Mobile Phone based Asthma Management System (이동전화를 이용한 천식질환 원격관리시스템)

  • Asthma requires continuous long-term management with at least 5% outcome of general population as well as being the second cause of death and disability after cancer. The present study developed an efficient self management system based on the commercial mobile phone network. The spirometric test results are input to the mobile phone through the communication line connected to the portable spirometer. The doctor or the care-giver can search, identify, and review the data accumulated daily by the patient, and feedback to the patient necessary recommendations by short message and color mail services. Patient can also send an emergency call to the doctor and/or the care-giver. User interface was designed as convenient as possible for maximum efficiency of these operations. The present system provides a desired remote medical services, thus would enhance health management of chronic diseased patients.

The Expression of Cytokines in the Airways from Patients with Bronchial Asthma (천식환자의 기도내 Cytokines 표현에 대한 연구)

  • Background: It has been well known that bronchial asthma is a chronic inflammatory disorder. The "activation" of lymphocytes has a significant role in the pathogenesis of bronchial asthma. Among these lymphocytes, TH2-like rather than TH1-like lymphoytes are activated in the bronchial tissues from patients with atopic bronchial asthma. However, the difference of cytokines expression is not well documented between the atopic normal subjects and atopic asthmatics. Methods: Bronchial tissues were obtained from the tweleve atopic and non-atpoic asthmatics and tweleve atopic and non-atopic healthy subjects for in stiu hybridizatin of IL-2, IL-4, IL-5, and INF-$\gamma$. The probe of cytokines were tagged with digoxigenin by random priming method. Results: The infiltration of many inflammatory cells on submucosa and denuded epithelium were observed in the bronchial tissue from patients with bronchial asthma. The RNase-treated bronchial tissues did not have the brown signal on the tissue, but, RNasc-untreated bronchial tissues had the positive brown signal on the inflammatory cells under the basement membrane. The IL-2 positive signals were detected in 2 cases, IFN-$\gamma$ in 1 casc, IL-4 in 2 cases, IL-5 in 2 cases among 6 non-atopic healthy subjects. The atopic healthy subjects showed 1 case of positive signal of IL-2 and IFN-$\gamma$, but did not show any signals of IL-4 and IL-5. The positive signals of IL-2 were detected in 4 cases among 6 atopic and 6 non-atopic asthmatics, 2 cases and 1 case of IFN-$\gamma$ respectively, 4 cases and 3 cases of IL-4 respectively, 4 cases and 3 cases of IL-5 respectively. Conclusion: The lymphocytes were activated in the bronchus of asthmatics. Among lymphocytes, TH2-like lymphocytes may be involved in the pathogenesis of bronchial asthma. However, futher study with immunohistochemical stain may be necessary for defining the source of cytokines, because of TH2-like lymphocytes were also activated in some atopic healthy subjects.

Clinical Features of Sulfite-Sensitive Asthmatics (설파이트(Sulfite) 과민성 천식 환자의 임상적 특성에 관한 연구)

  • Background:Sulfiting agents are widely used as preservatives and antioxidants in foods, beverages and drugs including bronchodilators. There have been reports of sulfite-related reactions such as anaphylaxis, urticaria, angioedema, abdominal discomfortness as well as bronchospasm. Several investigators reported that sulfite-sensitive asthmatic patients comprised from 3.9% to 8.2% of all asthmatic patients and its prevalence was higher in steroid-dependent group than in steroid-independent group. Subjects and Method:We performed oral provocation test with sodium bisulfite and aspirin in 17 asthmatic patients who have experienced aggravation of their symptoms after administration of drugs or foods. All of them were steroid dependent asthmatics. We observed clinical symptoms and steroid requirements from 1 to 18 months. Result:Ten of them showed severe bronchoconstriction after the ingestion of sodium bisulfite (50 to 200 mg) within 30 minutes. Concurrent aspirin intolerance was noted by oral provocation test in four cases (40%). Three of them showed positive responses on skin prick test with sulfite (10 or 100 mg/ml). Mean total eosinophil counts was $844/mm^3$ at asthmatic attack. And there was no significant responses on skin prick test and IgE-RAST to common inhalant allergens. After complete avoidance from sulfite containing foods and drugs as well as antiasthmatic medication for 1 to 18 months, nine of them (90%) could stop or reduce the steroid requirements. ConcIusion:It was suggested that severe steroid dependent and intrinsic type of asthmatic patients should be evaluated for sulfite-sensitivity.

