A Study on Dental Hygienists' Health Management and Their Use of Medical Care Services (치과위생사의 건강관리 및 의료 이용 행태에 관한 조사 연구)

  • The purpose of this study was to examine how dental hygienists health management and to what extent they used medical care services due to occupational diseases. It's ultimately attempted to improve the health care of dental hygienists and promote their health. After relevant literature and data were reviewed, a survey was conducted for five months from June through October, 2003. The subjects in this study were 160 dental hygienists who worked at dental hospital and clinics across the nation. And the collected data were analyzed. The findings of this study were as follows: (1) Concerning job-related health education experience, 83.1 percent, the greatest rate, had no experience to receive education, and the type of service made a significance difference to that(p<0.05). (2) As to in-house medical examination, 98.6 percent felt the need for medical examination, and 56.3 percent had ever their bodies examined. Their health examination experience was significantly different according to age(p<0.01), marital status(p<0.01), type of service(p<0.01), career(p<0.01) and yearly income(p<0.01). (3) Concerning how much they used hospitals after working as dental hygienists, 85.6 percent, the biggest group, had no experience to be hospitalized, and that was significantly different according to age(p<0.05), type of service(p<0.05), career(p<0.01) and yearly income(p<0.05). 51.9 percent had no experience to receive treatment as outpatients, and their career(p<0.01) made a significant difference to that. (4) As for how to cope with casual diseases, 34.4 percent, the largest group, visited pharmacies, and 32.5 percent, the next largest group, saw the doctors. In regard to herb restorative, 68.1 percent had no experience to take herb restorative, and that experience varied significantly with age(p<0.01), career(p<0.05) and annual income(p<0.01).

A Study on the Perception of Quality of Care Services by Care Workers using Big Data (빅데이터를 활용한 요양보호사의 서비스질 인식에 관한 연구)

  • Background: This study was conducted to confirm the service quality management of care workers, who are direct service personnel of long-term care insurance for the elderly, using unstructured big data. Methods: Using a textome, this study collected and analyzed unstructured social data related to care workers' service quality. Frequency, TF-IDF, centrality, semantic network, and CONCOR analyses were conducted on the top 50 keywords collected by crawling the data. Results: As a result of frequency analysis, the top-ranked keywords were 'Long-term care services,' 'Care workers,' 'Quality of care services,' 'Long term care,' 'Long term care facilities,' 'Enhancement,' 'Elderly,' 'Treatment,' 'Improvement,' and 'Necessity.' The results of degree centrality and eigenvector centrality were almost the same as those of the frequency analysis. As a result of the CONCOR analysis, it was found that the improvement in the quality of long-term care services, the operation of the long-term care services, the long-term care services system, and the perception of the psychological aspects of the care workers were of high concern. Conclusion: This study contributes to setting various directions for improving the service quality of care workers by presenting perceptions related to the service quality of care workers as a meaningful group.

A survey on the perceptions of dental hygienists and dentists on the legal scope of dental hygienists (치과위생사의 법적업무범위에 관한 인식 조사: 치과위생사와 치과의사를 대상으로)

  • Dentists and dental hygienists are major collaborators, as expressed by the concept of "Four-handed dentistry." Dentists are guaranteed their legal duties and rights in accordance with the Medical Act, whereas dental hygienists are currently stipulated in the scope of their legal duties under the Act on Medical Technicians. However, there is a difference between the actual work and the legal work performed by dental hygienists; therefore, the work is dependent on many legal controversies and authoritative interpretations. The purpose of this study is, first, to develop a reliable questionnaire tool regarding the actual work performed by dental hygienists in dental hospitals and clinics, and second, to examine the awareness of the appropriateness of legal work based on the developed questionnaire. The target subjects of the survey were dental hygienists and dentists, and the dental hygienists were those who worked in dental hospitals and clinics as license reporters of the Korean Dental Hygienists Association. A total of 1,294 dental hygienists and 39 dentists were included in the final analysis. In the dental hygienist group, 19 items received the response "appropriate for legal work" in over 90% of cases, accounting for 25% of the total 76 items. In addition, in a total of 31 items, more than 80% of the responses were appropriate for legal work. Among them, the highest was 'Scailing' (97.7%), followed by "Extraoral-Panorama" (97.1%). In the dentist group, 10 out of 76 items showed over 90% adequacy for legal work, and among them, "instrument cleaning and sterilization" was the highest at 100%. It was followed by "Intraoral radiography," "Scailing," and "Toothpick method, TPM" with 97.4%. In the case of dentists, 28 out of a total of 76 items showed an appropriateness of more than 80% for legal work. This study comprehensively investigated the actual work of dental hygienists in line with timely and appropriate social issues and provided reliable statistics in evidence-based dentistry.

