Adenoid Cystic Carcinoma of the Parotid and Submandibular Glands: A Comparative Study (이하선과 악하선에 생긴 선양낭암의 비교)

  • 선양낭암은 타액선종양중 비교적 드문 악성종양으로 병리조직학적 및 임상적으로 독특한 양상을 보인다. 병리조직학상 종양세포가 신경주변으로 침범하여 수술당시 적절한 수술연을 얻기 어렵고 이에 따른 국소재발이 거듭되고 임파선전이 및 예측치 않는 원격전이가 생기기도 한다. 지금까지 선양낭암환자의 예후에 영향을 미치는 여러 요인들에 대한 연구들이 이루워지고 있는데 그중 조직학적 분화도, 원발부위, 종양의 크기, 수술연의 상태와 임상적 병기들이 중요시되고 있다. 이에 저자들은 1960년부터 1980년 까지 텍사스대학 M.D. Anderson 암센터에서 치료받은 이하선 및 악하선 선양낭암 각각 13례와 26례를 대상으로 원발부위가 환자의 생존 및 치료에 미치는 영향을 알고자 다른 예후인자로 알려진 조직병리학적 소견들과 함께 통계학적 분석을 실시하였다. 이하선과 악하선에 생긴 선양낭암을 비교한 결과 이하선에 생긴 경우 조직학적 분화도가 더 낮았고 악하선 종양에서 수술연에 종양침범율이 더 적었다. 경부 임파절 전이는 악하선 종양에서 많았으나 반면 원격전이는 이하선 종양에서 더 많이 발생하였다. 그러나 종양의 크기, 선경주위침범 유무, 국소재발과 환자의 생존율사이에는 의의있는 상관 관계는 없었다. 이하선과 악하선에 생긴 선양낭암의 여러 특성들과 생존률간의 univariate 분석결과 악하선에 생긴 선양낭암 환자에서 종양이 크거나 조직학상 분화가 나쁘거나(solid pattern), 경부임파절 전이와 원격전이가 생기면 예후에 나쁜 영향을 끼침이 밝혀졌다. 그러나 이하선에 생긴 경우 분석례가 너무 적어 결론을 내리기에 불충분하였다. 원발부위에 따른 선양낭암 환자들의 특성을 chi-square 검정법을 이용하여 통계학적으로 분석한 결과 이 두군간에는 의의있는 통계학적 차이는 발견할 수 없었다.

A Review of Minor Salivary Gland Tumor (소타액선 종양의 고찰)

  • Background and Objectives: Minor salivary gland tumors vary in their primary sites, histopathology and biological behavior. Therefore, various factors are considered in selecting the treatment modality and predicting the prognosis. We performed this study for the purpose of getting further understanding and more supporting ideas for the diagnosis and treatment of minor salivary gland tumor. Materials and Methods: A retrospective analysis of the patients with 52 cases of minor salivary gland tumor who were treated at the Hanyang University Hospital from 1996 to 2003 was performed. We analyzed demography, symptoms, histopathology, treatment and outcomes by the review of medical records. Results: Among 52 cases of minor salivary gland tumor, 46% were classified as benign and 54% were classified as malignant tumors. The most common benign tumor was pleomorphic adenoma. Adenoid cystic carcinoma(15/28) was the most common in malignant tumors. Eight patients were males and sixteen patients were females in benign tumors and 10 patients were males and 18 patients were females in malignant tumors. The most common site of benign tumor was the palate(17/22), whereas malignant tumors were most common in the nasal cavity and paranasal sinus(9/28). Asymptomatic mass was the most common symptom. According to the criteria given by the AJCC on staging, stage III and IV(21/28) were more common than stage I and 11(7/28). All benign tumors were treated with simple excision and had no recurrence. In malignant tumors, 25 patients underwent radical excisional operation and 13 patients of them had postoperative radiation therapy. Three of them were treated with additional chemotherapy. In whom treated with radical operation, 9 patients had recurrence. Three were recurred at the primary site with neck node metastasis, 3 were recurred at the primary site with lung metastasis, 1 was recurred at the primary site with neck node and lung metastasis, 1 was recurred only at neck node. Conclusion: In minor salivary gland tumor, malignant was more common than benign. Malignant tumor originated from minor salivary gland were more frequently diagnosed at advanced stage with high recurrence rate and distant metastasis. Early detection of the disease is needed to improve the prognosis of the patients with malignant tumors of the minor salivary glands.

