119 구급대원의 구강악안면 외상환자의 지식, 태도, 수행에 관한 연구 (A study on the knowledge, attitudes, and performances of 119 EMTs regarding oral and maxillofacial trauma patients)

  • Purpose: This descriptive research study aimed to identify and strengthen the knowledge, attitudes, and performances related to first aid for patients with oral and maxillofacial trauma among 119 EMTs working at 119 safety centers and local stations in Jeollanam-do, South Korea. Methods: An online questionnaire was distributed among 144 119 EMTs working at fire stations in Jeollanam-do from August 1 to August 15, 2022. Results: The factors that influenced the knowledge of oral and maxillofacial trauma among the 119 EMTs included qualification (β=.113, p=.005) and performance (β=.837, p=.005), and those influential to attitudes were qualification (β=0.194, p=.005), ordinary interest (β=0.242, p=.005), and the need for education (β=.285, p=.005). The only factor that influenced the performance of first aid for patients with oral and maxillofacial trauma was knowledge (β=.900, p=.005). Conclusion: Considering the nature of first aid for patients with oral and maxillofacial trauma, it is necessary to organize practical knowledge and educational content tailored to the characteristics of each task. This should be simultaneously conducted with practice-oriented simulation education.

119구급서비스 품질관리가 병원 전 심정지 환자의 생존율에 미치는 영향 (The Effectiveness of Quality Control of 119 Emergency Medical Services on Survival Rate of Cardiac Arrest Patients)

  • Hospitals in Korea are enforcing a quality control over 119Emergency Medical Services to increase the survival rate of Out of Hospital Cardiac arrest patients. This study is to analyze the factors that effect the survival rate of Out of hospital Cardiac arrest patients by comparing the results of before and after the quality control enforcement. Cardiac arrest patients can be assorted into pre-decision group and decision group. The study analyzed the survival factors which was based on the adequate use of 119 BLS and ACLS usage rates, response time of 119EMS, qualification of ambulance worker, number of EMS team members, and adequate use of AED according to ECG diagnosis. The analyzation was done over total 1,233 of Cardiac arrest patients from January 1, 2010 to December 31, 2011 After the enforcement of the quality control, the usage of Vital sign check and BLS showed a big difference. Especially, as the usage of ECG showed a significant difference, Shockable rhythm, which is the most important to Cardiac arrest patients, also showed a significant difference. After the enforcement of quality control, the performance of ACLS showed a significant difference in IV. The study showed 119 ambulance workers provided better service in Vital sign and BLS and ACLS after the enforcement of quality control. It is considered a 119 ambulance service effects the survival rate of Cardiac arrest patients. Therefore, it can increase the survival rate and it is necessary to continue a quality control.

  • 본 연구는 코로나19 상황에서의 119구급대원들의 공감능력과 공감피로가 삶의 질에 미치는 영향을 살펴봄으로써 구급대원들의 삶의 질을 향상시키기 위한 구체적인 방안 마련에 기초 자료를 제공하기 위하여 실시되었다. 2021년 3월 10일부터 6월 15일까지 K도 및 D시의 119구급대원 250명을 대상으로 설문조사를 실시하였으며, 수집된 자료는 SPSS/WIN 25.0을 사용하여 빈도분석, 상관관계 및 다중회귀분석로 분석하였다. 다중회귀분석 결과, 119구급대원의 코로나19 관련 심리 변인이 공감능력, 공감피로 및 삶의 질에 미치는 영향 분석에서는 각각의 하위요인에 유의한 영향을 미치는 것으로 나타났다. 이러한 결과를 토대로 119구급대원들의 삶의 질을 높일 수 있는 제도가 만들어져야 할 것으로 제안한다.

119구급대원의 감염관리에 대한 인지도와 수행도 관계 연구 (A Study on the level of Recognition and Practices of 119 Emergency Medical Technician about Infection Control)