Effects of Smoking on Oral Health : Preliminary Evaluation for a Long-Term Study of a Group with Good Oral Hygiene (흡연이 구강 건강에 미치는 영향 : 구강 위생이 양호한 집단의 장기적 연구를 위한 예비 평가)

  • Tobacco smoking is a major risk factor of systemic health and also impairs oral health, which is related to development of oral cancers, periodontitis, delayed wound healing, tooth loss, failure of implant, etc. Aside from smoking, many other risk factors can be related to oral health and long-term effects of smoking on salivary flow and taste threshold are still in controversy. Authors considered dental students to be an appropriate group with good oral hygiene for a long-term study to reveal effects of smoking on oral health. This study was performed to compare smoking patterns and current oral health conditions between smokers and nonsmokers in dental students prior to long-term evaluation. 192 volunteers (85.7%) of 224 male dental students in Dankook University were evaluated through questionnaires and clinical examination in 2010. Questionnaires included smoking pattern, alcohol use, nicotine dependence, preventive care, psychological profile and clinical examinations comprised assessment of teeth or periodontal status, nicotine pigmentation, salivary flow, electrical taste thresholds and halitosis. From the study, (current) smokers were older, and drank more frequently with more alcohol intake compared to former smokers and nonsmokers(p<0.05). There was no significant difference among them in salivary flow rate, halitosis and electrical taste threshold. However, there was significant difference in DMFT rate, periodontal treatment need, nicotinic pigmentation between smokers and nonsmokers(p<0.05), irrespective of their levels of preventive care. The smokers in this study, who are young dental students with relatively shorter duration of smoking, less use of cigarettes and low level of nicotine dependence, did not reveal significant impairment of oral health. However, their oral health was found to be relatively impaired compared to nonsmokers', which suggests negative effect of smoking on the oral health and a need of smoking cessation.

Study about Vocational Consciousness and Job Value of Dental Hygiene Department Graduating Students (치위생과 졸업예정자의 직업의식과 취업가치관에 대한 조사연구)

  • This study has an objective not only to be helpful to the understanding about students' career and employment preparation by investigating the present conditions which are related with vocational consciousness and job values of dental hygiene department graduating students. Study subjects were 350 female graduating students in 3-year-course College, dental hygiene department at five areas of Seoul, Gyeonggi, Gangwon and Gyeongnam. Data were analyzed using IBM SPSS Statistics ver. 19.0. Regarding job values, job prospect of dental hygienist is a little developmental (55.2%) and institution hoping to work is dental clinic (43.2%), reasons for employment in major area are good wage and working environment (24.0%). And regarding opinion about job, dental hygienist job is a measure for living income (69.7%). Regarding job selection conditions, 35.0% subjects replied good human relationship. In the questionnaires regarding professional job in vocational consciousness category, results showed high percentages in that 'the occupation is a job in charge of oral health improvement'; regarding academic area, 'scaling should be done better than dentist'; regarding human relationship, 'harmony between colleagues is closely related with job efficiency'; regarding work ethics, 'development of dental hospital (clinic) and dental hygienist is correlated.' In vocational consciousness of study subjects, higher major satisfaction showed significant differences in professional job consciousness and work ethics consciousness (p<0.05) and the correlation results in vocational consciousness areas showed all statistically significant correlations (p<0.01). In case that the characteristic in one type is higher among 4 types, all other types showed high characteristics and also showed high general vocational consciousness.

A Study on the Job Performance of Dental Coordinators and Their Perception (치과코디네이터의 업무수행 및 인식도에 관한 조사연구)

