  • After the introduction of ultrasound enabling a detailed view of both neonatal hip instability and morphology, two different streams have developed: one arguing that neonatal hip instability is the major pathology warranting treatment, the other including acetabular dysplasia as an important feature. Graf's method including the acetabular dysplasia led to higher treatment and follow-up rate than that based upon neonatal hip instability only. Recent report suggested that improved examination techniques and a better understanding of the findings have enabled a more tailored approach, and no differences in treatment rates exist due to different ultrasound techniques. I'd like to emphasize that enough experiences and the understanding enabling standardized examination and high repeatbility is more important than the choice of different techniques. Here the mothods and meanings of different techniques of ultrasound and its usefulness in the diagnosis and treatment of DDH is discussed.

  • Nearly 20 years after the Boramea Hospital case, the act on decisions on life-sustaining treatment for patients in hospice and palliative care or at the end of life has taken effect on February 4, 2018 as recommended by the National Bioethics Committee. However, during the legislation process, some parts of the bill that stakeholders and concerned parties did not see eye to eye were either revised or removed. Moreover, the hospice and palliative care part was added in the last minute before the enactment. As a result, the law includes parts that are not in line with the recommendations from the National Bioethics Committee, thereby causing various problems. Therefore, it is crucial to monitor how the decisions on life-sustaining treatments are made in the field and gather various opinions of concerned parties to identify and address problems in the early stage of the implementation of the law. Based on the data, the legislation must be amended to fulfill its purpose that is "to protect the dignity and value of human beings by assuring the best interests of the patients and by respecting their self-determination".

  • This study seeks to measure the level and root causes of occupational and interpersonal conflicts between nurses and nurse aids against hospital doctors, in order to help increase motivation of both groups in their workplace and enable more effective nursing care to patients. 271 nurses and nurse-aids working in small- and medium-sized hospitals were therefore given a series of surveys on topics such as conflicts with doctors, communications, cause of conflicts and solutions. Analysis of the survey result shows that both nurses and nurse-aids share relatively similar level and causes of conflicts in their place of work; however, nurse-aids felt more stress when their responsibilities seem to be duplicated or blurred from those of doctors and nurses. When conflicts actually occur with doctors, both groups would first try to avoid it one way or another. Then, nurses would make others cave in or file complaints through union, while nurse aides would seek help from supervisors or try to meet them half way.