Modification of Late Radiation Response of Rat Salivary Glands by Pentoxifylline and Diltiazem (쥐의 타액선 방사선조사 후 만성반응에 Pentoxifylline과 Diltiazem이 미치는 영향)

  • Purpose : To elucidate the effects of pentoxifylline and diltiazem on the late response of the salivary glands of the rat after irradiation. Materials and Methods : Sixteen Sprague-Dawley rats were divided into 4 groups : (a) irradiation alone (b) irradiation with pentixifylline (PTX) (c) irradiation with diltiazem (DTZ) (d) irradiation with both PTX and DTZ. Irradiation was given in a single fraction of 16 Gy using 4 MV photon energy through an anterior port encompassing the left side of the salivary gland leaving the right side of salivary gland as a control. PTX, 20 mg/kg and/or DTZ, 50 mg/kg were infused intraperitoneally before irradiation, Two rats from each group were sacrificed on the 10th week and the rest was sacrificed on the 16th week after irradiation. Histopathologic examinations were undertaken for each section and the proportion of vacuolated cells out of the total number of cells under light microscopic fields was calculated. The statistical significance in the difference of the proportion of the vacuolated cells among the experimental groups was evaluated by a $x^2$-test. Results : Irradiated salivary glands of the 10th week group revealed markedly increased number of vacuolated cells compared to those of unirradiated control. The proportion of vacuolated cells was significantly reduced in both the PTX group (p value=0.001) and the combined PTX and DTX group compared to those of irradiation alone group. The DTZ alone group did not reveal the significant reduction of vacuolated cells compared to those of irradiation alone group (p value, >0.05). The 16th week groups revealed similar findings to those of the 10th week group, but the degree of chronic inflammatory cell infiltrates and interstitial fibrosis was increased and the number of acinar cells was reduced compared to those of the 10th week group. Conclusions : PTX significantly reduced the late radiation response of salivary glands, but DTZ did not reduce the same degree as PTX did. Taking the positive results of this study into consideration, it seems reasonable to apply PTX into the clinical trial for the head and neck irradiation to reduce the late radiation sequelae of salivary glands in the near future. A Case of Malignant Oncocytoma of the Parotid Gland (이하선에 발생한 악성 타액선 호산성 과립세포종 1례)

  • Malignant oncocytoma(oxyphil cell adenoma) is a extremely rare malignant tumor of the salivary glands and almost occurs in the parotid gland. A case of malignant oncocytoma of the parotid gland occurring in a 63 year old man is reported. Establishing a diagnosis of malignant oncocytoma essentially requires the application of two sets of criteria to the tumor under examination. First, the tumor cells must be identified as oncocyte and second, malignancy must be determined. Histochemical stains are not as useful as generally believed. Electron microscopy confirms the diagnosis by demonstrating an abundance of mitochondria. The diagnosis of malignant oncocytoma depends on several additional criteria in addition to cellular and nuclear pleomorphism, including lack of encapsulation, local infiltration, perineural and intravascular growth, and regional and/or distant metastasis. Satisfactory results from surgical resection and postoperative radiotherapy in our patient serve as additional experience in the treatment of this rare malignancy.

A Case of Mucoepidermoid Carcinoma Arising from the Intraoral Minor Salivary Gland (구강 내 소타액선에 발생한 점액표피양 암종 1예)

  • Salivary gland tumors comprise almost 5% of head and neck malignancies, and minor salivary gland tumor which account for 10-15% of all salivary gland neoplasm are infrequently malignant. The mucoepidermoid carcinoma (MEC) is second most common tumor in minor salivary gland. It usually presents as a painless, rubbery-hard or soft mass, which may be fixed or mobile into the underlying structure. The predilection sites of intraoral MEC are palate, cheek, mandible, lip, and tongue, etc. There are very few published reports of MEC occurred in retromolar trigone. Only one case has been reported so far. Recently, we experienced a-70-year old man with a mass in retromolar trigone, which was finally diagnosed as MEC. We report the unique case with literature review.

Expression of p53 and Ki-67 in Salivary Gland Tumors (타액선 종양에서 p53과 Ki-67의 발현)