Factors Affecting the Perception, Knowledge, and Preventive Behaviors of Chronic Pulmonary Disease Patients on Particulate Matter (만성호흡기질환자의 미세먼지에 대한 인식, 지식, 예방행위와 관련 요인)

  • Objectives: This research aimed to identify the level of perception, knowledge, preventive behavior, and factors affecting preventive behavior of patients with asthma and chronic obstructive pulmonary disease against particulate matter. Methods: This research was a descriptive survey research, and the subjects were chronic pulmonary disease patients over the age of 19 and under 80 who visited a university hospital in Daegu City. Data was collected by convenience sampling through structured self-administered questionnaire survey from December 2019 to January 2020, and a total of 212 copies were used for analysis. Results: Out of 212 total subjects, 112 were asthma patients (52.8%) and 100 were chronic obstructive pulmonary disease patients (47.2%). The average score (out of 10) of perception, knowledge and preventive behavior of patients with asthma for particulate matter was 7.92, 6.99, and 7.10, respectively, while those with chronic obstructive pulmonary disease scored 7.72, 6.24, and 6.80, respectively. The knowledge score was significantly higher in patients with asthma than those with chronic obstructive pulmonary disease (p=0.007). Factors affecting particulate matter preventive behavior were perception score, knowledge score, and age for asthma patients, and perception score for chronic obstructive pulmonary disease patients. Conclusions: As a result of the above, the factors relate to the preventive behavior of patients with asthma and chronic obstructive pulmonary disease were perception score, knowledge score, and age for asthma, and perception score for chronic obstructive pulmonary disease. Raising the level of particulate matter preventive behavior can prevent the deterioration of chronic pulmonary diseases caused by particulate matter, so the development of arbitration programs considering the characteristics of the patients according to the disease and continuous and repetitive education are required.

Chronic Cough : The Spectrum and the Frequency of Etiologies (만성기침 환자의 원인질환과 빈도)

  • Background: Chronic cough is a common symptom that requires the systematic diagnostic approach for proper evaluation. Postnasal drip syndrome(PNDS), bronchial asthma, gastroesophageal reflux disease(GERD), and chronic bronchitis are among the common causes. This study was conducted to evaluate the spectrum and the frequency of the causes of chronic cough. Methods: We prospectively evaluated 93 patients who had chronic cough despite normal chest radiographic finding. History and physical examination were done along with paranasal sinus radiograph, spirometry, bronchoprovocation test and 24-hours' ambulatory aesophageal pH monitoring as necessary. Results: Forty-nine(52%) of the 93 patients had PNDS, 15 patients(16%) bronchitis, 10 patients(11%) asthma, 4 patients (4%) GERD, 7 patients (8%) both PNDS and asthma, 4 patients (4%) undiagnosed condition and 4 patients(4%) were taking ACE inhibitor. Sixty-nine percent of the patients with PNDS improved after follow up, 73% patients with bronchitis, 80% patients with asthma, 50% patients with GERD, 100% patients with both PNDS and asthma, and 100% patients with ACE inhibitor. Conclusion: PNDS was the most common causes of chronic cough. Bronchitis was the second and asthma the third in frequency. The etiology of chronic cough can be determined easily by history and physical examination, successful therapy initiated in most patients. The response to specific therapy also was important in evaluation of chronic cough.

