에 거주하는 중노년층의 체격지수와 영양소 섭취상태)

  • As the older adult period $({\geq}65y)$ is increasing, it is needed to investigate the trend of aging-dependent anthropomeric index and nutrient intake, and establish the more specific dietary guide for the different stages of aging period. To find the difference in nutrient intake among the Koreans aged 50-64, 65-74 and 75 years and older, and also any characteristics of dwellers in longevity area, we recruited 1,083 subjects (385 male and 698 female) aged 50-95 years (mean age, 71.3 yrs) living in Kugoksoondam area (Kurye, Goksung, Soonchang and Damyang counties), known as a longevity-belt region in Jeonlaprovince, Korea. We measured some anthropometric index and collected 2 day-dietary record. Nutrient intakes were analyzed by using DW24 program. The mean height and weight of subjects aged 75 years and older were lower than Korean national reference. BMI and obesity $({\geq}25kg/m^2)$ prevalence were significantly decreasing with aging. Underweight $(BMI<18.5kg/m^2)$ prevalence was also increasing with aging, especially in males, and it was slightly higher than national average, but similar to that in some other rural area. Obesity prevalence of male subjects was lower compared to national prevalence, but abdominal obesity prevalence $(waist{\geq}80cm)$ was very high in females (about 89%). In both genders, the average proportional contribution of carbohydrate, protein and fat to energy intake was not different between 65-74 years and 75 years and older. On overall, nutrient intake and quality of diet of females were inferior to those of males so that many of females aged 75 years and older assumed to be at risk of malnourished status. Fiber, folate and vitamin E intakes were substantially higher compared to those in 2007 KNHNES and other some studies in rural area. While almost nutrient %EAR was significantly decreased with aging in females, there was no significant difference in %EAR for protein, vitamin A, $B_1$, $B_6$, $B_{12}$, niacin, Ca and Zn between 65-74 years and 75 years and older in males. Vitamin $B_2$ for male aged 50-64 years, vitamin $B_2$ and Ca for male aged 65 years and over and female aged 50-74 years, and vitamin $B_2$, vitamin C, Ca and folate intake for female aged 75 years and older were assessed to be at risk to undernutrition based on the prevalence of intake below EAR. MAR of 13 nutrients and the number of nutrients consuming below EAR were significantly decreasing with aging in both genders, however, the number of nutrients of INQ < 1 and the average mini-nutritional assessment score were not significantly different between 65-74 years and 75 years and older. Taken together, decreasing tendency of nutrients intake and the quality of diet with aging was more evident in females than in males, and it is unique that our subjects consumed substantially higher fiber, folate and vitamin E compared to not only urban but also some other rural areas.

Impact of Age on Clinicopathological Features and Survival of Patients with Noncardia Gastric Adenocarcinoma

  • Purpose: Gastric cancer often occurs in the elderly but is uncommon in young individuals. Whether young patients have different clinical behaviors and outcomes from those of older patients remain unclear. Materials and Methods: We identified 1,366 cases of newly diagnosed noncardia gastric adenocarcinoma from the Kaiser Permanente Northern California Cancer Registry between 2000 and 2010. We then compared the clinicopathological features and survival among the different age groups. Results: The male : female ratio differed significantly between the younger and older patient groups (0.84 in age <50 years vs. 1.52>60 years, P<0.01). More younger patients were Hispanic (54% patients <40 years vs. 19% patients ${\geq}70$ years, P<0.0001), while more older patients were Caucasian (49% patients ${\geq}70$ years vs. 15% patients <40 years; P<0.0001). The diffuse/mixed histological type was more prevalent in younger patients (70% patients <40 years vs. 27% patients ${\geq}70$ years; P<0.0001), whereas the intestinal type was more frequent in older patients (71% in patients ${\geq}70$ years vs. 30% in patients <40 years; P<0.0001). Poorly differentiated adenocarcinoma was more common in the younger patients (80% in patients <40 years vs. 60% in patients ${\geq}70$ years; P=0.016). Survival rates at 1, 2, and 5 years gradually declined with increasing age (overall P=0.0002). Conclusions: Young patients with gastric cancer had more aggressive disease but higher overall survival rates than older patients. Younger Hispanic patients and older Caucasian patients were more likely to be diagnosed with gastric cancer. These differences may be due to biological predisposition and/or environmental exposure.

