A 10-year retrospective study on the risk factors affecting the success rate of internal connection implants after prosthetic restoration (내부연결 임플란트의 보철 수복 후 성공률에 미치는 위험요소에 관한 10년간의 후향적 연구)

  • Purpose. The purpose of this study is to help increase the success rate by analyzing the types and characteristics of implant prosthesis and the survival rate. Materials and methods. Among implants placed between 2011 and 2020 at Sanbon Dental Hospital, College of Dentistry, Wonkwang University, a case restored by a prosthetic surgeon was investigated for the characteristics and correlation of failure. The causes of failure were classified as failure of osseointegration, peri-implantitis, fixture fracture, abutment fracture, screw fracture, screw loosening, prosthesis fracture, and loss of prosthesis retention. Prosthetic method, cantilever presence, placement location, etc. were analyzed for their correlation with implant failure. Results analysis was derived through Chi-square test and Kaplan-Meier survival analysis using SPSS ver 25.0 (IBM, Chicago, IL, USA). Results. A total of 2587 implants were placed, of which 1141 implants were restored with Single Crown and 1446 implants with Fixed Partial Denture, and the cumulative survival rate was 88.1%. The success rate of SC was 86.2% (984) and the success rate of FPD was 89.6% (1295), showing statistically significant differences, among which factors that had significant differences were abutment fracture, screw fracture, and screw loosening (P < .05). Conclusion. As a result of the 10-year follow-up, more failures occurred due to biomechanical factors than biological factors. Further studies on the success of implants will be needed in the future.

Success rate and marginal bone loss of Osstem USII plus implants; Short term clinical study (Osstem USII plus 임플란트의 단기간 성공률 및 변연골 흡수량 평가)

  • Purpose: The aim of this study was to evaluate the clinical value of Osstem$^{(R)}$ USII plus system implants. Clinical and radiographic data were analyzed for 88 implants placed and functionally loaded for a 12 month period at the Yonsei University Dental Hospital. Materials and Method: Based on the patient's medical records, clinical factors and their effects on implant marginal bone resorption, distribution and survival rate were analyzed. The marginal bone loss was evaluated at implant placement and during a 6 to 12 months functional loading period. The independent sample t-test was used to evaluate the interrelationship between the factors (${\alpha}$=0.05), and one way repeated measures ANOVA was used to compare the amount of marginal bone resorption. Results: The cumulative survival rate for 88 implants was 100%. The marginal bone resorption from implant placement to prosthetic delivery was 0.24 mm and the average marginal bone resorption from prosthetic delivery to 12 months of functional loading was 0.19 mm. The total average bone resorption from implant placement to 12 months of functional loading was 0.43 mm. There were no statistically differences in the amount of marginal bone resorption when implants were placed in the maxilla or the mandible (P>.05), however, implants placed in the posterior areas showed significantly more marginal bone loss than those placed in the anterior areas (P<.05). Conclusion: Based on these results, the short term clinical success rate of RBM surface treated external connection domestic implants showed satisfactory results and the marginal bone loss was in accord with the success criteria of dental implants.

  • 치과용 임플란트의 세라믹 지대주를 위해 개발된 지르코니아 함유 세라믹 시편의 생체 적합성을 평가하기 위하여 시험관내 세포 독성을 검사를 시행하였다. L929 섬유모세포를 $37^{\circ}C$, 90% 습도, 5% CO2 및 95% 공기의 조건을 유지하는 세포 배양기에서 배양하여 실험에 사용하였다. 배양 2일, 4일, 6일 마다 시편을 넣은 배양 접시 내의 전체 세포 수와 생존 세포 수를 세어 세포 증식과 세포 생존율 검사를 시행하였다. millipore filter test를 이용하여 succinate dehydrogenase 효소 활성을 검사하였으며, 세포막 투과성의 변화를 관찰하기 위해 agar overlay test를 시행하였다. 음성 대조군은 시편을 사용하지 않았으며, 양성 대조군은 시편과 같은 크기의 구리를 사용하여, 다음과 같은 결과를 얻었다. 1. 세포 증식과 세포 생존율 검사에서는 지르코니아 함유 세라믹을 넣은 실험군과 음성 대조군 모두에서 시간이 경과함에 따라 세포가 증식하는 양상을 보였다. 세포 생존율 검사에서도 실험군과 음성 대조군이 유사한 결과를 나타내었다. 2. millipore filter test에서는 실험 시편 모두에서 염색 정도의 변화가 없이 음성 대조군과 동일한 결과를 나타냈다. 반면에 구리 시편을 넣은 양성 대조군에서는 중등도의 세포 독성을 나타냈다. 3. agar overlay test에서도 시편을 넣지 않은 음성 대조군에서는 세포 성장에 변화가 나타나지 않았으며, 실험군에서도 시편 주위로 탈색이 관찰되지 않아서 음성대조군과 같은 결과를 나타냈다. 양성 대조군에서는 심한 세포 독성을 나타내었다. 4. 실험결과, 치과용 임플란트의 세라믹 지대주를 위해 개발된 지르코니아 함유 세라믹 시편은 시험관내 세포 독성을 나타내지 않았다.

