  • 본 논문에서는 대규모 평시 또는 감염병 발생이전 지자체별 취약성 평가를 통해 감염병 발생 및 확산에 영향을 미치는 취약지표를 평가해보는 것이다. 이를 위해 국내 코로나19 대처사항(각종 회의자료 및 보고자료 등), 선행연구 등을 기반으로 확산취약시설, 감염취약계층, 사회적 취약여건, 대응능력(보건·의료/재정적, 사회적 대응역량) 총 4개의 증감유형으로 분류한 후 세부 평가지표로 약 24개로 세부지표를 선정하였다. 이러한 취약성 평가 결과는 지자체별 특성에 맞는 감염병 대응정책 수립에 필요한 근거자료를 마련하는 기초자료로 활용 가능할 것이다.

  • 본 논문은 보건의료서비스 공급을 민간이 주도하고 있는 현실에서 인구가 밀집한 도시 지역 병 의원의 집중과 과잉으로 농촌지역의 효율적인 의료서비스 공급이 절대적으로 부족한 현실을 고려하여 의료서비스의 질적 수준을 높일 수 있는 On-line 의료서비스 공급기반 확충과 저비용-고효율의 의료전달체계를 확립하기 위한 U-healthcare service 모델 연구와 발굴을 제안하고 IT기술을 기반으로 한 의료취약계층에 대한 건강관리 및 치료나 수술 후 질병의 예후관리에 대한 보다 효율적이고 기술 집약적인 의료서비스 제공을 통한 농촌지역 의료서비스 활성화를 제안한다.

  • XML기반 웹서비스의 활성화와 더불어 효율적인 데이터 호출 및 통합을 위해, XML 문서 안에 웹서비스 호출 노드를 내장할 수 있는 Active XML(AXML) 문서가 개발되었다. 본 논문에서는 기존 전송계층 보안 프로토콜 및 WS-Security의 제한점을 방지하기 위한, AXML 문서의 내장형 웹서비스 호출과 XML-Encryption 및 XML-Signature 보안 표준을 통합한 데이터 암호화 및 전자서명 방식에 대해 소개한다. 또한, '전자의료 시스템' 예제를 통해 AXML 문서를 이용한 중첩된 웹서비스 호출 및 쿼리위임 방식에 대해 소개한다. 마지막으로, 기존 보안성 정형분석 방법의 확장을 통한 AXML 시스템의 보안 취약 가능성에 대해 언급하고자 한다.

  • As the variety of scientific technology has grown repeatedly since the 19th century, innovative technology is developing high-level in healthcare field. The system to improve patient's satisfaction for silver generation introduced in domestic medical process result from promotion of convergence technology. But utilization of small and medium hospital is inevitable with maintain limited performance around existing large hospitals and high cost service system. Therefore phenomenon that weakness of patient's satisfaction and service accessibility for silver generation occurs. This study propose the design method that Android-based low-cost smart medical treatment information service system to improve accessibility to user of small and medium hospital for effective patient's satisfaction services management and medical services.

How Much should the Poor Pay for their Health Care Services under the National Health Insurance System? (계층간 진료비 본인부담의 형평성에 관한 연구)

  • Although National Health Insurance(NHI) in the South Korea has guaranteed access to health insurance coverage to virtually all the people, a significant portion of out-of-pocket spending can create substantial financial burdens for some beneficiaries, particularly those with low incomes. Previous studies have estimated the magnitude of out-of-pocket spending by types of chronic illness or in- and out-patients. Prior estimates, however, have not given a complete picture of the impact of health care costs on lower-income populations. The result from this study shows that 20 percent of beneficiaries in the lowest-income quintile spent more than twelve percent of their household equivalent income out-of-pocket health services, whether they were enrolled in a Health care services or not. In comparison, the beneficiaries in the highest-income quintile level spent only 2 percent of their income out-of-pocket on health care. Also, the regression analysis suggests that age, household income, number of chronic illness, type of hospital in addition to the number of usage may affect the size of out-of-pocket spending.

A qualitative study of home-visiting oral health care experience in vulnerable populations (취약계층의 재가 거주자를 대상으로 수행한 방문구강건강교육 경험에 관한 질적연구)

  • Objectives: The purpose of this study was to explore and analyze experiences with home-visiting oral health education in vulnerable populations and to provide foundational data necessary for the development of preventive strategies for oral health promotion among these patients. Methods: Using a phenomenological research, in-depth interviews were conducted between August 29 and October 31, 2023 with 20 vulnerable individuals participating in the Home Visit Oral Health Education Program under the Gwangju+ Gwangsan Integrated Care Service project spearheaded by the Gwangju Medical Welfare Cooperative. Results: After analyzing the contents of the participants' experiences, 130 meaningful words and 14 subcategories were identified and categorized into five major themes. Participants expressed discomfort when chewing or swallowing food and felt burdened by visits to the dentist. Although they experienced considerable loneliness because they spent a significant amount of time alone, they enjoyed communicating through visitation care and expressed gratitude for receiving oral health education. Conclusions: Oral health education through home visits cam increase awareness of oral health management among vulnerable populations and serve as an important means of improving their quality of life.

