Exploring Job Stress, Job Satisfaction, and Turnover Intention of Nurses in the Comprehensive Nursing Service (상급종합병원의 간호·간병통합서비스 병동에 근무하는 간호사의 직무스트레스, 직무만족 및 이직의도)

  • The purpose of this study was to examine job stress, job satisfaction, and turnover intention of the nurses who work at the comprehensive nursing service units in two academic medical institutions that provide healthcare service at the tertiary care level in Korea. A descriptive study was conducted with the nurses by using self-reported questionnaires. We used descriptive statistics to summarize the data and Mann-Whitney U test for group comparison. The mean scores of job stress, job satisfaction, and turnover intention were $89.17{\pm}12.56$, $107.25{\pm}6.85$, $37.19{\pm}4.86$, respectively. Nurses with religion showed more job stress than those without. Nurses with their age below 30 years old had significantly less job satisfaction than those older than 30 years old. Monthly income was significantly associated with turnover intention in our data. The top three sub-category of the job stress were conflict with physicians, patients and their caregivers, and conflicts with others; and those of the job satisfaction were pay and promotion and task requirement. Job stress, job satisfaction, and turnover intention were significantly correlated. In order to reduce turnover intention of nursing professionals in the comprehensive nursing service, job stress related to issues on relationships with patients, their family, health care providers need to be taken into consideration. Policy and administrative support are needed to improve their work environment.

Analyzing Vocabulary Characteristics of Colloquial Style Corpus and Automatic Construction of Sentiment Lexicon (구어체 말뭉치의 어휘 사용 특징 분석 및 감정 어휘 사전의 자동 구축)

  • In a mobile environment, communication takes place via SMS text messages. Vocabularies used in SMS texts can be expected to use vocabularies of different classes from those used in general Korean literary style sentence. For example, in the case of a typical literary style, the sentence is correctly initiated or terminated and the sentence is well constructed, while SMS text corpus often replaces the component with an omission and a brief representation. To analyze these vocabulary usage characteristics, the existing colloquial style corpus and the literary style corpus are used. The experiment compares and analyzes the vocabulary use characteristics of the colloquial corpus SMS text corpus and the Naver Sentiment Movie Corpus, and the written Korean written corpus. For the comparison and analysis of vocabulary for each corpus, the part of speech tag adjective (VA) was used as a standard, and a distinctive collexeme analysis method was used to measure collostructural strength. As a result, it was confirmed that adjectives related to emotional expression such as'good-','sorry-', and'joy-' were preferred in the SMS text corpus, while adjectives related to evaluation expressions were preferred in the Naver Sentiment Movie Corpus. The word embedding was used to automatically construct a sentiment lexicon based on the extracted adjectives with high collostructural strength, and a total of 343,603 sentiment representations were automatically built.

A Study on the Current State of Pediatric Dentists and the Adequacy of Supply and Demand Based on Covered Services (소아치과 전문의 인력 현황 및 공급 적정성에 관한 연구 - 급여 진료 항목을 기준으로)

  • The aim of this study was to identify the current state of pediatric dentists, evaluate the adequacy of pediatric dentist supply and demand, and find out the perception of all pediatric dentists on the current state of pediatric dentists and policy establishment. An Online survey was conducted among pediatric dentists. The questionnaire was subdivided into 'general characteristics', 'number of dental treatments and working days per year', 'proportion of covered services', 'perceptions of supply and demand of pediatric dentists'. Through the Korean Academy of Pediatric Dentistry, the Health Insurance Review and Assessment Services, the National Health Insurance Service (NHIS), and the Korean Statistical Information Service, the current state of pediatric dentists, the number of claims for covered services, and the decrease in births per year were investigated. Dental clinics claiming to be pediatric dentistry reached half of all medical institutions, but only 3.78% of pediatric dentists actually worked. 61.36% of all pediatric dentists were concentrated in the metropolitan area, showing a national imbalance. Although the population of children and adolescents have continuously decreased over the past 20 years, the number of NHIS-covered services has shown a continuous increase. Over the past 10 years, the optimal supply of pediatric dentists has been maintained at around 4,000. According to the analysis, 92.15% of pediatric dentists thought that it was necessary to prepare policies and support measures at the government level. This study is expected to be used as basic data for establishing a demand estimation method for pediatric dentistry specialists in the future.

