QUANTITATIVE COMPARISON OF PERMEABILITY IN THE ADHESIVE INTERFACE OF FOUR ADHESIVE SYSTEMS (열순환 후 상아질 접착 계면의 수분 투과성 변화에 대한 정량적 분석)

  • The purpose of this study was to perform quantitative comparisons of water permeable zones in both the adhesive and the hybrid layer before and after thermo cycling in order to assess the integrity of the bonding interface. Twenty eight flat dentin surfaces were bonded with a light-cured composite resin using one of four commercial adhesives [OptiBond FL (OP), AdheSE (AD), Clearfil SE Bond (CL). and Xeno III (XE)]. These were sectioned into halves and subsequently cut to yield 2-mm thick specimens; one specimen for control and the other subjected to thermocycling for 10,000 cycles. After specimens were immersed in ammoniacal silver nitrate for 24 h and exposed to a photo developing solution for 8 h, the bonded interface was analyzed by scanning electron microscopy (SEM) and wavelength dispersive spectrometry (WDS) at five locations per specimen. Immediately after bonding. the adhesive layer of OP showed the lowest silver uptake, followed by CL, AD. and XE in ascending order (p < 0.0001); the hybrid layer of CL had the lowest silver content among the groups (p = 0.0039). After thermocycling, none of the adhesives manifested a significant increase of silver in either the adhesive or the hybrid layer. SEM demonstrated the characteristic silver penetrated patterns within the interface. It was observed that integrity of bonding was well maintained in OP and CL throughout the thermocycling process. Adhesive-tooth interfaces are vulnerable to hydrolytic degradation and its permeability varies in different adhesive systems, which may be clinically related to the restoration longevity.

Full mouth rehabilitation for a patient with vertical dimension loss using digital diagnostic analysis: A clinical report (수직고경이 감소된 환자의 디지털 진단 분석을 이용한 완전 구강 회복 증례)

  • Full mouth rehabilitation is re-organizing the occlusion of the remaining teeth and missing teeth considering the functions, esthetics, and neuromuscular harmony. With the loss of multiple teeth, the patient's occlusal plane gradually collapses and the vertical dimension can be reduced. Since reduced vertical dimension can be a potential etiology of the temporomandibular joint and masticatory muscles, prosthetic restoration with increased vertical dimension is required. This case report is about a 68 years old patient with vertical dimension loss due to worn dentition and multiple loss of teeth. In this case, the loss of vertical dimension is assessed carefully using the digital dentistry technology. Using CAD software in digital analysis step, the occlusal plane was established and evaluated using several criteria. Orienting the position of the bone and teeth using CBCT image, patient's condition was visualized in 3 dimension and treatment planning was possible virtually. The information that matches the patient's condylar position with the articulator, which is the virtual face bow, is reproduced on the actual articulator, and evaluated again. After the evaluation, provisional prosthesis was fabricated and it was confirmed that the patient adapts without any abnormality. This was implemented as a final prosthesis. As a result, the patient obtained satisfying results, utilizing the benefits of digital dentistry technology and traditional methods.

  • We described two cases of odontogenic keratocysts of the maxilla. Odontogenic keratocysts (OKCs) are jaw cysts with a proclivity for local invasion and recurrence. Clinically, OKCs are characterized by aggressive, local growth. The diagnosis of mandibular OKC is based on physical examination and plain film radiographic findings. However, histopathological confirmation is required to make the diagnosis with certainty. The molar regions of the mandible and maxilla are the principal primary locations. The maxillary antrum is also a common site. Initial therapy is typically enucleation with or without extraction of the associated teeth. Long-term follow-up is necessary because of the aggressive nature and recurrence rate of OKCs.

  • Although the first case of supernumerary teeth had been documented almost 20 centuries ago, the etiology of supernumerary teeth still remains unclear. The prevalence of supernumerary teeth in the general Asian population is between 2.7% and 3.4%. The pathogenesis of supernumerary teeth has been attributed to phylogenetic reversion(atavism), splitting of the tooth bud(dichotomy theory), locally induced hyperactivity of the dental lamina and a combination of genetic and environmental factors(unified etiologic explanation). This report describes 3 cases of monozygotic twins with mesiodens who visited the pediatric dental clinic of Kyungpook national university hospital, and this is significant to support genetic factors involoved in the development of supernumerary teeth.

