Purpose : The purpose of this study was to provide appropriate direction for triage education by investigating the knowledge level of triage in 119 EMTs before and after the triage education. Methods : The questionnaire was filled out by newly assigned 33 EMTs in the fire service academy in I metropolitan city from November 1, 2013. The data were analyzed using SPSS WIN 21.0 program. Results : The lowest knowledge level by 119 triage was the potential emergency related question, but that by case-based triage was the semi-emergency related questions. The knowledge score by case-based triage before education was 51.14 points. This was lower than knowledge score by 119 triage, that is, 75.70 points. After education, the knowledge level by 119 triage was significantly improved(p =.000). However, there was no significant difference in the knowledge level by case-based triage(p =.236). Conclusion : It is necessary to provide systematic and periodic education and training for 119 EMTs toward triage to improve triage accuracy and efficient circulation of the emergency medical service system. Especially, it is very important to provide case-based triage education for field application.
Even now, 119 rescue services have dissatisfactory aspects in operation, system and equipments as discussed above, It is the most urgent subject to systemize rescue services so that they can be suitable for our status, for we will make 21C welfare state come true before long. So, this author suggest that the followings have to be raised to activate 119 rescue service. 1) Bring up experts and offer high-quality rescue service 2) Prepare more up-to-date equipments 3) Operate transfer joint organizations 4) Promote the ability to meet with a press at the time of rescue service activities 5) Adjust regulations related to rescue services 6) Make up for a countermeasure to traffic accidents of ambulances 7) Adjust regulations making it mandatory to establish heliport at the target on hospitals more than a defined scale 8) Install more rescue service teams 9) Educate and train officials belonging to briefing rooms, where the officials with long experiences are arranged 10) Minimize the time for rescue team to reach fields 11) Establish legal protection system for rescue the team Nowadays, our country operates the department of fire fighting and rescue services without great difficulty, even though the circumstances are bad - insufficient members and the inferior circumstances. All of the fire fighting officials are given heavy duties in bad circumstances, and so are the team of rescue service. The rescue service team, taking charge of some emergency medical system, do a fire fighting inspection as a non-duty service, though they are scanty of sleep due to prevention and protection services of the fire fighting service team. But, they can not engage in rescue services completely and have to deal with miscellaneous duties. So they can not offer professional emergency medical services. But now, almost every fire fighting organization, belonging to National Emergency Management Agency, are separating rescue services, which shows a lot of good results. People recognize rescue services to get better and better gradually and the demands for this rescue services increase. So, this is the best time when rescue service teams should offer qualitative services rather than quantitative services. The people will recognize this rescue service team to be an organization sacrificing and serving for them. However well institutes and operation systems should be established, the rescue service team can not come true their aim without strong wills that they will serve and sacrifice themselves for people from their hearts. In addition, it is essential for the officials in charge of policies about emergency medical services to have a concernment on and practice the policy without failure.
In terms of reinforcing the functions and services of rescue 119 and emergency corps, we must review the following considerations : Building up security system along with operational expansion, building up a system for emergency medical treatment, buliding up a comprehensive information management system for rescue and emergency, constructing a provincial safety museum and so forth. For the ways to better the works of rescue 119, we can review the following considerations : Improving the education for fire fighting trainging corps under Jeju Provicial Fire and disaster management department, providing rescue members with more opportunity for clinical practices, enhancing the morale of rescue members, installing a comprehensive briefing room for emergency rescue members, building up medical networks along with reasonable policies for information service, operating the consulting system for rescue 119 and so on. Provide little more superior quality high fire fighting service to jeju islander, and need long-term and elaborate research to correspond in rising fire fighting man-power demand newly and investment. Construction of well-matched disaster administration system is assignment that must hurry in international free city. That jejudo receives time factor about that is received for another thing area's number of persons or equipment when regional special quality large scale or disaster happens is that of course the support receives limitation immediately. Jeju fire fighting services is one story reform, specialization taking into account particular situation of jeju area to grow as sightseeing and center of culture intending international free city, should be upgraded. An analysis was performed to evaluate the prehospital advanced emergency care in Emergency Medical Service system(EMS) through the review of the 119 transfort chart of 190 patients who visited to Jecheon Seoul hospital emergency center at Chungbuk Jecheon via 119 system during the period from October 2002 to September 2003. The results were obtained as follows: 1. Among 190 patients, male were 127(66.8%) and female were 63(33.2%) and nontraumatic patients were 81(42.6%) and traumatic patients were 109(57.4%). 2. The analysis of prehospital care by Emergency Medical Technician(EMT) revealed that Airway maintenance and keeping oral airway(37.7%), oxygen supply(75.9%), Tourniquett or MAST(18.9%), Immobilization of neck or spine(94.7%), Immobilization of extremities (51.4%), wound bandage and dressing(25.0%), BCLS(12.1%) and Application of AED(2 patients). 3. The analysis of prehospital care by EMT revealed prehospital care was limited to keep of intubation or LMA, medication and IV insertion, insertion of gastric tube and ACLS. 4. There was no case of the notification to medical institute or consultation to doctor.

