Purpose: This study was aimed to understand and describe the Korean critical care advanced practice nurses' work experience. Methods: Data was collected through 2 focus group interviews that were held in two different university hospitals in Seoul, Korea. All interviews were recorded and transcribed, and data were analysed using modified qualitative content analysis. Results: Ten themes were drawn from data analysis. They are "Various and complex tasks", "Excellency of advanced nursing practice", "Pressure and responsibility accompanied with pride", "Role identity confusion", "Role conflict", "Leaping and evolving role", "Gap between role and outcome evaluation", "Underestimated outcomes", "Where are my outcomes?" "Searching for outcomes: publicize the role". Conclusion: The results of this study help to understand the role of Korean critical care advanced practice nurses and to guide to outcome evaluation of their role effectiveness. Developing qualitative outcome indicators, cost-effectiveness ananlysis of Korean critical care advanced practice nurses' role, and accumulation of evidences through researches on outcome evaluation will be needed to successfully settle down advanced practice nursing in Korea.

Purpose: This descriptive study was conducted to project the number of critical care APNs needed in critical care units in an acute care hospital setting, up to the year 2020. Method: Necessary data and information were collected from various funded reports, professional literature, web-sites and personal visits to national and private institutions. The demand of critical care APNs were projected based on two critical care APNs per critical care units. Result: The projected number of critical APNs for the critical care units in acute care hospital settings as follows: 1) The total projected number of critical care APNs needed for critical care units were 1,270 in 2001. 2) By the year 2020, total number of projected critical care APNs needed in critical care units will be 1,080-1,700. Conclusion: In order to match the supply to the need, the professional organization should direct their efforts toward enacting legislation. Educational systems should identify strategies in initiation of critical care APN programs in masters level as well as standardizing curriculums across the programs.

Purpose: The purpose of this literature review was to describe the roles of advanced practice nurses related to advance directives. Methods: A comprehensive literature review was conducted utilizing Pubmed, Ovid, and CINAHL. The key words for search were 'advance directives', 'living wills', 'durable power of attorney', 'advanced practice nursing', 'advanced practice nurse', 'nurse practitioner', and 'nurse specialist'. Research articles published between January 1990 and September 2010 and written in English were included. Review papers and expert's opinion were excluded. Results: Five articles were yielded and analyzed. Among them, two focused on the advanced practice nurses' role during the process of advance directives. The other three presented the outcomes depending on the type of nurses' involvement in each health care delivery system. Conclusion: Recently, Korea established advance directives and expanded to diverse sites. Advanced practice nurses' role in the advance directive decision-making process should be examined and developed in research and practice.

Purpose: The purpose of this study was to develop a resource-based relative value scale (RBRVS) and its conversion factor for advanced nursing practices carried out by critical care nurse practitioners (CCNP) in intensive care units. Methods: The methodology was developed by calculating CCNP's RBRVS for 32 advanced nursing services based on CCNP's workload and time spent in the context of national health insurance. A cost analysis was performed to estimate the conversion factor of CCNP's RBRVS. The share of CCNP's contribution to fee-for-service in intensive care units was also analyzed. Results: Calculation of the RBRVS of 32 advanced nursing practices showed a range of points from 100.0 to 1,181.4 and an average of 296.1 points. The relevant conversion factor for advanced nursing practices in CCNP were estimated at 37.3-48.4 won. The contribution rate of CCNP's advanced nursing practices in the relative value scale of the national health insurance was estimated at 0.1-31.3%. Conclusion: Measuring the economic value of advanced nursing services will be a basis for esta-blishing a reimbursement system for CCNP's practices and thus encourage a social demand for advanced nurse practitioners.

Purpose : This study aimed to investigate the public's perception of reimbursement for the education and counseling services provided to advanced practice nurses in intensive care units. Method : This was a secondary data analysis study. The original data were collected utilizing an online panel. The sample comprised 615 individuals aged between 19 and 49 years nationwide. The study variables included the public's perception of reimbursement for education and counseling services, age, gender, education level, income, and health status. Variables such as past experience of being admitted to an intensive care unit by self, family, or relatives and the recognition of advanced practice nurses were also examined. Results : The mean of the perception score was 3.15 on a 4-point scale ranging from 1 (strongly disagree) to 4 (strongly agree). Among the participants, 89.2% answered "somewhat agreed" or "strongly agreed" to the question about the education and counseling services being covered by the National Health Insurance. Moreover, education level, past experience of being admitted to an intensive care unit by self, family, or relatives, and recognition of advanced practice nurses were significantly associated with the perception score. Conclusion : Efforts should be made to publicize the need for the education and counseling services that are provided to advanced practice nurses in intensive care units.