Differences of EEG and Sleep Structure in Pediatric Sleep Apnea and Controls (소아 수면무호흡증 환아와 정상 소아에서 수면구조와 뇌파 양상 차이)

  • Introduction: In this study, we compared sleep structure, EEG characteristic of pediatric obstructive sleep apnea (OSA) and normal controls which were matched in sex and age. Methods: Fifteen children (male:female=4:11) who complained snoring and were suspected to have sleep apnea and their age and sex matched normal controls (male:female=5:10) have been done nocturnal polysomnography (NPSG). Sleep parameters, sleep apnea variables and relative spectral components of EEG from NPSG have been compared between both groups. Results: Pediatric OSA group were distinguished from normal controls in terms of apnea index, respiratory disturbance index and nadir of oxyhemoglobulin desaturation. Pediatric OSA group showed increased percent of sleep stage 1, decreased rapid eye movement sleep percent and increased delta power in O1 EEG channel. However other sleep parameters and spectral powers were not different between two groups. Conclusion: In pediatric OSA group, sleep structure parameter disruption may be not prominent as the previous studies for adult OSA group because of including mild OSA data in diagnostic criteria. In addition, EEG changes might not be distinct due to low arousal index compared to adult OSA patients. We can observe general characteristics and particularity of pediatric OSA through this study.

  • A shopping mall sie is the area which males use of the Internee activity as a medium in business transactions, But their growth rate of revenues does not keep up with the growth rate in the number of internet users. One of the reasons why they fall behind of the expected earning proportionate to the growing number of internet user is that the shopping sites do not satisfy various need of customers on the web. This comes from that there has been not enough efforts on a shopping site design satisfying their needs. This paper is to find the elements of the shopping site design to affect the my customers purchase goods on the internet on the premise that a design being closely related with a marketing strategy makes a good design. This paper consists of three parts. The first chapter is about related works inducing e-commerce, marketing as wet as design theory. In the second chapter, data are analyzed to find the elements of a good design. The data are categorized by criteria of general properties, elements in design theory, demographic characteristics and hypothesis tests are done. The third chapter is the conclusion.

  • As medical practices and procedures become more specialized and information technology develops in clinical settings, health organizations need medical personnel with special skills, knowledge and competency. But the lack of practical experience in clinical settings may impair competency in basic nursing skills among nursing students. This study used a cross-sectional survey to analyze factors related to nursing competency among nursing students in order to establish effective teaching methods to increase the clinical competency of nursing students. The data were collected from a questionnaire distributed in several universities among 106 nursing students who expressed a willingness to participate in the study during December 2011 in order to measure self-directedness, professional self-concepts, communication ability, learning satisfaction. The data were analyzed using the SPSS window program. There were differences regarding competency in basic nursing skills according to interpersonal relationships, grades, existence of an open lab. The level of learning satisfaction, communication skills and self-directedness were deemed influencing factors regarding competency in basic nursing skills. These 3 elements account for 49.9% of competency with regard to basic nursing skills. According to existing research, blended learning methods which consist of problem based learning, cyber education or case centered education should be considered as effective teaching methods for developing clinical nursing skills.

Effects of Foot Massage and Supportive Communication by Hospice Volunteers on Depression, Anxiety, and Mood of Cancer Patients Who Undergo Intravenous Chemotherapy at Out-patient Department (호스피스 자원봉사자의 발마사지와 지지적 의사소통이 외래 항암화학요법 환자의 우울, 불안 및 기분에 미치는 효과)

  • Purpose: This study was done to investigate effects of foot massage and supportive communication provided by hospice volunteers on depression, anxiety, and mood of cancer patients who are going under intravenous chemotherapy at out patient department (OPD). Methods: A non-equivalent control group non-synchronized design was used. Sixty-four participants were recruited from out-patients setting in a university hospital by a convenient sampling. Two trained- hospice volunteers first provided regular hospice volunteer service to 34 patients in the control group. Subsequently, foot massage and supportive communication were provided to 30 participants in the experimental group. After the intervention, all participants answered the structured questionnaire with visual analog scales on depression, anxiety, and mood including general characteristics. Data were analyzed using descriptive statistics, two sample t-test, and paired t-test by PASW 17. Results: After the intervention, changes in negative moods, i.e. strain and powerlessness in the experimental group were significantly different from those in control group; strain and powerlessness in the experimental group were significantly decreased by foot massage and supportive communication provided by hospice volunteers. Conclusion: The current findings provide an empirical evidence of a brief and efficacious intervention for cancer patients. Further studies are warranted with larger sample size to validate the effectiveness of foot massage and supportive communication as a solid intervention for cancer patients who are undergoing intravenous chemotherapy at OPD.