The Effects of Chewing Difficulty on the Prevalence of Osteoarthritis in Adults Aged 50 Years and Older (50대 이상 성인의 저작불편감이 골관절염 유병에 미치는 영향)

  • In this study, PASW Statistics ver. 18.0 was used to analyze the correlation between chewing difficulty and osteoarthritis in the population of 8,498 persons aged 50 years and older who had completed the health related questionnaire in the fifth Korea National Health and Nutrition Examination Survey in 2010~2012. The following conclusion was drawn: The distribution of the chewing difficulty status showed that 38.8% of the respondents had chewing difficulty and that there was no significant difference by gender. Older respondents tended to have significantly greater chewing difficulty 34.3% of the respondents aged from 50 to 64 years and 46.4% of those aged 65 years and older, those who had lower levels of education and income, who were smokers, and who had higher levels of depression and stress suffered from significantly greater chewing difficulty. Chewing difficulty and osteoarthritis were correlated with each other: the respondents having chewing difficulty had 1.44 (95% confidence interval [CI], 1.23~1.70) higher prevalence of osteoarthritis than those with no such difficulty. In addition, such correlation was not found in males: the former had 1.36 (95% CI, 0.98~1.88) higher prevalence of osteoarthritis than the latter, which was not statistically significant. Such correlation was found in females: the former had 1.47 (95% CI, 1.22~1.76) higher prevalence of osteoarthritis than the latter, which was statistically significant. Therefore, the chewing difficulty status affected osteoarthritis for females aged 50 years and older. Therefore, the efforts to solve oral health problems through oral hygiene would help prevent osteoarthritis from worsening and manage the condition.

The Estimated Proportion for Influenza Vaccination and Related Factors in Korea Adults Aged 50 and Older with Asthma (50세 이상 천식 환자의 인플루엔자 백신 접종률 및 관련 요인)

  • Objectives: This study aims to investigate the estimated proportion for influenza vaccination and to identify factors associated with influenza vaccination in Korean adults aged 50 and older with asthma. Methods: Data from 2010-2015 Community Health Survey(n=23,662) was used. A chi-square test was performed to investigate the estimated proportion for influenza vaccination, and a multiple logistic regression analysis was used to identify the factors associated with self-reported influenza vaccination. Results: In men, 42.3-49.5% of asthma patients 50-64 years of age, and 78.5-90.2% of elderly (over 65 years of age) asthma patients received influenza vaccination. In women, 49.7-61.9% of asthma patients 50-64 years of age, and 82.7-89.7% of elderly asthma patients received influenza vaccination(p<0.0001). Low education level, non-smoking, non-drinking, hypertension, diabetes and use of public center were related to high influenza vaccination in 50-64 years old men with asthma. Low education level, non-smoking, bad health status, hypertension, diabetes and use of public center were related to high influenza vaccination in 50-64 years old women with asthma. Non-smoking and use of public center were related to high influenza vaccination in over 65 years old men with asthma. White-collar jobs, smoking, absence of hypertension and use of public center were related to low influenza vaccination in over 65 years old women with asthma. Conclusions: Influenza vaccination remains relatively low in asthma patients 50-64 years of age. It is necessary to recommend vaccination to asthma patients, provide them with information, and devise other strategies to improve vaccination.

