Effect of Left Atrial Volume Reduction with Maze Operation on Sinus Conversion and Recovery of Left Atrial Transport Function (Maze 수술 후 동율동 및 좌심방 수축능 회복에 대한 좌심방 부피 감소 성형술의 영향)

  • Background: The Maze procedure considered the most effective method of treatment for atrial fibrillation. However, the sinus conversion rates decreased due to several factors, especially enlarged left atrium. The purpose of this study was to investigate the effects of Maze procedure with aen atrial volume reduction plasty on rheumatic mitral valve disease, Material and Method: From December of 200f to July of 2004, 45 patients received mitral valve and Maze operation. The patients were placed in either group f or group 2, based on the left atrial volume reduction plasty. The presence and onset of sinus rhythm and the incidence of trans mitral A waves were monitored during the postoperative 7 days and throughout the follow up period of 3 and 6 months. Mean follow up periods were 15.8 10.1 months in group 1 and $6.1\pm2.7$ months in group 2. Result: The sinus onset were $9.88\pm12.2$ days in group 1, and $1\pm3.6$ days in group 2 (p<0.01). The sinus conversion rates in the group 1 and 2 were $65\%,\;75\%$ (p=0.07) in the postoperative 7 days, $70.5\%,\;100\%$ (p<0.01) at postoperative 3 months, and $93\%,\;100\%$ (p<0.01) at postoperative 6 months, respectively. The wave detection rates in the postoperative 7days were $31.2\%\;and\;63.6\%$, and continued to improve over time to $83.3\%\;and\;100\%$ by 6months, respectively. Conclusion: The results suggest that Maze procedure with left atrial volume reduction plasty is effective for inducing sinus rhythm and for restoring left atrial contractile function after concomitant rheumatic mitral valve surgery. However further follow up of this patients for long time is necessary.

  • The purpose of this study focusing on the gather decorations shown in the modern fashion was to review needlework techniques and design features of gather decorations and thereupon, analyze the aesthetic characteristics of gather decorations shown in designers' works to examine the gather decoration trend in today's fashion. First, the decorative features of gather decorations different from the conventional costumes are shirring, lace and trimming combined or shirring repeated. Second, in view of sensualism, the gather decorations seem to play a role of enhancing the exposure as a mode of expression, while the indirect exposure or beach is mixed with the materials to widen the range of gather decoration expressions. Third, in terms of history, the conventional decorative techniques are combined with new techniques and materials to create a fantastic and romantic mood different from the conventional costumes. Fourth, in view of rhythms, the wave line of the rim for gather decorations serves to express some vivid effects; on the human body, its monotonous plane repetition will turn into a new expression or a voluminous physical sense of motion. However, since expressions or meanings of the gather decorations are much diversified, they should not be regarded as simple decoration details. Our modern designers searching for new expression of creases use unique structural forms to explore the possibility of creative gather decorations. In all, since gather decorations seem to be significant as unique details conducive to a structural pragmatism and aesthetics, it is deemed necessary to continue to develop gather decorations.

A Case of Post-stroke Rhythmic Akathisia (뇌졸중후 율동성 정좌불능증 1예)

  • Dyskinesia can occur as a neurological abnormality due to stroke, and its incidence in stroke patients is reported to be about 1%. It is possible to classify dyskinesia into one of the morphologic types already classified clinically. However, a specific type of dyskinesia can occur; one which does not fall into the existing morphologic types. We experienced such a case of specific type dyskinesia, which couldn't be classified into the existing classification system. A 50-year-old man visited our hospital due to rhythmic dyskinesia of the right hand, which appeared during the resting state, and had developed one month after left subcortical infarction. Flexion and extension movements of the fingers at 3Hz appeared due to the impatient impulse to move. However, this abnormal movement could be easily suppressed under the patients will. We suggested that the abnormal movement was similar to akathisia from the fact that it occurred due to the internal desire to move and that the patient could suppress dyskinesia. However, the rhythmic tendency and lack of medication history of antipsychotics suggested that the movement was not the typical form of akathisia. The present case may represent a new clinical type of movement disorder developed after stroke. Considering the clinical pattern of the present case and following a review of the literature, we believe that it can be labeled, post-stroke rhythmic akathisia.