  • Objectives: The p53 tumor suppressor gene has a key role in cellular control mechanisms involving apoptosis and DNA repair, leading to the G1 arrest following DNA damage. Its mutation is one of the most frequent alterations in human cancers. Ki-67 is identified in replicating cells of both benign and malignant lesions, so it can be the predictor of proliferative activity. The aim of this study is to evaluate the expression of p53 and Ki-67 in salivary gland tumors. Materials and Methods: Immunohistochemical analysis was used to detect expression of p53 and Ki-67 in paraffin-embedded samples from 31 benign and 27 malignant salivary gland tumors. Results were analyzed between benign and malignant tumors and compared with the clinical parameters such as stage and recurrence in malignant tumors. Results: p53 overexpression was detected in 19.6% of benign tumors and 40.7% of malignant tumors, but there was no statistical significance. p53 was significantly expressed in Warthin's tumor(45.5%) compared with pleomorphic adenoma(5.9%). Only 5.9% of pleomorphic adenoma were positive for p53, while 60% of carcinoma ex pleomorphic adenoma were positive for p53. Ki-67 was expressed in 3.2% of benign tumors and 51.9% of malignant tumors, which showed significant higher expression in malignant tumors. In malignant tumors, p53 and Ki-67 expressions bore no correlation to stage and recurrence. Conclusion: p53 overexpression is not associated with the progression of malignant tumors, and Ki-67 overexpression can be used as biologic indicator of malignant salivary gland tumors.

A case of Primary Hard Palate Adenocarcinoma Metastasizing to Cervical Lymph Nodes 9 Years after Resection of Primary Lesion (원발병소의 외과적 절제 9년 후 경부 림프절에 전이된 경구개 선암종 1예)

  • Primary malignancies of hard palate are rare, espicially adenocarcinoma of minor salivary gland in hard palate. The clinical stage of the tumor and its histologic grade at the time of treatment were significant predictors of survival. A 33-year-old woman was diagnosed with stage II, polymorphous low-grade adenocarcinoma of the hard palate and treated with surgery. We thougt that she would have excellent prognosis. After that, there was no definite evidence of recurrence during 3 months. Since then, the patient herself decided to discontunue follow-up. 9 years after that she came to the hospital due to both submandibular mass without any sign of local recurrence. CT scan and biopsy of the mass revealed multiple cervical metastatic adenocarcinoma without distant metastasis and the histologic findings were similar to that of the primary hard palate adenocarcinoma. So, we present this case with the review of literature.

  • We reviewed retrospectively 152 patients' records with salivary gland tumors confirmed histologically at Hanyang University Hospital from Jan. 1984 to Dec. 1993. The results obtained were as follows: 1) Sites of tumors were parotid gland in 102 cases(67%), submandibular gland in 29 cases(19%) and minor salivary glands in 21 cases (14%). 2) 113 cases(74%) were benign. and 39 cases(26%) were malignant. 3) The incidence of benign and malignant tumors were 81% & 19%, 69% & 31% and 48% & 52% in the parotid gland, the submandibular gland and the minor salivary glands respectively. 4) The most frequent histologic types of benign and malignant tumors were pleomorphic adenoma (64%) and mucoepidermoid carcinoma (11%) respectively. 5) The surgical procedures include superficial parotidectomy. total parotidectomy. extended total parotidectomy & neck dissection in the parotid gland. wide excision & neck dissection in the submandibular gland and wide excision, partial maxillectomy & neck dissection in the minor salivary glands. 6) The postoperative recurrence rate were 4.4% in benign tumors and 27% in malignant tumors.

A Clinical Analysis of Major Salivary Gland Tumors (주타액선 종양의 임상적 고찰)

  • Background and Objective: Even though major salivary gland tumor is a rare disease, the diversity of histopathologic characteristics makes treatment decisions difficult. The aim of this study is to analysis the clinical experience of our major salivary gland tumor and to suggest a guideline of treatment. Materials and Method: Sixty-eight major salivary gland tumors and tumor-like glandular enlargements treated at Sanggye Paik Hospital during the past seven years between June, 1995 and January, 2002 are analyzed for histopathologic diagnosis, treatment modality, clinical manifestation, local control, and treatment-related morbidity, recurrence rate, retrospectively. Results: In twenty-eight patients the swellings were diagnosed as non-tumorous condition. The clinical features, diagnostic and surgical management of fourty salivary neoplasms involving the parotid and submandibular glands are correlated with their histological features. Conclusion: We have concluded that salivary gland neoplasms are needed multimodal treatment, because of their highly variable biologic behavior in each tumor type. Thirty-four cases were benign and six cases were malignant. Most of benign cases were pleomorphic adenoma and they showed wide age-distribution. In six malignant cases, there were acinic cell carcinoma, adenocarcinoma, carcinoma ex-plemorphic adenoma, myoepithelioma, and adenoid cystic carcinoma.

  • Adenoid cystic carcinoma (ACC) commonly originates in the major salivary glands and respiratory tract, but it is extremely rare to find ACC in the esophagus. ACC of the esophagus is clinopathologically different from the salivary gland variant. It shows more aggressive malignant behavior and a very poor prognosis. We report here on the surgical and clinopathologic findings of ACC of the esophagus in a 65-year-old man, and we also include a review of the relevant medical literature.
