  • The purpose of this study was to develop a group validation therapy(V/T) which could be implemented for the elderly with dementia in nursing home, and to evaluate the effectiveness of the program on cognition, ADL(Activity of Daily Living), depression, problematic behavior and QOL(Quality of Life). Subjects were recruited from 4 nursing homes in D city. The sample comprised forty elderly with dementia, capable of verbal communication. Each twenty were in an experimental and control groups. However, four elders with dementia dropped out in experimental and control groups due to personal affairs. Experimental group completed twelve consecutive group validation therapy sessions that combined centering, asking factual questions, rephrasing, identifying and using the preferred sense, asking the extreme, imagining the opposite, reminiscing, touching, maintaining eye contact and a caring tone of voice, observing, matching and expressing the emotion with emotion, using ambiguity, linking behavior with a basic human need, using music and mirroring techniques. Following the intervention, experimental group experienced a significant improvement in cognition, ADL, depression, and QOL. But it is a nonsignificant in problematic behavior on statistically. As a result, a group validation therapy should be applied as an effective and practical psychosocial intervention for the elderly with dementia.

  • The purpose of this study was to investigate the caregiving stress and depression of spouses who support adults with physical disabilities, calculate the activity of daily living (ADL) of the disabled and determine the correlation of each area and the influence of their ADL on the caregiving stress and depression of spouses. This study was performed on 86 spouses of the disabled who were receiving rehabilitation treatment in two hospitals located in S and K cities from July to August 2016. According to the results, the total caregiving stress scores were $71.43{\pm}17.78$ and the economic and psychological stresses among low items were higher than the other items. The total depression score was $50.34{\pm}26.41$, which was slightly higher than the average of normal adults. The caregiving stress and depression were high for aged women, for cases of high caregiving expense and time, for young disabled and for stroke disease. Factors that affected the caregiving stress were the age of the disabled and the duration of their disability; their ADL had no influence on the caregiving stress. The result showed that the depression affected their age, disabled type and grade, and ADL. Actual policy support capable of giving economic, physical, and psychological assistance should be prepared to reduce caregiving stress and depression.

  • The aim of this study was to examine empirically the causal relationship between significant adverse outcomes of elderly frailty. The 2017 National Survey on Old Adults was used to test this, but only nine variables necessary for the hypothesis test were abstracted from it. The results of the hypothesis test are summarized as follows. The sub-variables of adverse outcomes, such as disabilities in daily living activities, disabilities in instrumental daily living activities, and health care utilization, were analyzed empirically by inputting sub-variables of six frailty factors into hierarchical regression analysis. In the final model, the physical activities, emotional control, and social support were verified as significant variables influencing the disabilities in daily living activities. These included the following: physical activities, sensory functions, cognition, emotional control and social support as those influencing disabilities in the instrumental daily living activities; and nutrition, physical activities, sensory functions, emotional control, and social support as those influencing the health care utilization. Based on the results of the hypothesis test, it is necessary to develop and implement personalized health management programs that specifically identify elderly weakness factors that affect adverse outcomes, such as disabilities or health care utilization.

  • This study examined the effects of a computer-assisted cognitive rehabilitation program on the depression, cognitive functions, and daily functions of stroke patients. Forty-four stroke patients were divided into the treatment group and control group, consisting of 22 patients each. The patients in the treatment group were treated with a computer-assisted cognitive rehabilitation program for 6 weeks, 5 times a week, for a total of 30 times. Depression, cognitive functions, and daily functions was gauged by the BDI, NSCE, and k-MBI, respectively. The treatment group showed decreases in depression, and increases in both cognitive functions and daily functions by the end of the rehabilitation program. Compared to the treatment group, the control group did not show significant improvements in depression, cognitive functions, and daily functions. Therefore, these findings indicate that a computer-assisted cognitive rehabilitation program has a positive effect on depression, cognitive functions, and daily functions in stroke patients.

  • Objective : Questionnaires completed by occupational therapists and patients in institutions conducting a "Pilot Project of Rehabilitation Medical Institutions" were used to determine whether occupational therapy was able to establish appropriate patient support systems for return to home and community. Methods : The questionnaire was completed by 293 occupational therapists from 13 institutions conducting pilot projects and 296 patients from 8 institutions who were able to respond. Results : Although the Pilot Project of Rehabilitation Medical Institutions is being implemented to enable early return of patients to activities of daily living, the most important therapeutic goal is not efficiently applied. In addition, since the health insurance systems utilized in medical institutions are applied as in the pilot project, there is no basis for evaluation and mediation of practical daily activities and vocational rehabilitation for actual return to the community. Conclusion : Through a detailed review of the pilot project, it will be necessary to improve the quality of life through the early return of patients to social activity, and to make practical improvements to reduce unnecessary societal expenditures.

  • This study examined the effectiveness of the cognitive activity integration program to delay the progression of the mild demented elderly. The subjects were divided into experimental group and comparative group for 12 cases of mild dementia, and the cognitive activity integration program was conducted for 8 weeks and compared with the MMSE-K, ADL & IADL, CDR and GDS scales. The results show that the effectiveness and continuity of the program are as follows. First, as an implication of practical dimension, integrated program of fusion and complexity is more effective than cognitive activity program of single domain. Second, the cognitive activity integration program must be continuously provided to maintain its effectiveness. Third, the cognitive activity integration program is as important as the practice process. It is necessary to develop and disseminate a cognitive activity integration program that reflects fusion and complex treatment techniques applicable to the elderly with mild dementia by practical suggestion. Policy suggestions are needed to continue the ongoing cognitive activity integration program, to maintain effectiveness, and to delay the progress of dementia.

  • Objective: The purpose of this study was conducted to systematic review about assessment tools for Activities of Daily Living (ADL) for stroke patients. Methods: Studies tools was administered by using four electronic databases (Pubmed, Embase, Cochrane and NDSL). For the main key words,"stroke AND activities of daily living AND occupational therapy AND assessment OR evaluation OR measurement"was used. We analyzed the types and frequency of evaluation tools. In addition, the evaluation tools for activities and participation were classified based on the classification criteria of International Classification of Functioning, Disability and Health (ICF). Results: In this study, 111 studies were analyzed and 30 assessment tools were identified. As the number of studies on stroke patients has been increased recently, the types and frequency of evaluation tools have been also increased. The most commonly used evaluation tools were Functional Independence Measure (FIM), Barthel Index (BI) and Canadian Occupational Performance Measure (COPM). In addition, according to classification based on ICF, we found that the types of assessment tools which assess participation were few. Conclusion: Although there were many kinds of assessment tools, the types of evaluation tools which were used in the research or field were very limited. Using various assessment tools, more research should be conducted to support evidence-based occupational therapy. Evaluation tools for participation also should be developed.
