Synergistic Effect of Bacteriophage and Antibiotic against Antibiotic-Resistant Salmonella Typhimurium

  • In this study, we investigated the efficacy of Salmonella phage P22 combined with antibiotics to inhibit antibiotic-resistant S. Typhimurium CCARM 8009. The synergistic effect of phage P22 and antibiotics was evaluated by using disk diffusion and broth dilution assays. The development of Antimicrobial resistance was determined after time-kill assay. The antibiotic susceptibility assay showed the inhibition zone sizes around the antibiotic disks were increased up to 78.8% in the presence of phage (cefotaxime; 13.6%, chloramphenicol; 19.3%, ciprofloxacin; 12.7% and erythromycin; 78.8%). The minimum inhibitory concentration values of the combination treatment significantly decreased from 256 to 64 mg/mL for tetracycline, 8 to 4 mg/mL for chloramphenicol, 0.0156 to 0.0078 mg/mL for ciprofloxacin, 128 to 64 mg/mL for erythromycin and 512 to 256 mg/mL for streptomycin. The number of S. Typhimurium CCARM 8009 was approximately 4-log lower than that of the control throughout the combination treatment with phage P22 and ciprofloxacin delete at 37℃ for 20 h. The results indicate that the development of antimicrobial resistance in S. Typhimurium could be reduced in the presence of phage treatment. This study provides promising evidence for the phage-antibiotic combination as an effective treatment to control antibiotic-resistant bacteria.

The Effect of Essential Oils on Antimicrobial Activity (에센셜 오일이 항균 활성에 미치는 효과)

  • We investigated the best antibiotics using blending oils after screening 11 kinds of essential oil known as antibiotics from plants. The minimum inhibitory concentration (MIC) and the minimum killing concentration (MBC) were found to be essential for essential oils B and E to inhibit target bacteria. All gram-positive bacteria containing S. aureus used in this experiment were shown highly antibiotic activity. And only A. baumanii in gram-positive bacteria and C. albicans in fungi were shown highly antibiotic activity. The essential oils used in our experiments showed better antibiotic activity compared to major studies using natural antibiotics with excellent antibiotic activity and essential oils from natural medicine. It is not known what mechanism of antimicrobial activity the essential oil used in the test has, but it is interpreted as a synthetic inhibitory mechanism of cell wall compared with other previous studies. From these results, it is expected that some substances or functional products with antibiotic activity will be developed.

  • The antimicrobial susceptibility and antibiotic resistance patterns of 67 eae positive Escherichia coli strains isolated from pigs were investigated by disc diffusion method. Sixty-seven E. coli isolated from pigs showed susceptibility to Ceftiofur (98.5%), Lincomycin+Spectinomycin (74.6%), Danofloxacin (73.1%), Enrofloxacin (64.2%), and Neomycin (41.8%). However, the multiple resistance patterns were also seen in eae+E. coli isolates. Neomycin+Tylosin+Penicillin+Tetracycline, Tylosin+Penicillin+Tetracycline, and Neomycin+Tylosin+Danofloxacin+Penicillin+Tetracycline+Enrofloxacinwere the most prevalent patterns of multiple antibiotic resistance.

The Factors Affecting the Use of Empirical Antibiotics in Febrile Infants from 1 Month to Less than 3 Months (30일 이상 90일 미만의 발열 영아에서 경험적 항생제 사용에 영향을 미치는 요소)

  • Purpose: This study investigated the factors affecting the use of empirical antibiotics in febrile infants from 1 month to less than 3 months. Methods: We retrospectively reviewed the medical records of hospitalized previously healthy infants with fever in Pusan National University Children's Hospital from January 2010 to December 2016. Clinical features, laboratory findings and antibiotic therapy were analyzed. Respiratory viruses were identified by multiplex reverse transcriptase polymerase chain reaction (RT-PCR) and were reported after 1-3 days. Enterovirus were identified by real time polymerase chain reaction (PCR) and were reported in several hours. Results: The 129 of 366 subjects used empirical antibiotics and 237 patients didn't used empirical antibiotics. Empirical antibiotics were used more frequently when the fever was longer before admission, respiratory symptoms and ill being appearances were present and C-reactive protein was elevated. The rate of readmission was low in the group not used empirical antibiotics. Most of the patients detected by enterovirus PCR in cerebrospinal fluid didn't used empirical antibiotics. The results of respiratory virus multiplex RT-PCR showed no difference in the use of empirical antibiotics. Conclusions: In our study, empirical antibiotic prescriptions were affected not respiratory virus multiplex RT-PCR but enterovirus PCR. If multiplex RT-PCR were reported more rapid turn around time, it will affect antibiotic use.