  • In order to protect Rescue 119 workers exposed on the spot from potential infection, this study identified their awareness and practices of infection control so that it could help preventing them from infection and also provide basic materials necessary for pre-hospital infection control. This study applied questionnaire survey to total 215 Rescue 119 workers at fire stations in Jeonbuk province, Jeonnam province and Gwangju city from July 14 to Sept. 14, 2006 for the benefit of data collection. The questionnaire about possible associations between awareness and practices of infection control consisted of total 46 times across 6 categories such as washing hands during emergency activities ; fluid therapy and injection ; respirator maintenance; individual hygienics ; disinfectant supplies and equipments maintenance ; and control of infectious wastes. And collected data were processed using SPSS statistic program to analyze frequency and percentage, mean and standard deviation, Pearson's correlation coefficient, t-test and one-way ANOVA. As a result, this study came to the following conclusions : In terms of awareness about infection control, our respondents showed highest awareness about infectious waste control, and also showed highest level of practices in washing hands during emergency activities. Throughout all domains, awareness means were higher than practice means. In particular, infectious waste control was the domain of significant differences between awareness and practices. In terms of associations between awareness and individual characteristics, it was found that female rescue worker group and hospital/general hospital career group (before joining the Rescue 119) showed significantly higher awareness on statistic level. In regard to associations between individual characteristics and practices, it was found that female rescue worker group showed higher level of practices than male group on statistic level. This study also analyzed correlations between rescue workers' awareness and practice of infection control. As a result, it was found that the higher awareness was in correlations with the higher practices across all 6 domains including washing hands. In addition, the higher awareness of a questionnaire item was in significantly positive correlations with the higher practice of other items. However, our respondents showed high awareness about anti-infection, but low practices in reality. This indicates necessity of devising possible solutions to improve the practices as much as awareness. Especially, it was noted that major reasons for insufficient practices of infection control guideline come from unhabituated practices and lack of supports for infection-preventing supplies and protective device (mask, etc). Hence, it is necessary to provide more infection-preventing supplies for local rescue workers sufficiently, in parallel with steady habituation of infection control. Furthermore, it is required to manage and study infection control policies even at pre-hospital step in efforts for effective infection control, education and activities.

119구급대원들의 근로생활의 질이 조직유효성과 고객지향성에 미치는 영향 (Influence of The Quality of Work Life on Organizational Effectiveness and Customer Orientation in 119 Paramedic's)

  • 본 연구는 119 구급대원들의 근로생활의 질이 조직유효성 및 고객지향성에 미치는 영향을 규명하고, 응급의료서비스의 질적 개선을 위한 기초자료를 마련하기 위하여 실시되었다. 대구 및 경북 소방안전센터에 근무하고 있는 119 구급대원 남녀 308명을 대상으로 2019년 2월 3일부터 5월 7일까지 설문조사를 실시하였다. 수집된 자료는 SPSS 20.0을 사용하여 빈도분석, 상관관계 및 다중회귀분석을 실시하였다. 연구결과는 첫째, 119구급대원들의 근로생활의 질이 조직유효성 및 고객지향성 간의 정적상관관계를 보이고 있는 것으로 나타났다. 둘째, 119 구급대원들의 개인차원, 관리차원의 근로생활의 질이 조직유효성의 조직몰입, 직무만족에 유의한 영향을 미치는 것으로 나타났다(p<.001). 셋째, 119 구급대원들의 개인차원, 조직차원, 관리차원의 근로생활의 질이 고객지향성에 유의한 영향을 미치는 것으로 나타났다(p<.001). 넷째, 119구급대원들의 조직유효성이 고객지향성에 미치는 요인으로 조직몰입, 직무만족도에 유의한 영향을 미치는 것으로 나타났다(p<.05). 이러한 결과를 감안할 때, 119 구급대원들의 복지제도를 개선함으로써 응급의료서비스의 질향상을 도모하기 위한 정책마련이 필요하다.

일부지역 유아 교육 시설의 안전사고에 대한 교사들의 실태 분석 (An Analysis of Teacher's Perceptions on Safety Accident in Facilities for Children's Education)

  • The purpose of this study was teacher's perception on young children's safety life, safety accident, and safety education and provided basic data of administrating teacher's education for young children's safety. Subjects of this study were teachers of young children attending for their education. 230 questionnaires were provided and 181 were collected and 170 were used for data analysis. Data collected were analyzed with SPSS WIN 2.0 program. The results of the study were as follows : 1. Regarding teacher's perception on types of young children's safety accident, play accident was high(70.0%). 2. With regard to teacher's perception on causes of accident, lacks of perception was high(64.1%). 3. Of transportation means in accident, 119 ambulance use was high(60.5%) 4. Regarding teacher' perception on accident prevention, direct attention of education by paramedics was high(48.2%).