Association between Metabolic Syndrome and Macronutrient Intakes in Adults aged 50 years and older based on the 2013-2017 Korean National Health and Nutrition Examination Survey Data (2013-2017년 국민건강영양조사 자료에 근거한 50세 이상 성인의 대사증후군과 다량영양소 섭취의 관련성)

  • The purpose of this study was to investigate the association between dietary macronutrient intakes and prevalence of metabolic syndrome (MetS) in Korean adults. Data were obtained from the Korean National Health and Nutrition Examination Survey (2013-2017), and a total of 11,600 Korean adults (4,918 men, 6,682 women) aged 50 years and older were analyzed. The daily intakes and percentages of energy from carbohydrates or fat in men or women with MetS were significantly lower than in normal subjects, respectively. High carbohydrate or fat percentages were negatively associated with MetS based on adjusted odds ratios (OR) of 0.804 in men (p=0.034) and 0.820 in women (p=0.045), respectively. A high percentage of energy from carbohydrates was positively associated with reduced waist circumference, diastolic blood pressure, triglyceride and high-density lipoprotein (HDL) cholesterol in men. On the other hand, a high percentage of energy from fat was positively related with elevated HDL cholesterol and reduced triglycerides in women. In conclusion, our study indicates that high carbohydrate or fat intake is associated with risk of MetS in Korean men or women aged 50 years and older, respectively. Further prospective studies are necessary to elucidate the association between macronutrient intakes and MetS among Korean adults according to age.

Comparative Study on Nutrition Attitudes, Food Purchase Behaviors, and Dietary Habits of Housewives Living in Seoul and Kyunggi Area (주부들의 영양태도, 식품구매 및 식생활 평가의 차이에 관한 비교 연구 -서울특별시와 경기도 일부 지역에 거주-)

  • This study focused on the nutrition knowledge, food purchase behaviors, and dietary habits of 213 housewives (41 women under 39 years, 98 women 40~49 years, and 74 women over 50 years). Younger housewives held more special jobs, whereas older housewives worked as housewives or had no jobs ($p$<0.05). The average monthly income of the three family groups was 3,000,000~5,000,000 won, and the monthly dietary expenditure for all three groups was 200,000~500,000 won. Familial frames for the three groups was nuclear family (over 80%), but housewives younger than 39 years (19.5%) lived together with their parents and let them take care of their children. Housewives (over 50 years) prepared a balanced diet for every meal, utilized less drastic cooking methods to prevent loss of nutrients, and shopped at many places in order to save money on various food materials. Further, they purchased food materials that resulted in the least amount of left overs. On the other hand, housewives under 39 years visited large supermarkets and purchased cooked foods more than other groups. Housewives under 39 years considered nutrients content as the most important factor when purchasing food materials, whereas 74.5% of housewives between 40~49 years and 51.4% of housewives over 50 years put the most importance on flavor ($p$<0.05). Housewives under 39 years were interested in manufacture date, nutrient content, and food price, in decreasing order, whereas housewives between 40~49 years and over 50 years were interested in manufacture date, food prices, and nutrient content. Older housewives were interested in brown rice and fruits, whereas younger housewives under 39 years were interested in meat, fish, eggs, fried foods, and fast food. Older housewives over 50 years did not consume fried foods more than two times per week and consumed three regular meals more often than younger housewives. Accordingly, for younger housewives, it is necessary that healthy foods be cooked for their families.

Treatment of Spontaneous Pneumothorax; in Patients 50 Years of Age or Older (노인성 자연 기흉의 치료방법 및 그 결과;50세 이상환자 60례 대상)

  • To assess the therapy of spontaneous pneumothorax in the aged patients, the treatment methods and results in 60 patients 50 years of age or older were retrospectively reviewed. Most of the patients were male [56 of 60 patients] and the major underlying lung diseases associated with spontaneous pneumothorax were tuberculosis [32 patients] and chronic obstructive lung disease [20 patients]. The recurrence rate of thoracostomy tube drainage with or without chemical pleurodesis using tetracycline was 39.6% [21 of 53 patients], but there was no recurrence in the patients treated with open thoracotomy, pleural abrasion, and chemical pleurodesis using talcum powder[Asbestosis free]. In the patients treated with open thoracotomy, the bullous or bleb lesions were placed in the various sites of both lungs. We concluded that even though thoracostomy tube drainage is the first choice of therapy for spontaneous pneumothorax in the aged patients, the recurrence rate is high, especially in the patients with persistent air leakage for more than 2 days, and the open thoracotomy with pleural abrasion and chemical pleurodesis using talcum powder can prevent the recurrence in the selected patients.