보건관리대행기관 산업간호사의 업무수행과 직무만족도에 관한 연구

  • 본 연구는 보건관리대행기관에 근무하는 산업간호사들의 보건관리업무실태 및 근무만족도와 이들간의 관계를 파악함으로써, 산업간호업무향상 및 효율적인 산업보건관리를 위한 기초자료로 제시하고자 시도되었다. 조사대상은 38개 보건관리대행기관중 24개 기관에 근무하는 산업간호사 118명중 97명을 대상으로 하였다. 연구기간은 '92년 5월부터 6월까지 설문조사를 실시하였으며 그중 89명이 설문에 응답하여 이를 분석 대상으로 하였다. 연구결과는 다음과 같다. 첫째, 보건관리대행기관의 간호사는 산업간호사의 일반적 특성과 비교해볼 때 기존 산업간호사와 유사하였다. 그러나 법정휴가를 받는다는 간호사가 59.1%로 더 낮았다. 둘째, 보건관리대행기관 산업간호사의 담당사업장수는 법적으로 30개 사업장, 2,000명의 근로자를 담당하도록 되어있는데 조사결과 법적기준인 보건관리사업장의 수를 담당하는 간호사는 57.1%를 차지하였고, 담당 근로자 수로 볼때는 2,000명이하가 29.8%를 차지하여 많은 산업간호사가 법적인 기준을 초과하여 산업보건관리를 대행하고 있는 것으로 나타났다. 셋째, 보건관리대행의 방문방법으로 교통편은 대행 기관차량을 이용하며, 간호사 혼자 산업장을 방문하는 것이 대부분으로 나타났고, 방문횟수는 100인미만이 월1회, 100인이상이 월2회로 규정된 것과 비교해보면 규정을 따르는 곳이 100인 미만의 경우 97.4%, 100인이상의 경우 86.4%였다. 또한 의사나 위생사는 방문을 안하거나 년1-5회 방문하는 것으로 나타나 보건관리대행의 주제공자는 간호사인 것으로 냐타났다. 또한 사업장 방문시 업무수행장소는 휴게실이나 사무실이 87.5%, 현장이나 수위실에서 하는 곳이 70.5%나 되어 뚜렷이 업무를 수행할 장소가 마련되지 않은 실정을 나타냈다. 네째, 보건관리대행에 근무하는 산업간호사의 애로사항은 사업장내에서는 조직과 체계상의 문제, 간호사 역할에 대한 인식부족, 권한과 자율성 부족 등을 들었고, 사업장 방문시에는 사업주와 근로자의 인식부족이 우선적으로 지적되었다. 또한 산업간호사는 보건관리 대행업무에서 사업장의 참여유도, 사업장 순회점검 및 개선지도, 보건교육, 건강상담이 우선적으로 실시되어야 한다고 하였다. 다섯째, 보건관리대행기관 산업간호사의 업무수행정도는 건강검진이나 직업병관리, 보건교육, 환경위생관리업무를 비교적 많이 실시하는 것으로 나타났다. 여섯째, 보건관리대행기관 산업간호사의 직무만족도는 전문적 위치나 상호작용, 업무요구등은 높게 나타났으나 보수와 산업보건관계자와의 관계등은 만족도가 낮게 나타났다. 또한 직무만족도에 대한 총화점수는 사업장근무자가 240점중 143.8인데 비해 보건관리대행기관 근무자는 230점중 129.61점으로 더 낮게 나타났다. 일곱째, 일반적 특성별 보건관리대행 산업간호사의 직무수행정도는 연령별 30세이상군이, 결혼상태별로 기혼군이, 경력은 1년이상인 군이, 근무시간 8시간인군이, 급여수준은 50만원이상인군이, 법정휴가가 있는 군이, 자질향상교육을 받은군이 담당사업장수가 30개이하인 군이, 더 높게 나타났다. 또한 연령군이 20-24군과 30세이상인군 (p<0.05), 결혼상태별로 기혼군과 미혼군(p<0.01), 급여수준별로 50만원미만군과 이상인 군(p<0.05), 법정휴가가 있는 군과 없는군(p<0.05), 자질향상 교육을 받은군과 받지못한 군(p<0.01), 담당사업장수가 30이하인 군과 31인이상인 군(p<0.05) 간에 유의한 차이를 보였다. 즉 산업간호사중 나이가 많고, 기혼이며 급여수준이 높고 법정휴가와 교육을 받고, 담당사업장수가 30이하인 군이 보건관리대행업무를 잘 수행하는 것으로 나타났다. 여덟째, 일반적 특성별 보건관리대행 산업간호사의 직무만족도는 연령별 30세이상군이, 결혼상태별로 기혼군이 경력은 1년이상인 군이, 근무시간 8시간인군이, 급여수준은 50만원이상군이, 법정휴가가있는군이, 자질향상교육을 받은군이, 담당사업장수가 30개이하인 군이 더 높게 나타났다. 또한 결혼상태별로 기혼군과 미혼군(p<0.01), 근무시간별로 8시간군과 9시간 이상인 군(p<0.01), 급여수준별 50만원 미만군과 이상군(p<0.05), 자질향상교육을 받은군과 받지 않은군(p<0.05) 사이에 유의한 차이를 보였다. 즉 산업간호사중 기혼이고 근무시간은 8시간이며 급여수준이 높고 자질향상교육을 받은 군이 만족도가 높은 것으로 나타났다. 아홉째, 보건관리대행기관 산업간호사의 업무수행정도와 직무만족도 사이의 상관관계를 보면 보수, 자율성, 업무요구가 높을수록 산업간호업무를 수행을 잘하는 것으로 나타났으며(상관관계 r>0.6, P>0.001), 전체적으로 총직무만족도와 업무수행정도와의 상관관계는 6346, P<0.001로서 만족도가 높을수록 업무수행정도가 높은 것으로 나타났다. 이상의 결과를 토대로하여 보면, 보건관리대행기관에 근무하는 산업간호사는 그 특성을 고려하여 업무도 사업장 근무자와 다르게 규정되어야 하며, 담당업무의 명확한 한계를 정해 중소규모사업장의 산업보건관리업무를 책임있게 수행할 수 있도록 뒷받침이 필요하다. 더불어, 직무만족도를 높일수 있도록, 산업간호사에 대한 다방면의 지원이 필요하다고 하겠다. 또한 보건관리대행기관 산업간호사는 나이가 많고 기혼이며 경력있는 간호사를 채용하고 근로조건을 잘 개선하면 직무수행정도가 더 높아질 것으로 보인다. 좋은 근로조건의 형성과 직무만족도 등을 높여 산업간호사의 직무를 효율적으로 수행할 수 있도록 하기 위하여는 보건관리대행기관에 관한 규정이 강화 보완되어야 하겠으며, 민간단체와 의료기관 뿐아니라 공공기관에서도 중소규모사업장의 보건관리업무를 담당하는 방향으로 전환하는 방안을 모색해 보아야 할 것이다.