Joubert syndrome with peripheral dysostosis - A case report of long term follow-up - (말초 이골증을 동반한 Joubert Syndrome 1례)

  • This report describes the long-term follow-up of a 10-year-old female patient with Joubert syndrome with short stature and brachydactyly. She presented with hyperpnea alternated with hypopnea, uncontrolled jerking eye movements, and hypotonia during early infancy. She was diagnosed with Joubert syndrome based on clinical symptoms and typical MRI findings at 5 months of age. Abnormal ventilation and eye movements disappeared at around 4 years of age. Head circumference kept within normal range for her age, but her height and weight growth were markedly retarded. Simple X-ray showed an enlarged skull with increased digital markings, hypoplasia of facial bones, and abnormal enchondral bone formations in hands and feet. This article is the first report of Joubert syndrome with peripheral dysostosis.

The relationship between general creativity and musical creativity of children (유아의 일반적 창의성과 음악적 창의성과의 관계)

  • The goal of this study was to examine the relationship between general creativity and musical creativity of children, and utilize the results as the basic data to develope of children' music education. The subjects were 50 children of a kindergarten located in the Jeonlabukdo area. The instruments employed were general creativity and musical creativity scales. The data were analyzed with the SPSS program. They were analyzed by Pearson Correlation. The results of this study are as follows: First, creativity was correlated positively with 'making of the melody'. Second, fluency was correlated positively with 'making of the melody', abstraction of title was correlated positively with 'making of the song words', and elaboration was correlated positively with making of the melody and expression by rhythmic movement.

Outcomes of the Modified Maze Procedure for Chronic Atrial Fibrillation Combined with Rheumatic Mitral Valve Disease (류마티스성 승모판막질환과 동반된 만성 심방세동 치료에 대한 변형 Maze 술식의 결과)

  • Background: The aim of this study was to investigate the mid-term outcomes of our modifications to the maze procedure using cryoablation for treating atrial fibrillation associated with rheumatic mitral valve disease. Material and Method: Between March 2000 and February 2004, 177 consecutive patients underwent the modified maze procedure with the use of cryoablation concomitant with mitral valve surgery for atrial fibrillation associated with rheumatic mitral valve disease, and were divided into three groups: (1) modified Cox-maze III (CM group, n=88): (2) modified Kosakai-maze (KM group, n=63): and (3) left atrial maze procedure (LA group, n=26). The postoperative and follow-up results were analyzed and compared between the groups. Result: There were three hospital deaths (1.7%) and no significant differences in the incidence of postoperative complications between the three groups. The operative time, such as the cardiopulmonary bypass and aortic crossclamp time, were significantly longer in the CM group than in the KM and LA groups, respectively (p<0.0001). The mean follow-up was $22.4{\pm}15.1$ months ($1\sim52.6$ months) for all patients. One late death developed in the CM group (0.0%). At last follow-up, 139 patients exhibited sinus rhythm (79.9%), which was also regained in 67 patients (77.9%) in the CM group, 50 (80.7%) in the KM group and 22 (84.6%) in the LA group (p=0.743). The actuarial freedom from stroke at 4 years was $84.5{\pm}9.4%$ in the CM group, $95.0{\pm}4.9%$ in the KM group, and $92.9{\pm}6.9%$ in the LA group (p=0.916). Conclusion: The modified maze procedure using cryoablation is safe and effective in treating chronic atrial fibrillation associated with rheumatic mitral valve disease.