Residue and risk assessment of veterinary antibiotics in manure-based composts and agricultural soils (가축분뇨 유래 퇴비 및 농경지 중 축산용 항생제의 잔류 및 위해성 평가)

  • While veterinary antibiotics are used only in a part of the dose administered, the rest are excreted as urine or feces. Residual antibiotics enter the adjacent agricultural environments by spraying manure-based composts on farmlands and lead to secondary pollution. Therefore, it is necessary to develop the technique for post management such as regulatory levels of antibiotics in the agricultural environments. This study was conducted to compare by different matrices the amount of residual antibiotics such as tetracyclines and sulfonamides, which are known to be frequently used in Korea and to practice risk assessment by different antibiotics in soils before and after application of composts. Pre-treatment with modified typical method using buffer and solid phase extraction showed the recovery of composts and soils was more than 70% at ppb level and the limits of detection were 0.13-0.46 and 0.05-0.25 ㎍/kg, respectively. Analysis of manure-based composts revealed concentrations from 5.38 to 196.0 ㎍/kg for tetracyclines, from below the detection of limit (BDL) to 259.0 ㎍/kg for sulfonamides. In case of agricultural soils, residual concentrations of selected veterinary antibiotics were ranged 0.30-53.3 ㎍/kg, BDL-4.16 ㎍/kg respectively and the concentration level of tetracyclines, which had higher soil distribution coefficient (Kd) values, was higher than that of sulfonamides. There was a difference in human risk assessment by different antibiotics in soil before and after application of composts. But, it was indicated that detection values of all of 5 antibiotics were very safe on the basis that Hazard Quotient was safe below 1.

  • 우리회에서는 농림부에서 진행중에 있는 '항생제사용절감연구모임'에 참여하고 있으며, 지난 9월 26일 개최된 제4차회의에서 구체적인 항생제의 사용절감방안으로 "수의사처방제 도입방안"에 대하여 발표(발표 : 우연철 기획실장)하였습니다. '항생제사용절감연구모임'은 제5차회의를 거쳐 최종보고서를 발표할 예정이며, 본원고는 제4차회의 자료집에 발표한 내용으로 회원여러분께 안내하고자 10월호 특집으로 다루게 되었습니다. 이후 항생제의 안전한 사용과 수의사처방제 도입에 회원여러분의 많은 관심과 협조를 부탁드립니다.

  • ‘국가항생제내성안전관리사업’을 총괄하고 있는 식약청은 수산용 항생제 사용이 인체에 미치는 영향을 평가하고 인체 위해를 최소화하기 위한 국제 전략을 수립하고자, 관련 국제기구인 FAO/WHO/OIE/Codex와 공동으로 2006년 6월13일부터 6월16일까지 제3차 국제전문가 회의(Joint FAO/WHO/OIE Expert Consultation on Antimicrobial Use in Aquaculture and Antimicrobial Resistance)를 개최하였다고 밝혔다. 이번 전문가 회의는 항생제내성에 대한 제1차 Geneva, 제2차 Oslo 회의에 이어 아시아에서는 처음으로 Seoul (식약청)에서 개최되었다.

Use of Antimicrobial Agents for the Treatment of Inpatients in Chonbuk National University Hospital (전북대학교병원 입원환자에서의 항생제 사용 실태)

  • Purpose : The use antimicrobial agents is one of the important strategies for the treatment and prophylaxis of microbial infections. But injudicious abuse and misuse of antimicrobial agents is problem to add an extra weight on medical fee, increase of resistant bacteria and side effects according to the antibiotic use. This study was performed to establish the pertinent use of antimicrobial agent in Chonbuk National University Hospital(CNUH). Characteristics of antibiotics use was analysis by reviewing the medical records of patients admitted to CNUH during the period of May 1998. Methods : One thousand eight hundred and thirty three patients were enrolled in this study(medical division 1,014 cases, surgical division 819 cases). Medical records were retrospectively reviewed to classify the rate of antibiotics use, name of antibiotics used, appropriateness of antibiotics use. Results : The overall rate of antibiotic usage in CNUH was 67.2%(1,231/1,833), showing higher rate in surgical division(89.6%) compare to that of medical division(49.0%). Among 1,231 patients to whom antimicrobial agents were given, only 125(10.2%) were treated with single antimicrobial agents. 311(25.3%) were treated with two antimicrobial agents, and 795(64.5%) patients received 3 or more antibiotics. ${\beta}$-lactams(56.4%) were most frequently used followed by aminoglycosides(35.3%), the others(4.9%) and quinolons(3.4%). Amoxicillin-clavulanate was the mostly commonly used antibiotics followed by amoxicillin and unasyn. Prophylactic use of antibiotics was carried in seven hundred six patients(57.4%), mostly in surgical division, which can be considered somewhat inappropriate in the initiation time and duration of antibiotic use. Conclusion : Importance of monotherapy and appropriate prophylactic antibiotic use should be emphasized. Strategies of antibiotics use, such as restriction of drug use, continuous monitoring system, flow sheet system should be considered to reduce antibiotics use and establish the appropriate use of antibiotics as well as inhibiting the occurrence of resistant strains.

