서 론 : 두경부암의 예후에 가장 중요한 인자는 진단시 병기와 치료 후 재발의 유무이다. 따라서, 재발 병변을 조기에 발견하는 것이 매우 중요하다. 본 연구에서는 두경부암 환자의 추적 관찰에서 $^{18}F$-FDG PET/CT의 유용성에 대해 알아보고자 하였다. 대상 및 방법 : 2004년 4월부터 2008년 8월까지 고려대 안암병원에서 두경부암으로 치료를 완료한 후 추적관찰이 가능하였던 88 예의 환자를 대상으로 하였다. 환자는 본원의 두경부암 추적관찰 양식에 따라 술 후 6, 12개월째 $^{18}F$-FDG PET/CT를 시행하였으며, 원발부위 및 경부 재발, 원격전이와 중복암의 진단에 있어 $^{18}F$-FDG PET/CT의 민감도, 특이도, 양성예측율, 음성예측율을 평가하였고, 기존의 추적관찰 방법과 이를 비교하였다. 결 과 : 원발부위 재발의 진단에 있어 민감도, 특이도, 양성예측율, 음성예측율은 각각 95%, 97%, 90%, 98% 였다. 경부전이 재발의 진단에 있어서는 각각 94%, 98%, 83%, 99%였으며, 원격전이 및 이차암 진단에 있어서는 94%, 100%, 100%, 99%였다. 원발부위 재발은 고식적인 진단 방법, 원격전이와 이차암은 $^{18}F$-FDG PET/CT가 진단에 의미있게 유리하였다. 결 론 : $^{18}F$-FDG PET/CT는 두경부암의 추적관찰 과정에서 재발 여부를 판정하는데 있어 매우 유용하며, 특히 원격전이 및 이차암의 발견에 있어 유용하다. 그러나, 위양성 및 위음성의 가능성으로 고식적인 진단 방법과 병행해야 진단의 효과를 높힐 것으로 생각된다.
Radiotherapy and chemotherapy can effectively control cancer but can also cause new second cancers to develop as long-term complications especially in childhood cancer. We experienced two patients with second malignant solid neoplasm who had been treated with radiation and chemotherapy for childhood cancers One female patient with rhabdomyosarcoma of the right popliteal fossa was treated with radiotherapy at total dose of 54Gy Three years and seven months later, osteosarcoma developed in the field of radiation therapy The other male patient with non-Hodgkin's lymphoma of the small bowel was treated with radiotherapy and leiomyosarcoma developed in the field of radiotherapy 18 years later. We reviewed the literature of the second malignant neoplasm in children in respect of risk factors. The risk for a second primary cancer following radiotherapy or chemotherapy emphasizes the need for life long follow-up of patients receiving such treatments, particularly patients treated for childhood cancers.
Purpose: This study was conducted to identify the factors influencing second primary cancer (SPC) screening practice by examining the relationships of physical symptoms, knowledge and attitudes regarding SPC screening, perceived risk, primary cancer type, and demographic factors of cancer survivors. Methods: Participants were 308 survivors of stomach, colon, or breast cancer recruited from 2 university hospitals in Korea. Data were collected using a questionnaire and analyzed using IBM SPSS 21.0 and AMOS 18.0. Results: The proportion of participants taking all cancer screenings according to national guidelines was 40%. They had moderate knowledge and a relatively positive attitude regarding SPC screening and high cancer risk perception. The participants had taken fewer SPC screenings after than before cancer diagnosis. The factors influencing cancer risk perception were age, physical symptoms, knowledge regarding SPC and primary cancer type (stomach). The factors influencing SPC screening practice were age, gender, economic status, knowledge regarding SPC screening, and primary cancer types (colon). Conclusion: It is important for clinical professionals to recognize that survivors of cancer are susceptible to another cancer. Education on SPC screening for these survivors should focus on communicating with and encouraging them to have regular cancer screenings.
The clinical staging system for laryngeal cancers is not sufficient for prognosticator due to different biologic characteristics and their microenvironment according to primary sites. For determining the prognosticators, the authors peformed immunohistochemical staining to EGFR, p53 protein, and pRB in 40 cases of surgically treated squamous cell carcinomas of larynx in our institute during the past 5 years. The results are as followings; 1. The positive expression rate of p53 protein and negative expression rate of pRB showed correlations with clinical parameters. 2. The three-year survival rate for p53 protein positive cases was worse than the p53 protein negative cases. 3. Expression rate of EGFR was not correlated with the clinical parameters. As a conclusion, expression rates of p53 protein and pRB not only reflect well the biologic behavior of laryngeal cancer, but correlate closely with the tumor factors. Therefore they may be useful as the prognosticator to predict the malignant potency of laryngeal squamous cell carcinomas.