A Clinical Study on the Relationship between Dental Implant and Systemic Disease (치과 임플란트와 전신질환과의 연관성에 관한 임상적 고찰)

  • Background: In order to emphasize the importance of clinical dental hygienists-led dental hygiene management processes for those with systemic diseases, we tried to identify systemic diseases affecting dental implants based on clinical data. Methods: In order to identify systemic diseases affecting dental implants, literature review was conducted from March 1 to May 31, 2023, and the search period was for research papers published in domestic and foreign academic journals from January 2000 to December 2020. Domestic databases used for search use RISS, Nuri Media(DBpia), and Korea Academic Information (http://www.papersearch.net ; KISS), while overseas databases searched Pubmed for dental implant failures, implants, systemic diseases, and Dental implant and system disease. Results: The cumulative survival rate of implants averaged 94.3 percent and the failure rate was 5.7 percent. Clinical analysis of systemic diseases related to implants accounted for the highest frequency with 13 (100.0%), followed by 8 (61.5%) studies on high blood pressure and smoking, 7 (53.8%) cardiovascular diseases, and 5 (38.5%). In addition, liver disease, thyroid abnormalities, blood abnormalities, organ transplants and infectious diseases were confirmed. Conclusion: Since unregulated systemic diseases are a risk factor for implant failure, clinical dental hygienists should continue to maintain healthy oral conditions by sharing information with patients during periodic preventive dental hygiene management processes such as dental hygiene assessment.

Clinical Evaluation of Implant-Supported Fixed Prostheses (임플란트-지지 고정성 보철물의 임상적 상태에 대한 평가)

  • This study was to compare the cumulative survival rate of implant-supported fixed prostheses and to analyze association between risk factors and cumulative survival rate of implant-supported fixed prostheses. In order to assess the clinical status of implant-supported fixed prostheses, individuals who treated in the Department of Prosthodontics, Pusan National University Dental Hospital, between 2000 to 2007 were examined. The results of this study were as follows: 1. Length of service of implant-supported fixed prostheses was $6.6{\pm}32.0$ years (mean), 11.7 years (median). 2. Age and sex of patient was found to have no statistically significant influence on longevity of implant-supported fixed prostheses (P>.05). 3. Reason of tooth extraction wax found to have statistically significant influence on implant-supported fixed prostheses (P<.05). The longevity of fixed prostheses was low in tooth extraction case due to periodontal disease (median:9.0 years). 4. Location of implant-supported fixed prostheses was found to have statistically significant influence on longevity of fixed prostheses (P<.05). The longevity of fixed prostheses was low in molar region (median:8.8 years). 5. Number of units in implant-supported fixed prostheses was found to have no statistically significant influence on longevity of fixed prostheses (P>.05). 6. Condition of opposing dentition was found to have no statistically significant influence on longevity of implant-supported fixed prostheses (P>.05). 7. Food impaction (40.5%), porcelain fracture (25.8%), screw loosening (23.6%) were frequent complications.

Clinical evaluation of retained preload and cement washout in screw- and cement-retained implant prosthesis (나사 시멘트 유지형 임플란트 보철물의 잔여 전부하 및 시멘트 파손에 대한 임상평가)

  • Purpose: The aim of this study was to evaluate the clinical performance of screw- and cement-retained implant prosthesis (SCP) design in terms of retained preload of abutment screws and cement washout. Materials and methods: Patients with the partial posterior edentulous areas comprised the study group. Implants were placed, and SCPs were delivered after 3 to 6 months healing. Follow-up examinations were performed. The implant survival rate and the prosthetic success rate were evaluated. The retained preload ratio of abutment screws and the prosthetic decementation ratio were measured. Results: Twenty one SCPs (forty three implants)in twenty patients were followed up to 64 months. All of the implants survived during the follow-up period (mean follow-up: 34 months). The prosthetic success rate was 100 % considering no abutment, screw, porcelain or metal frame fractures, as well as no screw loosening. The retained preload ratio of SCPs at the end of follow-up period was 97.61% (${\pm}16.29$) and the decementation ratio was 9.5 %. Conclusion: Within the limitations of this clinical study, SCP design showed favorable short-term clinical performances in respect of screw loosening and cement washout.