Purpose: The purpose of the study is to check up the status of 119 emergency control centers usage. Therefore, the status of use of 119 emergency control centers and the incidence of pre-hospital cardiac arrest patients were investigated. Methods: The emergency activity daily reports and first aid diaries of 119 emergency control centers from January to December 2018 were reviewed. For more accurate status analysis, Among the first aid guidance received in the emergency rescue standard system, the cardiopulmonary resuscitation guide log was reviewed. Results: In 2018, the total usage of the 119 emergency control centers was 1,358,356 calls, hospital guidance werethe most commom (n=629,676, 46.4%), followed by first aid (n=428,027, 31.5%), disease consultation (n=170,238, 12.5%), medical oversight (n=111,188, 8.2%), and interhospital transfer (n=5,052, 0.4%). Regarding the user number per 1,000 persons, Jeju was the greatest at 48.0, whereas Changwon was the lowest at 13.0. A total number of dispatcher-assisted cardiopulmonary resuscitation was 12.181. The time from report to chest compression were 156.2±80.8 seconds for those with previous cardiopulmonary resuscitation training and 168.0±79.3 seconds for those without such training (p<.05). Conclusion: The ratio of first aid instructions, including dispatcher-assisted cardiopulmonary resuscitation, among total usage of the 119 emergency control centers increased. Therefore, additional efforts are required to improve the quality and expertise of information provided through the 119 emergency control centers.
Purpose: The aim of this study was to provide basic data to aid in the effective adoption of the preceptor system for new paramedics by assessing the preceptor role awareness among senior 119 paramedics in the field. Methods: A questionnaire was administered to 182 senior paramedics who each had more than 3 years field experience. The questionnaire measuring preceptor role awareness was composed of 64 items with responses based on a 5 point Likert scale. Data were collected from July 31, 2018 to August 29, 2018, and were analyzed with IBM SPSS version 24.0. Results: The average score for preceptor role awareness was 3.87 (supervisor = 3.95, resource person = 3.88, role model = 3.85, and instruction designer = 3.81). There were significant differences in preceptor role awareness among the participants based on competence (p =.002) and clinical skill (p =.000). Also, there were high positive correlations between preceptor role awareness and its subdivision (p <.01). Conclusion: For effective operation of the preceptor system in the future, the preceptors' role as designers of instruction and role models should be enhanced, Also, future preceptors are very important in the improvement of competence and clinical skill in the preceptor system. Therefore, it is necessary to provide training that enables new preceptors to effectively adopt and operate the preceptor system.
The results of analyzing the questionnaires with 119 emergency staffs working in Gwangju and Jeonnam region are as follows. 1. The whole stress score was mean 2.94 and the degree of stress from personal relationship was highest as 3.22. 2. There were significant differences in possession of qualification related to public health and medical treatment (F=3.090, p=.030) in service factors, total career of emergency staffs (F=2.979, p=.023) in working environment factors, age (F=2.835, p=.042) and sex (F=2.375, p=.019) in social position factors, possession of medical qualification (F=3.995, p=.010) and household account burden (F=2.777, p=.045) in personal relationship factors. 3. There were correlations (p=.475) between working environment factors and social position (p<0.01) and correlations (p=.508) between working environment factors and personal relationship factors (p<0.01). There was correlation (p=.415) between social position and personal relationship factors(p<0.01). More concrete research that examines whether current fire station operation system is desirable or not by comparing the degree of stress of the whole 119 emergency staffs in Korea is needed and it is considered that real allowance level should be secured to reduce their stresses and facility and guarantee system to satisfy their demands for welfare are required.
Based on 2009 Korea Health Panel, this study investigated socio-economic and clinical characteristics associated with emergency medical transport use, and analyzed a simple predictive model of emergency medical transport use. Analysis results were summarized as follows: First, emergency medical transports such as 119 ambulance were more used than private cars, taxis, or walk-in. Second, between a user group and a non-user group of emergency medical transports, there were statistically significant differences in age, the level of education, family composition, house type, household income, the relationship with the head of household, insurance types, the presence of handicap, the presence of chronic disease, reasons to emergency medical service use, and treatment after emergency medical service completed. Third, age, household income, the presence of handicap, reasons to emergency medical service use, and treatment after emergency medical service completed were statistically significant predictors associated with emergency medical transports use. To improve emergency medical service system, the characteristics and predictors associated with emergency medical transports are more concerned.
Glucose is the most importantenergy source in the central nervous system. Because the central nervous system cannot synthesize or store glucose. low blood glucose even for a short period results in no functionally normal activity of the brain. In addition, continuous low blood glucosecan cause irreversible brain damage and brain death. Therefore, the current status of emergency care by first aid staffs during transfer of patients with hypoglycemia has been investigated in this study, 114 patients who have visited the hospital using 119 Emergency Medical Service from January 1, 2004 till December, 2004 and shown blood glucose of 2.8 mmol/L and less on arrival have been involved. In case of nurses, assistant nurses, first aid staffs receiving first aid training and second-class emergency medical technicians(EMTs), they provided basic life support (BLS). In case of first-class EMTs, they measured blood sugar and then they orally administered glucose to conscious patients or provided simple first aids and prompt transfer for unconscious patients. As a result of analysis, it is suggested that first aid retraining is required.

Since a few years ago, great disasters have often took place in domestic and abroad, such as Sampoong Department store collapse, Sea-land disaster accident, Incheon Hof fire accidents. Whenever a lot of accidents occurred such as fire, gas explosion, traffic accident, mountain accident, various disease, people required 119 first aid service. As the result, even for the last year, average 2,608 person per day were temporary treated and carried to near hospital by 119 system. But first-aid team or first grade EMT(emergency medical technician) that should provide high quality service and save important lives, might have negatively treated or made their duty only for simple transfer, when they were imminent of emergency case in every kinds of accident spot. Because they worried about medical dispute for "task restriction for EMT" decided by medical laws or rules of first aid. So it was proposed that "task restriction for EMT" should be mitigated or released in the possible range on this occasion.