With advances in health care, the cancer survival rate has significantly increased. Even though cancer treatment has finished, cancer survivors may continuously suffer from physical and psychosocial problems. In order to investigate the problems cancer survivors have experienced, a comprehensive rehabilitation program must be developed. This study sought to determine the content and structure of rehabilitation programs for cancer survivors, analyze the effectiveness of such programs, and develop a basis for future comprehensive rehabilitation program development. Through this review, thirteen programs were selected and classified according to exercise, psychosocial, and integrated or comprehensive aspects. Exercise programs improve physical function, fatigue, and hormone-related cancer effects. Psychosocial programs improve quality of life, mood changes such as depression, anxiety, and post traumatic factors. Comprehensive programs improve both physical and psychosocial aspects.
Purpose: With the development of diagnostic techniques, second primary neoplasms such as synchronous or metachronous cancers in gastric cancer patients are being increasingly found. In this study, we investigated the clinicopathological features and clinical significance of gastric neoplasms combined with synchronous and metachronous cancers. Materials and Methods: 1,048 patients who were diagnosed with gastric cancer in Chosun University Hospital from January 1998 to March 2008 were retrospectively reviewed. Results: 38 of the 1,048 patients with gastric cancer (3.6%) had synchronous and metachronous cancers. Of the 38 patients, 16 patients (42.1%) had synchronous cancer and 22 patients (57.9%) had metachronous cancer. The average time interval between gastric cancer and the secondary primary cancer was $27.08{\pm}31.25$ months. The most common second primary neoplasm was lung cancer (8/38, 21.1%), followed by colorectal cancer (8/38, 21.1%). Among the 27 patients who underwent surgical resection for gastric cancer, 5 patients (18.5%) were in the synchronous group and 22 patients (81.5%) were in the metachronous group. The mean survival time of the 38 patients was 49.8 months. The mean survival time was 24.6 months for the synchronous cancer patients and 68.1 month for the metachronous cancer patients. The 3 year survival rate of the synchronous group and the metachronous group was 33.3% and 81.1%, respectively. Conclusion: We must pay attention on the preoperative workup for synchronous cancer and on the postoperative follow-up for metachronous cancer in gastric cancer patients.

Pancreatic cancer is a highly aggressive cancer with poor prognosis. Although, gemcitabine is the current standard regimen as first-line chemotherapy for advanced pancreatic cancer, effective regimens of second-line chemotherapy after gemcitabine failure have not been established yet. We report a case of gemcitabine refractory pancreatic cancer treated with second-line chemotherapy with FOLFIRINOX regimen. A 57-year-old-woman visited our hospital for pancreatic body mass detected by computed tomography (CT). The patient underwent distal pancreatectomy and splenectomy and the pathologic results after surgery demonstrated adenocarcinoma. Follow-up was performed after surgery and CT and positron emission tomography (PET) 4 months after surgery revealed multiple hepatic metastases. The patient underwent first-line chemotherapy with gemcitabine and erlotinib for recurred pancreatic cancer. However, CT after 7 cycles of the chemotherapy showed the progression of multiple hepatic metastases and switch to second-line chemotherapy with FOLFIRINOX was initiated. CT after 16 cycles of the FOLFIRINOX showed the complete remission of multiple hepatic metastases. The patient was admitted for infective endocarditis with septic pneumonia 17 months after the initiation of FOLFIRINOX. However, the patients died from the progression of septic embolism and acute respiratory distress syndrome.

In proton therapy, the analysis of secondary particles is important due to delivered dose outside the target volume and thus increased potential risk for the development of secondary cancer. The purpose of this study is to analyze the influence of secondary particles from proton beams on fluence and energy deposition in the presence of inhomogeneous material by using Geant4 simulation toolkit. The inhomogeneity was modeled with the condition that the adipose tissue, bone and lung equivalent slab with thickness of 2 cm were inserted at 30% (Plateau region) and 80% (Bragg peak region) dose points of maximum dose in Bragg curve. The energy of proton was varied with 100, 130, 160 and 190 MeV for energy dependency. The results for secondary particles were presented for the fluence and deposited energy of secondary particles at inhomogeneous condition. Our study demonstrates that the fluence of secondary particles is neither influenced insertion of inhomogeneties nor the energy of initial proton, while there is a little effect by material density. The deposited energy of secondary particles has a difference in the position placed inhomogeneous materials. In the Plateau region, deposited energy of secondary particles mostly depends on the density of inserted materials. Deposited energy in the Bragg region, in otherwise, is influenced by both density of inserted material and initial energy of proton beams. Our results suggest a possibility of prediction about the distribution of secondary particles within complex heterogeneity.