A Case of Repeated Ventricular Tachycardia Caused by Cardiac Rhabdomyomas in an Infant with Tuberous Sclerosis (반복되는 심실성 빈맥을 보인 대동맥판하 횡문근종을 동반한 결절성 경화증 1례)

  • Cardiac rhabdomyomas are relatively uncommon and associated with tuberous sclerosis in 40-50% cases. We report a 10-month-old infant with tuberous sclerosis who presented with ventricular arrythmias and status epilepticus. There were hypopigmented macules on the body, periventricular calcifications, renal cyst and cardiac rabdomyomas just below the aortic valve. The patient required resection of left ventricular subaortic masses due to sustained arrythmia in spite of intravenous amiodarone therapy. The pathologic examination confirmed the diagnosis of rhabdomyoma. The patient had no more arrythmia during the 14 month follow up period. Although cardiac rhabdomyomas may spontaneously regress, surgery is often necessary and frequently resolves the underlying arrythmia.

  • By using deterministic dynamic models, we observe the behavior of the foreign exchange rate of a small open economy with rational expectation formation and different restrictions on the international economic integrations. First, an economy connected to the world by purchasing power parity and uncovered interest parity is studied in the next section. In both sections, financial assets available in the economy are domestic money and bonds. Stocks are added as a financial instrument in the next section, and real capital accumulation is also taken into account. Furthermore, the economy concerned there is fairly autonomous, and not directly governed by either purchasing power parity or uncovered interest parity. The expectation formation used throughout the whole paper is complete perfect foresight, which is the certainty version of rational expectation and free from any forecast errors. It is found that upon monetary expansion the short run depreciation of the foreign exchange rate is a fairly robust result regardless of the degree of the international economic integration, while it is not true for fiscal expansion. The expectation on the long run state significantly affects the short run response of the exchange rate. All of our models postulate that the current account should be balanced eventually. As the result, the short run behavior of the exchange rate is affected by the expectation on the long run balance and may well be a blend of the traditional flow view and modem asset view. The initial overshooting of the exchange rate is easily observed even in the fairly autonomous economy Furthermore, the initial overshooting is not reduced over time, but augmented for some time before it is eventually eliminated. As long as we maintain rational expectaion, introducing time delay in the adjustment of the foreign goods price to the foreign exchange rate does not make much difference.

  • Purpose : The purpose of this study was to examine the effects of Rhythmic Stabilization(RS) and Combination of Isotonic(CI) of Proprioceptive Neuromuscular Facilitation on balance in patients with hemiplegia. Methods : Ten patients with hemiplegia were trained with RS and CI for six weeks. Two Standard Scale(TSS), Functional Reach Test(FRT), Time Up and Go test(TUG) and Berg Balance Scale(BBS) were used to prove their improvement of balance. Results : Body weight bearing decreased on less affected side and increased on more affected side. Distance of forward reaching for FRT increased. Time for TUG decreased. BBS score increased. They all showed significant difference. Conclusion : This study suggests that balance training with RS and CI of Proprioceptive Neuromuscular Facilitation improved balance in patients with hemiplegia.

About Ahn, Jasan's theory of Sijo (안자산의 시조론에 대하여)

  • This paper is an analysis about Ahn, Jasan's theory of Sijo. He comprehended that Korean classical poetry is not a literary genre written in letters but a music genre sung orally, because he minutely investigated old books under the situation without preconceptions. Ahn, who had a thorough knowledge on our native music or literature, had an exact recognition about Sijo. He had analyzed existence of Sijo after commenting Sijo's true nature, whenever he discuss. His attitude was possible for his recognition like that. The first step of Ahn, Jasan's theory of Sijo is source of Sijo searched for identity of Sijo. Generic source, metrics, poetical idea, and history are written well in the paper. Establishment of Sijo's poetic identity is second step's discourse about Ahn's theory of Sijo, the contents are integrated in his book Poetics of Sijo poetry. He became to discuss Sijo as a regular poetry, even in this book. Especially a minute discussion about theory of Sijo's phrase and lines was accomplished in this time, theory of metrics based on meter and rhythm was a core discussion of this time too. Although his scholarly opinions are not refined comparing to the scholars of later ages, we can say that his opinions clearly worked a role of exemplary precedent. His opinions were other scholars ahead, nevertheless he had been neglected by researchers for not belonging to major group against his will. For this reason, I think Ahn's status in history of Sijo study has to be readjusted.