  • Restorative dental materials have advanced rapidly, with improved physical properties that improve survival rates. Accordingly, various materials can be selected. Amalgam, composite resin, glass-ionomer cement, and preformed stainless steel crowns have all been used widely for the restoration of dental caries in primary molars. The various dental materials used to treat proximal caries in the primary molars have distinct advantages and disadvantages. However, few studies have examined their survival rates. This retrospective study examined the 2-year survival rates of more than 700 class II restorations of proximal caries in primary molars clinically and radiologically according to the type of restoration. The study results should help in the selection of class II restorations for molars, one of the biggest concerns of pediatric dentists.

  • Recentrly, early dental care for children is becoming more important, and the number of children patients' dental services is on the rise. So, a variety of seduction therapies are needed to alleviate the tension in children who are anxious about dental care. This paper describes the development process and contents of system that can display the animation of different stories according to the treatment stage by grasping the use state of the treatment device in real time. In the animation, the hreo character and the villain, the cavity character, appear and the hero defeat the villain at each treatment stage. Animation is played when treatment starts and ends after treatment. Current existing one-side played animation of dental clinics is not enough to attract children's attention. If the flow of animation changes depending on the treatment process, it will give children a different enjoyment and consequently ease the tension over the treatment.

저온 대기압 플라즈마 제트를 이용한 우치 미백효과

  • 현재 치과에서 상용되는 치아미백법은 과산화수소와 레이저를 사용하여 의사가 직접 치료를 하고 있다 [1]. 단기간에 높은 미백효과를 얻기 위해, 고농도의 과산화수소를 이용하게 되는데, 이는 암 또는 심장병 등을 유발시키는 원인이 될수 있음으로 인체에 매우 유해하다 [3,4]. 우리는 식품의약품안정청에서 규제하고 있는, 카바마이드 퍼옥사이드(15%)를 사용하였다. 카마바이드 퍼옥사이드(15%), 수증기, 저온 대기압 플라즈마 제트를 사용하여 미백효과를 관찰하였다. 기체 유량은 1,000 sccm 이며, 공기와 질소를 사용하였다. 미백효과를 보기 위한 대상으로는 우치(牛齒)를 사용하였으며, 플라즈마를 처리하여 미백효과를 관찰하였다. 실험 대조시료군으로는 카바마이드 퍼옥사이드(15%)와 수증기(0.4%)를 첨가한 다음, 공기 플라즈마와 질소 플라즈마를 조사하여 비교해보았다. 수증기를 첨가한 이유는 활성산소의 농도를 높이기 위함이며, 탁월한 미백효과를 얻을 수 있다. 실험을 통하여 우치에 카바마이드 퍼옥사이드(15%)와 수증기(0.4%)를 처리한 경우 플라즈마의 미백효과가 탁월함을 보였다. 이때 CIE색좌표 ($L^*a^*b^*$)에서 명도도가 높아짐을 보았다. 미백효과에 대한 측정은 측색분광기(cm-3500d)를 이용하였다. 라만은 빛이 어떤 매질을 통과할 때 빛의 파장을 변화시켜 빛의 일부는 진행방향에서 이탈해 다른방향으로지행하는 현상을 산란이라고 한다. 이를 이용하여 빛의 파장을 변화시키는 현상을 라만산란이라고 한다. 이것을 이용하여 같은 우치의 표면을 플라즈마 처리 전 후를 라만을 통해 측정하였다. 대기압 저온 플라즈마에서 발생되는 ROS는 미백효과에 큰 영향을 미친다. 모든 실험의 플라즈마 처리시간은 최대 20분까지로 하였다.

