The Development of Interactive Wireless-Game Device based on Multi-Channel Electromyography for Rehabilitation in Patients with Hemiplegia (편마비 환자의 재활을 위한 다채널 근전도 기반의 체감형 무선 게임 장치 개발)

  • An interactive wireless-game device based on multi-channel electromyogram has been developed to help rehabilitation of patients with hemiplegia. The device is able to provide a visual feedback and rehabilitation motivations to the patients. The game contents were made according to the number of applied channels and, their level and speed can also be controlled depending on the recovery states of patients. Patient satisfaction survey was conducted to evaluate the necessity and usefulness of the proposed device after playing the game. The results show that the proposed system is expected to give hemiplegia patients exciting and effective rehabilitation chances, if more diverse and specialized rehabilitation games are developed.

Accelerometry of Unimanual and Bimanual Activities in Stroke Patients (뇌졸중 환자에서 한손 및 양손 활동 수행 시 가속도 분석)

  • Objective : This study evaluated the differences between the upper and lower arm activities of the affected and non-affected sides in stroke patients performing activities of daily living. It was intended to gather basic data for the measurement of daily activities. Methods : The participants in the study consisted of 20 patients, 10 had left hemiplegia and 10 had right hemiplegia. Between September and November 2017, they performed drinking and dressing as unimanual and bimanual tasks. The 3-axes accelerometers were placed 2cm below the lateral epicondyle on the upper arm and 2cm below the radial styloid on the lower arm. The data were collected at intervals of 1/32 Hz and analyzed. Results : In patients with right hemiplegia, the tendon activity was significantly higher in the lower arm when performing a bimanual activity (p<.05), for all the other activities, those of the lower arm were significantly higher than those of the upper arm (p<.01, p<.05). There was no significant difference between the unimanual and bimanual activities of the affected and tendon sides in patients with left hemiplegia (p>.05). In both hands, the activity of the lower arm was significantly higher than that of the activities of the upper and lower arms, except on the affected side (p<.05). In all patients, the activities of the upper and lower arms were significantly higher (p<.05), and the activity of the lower arm was significantly higher than that of the upper (p<.001, p<.05). Conclusion : The activity was higher on the non-affected side than the affected side during the dressing task, and the overall activity was higher in the lower arm than the upper arm.

The Effect of Height of Cane for Health Promotion on Mobility of Patients with Stroke (뇌졸중 환자의 건강증진을 위한 지팡이 높이가 보행과 체중지지율에 미치는 영향)

  • This study examines the effect of the height of cane that hemiplegic patients due to stroke use on their plantar pressure in standing position, gait speed. The study suggests a new standard for appropriate cane height that considers the change of average height of population. Research subjects included 12 patients hospitalized in S Medical Care Hospital located in Gwangju Metropolitan City in South Korea who were diagnosed with stroke. Group A uses a cane of the height of the greater trochanter of femur, group B uses a cane of 5cm above the height of the greater trochanter of femur, and group C is a cane of 10cm above the height of the greater trochanter of femur. In the study result, non-affected side plantar pressure and affected side plantar pressure showed a significant difference among the cases where the cane height was the same as the A group, B group, C group. In the post-hoc analysis result, a significant difference was observed between the case of A group and C group. Gait speed showed no significant difference among the case of the A group, B group, C group. The asymmetry of the stroke affects not only the posture but also the walking that is related to daily life. Changes in the height of the cane did not affect walking speed. The change in the height of the cane showed a change in the weight support ratio, which is thought to have a positive effect on the asymmetry. In future clinical setting, this study result will be able to provide fundamental data regarding the cane height in the standing or walking therapy for hemiplegic patients due to stroke with cane application.

Distribution of Weakness at the Lower Extremity of Hemiparesis Patients (편측부전마비환자의 하지에서의 위약증상의 분포)

  • The maximal voluntary strength of knee extension and flexion on both the right and left sides was measured in patients with hemiparesis of upper motor neuron type and in a group of normal subjects. Significant differences of maximal voluntary strength were found between male and female but the ratio of flexor to extensor strength did not vary significantly between the sides, between the sex in normal subjects. The maximal voluntary strength of uninvolved side were not reduced significantly but involved side reduced significantly in patients. The ratio of flexor to extensor strength in hemiparetic side was significantly less than the ratio for the normal subjects but not significant difference in uninvolved side of patients. According to the above results, the maximal voluntary strength of flexion was more reduced than that of the extension in lower extremity of hemiparesis patients. The strength ratio of flexion to extension was a useful parameter for guiding the rehabilitation of hemiparesis.