Association of Plasma Eotaxin with Asthma Exacerbation and Severity (혈장 eotaxin과 천식의 급성악화 및 중증도와의 연관성)

  • Background : The eosinophil chemotactic and activating effects of eotaxin and the known association of eosinophils with asthma suggest that eotaxin expression is increased during an asthma attack. This study was aimed to determine whether the plasma eotaxin levels are higher in patients during an asthma attack and to correlate the eotaxin levels with the disease activity, severity and response to therapy. Method : A case-control study of the plasma eotaxin levels was performed in 100 patients with exacerbated asthma and 48 age- and sex-matched subjects with stable asthma. Results : The plasma eotaxin levels were significantly higher in the 100 patients with exacerbated asthma($233{\pm}175\;pg/mL$) than in the 48 subjects with stable asthma($169{\pm}74\;pg/mL$). A trend toward higher eotaxin levels was observed in asthmatic subjects who were taking oral steroids ($332{\pm}225\;pg/mL$) than in those who were not ($214{\pm}159\;pg/mL$) and higher levels were found in those admitted to the hospital ($275{\pm}212\;pg/mL$) than in those discharged after receiving only emergency treatment ($190{\pm}115\;pg/mL$). The eotaxin levels inversely correlated with the $FEV_$ (r=-0.25, p<0.01). The eotaxin levels were higher in moderate persistent ($323{\pm}256\;pg/mL$) and severe persistent asthmatics ($276{\pm}170\;pg/mL$) than in mild intermittent asthmatics ($l60{\pm}60\;pg/mL$). Conclusion : Eotaxin expression is directly associated with asthma exacerbation, impaired pulmonary function and the disease severity.

Asthma Worsening factors of Adolescent Asthma Patients in Korea -Associated with Intake of Antioxidant Food- (청소년 천식 환자의 천식 악화의 영향 요인 -항산화식이 섭취와 관련하여-)

  • The purpose of this study was to investigate the association between antioxidant food intake pattern and asthma control status in Korean adolescents. This study was conducted using data from 1,578 patients diagnosed with asthma among the participants in the 11th online health behavior survey. Statistical analyses were performed for sociodemographic variables, known risk factors for asthma worsening, and all variables which were related to food intake pattern. For final decision, logistic regression analysis was used to estimate the associations between antioxidant food intake and asthma worsening. Male gender and smoking experiences were associated with asthma exacerbation. Low intake of antioxidant foods status also significantly increased asthma exacerbations. Antioxidant food intake and smoking prevention education is important for adolescent asthma patients. It is highly needed to recommend taking fruits and vegetables properly.

Impervious Surface as a Thematic Parameter of Analysis for Childhood Asthma Hospitalizations : Spatio-temporal Approaches (소아천식 유병율 분포의 분석변수로서 불투수면 : 시공간적 접근)

  • The impervious surface was frequently employed as a proxy of the total environmental burden in the urban area. The impervious surface was extracted from a satellite image and the GIS (Geographical Information System) database for childhood asthma hospitalizations was generated for a total of 62,136 children using the National Health Insurance database of South Korea. Children living in an impervious environment do result in almost twice as many hospitalizations (26.58%) for asthma, as compared to the sub-urban pervious living (15.82%). Furthermore, the risk zones with persistently high hospitalizations for three years were specifically identified over the impervious sub-district alone. The impervious zone showed a small inter-year variation of hospitalizations (r: 0.937) while the sub-urban pervious fringe was found to display a yearly variation(r: 0.371). The strong temporal autocorrelation means that the impervious areas have frequent long-lived hospitalizations for asthma, thus children living in impervious areas were likely to have had more chronic asthma attacks than those living in pervious areas. These experimental results indicate that an impervious surface as thematic parameter of analysis is a crucial explanatory variable for asthma hospitalizations and its longer persistence among children.