A STUDY ON THE COLORS OF THE ANTERIOR PRIMARY RESIN TEETH (유치레진치의 전치부 색조연구)

  • Removable or fixed space maintainer could be needed if one or some anterior primary teeth were missing, where resin pontics well-matched to natural primary teeth should be demanded to get an esthetic satisfaction. Resin Natural $Teeth^{TM}$(Nissin dental, Japan) is available currently in Korea, which consists of two shades of colors; type A1 and A2. The purpose of this study is to elucidate the colors of the anterior primary resin teeth and to establish the data to compare those with the natural anterior primary teeth. CIE $L^*a^*b^*s$ were measured each three times labially from 17 sets of maxillary four anterior teeth for type A1 and A2 Resin Natural $Teeth^{TM}$ using ShadeEye $NCC^{TM}$(Shofu, Japan) which is one of spectrophotometers. The data were analysed statistically using Kruskall-Wallis Test and Mann-Whitney U Test. The results were as follows : 1. There were smaller teeth color differences in group A1 than in group A2 when it comes to distributions of ${\Delta}E$, $L^*$ and $b^*$. There were no statistically significant differences of $a^*$ between teeth in the same group(P>0.05). 2. ${\Delta}E$ in group A1 and A2 (maximum ${\Delta}E{\le}1.23$) were very small, which could not be discerned by eyesight. ${\Delta}E$ between mean CIE $L^*a^*b^*s$ of group A1 and A2 was 3.97, which could be discernible by eyesight. 3. Mean measurements of group A1 were $L^*=73.8$, $a^*=-1.8$, $b^*=-4.7$, and those of group A2 were $L^*=75.8$, $a^*=-2.7$, $b^*=-1.4$. It would be recommended that resin teeth compatible to the colors of the natural primary ones needed to be developed by investigating in vivo study.

THE INFLUENCE OF THE VITAL BLEACHING AGENTS ON TRANSLUCENCY OF THE BOVINE ENAMEL (생활치 미백제가 우치 법랑질의 투명도에 미치는 영향)

  • This study was done to evaluate whether vital bleaching agents could influence on the translucency of the bovine enamel. The anterior bovine teeth that were extracted one day before and without any gross discoloration were obtained and then were preserved in physiologic saline. 6 mm cylindrical tooth specimens were fabricated with diamond puncher perpendicularly on labial surface of bovine tooth. After embedded in transparent acrylic resin with labial surface being exposed, they were cut to a thickness of 1.2 mm with low speed diamond saw (Isomat, Buehler Co., Lake Bluff, IL, USA). They were smoothly ground to 1 mm thickness of enamel with sandpaper. 24 specimens were randomly divided into 3 groups and control group respectively. Opalescence (10% carbamide peroxide, Ultradent, South Jordan, USA), Rembrandt (10% carbamide peroxide, DenMat, USA) and Opalescence F (15% carbamide peroxide with fluoride, Ultradent, USA) were applied on labial sides of the bovine enamel for 7 days (bleaching agents were reapplied every 24 hours) and the opposite surface was contacted to cotton that soaked in distilled water. The control group was soaked in distilled water. Three stimulus value X, Y and Z were evaluated with colorimeter (Color ' Color Differencerneter, Model TC-6FX, Tokyo Denshoku Co., Japan) on the labial surface of all specimen three times on white and black background plate before the bleaching agents were applied and on 3rd, 5th and 7th day after applied. The degree of translucency was normally assessed by measuring the inverse property, opacity (contrast ratio). 10% Opalescence, 15% Opalescence-F, and control group showed no significant variation in the translucency of bovine enamel, However Rembrandt decreased the translucency of it (p < 0.01).

  • In this study, we evaluated the virtual reality model for dental implant surgery and discussed about the method to make the surgical environment for virtual reality with practical patient data. The anatomical model for patient face was fabricated by facial and oral scan data based on CT data. The simulation scenario was composed step by step fashion with Unity3D. From incision and sinus bone graft procedure which is needed to this patient model to implant installation and bone graft was included in this scenario. We used the HMD and leap motion for immersiveness and feeling of real operation. Twenty training doctor was attended this simulation study, and surveyed their satisfactory results by questionnaire. Implant surgery education program was showed the possibilities of educational tool for dental students and training doctors. Virtual reality for surgical education with HMD and leap motion had advantages, in terms of cheap prcie, easy access.
