Maxillary removable partial overdenture within Locator Root Attachment in a retained root: a case report (소수 잔존 치근에 Locator Root Attachment를 활용한 상악 가철성 국소의치 증례)

  • If maintaining oral few retained root when planning the removable prosthesis, it is possible to obtain a more comfortable and functional result. In this case report, 'Locator Root Attachment' was used to maxillary removable overdenture in patients with a few teeth and retained root. A functionally proper clinical result from attachment after clinical and radiographic analysis was observed in this case.

  • The pulp infection of primary tooth is often caused by dental caries or trauma. But, if it is not managed properly, it can be produce the periapical lesion. The periapical lesion can cause some complications such as enamel hypoplasia, displacement, root dilaceration and impaction. Treatment options of displaced permanent successor are clinical and radiological follow-up after extraction of primary teeth, surgical opening, orthodontic traction, transplantation and extraction. In these cases, the premolars with abnormal eruption path caused by periapical lesion of the primary tooth have shown successful spontaneous eruption just as a result of extraction of infected primary tooth and space maintenance.

  • Pediatric dentists are frequently encountered with fractured root fragments of primary teeth caused either by the traumatic injuries or by the accidental fractures during the procedure of tooth extractions. In these situations, we often hesitate which method to choose, extract or retain it. In general, it is recommended to retain apical fragments, as the attempts to extract the apical fragments might harm the developing permanent tooth germ. This study was designed to ensure the validity of intentional retention of the root fragments of primary teeth in the situations described above. 6 children with intentionally root fragments who experienced root fracture in primary anterior teeth were available Periodic radiographic assessment was performed at 3 months interval for $7{\sim}37$ months. The results of this study showed that apical fragments had been resorbed through physiologic process in 5 patients. Apical fragment had been gingival emergence along with the erupting permanent tooth in 1 patient. There were no evidence of interference with eruption of permanent successors. In summary we have been ensured the validity of intentionally retention of the root fragments of primary teeth. Children with being remained apical root fragment should be recalled regularly for assessment and parents should be thoroughly informed about the situation with special emphasis on the necessity of periodic check-up.

  • The stability and retention of mandibular complete denture have been a continuing problem for dentists and patients. Magnets have been investigated or used in clinical dentistry as retentive aids. The obvious advantages are that these attachments do not need spacial equipments or require frequent adjustment of replacement with use, and they likely transmit less lateral forces to the abutment.   • Chemotherapy and radiotherapy used on pediatric oncology patients often causes dentofacial anomalies. Defects noted include tooth and root agenesis, root thinning, root shortening, localized enamel defect and maxillofacial underdevelopment. The effect of radiotherapy usually is confined to the radiation site but the effect of chemotherapy may be more wide spread becuase of its systemic distribution. Many pediatric cancers are treated with a combination of radiation and multiagent chemotherapy. Dental treatment affected by chemotherapy and radiation therapy damage to developing teeth and maxilloface includes retention of teeth, space maintenance, prosthetic considerations, requirements for oral hygiene.   • The purposes of this study were to observe the cavity wall morphology and to evaluate the degree of cavity cleanliness when root-end retrograde cavity preparation was done with ultrasonic diamond instruments. To observe the morphology of retrocavity and to evaluate the degree of cavity cleanliness. root-end resections were done on 20 palatal roots of extracted maxillary first molars after canal filling with gutta-percha. Retrocavities were prepared using either ultrasonic diamond instruments or stainless steel ones of medium power setting of level 6 ($Miniendo^{TM}$, EIE, CA, U.S.A.). Morphology of the cavity. degrees of the remaining canal debris and smear layer were evaluated under the scanning electron microscope. The results were as follows: Cavities prepared with ultrasonic diamond instruments showed scratched appearance of wall, while ultrasonic stainless steel preparation showed hatcheted appearance. Ultrasonic diamond instruments induced more smear layer than stainless steel ones did (p<0.01) in the cavity. However, there was no significant difference in canal debris (p>0.05).

  • In the case of the impacted teeth, the clinician has to consider development of tooth, site of impaction, eruption path, and cooperation of patient. Treatment options for the management of impacted teeth are separated into four categories: observation, intervention, orthodontic or surgical relocation and extraction. Autotransplantation may be defined as the transplantation of embedded, impacted or erupted teeth, from one site to another in the same individual into extraction site or surgically prepared sockets. Autotransplantation ensures preservation of natural tooth, induction of alveolar bone growth and root development, offers one of the fastest and most economically feasible means in the replacement of young patients' missing teeth. This case presents a malpositioned impacted mandibular premolar of an 11-year-old girl. It was thought that orthodontic traction was difficult because of its unfavorable impacted position. Therefore the tooth was treated by autotransplantation, we can observe good healing pattern during 12 months.