  • Background: Bronchial asthma is characterized by chronic eosinophilic inflammatory airway disease associated with bronchial hyperresponsiveness and reversible airway obstruction. Bronchial inflammation in asthma may depend in part on the activation of T helper lymphocytes that elaborate proinflammatory cytokines. T helper (Th) lymphocytes can be divided into two categories; Th1 lymphocytes, which secrete IL-2, IL-12 and IFN-$\gamma$, and Th2 lymphocytes, which secrete IL-4, IL-5, IL-6 and IL-10. Th2 lymphocytes appear to induce allergic responses, whereas Th1 lymphocytes induce delayed-type hypersensitivity response. Some infections, such as tuberculosis, cultivate a Th1 immunological environment and inhibit Th2 lymphocytes function. The presence of such infections might inhibit Th2 immune responses and thus protect development of atopic diseases. Method: 15 patients with allergic bronchial asthma, 10 patients with intrinsic bronchial asthma, and 10 healthy volunteers were studied. The serum concentrations of IFN-$\gamma$, IL-12, IL-4, IL-5, and IL-10 were measured by ELISA method and tuberculin skin test was estimated in different groups. Results: The positive response rates of tuberculin test were 46.7% in patients with allergic asthma, 100% in patients with intrinsic asthma and 60% in normal controls. The positive response rates were significantly lower in patients with allergic asthma than those of in patients with intrinsic asthma (p<0.05). Degree of responses to tuberculin test were $12.0{\pm}9.6mm$ in patients with allergic asthma, $18.4{\pm}4.5mm$ in patients with intrinsic asthma and $10.9{\pm}8.8mm$ in normal controls. The degree of responses were significantly reduced in patients with allergic asthma than those of patients with intrinsic asthma (p<0.05). The serum levels of IL-5 in patients with allergic asthma were significantly higher than in patients with intrinsic asthma and normal controls (p<0.05), although it was insignificant. the serum levels of IL-4 and IL-10 in patients with allergic asthma were higher than that of intrinsic asthma and normal controls. The serum levels of IL-12 and IFN-$\gamma$ in patients with allergic asthma and intrinsic asthma were significantly lower than those in normal controls(p<0.05). The serum levels of total immunoglobulin E (IgE) and peripheral blood eosinophile counts in patients with allergic asthma were significantly higher than those in normal controls. Peripheral blood esinophil counts had a significant correlation with the serum levels of total IgE, IL-5 and IL-10 in patients with allergic asthma (p<0.05). Conclusion: These results have showed that Th1 lymphocyte functions were lowered and Th2 lymphocyte functions were elevated in patients with allergic asthma than those in normal controls. Suppression of Th1 lymphocyte functions by activation of Th2 lymphocyte might be one of the important aspects of pathogenesis in allergic bronchial asthma.

  • Backgrounds: Although glucocorticoids are one of the most potent anti-inflammatory agents, they have limited effect on cysteinyl leukotriene biosynthesis. In addition, the response to inhaled corticosteroids (ICS) and inhaled long-acting ${\beta}_2-agonists$ (LABA) combination therapy in moderate to severe persistent asthmatics varies. Additional therapy with leukotriene receptor antagonists (LTRA) in patients with moderate to severe asthma suboptimally controlled with ICS and LABA combination therapy would be complementary to asthma control. Methods: One hundred and ninety eight asthmatics entered a 2 month, open-label descriptive study. Patients suffering from persistent asthma and suboptimally controlled on a combination therapy of fluticasone/salmeterol or budesonide/formoterol were given montelukast 10 mg daily as an add-on therapy. The level of asthma control was assessed using the Asthma Control Questionnaire (ACQ) including $FEV_1%$ predicted at the baseline and after a 2-month treatment with montelukast. A global evaluation of the treatment was also made by the patients and physicians. Results: The mean ACQ score decreased significantly on montelukast ($11.5{\pm}5.4$ at baseline vs. $6.7{\pm}5.0$), with a significant improvement in all individual symptom scores (p<0.01). The $FEV_1%$ predicted values did not show any significant change. 59.9% of patients and 59.4% of physicians reported global improvement in their asthma (${\kappa}=0.85$). Conclusion: These results suggest that the addition of montelukast in patients with persistent asthma that is suboptimally contolled by combination therapy of ICS and LABA might confer complementary effects on asthma control.