Retrospective Study of OSFE and Simultaneous Sintered Porous Surface Implant Placement (OSFE법을 이용하여 식립된 Sintered Porous Surface 임플란트에 대한 후향적 연구)

  • Various techniques and diversely designed implants have been developed to overcome anatomic limitations of the maxillary posterior alveolar bone. The OSFE (osteotome sinus floor elevation) technique has been used for maxillary sinus augmentation. Also, $Endopore^{(R)}$ implant was designed to increase the surface area by its sintered porous surface. The purpose of this study was to evaluate the survival rate of $Endopore^{(R)}$ implants placed in the posterior maxilla in association with the elevation of the sinus membrane using OSFE technique, and examine the new bone formation in the sinus. One hundred fifteen $Endopore^{(R)}$ implants in 66 patients were placed in the posterior maxilla by OSFE technique. The implants were clinically and radiographically followed up for an average of 26.3 months. Most implants were stable and radiographs showed that the bone regenerated in contact with the implants. But, 5 implants in 4 patients were removed after the prosthetic restoration and the survival rate was 95.6% during the follow-up period. The height of new bone formed in the sinus was $3.26{\pm}1.04mm$.

Korea Academy of Prosthodontics criteria for longevity studies of dental prostheses (보철물 수명 연구를 위한 대한치과보철학회 표준 방안: KAP Criteria)

  • Purpose: The most important factor in longevity studies of dental prostheses is objective and consistent evaluation of the prosthesis. The Korean Academy of Prosthodontics suggested developing a standardized method for longevity studies of dental prostheses. The purpose of this study is to evaluate previously-used criteria and to develop new criteria, in the form of a procedure flowchart and an evaluation sheet. These new criteria may be able to provide a unified standard for future longevity studies of dental prostheses. Materials and methods: A literature review was performed about the evaluation of dental prostheses. Taking into account the strengths and weaknesses of previously used criteria, a novel, intuitive and objective method was developed for assessment of dental prostheses. Then, a pilot survey was performed with the newly developed flowchart and evaluation sheet to determine problems and implement possible improvements. Results: Thirty cases of fixed dental prosthesis (FDP), 25 cases of removable dental prosthesis (RDP), and 13 cases of implant supported prosthesis (ISP) were evaluated. The average life expectancy estimate was 12.82 years for FDP, 5.96 years for RDP, and 4.82 years for ISP with Kaplan-Meier survival analysis. Additionally, possible improvements discovered by the pilot survey were reflected in the flowchart and evaluation sheet. Conclusion: The newly developed KAP criteria, flowchart and evaluation sheet enabled objective and consistent results in trial longevity studies of dental prostheses. It is expected that future studies will not only use the KAP criteria but also further improvement will be made on them.

Comparison of implant survival rate by sinus lifting technique in implantation with sinus lift (상악동 거상술을 동반한 임플란트 식립 시에 거상술 방법에 따른 임플란트 생존율의 비교)

  • Purpose: The purpose of this study was comparison of survival rate of implant as two sinus elevation techniques and when window opening procedure had done it was comparison of survival rate of implant between the procedure of implantation after 6 month of sinus elevation to allow healing period and that of implantation with sinus elevation simultaneously. Material and Methods: All 79 of patients treated at DanKook University Dental Hospital Dept. of Periodontics for 164 implantation of maxillary posterior edentulous area with sinus elevation. Sinus elevation technique was divided of the technique : Osteotome technique and window opening technique. Result: 14 implants among 96 implants was failed implants of using osteotome for sinus elevation whereas one implant of 68 implants was failed of using window opening technique. Conclusion: Window opening technique had higher survival rate than osteotome technique. In case of window opening, there was no significant difference of immediate implantation and delayed implantation.

A prospective clinical of lithium disilicate pressed zirconia and monolithic zirconia in posterior implant-supported prostheses: A 24-month follow-up (리튬 디실리케이트-지르코니아 이중도재관과 단일구조 지르코니아로 제작된 구치부 고정성 임플란트 지지 보철물의 전향적 임상연구: 24개월 추적관찰)

  • Purpose: The purpose of this study was to compare the clinical outcomes of lithium disilicate ceramic pressed zirconia prostheses and monolithic zirconia prostheses and to investigate the complications after two years of follow-up in posterior edentulous site. Materials and methods: A total 17 patients (male: 12, female: 5) were treated with 60 posterior fixed implant-supported prostheses (LP. lithium disilicate ceramic pressed zirconia prostheses: n = 30, MZ. monolithic zirconia prostheses: n = 30). After 24-month, clinical examination of Implant survival rate, marginal bone resorption, probing depth, plaque index, bleeding index, calculus and complications were evaluated. Results: There were no failed implants and all implants were normal in function without mobility. Marginal bone resorption was lower in LP group than MZ group at 12-month (P < .05), and 12-month probing depth and calculus deposit in LP group were significantly higher than MZ group (P < .05). Most common complications in MZ were marginal bone resorptions more than 1.5.mm and 2 chipping occurred in LP group. Conclusion: Within the limitations of the present study, lithium disilicate ceramic pressed zirconia is considered as a predictable treatment option as much as monolithic zirconia in posterior fixed implant-supported prostheses.