A Study on Changes in Antimicrobial Resistant Staphylococcus aureus from Wound Isolates in a South Korean University Hospital for the Past 10 Years (2006, 2016) (최근 10년 동안 일개 대학병원 상처 배양에서 분리된 포도알균의 항생제 내성 변화 연구)

  • Staphylococcus aureus, which is generally susceptible to the involvement route in community, is mostly MSSA. However, CA-MRSA is recently increased. Abuse of antibiotics and glycopeptides may increase VISA and VRSA. This study was conducted to investigate the changes on the antibiotic resistance prevalence and antibiotic susceptibility patterns of Staphylococcus aureus isolated from the wound of patients in a university hospital for the past 10 years. This study showed that antibiotic resistance was higher in males than in females; moreover, the antibiotic resistance rates increased with age. The resistance rate for penicillin, oxacillin, erythromycin, gentamicin, clindamycin, tetracycline, ciprofloxacin, fusidic acid, trimethoprim/sulfamethoxazole, clindamycin, and rifampicin was, respectively, 97.7%, 60.5%, 57.4%, 48.8%, 41.1%, 44.2%, 44.2%, 14.7%, 13.2% and 3.9% in 2006. The resistance rate for penicillin, oxacillin, erythromycin, gentamicin, clindamycin, tetracycline, ciprofloxacin, fusidic acid, trimethoprim/sulfamethoxazole, fusidic acid, clindamycin, and rifampicin was, respectively 95.9%, 62.6%, 55.7%, 28.6%, 50.3%, 34.7%, 38.8%, 34.0%, 2.7% and 8.2% in 2016. Vancomycin, linezolid, quinupristin/dalfopristin, and teicoplanin exhibited 100% in antibiotic susceptibility. In particular, fusidic acid resistance was increased by 19.3% in 2016. Compared with 2006, the decreased point, 12.4% was susceptible and was statistically significant. Therefore, this study suggests that periodic review and understanding of microbial and antibiotic changes should continue to investigate appropriate antibiotic susceptibility.

Antibiotic Resistance and Plasmid Profile of Vibrio parahaemolyticus Strains Isolated from Kyunggi-Incheon Coastal Area (경기인천 연안에서 분리된 장염비브리오균의 항생제 내성 및 플라스미드 보유 현황)

  • Vibrio parahaemolyticus is one of the major agents responsible for food poisoning during summer in Korea, which is transmitted via seawater or seafoods. Recently, distribution of the bacteria in the marine environment has been increased due to global warming. Great concern also has been raised regarding public hygiene as well as marine culture by the emergence of pathogens with antibiotic resistance. Therefore, distribution of V. parahaemolyticus and antibiotic resistance of the isolates were monitored in 7 coastal areas of Kyonggi Province and Incheon by sampling seawater, fishes and clams monthly. V. parahaemolyticus was detected from 47.7% of 966 samples (seawater 61.9%, seafoods 41.8%) analyzed using $CHROMagar^{TM}$ and TCBS agar plates as well as multiplex PCR. Among 13 antibiotics tested, resistance to vancomycin and ampicillin was observed in 97.3% and 87.3% of the isolates, respectively, and the ratios of them resistant to cephalothin (48.8%) and rifampin (46.1%) were also high. The isolates were most highly sensitive to chloramphenicol (91.7%) and trimethoprim-sulfamethoxazole (91.8%). The ratio of sensitivity for other antibiotics was also high in the descending order of gentamycin (82.3%), tobramycin (74.8%), nalidixic acid (71.6%), tetracyclin (69.4%), cefotaxime (63.0%). About 69% of the isolates showed multiple drug resistance toward 3 antibiotics including vancomycin and ampicillin. Two of them exhibited resistance for 11 antibiotics used in this study. Plasmid profile analysis of the isolates with antibiotic resistance revealed that 55.1% of them retained plasmids of 24 different types. However, no clear inter-relationship between the resistance and the plasmid profile has been observed.