The Demands and Awareness of Pediatric Dentists on the Korean National Health Insurance System (국민건강보험에 대한 소아치과 의사들의 요구와 인식 조사 )

  • The aim of this study is to investigate the awareness and perception of pediatric dentists regarding dental health insurance (DHI) system in Korea. Online surveys were sent to the members of the Korean Academy of Pediatric Dentistry (KAPD), and the responses were collected. The survey, consisting of 41 questions, was subdivided into 'general background of the respondents', 'the awareness on national health insurance (NHI) and DHI', 'the awareness on DHI pertaining to pediatric dentistry', 'issues that need improvement on DHI pertaining to pediatric dentistry', 'DHI claims', and 'the roles of KAPD in DHI'. In total, 302 responded, with a response rate of 28.9%. Excluding 2 questionnaires with insufficient answers, a total of 300 survey results were analyzed. According to the analysis, pediatric dentists thought that the coverage of DHI was not as sufficient compared to that of NHI, but were expecting its scope to broaden in the future. The satisfaction rate was higher in pit and fissure sealant and composite resin filling than in caries-detecting quantitative light-induced fluorescence. Pediatric dentists considered permanent tooth composite resin filling (65.7%) needed an increase in insurance copayment, while topical fluoride application (74.7%) to require insurance coverage. DHI claims were generally handled by dental hygienists and/or nursing assistants. Approximately half of the respondents answered that they have had experiences on appealing for insurance denials. Lastly, pediatric dentists generally had a positive attitude towards providing information for the DHI. This study is expected to be used as a sound dataset for the DHI policy development concerning pediatric dentistry.

Influential Job-Related Characteristics for the Job Satisfaction and Turnover Possibility of Dental Hygienists (치과위생사의 직무만족과 이직가능성에 영향을 미치는 직무관련특성 요인)

  • The purpose of this study was to examine the job-related characteristics of dental hygienists affecting their job satisfaction and turnover possibility. The subjects in this study were 297 dental hygienists who worked in dental hospitals and clinics in the metropolitan area. The dental hygienists who worked for three to less than five years, who were unmarried and who didn't benefit from any incentive systems expressed a greater satisfaction, and those who were married, whose workplaces were established jointly with other dental institutions in the form of a network and whose workplaces offered incentives were more likely to change their employment. As for influential factors for job satisfaction and turnover possibility, the respondents whose workplaces were established jointly with other dental institutions were less satisfied, and a larger number of patients and better communication led to higher satisfaction. Those who worked in general hospitals were more likely to change their employment, and better welfare benefits, more appropriate length of working hours and a larger number of patients led to lower turnover responsibility. The findings of the study ascertained that working environments were one of the factors to impact on job satisfaction and turnover possibility, and both of hospitals and dental hygienists should make an effort to improve working environments.