Development of EMG-Triggered FES System and Optimal Electrode Location for Chronic Hemiplegic Patients (편마비 환자를 위한 근전도 유발 기능적 전기자극기의 개발과 이상적인 전극부착 위치)

  • The purpose of this study was to develop EMG triggered FES system for restoration of upper extremity function in chronic hemiplegic patients and to identify the optimal location of electrode application for the EMG triggered FES system which produces effective muscle contraction and detects EMG activity for extension in the wrist and finger joints. The stimulus system was composed of EMG measuring component, constant current component and the program for muscle contraction by EMG triggered FES and passive FES. Parameter of electrical stimulation was 35 ㎐ in frequency, 150 ${\mu}\textrm{s}$ in pulse width and symmetric bi-phasic wave. In 15 hemiplegic patients, EMG triggered FES was applied to the proximal half of forearm which was divided into 12 areas. The most sensitive area for measuring EMG activities during extension of the wrist and fingers was area 4, 5 and the optimal location of electrical stimulation for producing extension of the wrist and fingers was area 4, 5, 7, 8. These results suggest that the area 4 and 5 was considered as the most optimal location of electrode application for measuring EMG activities as well as producing extension of the wrist and fingers by EMG triggered FES system.

Quantitative sensory Testing on Edema of Upper Extremity for Stroke Patients (뇌졸중 환자의 상지부종에 대한 정량적 감각 평가)

  • This study is conducted to provide basic data through the Quantitative Sensory Testing(QST) about edema of the upper extremity with hemiplegia for subacute and chronic patients in management and treatment. For the purposes of the study group I, subacute stroke patients(n = 15) and group II, chronic stroke patients(n = 17), was targeted, it was performed difference between the unaffected side and affected side of the upper extremity volume, MPT, CST, WST, CPT, WPT and QST of VST. As the results, all measurement items showed significant differences when comparing each group of all(p<.001), group I and group II of patients affected side, there was a significant difference in the measurements, with the exception of items MPT sensory test(p<.05). In subacute than chronic numbness due to increased sensory threshold appears in the upper extremity edema was able to confirm. It was focused on the difference in sensory properties according to the edema of stroke patients through quantitative test.

Effects of Action-observational training Program on Gait and Balance of Patients with Hemiplegia (동작관찰훈련프로그램이 편마비환자의 보행 및 균형에 미치는 영향)

  • The purpose of this study is to evaluate the effects of Action-observational training program on gait and balance of patients with hemiplegia. Subjects of this study, among the patients who received the diagnosis cerebral vascular accident, for patients total of 4 people have agreed to research. Experimental group 2 people, control group 2 people, was a total of 4 people. Group-specific arbitration method, was applied to Action-observational training program(experimental group) and general Task-oriented training program(control group). Each training courses 30 minutes for 4 weeks, examined the changes in 10 Meter Walk Test(10MWT), gait speed, Berg Balance Scale(BBS) and time up and go(TUG) ability to examine a total of 4-week course effectively. The intervention were compared by measuring before and after. There were significant improvements in the subscales of the gait and balance test of those who practiced with the Action-observational training program, while the control group showed no significant changes. Therefore, Action-observational training program is effective in improvement of to improve the gait and balance in patients with hemiplegia.

  • Rehabilitation of upper limb motor function of hemiplegic patient must maintain interest and demand a device for a quantitative evaluation of rehabilitation training. In this paper, we developed the device that is composed of arm cradle, handle, and balance ball for rehabilitation exercise. We have performed experiment for validity as to whether to use the rehabilitation device when tilting the upper extremity training device developed to measure changes in the EMG signal to the main upper limb muscles for 7 healthy volunteers. We have analyzed muscle activation signals on agonist and antagonist as a reference in the muscle contraction and relaxation in the upper limb extension and flexion when the balance-handle device is tilted to front-rear and left-right. The experimental results showed that a tendency of muscle activation of biceps, triceps, and deltoid used in upper limb motor function of hemiplegic patients from extension and flexion evaluation items of Fugl-Meyer Assessment(FMA). These results may be helpful for rehabilitation training for upper limb motor function of hemiplegic patients by utilizing a developed unit.