  • The purpose of this study was to examine the job performance of dental coordinators and their perception of their job to lay the groundwork for utilizing dental personnels more efficiently. The subjects in this study were dental coordinators who worked at selected dental hospitals and clinics in Seoul, Gyeonggi province and Incheon. A survey was conducted to gather data from May 1 to August 8, 2005 and answer sheets from 108 respondents were analyzed. The findings of the study were as follows: 1. As for the length of service, 43.5 percent of the dental coordinators investigated had worked at dental institutes for five years or more, which was followed by less than two years(19.5%) and three years to less than five years(19.4%). Concerning the length of service as dental coordinators, 39.8 percent had served for less than two years, and 19.4 percent had worked for two years to less than three years and for five years or more respectively. Regarding the name of position, 38 percent were called team leaders, and 30.6 percent were called coordinators. As to duties, the largest group of them that stood at 30.6 percent were in charge of receiving, and in regard to department, the largest group, 57.4 percent, belonged to the treatment backup department. 2. Concerning education, the greatest number of them, 45.4 percent, had received education at private institutes, and 73.1 percent found it necessary for dental coordinators to take an authorized qualification test. 43.5 percent, the largest group, looked upon the central government as the best organization to authorize their qualifications and 70.8 percent believed that what they learned enabled them to perform their job successfully. As to the necessity of follow-up education as a means to improve job performance, 96.3 percent consented to it. As for the reason, 63.9 percent considered that necessary to enhance their own ability and 22.2 percent were in want of systematic education. Regarding educational expenses, 29.6 percent were subsidized by the dental institutes where they had worked and 25.9 percent had totally been responsible for that. Regarding a required course, medical service and marketing was most widely pointed out(66.7%), followed by theory and practice(65.7%) and introduction to dentistry(57.4%). As to what sort of education they wanted to receive more, dental service and marketing was selected the most, followed by practical health insurance(35.2%). 3. In regard to what type of job they performed as dental coordinators, 88.9 percent were in charge of appointment in the field of customer service, and 87.9 percent paid attention to having good manners as service providers in the area of self-management. In the field of hospital affairs, 81.3 percent were in charge of receiving. 4. As to their awareness of dental coordinator job, the largest group took pride in the job they performed ($3.99{\pm}0.76$), and the second largest group believed that dental coordinators made a great contribution to hospital management ($3.92{\pm}0.70$). The third largest group gave a great weight to their own job ($3.91{\pm}0.84$) in light of overall dental duties and the fourth largest group found themselves to get along with other employees regardless of position ($3.86{\pm}0.74$). The fifth largest group believed their job was of great use for promoting the oral health of patients ($3.76{\pm}0.75$), and the sixth largest group thought the future of dental coordinators was promising($3.74{\pm}0.86$). 5. In regard to their perception by age group, those who were older had a better opinion on every item of their job in general. Their age made a statistically significant difference to their view of the weight of dental coordinator job(P < 0.001) in light of overall dental duties, of being approved and trusted by managers(P < 0.01), of social awareness of dental coordinator, and of being understood and approved by other employees and dentists. Their pride in current job and their satisfaction with the name of their position were statistically significantly different according to their age as well. Besides, their age made a statistically significant difference to their opinion about whether or not there was an age limit to their occupation and about their contribution to hospital management (P < 0.05). 6. As for their perception by type of job, the dental hygienists were generally most satisfied with their job, followed by nursing aids and others. There was a statistically significant gap among their opinions about whether to make a job-related decision on their own(P < 0.001). the weight of their job in terms of overall dental duties, whether their job improved their ability, whether their job made a great contribution to enhancing the oral health of patients, whether their job was understood and approved by other employees(P < 0.01), social awareness of their job, whether they conflicted with other employees during job performance, and whether dental hospitals or clinics offered a self-development opportunity for them to take their ability to another level(P < 0.05). And their satisfaction with current pay was statistically significantly different as well.

Awareness, need and demand for the amendment of medical device law according to the general characteristics of some dental hygienists (일부 치과위생사들의 일반적 특성에 따른 의료기사법 개정에 대한 인식, 필요 및 요구도)

  • This research has been conducted from June 1, 2017 to August 25, 2017 for the dental hygienists working in Busan and Gyeongnam area, to provide necessary basic data for the purpose of revision of the relating laws from the analysis of their understanding and perspective on such laws through 262 subjects' questionnaire. Its analysis of their perspectives are as follows. 1. The common characteristics of the subjects are that 40.1% of them are under 25, and their working period was under three years with the 38.9%, and as for their marital status, 70.2% were single, the final education of 80.2% were associated degrees. Their working areas are centered in Gyeongsangnam-do province with 91.2%, the workplace type is for the dental clinic with 80.2%. 2. The comparison of the view point of the medical(technician) law according to the characteristics of the research subjects differed only whether or not the completion of the education, and as a result of the comparison according to the characteristics of those subjects understanding the details of the medical(technician) law, there was a significant difference saying "the current law has clear job description" depending on the working area or "the job duty is definite" depending on the job experience and job details. As for those saying "the job duty is definite", there was also significant difference depending upon marital status, final education and work details. There was no significant difference in all characteristics from findings of the necessity of legal system according to the subjects not understanding the medical(technician) law. 3. As a result of comparing the necessity of the medical(technician) law according to the characteristics of the research subjects, it was found that all the subjects accepted the necessity of the medical law revision including the dental hygienist in the medical person. The "necessity of the professional dental hygienist system" showed a significant difference depending upon the final education and medical institution type. Among the triggering factors in its amendment, there showed significant difference in the "cooperation of other organizations" and "solution of medical law problems" only in the final education. 4. As a result of comparison of the needs of the medical(technician) law revision according to the characteristics of the research subjects, it was found that the significant traits related to the age and job details showed "Legal responsibility would be increased" when the medical law is revised, in case that "it will help broaden the job extension", there showed a significant difference in career, final education, and working institution, and job details. "Legal protection is possible" showed significant difference except the age group and working area, and "it help the system settlement" showed in the final education. There was a significant difference in career, final education, and job details that "I can regulate the education and field practice", and the same in "my status will be improved" depending upon the final education, work area, and job details. Accordingly, in this research, for the establishment of more professional and comprehensive dental health service as suggested from the demand and necessity toward the medical(technician) Law by the dental hygienists.