The purpose of this study is to investigate the nurses' needs in the general hospitals as a basic study to develop ICU clinical nurse from Aug. 3 to Sept. 26 1998 at 13 general or university hospitals that have ICU nurse training course and are accredited at KNA. The subjects of study were ICU nurses and nurse managers at the department of nursing. The questionnaire items were developed from literature review, interviews with 30 ICU nurses at an university hospital in Pusan. The results were as follows : The subjects of study educated in ICU nurse training course were 44.3% of total. The 93.1% responded the necessity of ICU CNS and 89.1% wanted to have ICU CNS certification. The 43.8% answered that ICU should be a center of CNS education and 32.0% answered the department of nursing. Most of subjects responded that the clinical experience of ICU should be needed prior to CNS education. Regarding the treatment of ICU CNS, the 34.2% of total subjects responded it would be suitable to pay additional allowance and the 28.3% answered to grade-up salary step, and then the 13.7% to promote level position. Concerning the assignment department, the 63.5% answered that the charge nurse would be proper than general nurse or head nurse. As to the working time of ICU CNS, the 93.2% responded that D-duty is needed, the 79.5% to E-duty and the 64.4% to N-duty. It is suggested that the consensus of ICU nurses for the concept, the role and the system of ICU CNS is needed.

Purpose: The purpose of this study was to reclassify the advanced nursing practices of critical care nurse practitioners(CCNPs) in intensive care unit and measure the time and frequency of CCNP's activities. Method: Practices of ICU nurses are divided into RN's and CCNP's practices by a panel of ICU nursing experts. Each practice of CCNP is defined and CCNP's working time and service frequencies are monitored in general hospitals. Result: Practices of CCNP were classified into 4 domains and 32 practices. Fourteen practices by CCNPs were completed in 10 minutes and the other 12 practices consumed 10-30 minutes. A priority of practice in respiratory therapy was given to artificial airway management, management of tracheostomy patient, lower respiratory care, and the priority of CRRT was management of anticoagulation. Conclusions: Advanced nursing practices of CCNPs were recognized from those of RNs. A further research of CCNPs practices should be extended to other advanced practices and it is required to evaluate economic value of advanced nursing practice in the national health insurance system.

본 연구는 구강점막욕창의 시각적 감별도구에 대한 신뢰성을 평가하기 위한 연구이다. 연구의 절차는 개발단계와 평가단계로, 개발단계에서는 자료수집, 자문단의 타당도와 신뢰도 평가, 최종도구개발이 수행되었으며, 평가단계에서 대상자들에게 9장의 사진도구를 평가하게 하였다. 세 군에서 측정자 간 신뢰도를 구한 결과, 상처 전문간호사군에서는 Krippendorff's alpha 가 .75이었고, 중환자 전문간호사군에서는 .70이었다. 치과의사군에서의 Cohen's weighted kappa값은 .78 로 나타났다. 측정자 내 신뢰도를 살펴본 결과, 중환자실 일반간호사군들의 intra-rater reliability는 .73으로 나타났다. 신뢰도가 입증된 9장의 사진은 실제 중환자실과 같은 구강점막욕창 위험을 가진 환자를 돌보는 임상환경에서 교육용으로 활용될 수 있을 것으로 보인다.

Purpose: The purpose of the study was to evaluate how advanced practice nurses (APNs) recognized their work by analyzing nursing outcomes. The ultimate goal was to examine the utility of APNs by connecting their work to the outcomes. Methods: Cross-sectional survey was conducted to collect the data from 59 APNs in a tertiary hospital lovated in Seoul, Korea. The questionnaire with 84 outcome indicators for advanced practice nurses was used. Descriptive statistics were used to analyze the data. Results: Among 84 outcome indicators, 11 indicators were associated with the work of APNs. Majority of these findings were included in the categories of clinical results and satisfaction. Items identified to be associated with the work of APNs were different among oncology nursing area, surgical speciality area, emergency care area, and critical care area. Conclusion: The results of the study indicated that the works of APNs were relevant to education/counseling/training and interpersonal relationship with other health professionals. The results of the study could be helpful to delineate the work characteristics of APNs and increase the utility of APNs in Korea where roles of APNs are not yet clearly delineated in most clinical settings.

Purpose : The purpose of this study was to identify the mediating effect of role clarity in the relationship between clinical decision-making abilities and job stress among advanced practice nurses at tertiary hospitals. Methods : The participants were 137 advanced practice nurses. The assessment tools were clinical decision-making in nursing scale, role ambiguity scale, and Korean occupational stress scale (KOSS-26). Data were analyzed with the SPSS/24.0 program and mediation analysis was performed according to the Baron and Kenny methods. Results : There were significant relationships between clinical decision-making abilities and job stress (r=-.33, p<.001), and role clarity and job stress (r=-.29, p=.001). Role clarity showed partial mediating effects in the relationship between clinical decision-making abilities and job stress (Z=2.02, p=.043). Conclusion : Therefore, to reduce advanced practice nurses' job stress, it is necessary to develop a program and strategies to increase their clinical decision-making abilities.