PRECEDE 모형에 기반한 119구급대원의 감염관리 수행 관련 요인 분석 (Analysis of the factors related to the infection control practice of 119 emergency medical service providers based on the PRECEDE model)

  • Purpose: Emergency medical service (EMS) personnel are at high risk of spreading infection. In this study, we used the PRECEDE model to identify the knowledge, status, and barriers to infection control among Korean paramedics to provide basic infection control data. Methods: A total of 164 respondents were analyzed for the study. A questionnaire was administered and collected through an online self-response platform. Descriptive analysis, t-test, ANOVA, multiple regression, and logistic regression analyses were performed to determine infection control practices and associated factors using SAS 9.4. To identify the pathways and direct, indirect, total effects based on the PRECEDE model, we used AMOS 26.0. Results: Highly rated self-efficacy (OR 8.82, 95% CI: 3.23-24.09), awareness (OR 6.05, 95% CI: 2.06-17.72), and enabling factors (OR 3.23, 95% CI: 1.18-8.78) led to superior infection control. As a result of the structural model analysis, the highly rated enabling factors and awareness led to superior practice patterns. Conclusion: Practice is related to self-efficacy, awareness, and enabling factors; however, further research is needed to develop strategies for infection control. In particular, institutional arrangements are needed to improve the enabling factors. Improving infection control performance may lead to better infection control and enhanced protection of EMS personnel and patients against infection risks.

119구급대원의 업무 관련성 근골격계 질환 위험성 평가 : 환자 운반 작업을 중심으로 (Assessing the risk of work-related musculoskeletal disorders in 119 EMT: a focus on patient-carrying tasks)

  • Purpose: This study aims to evaluate the risk of work-related musculoskeletal disorders (WMSDs) in 119 EMT during patient-carrying tasks using a long backboard (LBB) and a variable stretcher. Methods: Manikins were used as patients, and 45 new 119 EMT from K Fire Academy were filmed performing patient-carrying tasks on stairs. The tasks were analyzed using Ovako working posture analysis system (OWAS) and Rapid entire body assessment (REBA). Results: In using LBB, the OWAS score was Mode 3(Mean 2.37, Maximum 3), requiring as soon as possible corrective action, the REBA score was Mode 11(Mean 9.16, Maximum 11), requiring immediate improvement. In using variable stretcher, the OWAS score was Mode 1(Mean 2.33, Maximum 3), non-necessity for corrective action, the REBA score was Mode 9(Mean 8.0, Maximum 11), requiring as soon as possible. Conclusion: In conclusion, improvement was needed in one task (carrying a patient using a LBB) in the OWAS and in two tasks (carrying a patient using a LBB, carrying a patient using a variable stretcher) in the REBA. Thereby, required attention and management of WMSDs during training. In addition, it is essential to carry out objective and quantitative assessments through ergonomic analysis by occupational health professionals when designing future training programs to prevent WMSDs.

응급구조(학)과 학생들의 구급현장실습 표준 평가도구의 유용성에 관한 연구 (A study on the usefulness of standard evaluation tools for ambulance ride practices of paramedic students)

  • Purpose: This study tests the validity of a standard evaluation tool of ambulance ride practices with new evaluation items and supports its application by 119 preceptors. Methods: We tested the validity of standard evaluation items collected from 19 on-site professionals assessing ambulance ride practices. New evaluation items, 'understanding of emergency rescue equipment' and 'ability to communicate', were added as additional criteria. The modified Delphi technique was used to test the three evaluation areas and ten evaluation items. We used the analytic hierarchy process to analyze the weighting value of the reconstructed evaluation tool model. Results: All three evaluation areas and 10 evaluation items within the standard evaluation tools used for ambulance ride practices corresponded with the consistency index, degree of convergence, and agreement in the modified Delphi panel. Conclusion: These results provide evidence of the consistency and usefulness behind preceptors' use of this standard evaluation tool in ambulance ride practice.

가슴압박시 구급대원의 체간 각도와 근활성도 분석 (Analysis of trunk angle and muscle activation during chest compression in 119 EMTs)

  • Purpose: We aimed to investigate trunk angle and muscle activation of the extremity and back to evaluate the effect of chest compression on work-related musculoskeletal disorders in 119 emergency medical technicians (EMTs). Methods: Eighteen 119 EMTs performed 2-minute chest compression without interruption on a cardiopulmonary resuscitation manikin, during which we measured changes in the trunk and shoulder joint angles, muscle activation (triceps brachii, biceps brachii, erector spinae, gluteus maximus, pectoralis major, rectus abdominis, and rectus femoris) and chest compression accuracy. Results: The decrease in trunk angle by trunk muscle activation was the highest in event 2, the major direction of chest compression. Both shoulder joint angles had no significant difference. Muscle activation of the triceps brachii (p < .01), biceps brachii (p < .05), rectus abdominis (p < .05) and rectus femoris (p < .01) significantly increased during the compression phase compared with the decompression phase, with the rectus femoris showing an increase of 19%. Muscle activation of the erector spinae significantly increased in the decompression phase compared with the compression phase (p < .01). Conclusion: 119 EMTs mainly use the triceps brachii, biceps brachii and pectoralis major muscles during chest compression.