Exploring Social Service Demand and Policy Implications of the Vulnerable in Rural Areas (농어촌 취약계층의 사회서비스 수요탐색과 정책함의)

  • This study sought to explore the demand in the social service sector and present policy implications, focusing on the vulnerable in rural villages exposed to worse conditions amid the Covid-19 crisis. To this end, the social service needs of vulnerable groups by household type were analyzed by utilizing the raw data of the 2018 Survey on Rural Well-Being. Analysis showed that the greatest demand social services for all rural villages, the elderly, the disabled, one-person, one elderly and low-income households were income support services, while in household that included children the demand was for childcare and education-related services. The second-highest social services in terms of demand were cultural leisure vacation support services for all rural villages, healthcare-related services for the elderly, the disabled, one elderly and low-income households, cultural leisure vacation support services for households including children and daily life support services for single-person households. Based on these results, a measure was proposed to support social services, tailored to vulnerable groups in rural villages. In addition, the government's lack of a consultation system between urban and rural welfare policies, such as a basic plan for health and welfare in rural villages, led this paper to discuss the need for a feedback function and dedicated formulation of mid- to long-term policies in rural villages. It also proposed the establishment of conditions for providing customized social services for rural villages.

The Role of Occupational Therapist in Disaster Management (재난상황에서 작업치료사의 역할에 대한 고찰)

  • Objective : In October 2016, the most powerful earthquake, magnitude 5.1 and 5.8 struck the city of Gyeongju in Korea. Although it did not take a toll, this implicates potential disaster in the future. Taking this earthquake, this paper considers the healthcare system responding to disaster in non-government organization and other countries, and investigates the roles of occupational therapist in disaster management. Methods : This paper reviews literature related to healthcare system and roles of occupational therapist in disaster response. Results : Humanitarian recovery mission of Red Cross impacted and facilitated the recovery of vulnerable population including children, elderly and people with disabilities in disaster response. It was also emphasized by occurring large population with disabilities after disasters so that the concept of rehabilitation and occupational therapist's role was required. Occupational therapy practitioners play an important role in the stage of disaster preparedness, response and recovery and their target population is children, elderly and people with disabilities. Conclusion : The most of NGO and counties take the concept of rehabilitation into healthcare system responding to disaster. However, the system in Korea stays in emergency level. It is important to take the humanitarian recovery and rehabilitation concept to disaster relief. the survivors would be able to return to their normalcy and health life.

A Qualitative Research on the Evaluation of Healthcare and Welfare Network for Vulnerable Populations : Focusing on the Dalgubeol Health Doctor Services (취약계층 대상 보건의료·복지 네트워크 사업 성과에 대한 질적연구 : 달구벌건강주치의사업을 중심으로)

  • Objectives: This study examined the evaluation and potential improvements of 'Integrated Healthcare and Social Welfare service model' based on the experiences of practitioners from institutions participating in the 'Dalgubeol Health Doctor Services' and the service recipients. Methods: Qualitative research was conducted from September to November 2022 in this study, focusing on 4 providers from the dedicated Dalgubeol Health Doctor Services Team, 5 contact partners from affiliated organizations, and 6 service beneficiaries. The data gathered underwent thematic analysis. Results: The evaluation indicated that Dalgubeol Health Doctor Services has proven to be effective in addressing the complex needs of vulnerable populations. By providing integrated services through quick and simple beneficiary selection and resource linkage, it has contributed to the resolution of complex demands, recovery of positive attitudes towards life, and improvement in quality of life for users who have fear the use of medical and welfare services. Dalgubeol Health Doctor Services has established an integrated health care system involving not only public but also private organizations, from the referral agency to the service provider. Centered around Daegu Medical Center and involving five tertiary hospitals, it has established a model that supports treatment appropriate to the severity of the patient, from mild to severe. Conclusions: These findings indicate an enhancement in health equity, achieved through the active identification and subsequent health and welfare issue resolution of individuals marginalized from medical benefits.

  • 사회가 고령화, 초핵가족화 및 재난취약계층이 늘어나면서 요구호자에 대한 고품질 맞춤형서비스가 필요하게 되었고, 급성진환, 심뇌혈관 고위험 환자 및 자살 등 예방가능 사망률이 선진국에 비해 높음에 따라 신속하고 전문적인 구조 구급서비스가 요구된다. 따라서 본 논문에서는 응급환자가 발생하였을 때 병원 도착 전 환자의 정보를 이용하여 응급처치가 가능하도록 하고, 응급의료기관에서는 환자 진료준비를 사전에 할 수 갖출 수 있도록 하여 응급환자 진료의 효율성을 극대화하였다. 또한 사회적 인지도가 높은 119번호를 이용한 다양한 복합 응급신고 접수 시 유관기관과 통합적 대응체계를 구축하는 효율적 응급의료서비스 고도화 전략을 제시하고자 한다.