The Demands and Awareness of Pediatric Dentists on the Korean National Health Insurance System (국민건강보험에 대한 소아치과 의사들의 요구와 인식 조사 )

  • The aim of this study is to investigate the awareness and perception of pediatric dentists regarding dental health insurance (DHI) system in Korea. Online surveys were sent to the members of the Korean Academy of Pediatric Dentistry (KAPD), and the responses were collected. The survey, consisting of 41 questions, was subdivided into 'general background of the respondents', 'the awareness on national health insurance (NHI) and DHI', 'the awareness on DHI pertaining to pediatric dentistry', 'issues that need improvement on DHI pertaining to pediatric dentistry', 'DHI claims', and 'the roles of KAPD in DHI'. In total, 302 responded, with a response rate of 28.9%. Excluding 2 questionnaires with insufficient answers, a total of 300 survey results were analyzed. According to the analysis, pediatric dentists thought that the coverage of DHI was not as sufficient compared to that of NHI, but were expecting its scope to broaden in the future. The satisfaction rate was higher in pit and fissure sealant and composite resin filling than in caries-detecting quantitative light-induced fluorescence. Pediatric dentists considered permanent tooth composite resin filling (65.7%) needed an increase in insurance copayment, while topical fluoride application (74.7%) to require insurance coverage. DHI claims were generally handled by dental hygienists and/or nursing assistants. Approximately half of the respondents answered that they have had experiences on appealing for insurance denials. Lastly, pediatric dentists generally had a positive attitude towards providing information for the DHI. This study is expected to be used as a sound dataset for the DHI policy development concerning pediatric dentistry.

A study on the knowledge and awareness of care workers on the geriatric oral health (요양보호사의 노인 구강보건에 대한 지식과 인식도 연구)

  • The study aims to examine the knowledge and awareness of care workers on oral health knowledge at the geriatric care facilities in order to improve and develop oral health services. For the research method, the study carried out a questionnaire survey to 163 care workers working at the geriatric care facilities (10) in D Metropolitan City from May 1~31, 2015 and then the collected data were statistically analyzed. As a result, the oral health knowledge of care workers was 20.09 points out of total 26 points (77.2%). The highest knowledge was 'proper toothbrushing method' with 93.0% and the lowest one was 'knowledge on the use of oral supplementary goods' with 58.1%. Those with experience of oral health education had higher demand on oral health education (P<0.001), and those with higher experience of oral health education had significantly higher oral health knowledge (P<0.001), demand (P<0.01) and awareness (P<0.05). As for factors influencing care workers' oral health awareness, if they had higher oral health knowledge points (P<0.01), oral health education demand (P<0.01), they had higher oral health awareness level. Accordingly, it is required to provide efficient, sustainable and practical oral health education of care workers considering oral characteristics of the long-term hospitalized patients at the care hospital. And, the role of dental hygienist as the person in charge of oral health education is necessary. Therefore, institutional support from the government is required to assign professional personnel.

A Study on Effective Smoking Cessation Policy of Smoker Patients (흡연환자의 효과적인 금연정책에 대한 연구)

  • The purpose of this study was to examine the subjective awareness of smoker patients about the preparation of smoking cessation plans in an effort to lay the foundation for smoking cessation policy setting. The subjects were the selected patients at a dental clinic. A self-administered survey was conducted to grasp their personal characteristics, and the selected answer sheets from 236 respondents were analyzed. The findings of the study were as follows: 1. As for the extension of existing smoking cessation plans, the largest group (32.6%) preferred smoking cessation education, followed by smoking cessation counseling (28.8%), prescription of an smoking cessation aid (18.6%). 2. Concerning the necessity of a smoking cessation law, the men and the women respectively gave 2.37 and 3.00 to that, and the gender gap was significant. The patients had a significantly different opinion on the necessity of a rise in tobacco price according to their occupation and monthly mean household income, and their took a significantly different view of the necessity of smoking cessation counseling according to their residential area. 3. As a result of analyzing the correlation between their on effective smoking cessation policies and the related variables, all the variables had a statistically significant correlation to each other. 4. As a result of analyzing their opinions on what institution should be in charge of smoking cessation plans, the biggest group answered that dental clinics should be responsible for smoking cessation-aid prescription (50.0%), and the largest group replied that smoking cessation education and smoking cessation counseling should respectively be provided by public dental clinics (37.3%) and dental clinics (44.1%).The above-mentioned findings suggest that the preparation of new smoking cessation measures and the extension of existing smoking cessation plans are urgently required, and that dentists and dental hygienists should make a concerted effort to offer counseling and education to stimulate dental patients to abstain from smoking.
