The Relationship between Practice in Lymphedema Self-care Management and Health-related Quality of Life in Breast Cancer Patients with Mastectomy (림프부종 자가 관리 실천 정도와 유방절제술 환자의 삶의 질과의 관계)

  • Purpose: The purpose of this study was to examine the relationship between lymphedema self-care management and quality of life in breast cancer patients with mastectomy and lymphedema. Methods: One hundred and eighty-six breast cancer patients with mastectomy and lymphedema (n=186) were recruited at a medical center located in Seoul. The levels of lymphedema self-care management and quality of life were measured by the scale for measurement of practice in lymphedema self-care management, European Organization for Research and Treatment of Cancer-Quality of Life Core 30 (EORTC QLQ-C30) and European Organization for Research and Treatment of Cancer-Quality of Life Questionnaire Breast Cancer Module (EORTC QLQ BR23), respectively. Data were analyzed with SPSS 18.0 program. Results: There were statistically significant correlations between lymphedema self-care management and general health status/quality of life in mastectomy patients with lymphedema (r=.30, p<.001). The physical function score of cancer related function scale (r=-.15, p=.033), fatigue score of cancer related symptom scale (r=.15, p=.036), systemic side effect score of breast cancer related symptom scale (r=.45, p=.034), and upset by hair loss (r=.27, p=.004) were significantly correlated with quality of life. Conclusion: The findings suggest that these significant factors should be considered when caring for lymphedema patients.

  • Mammoplasty is currently increasing not only for cosmetic surgery, but as well as for the recovery after breast cancer surgery. The prostheses inserted into the breasts of women who have undergone mammoplasty, hide the breast substances and it is becoming increasingly difficult to diagnose breast disease, and fear is growing by the concern of the prostheses bursting by the strap. So we want to develop a strap applicable to women with prostheses inserted, to determine the usefulness, and we also want to compare the utility by comparing the total area of the Inner and Outer parts of the breast and Posterior Nipple Line (PNL), after obtaining video by applying the existing strap and the developed strap to phantom of the prostheses inserted patient shape. When the pressure by the developed pressure, the total area increased by 10.09% from CC view to $9,813.797mm^2$, 3.88% from CC-ID view to $7,621.531mm^2$, PNL increased by 3.41% from CC view to $90.916mm^2$, 1.64% from CC-ID view to$75.357mm^2$. And the breast tissue of the thorax side increased 3.53% from CC view to $177.725mm^2$, and 6.57% from CC-ID view to $152.510mm^2$, and we could verify that the prostheses were completely eliminated in the CC-ID images of developed strap, compared with the existing strap.

Sparganosis existed for more than three years that misdiagnosed as a breast cancer (3년이상 존재한 유방암으로 의심되었던 스파르가눔증)

  • Objective.: Sparganosis locations in humans are usually presented with a subcutaneous tissue of abdominal wall, chest, abdominal vicera and brain, but are rarely found in the breast. Methods. A case of sparganosis was confirmed by surgical excision of two parasites in a 76-year-old female patient present to a palpable mass in the right breast (presumed to have been sparganosis approximately 3 years ago). She had no history to direct ingestion of snakes or frogs, but had the history of drinking contaminated water. Mammography, ultrasonography, MRI, and FDG PET/CT imaging findings for patient were characteristic of sparganosis due to suspicion of breast cancer. Conclusions: The first route of infection in humans is drinking contaminated water. The second route is the ingestion of raw or partially cooked snakes or frogs. The third route is infected wound snake, frog muscle that attach to the case. However, only a few cases of drinking contaminated water have been reported in the country. Ultrasonography, MRI is known to be helpful for diagnosis of breast sparganosis. However, Mammography, ultrasonography, MRI, and FDG PET/CT for breast sparganosis is not reported present in the country. Reported the case and reviewed the related literature briefly.

Angiosarcoma in a Chronically Lymphedematous Leg - Stewart-Treves Syndrome - (하지의 만성 림프부종에 발생한 혈관육종 - Stewart-Treves 증후군 -)

  • Angiosarcoma is a very rare disease of soft tissue sarcoma, and angiosarcoma arising in a region of chronic lymphedema is referred to as Stewart-Treves syndrome. it typically occurs in postmastectomy lymphedema of the arm and sporadically in a lymphedematous leg. The prognosis, even with wide surgical excision and subsequent radiotherapy, is poor. The authors experienced a case of angiosarcoma in a chronically lymphedematous leg which had undergone hysterectomy due to cervical cancer 17 years ago and lymphedema of leg persists for about 16 years. We report a case of angiosarcoma with a brief review of the literature.

Relationships of Perceived Health Status, Depression and Quality of Life of Breast Cancer Survivors (유방암 절제술을 받은 여성의 지각된 건강상태, 우울 및 삶의 질의 관계)

  • Purpose: The purpose of this study was to describe the degree of perceived health status, depression and quality of life(QOL) and to examine the relationships among these factors in breast cancer survivors. Methods: The subjects were 105 women with mastectomy after breast cancer diagnosis who were in the out-patient department of surgery at SNUH in Seoul. Data was collected using self-report instruments. The instruments included 1-item for perceived health status, Chae and Choe's QOL scale for Korean breast cancer survivors, and Beck Depression Inventory(BDI) as well as socio-demographic and medical information. Data were analyzed with SPSS WIN 10.0 program. Results: The levels of perceived health status and QOL were moderate. Mild depression was found on the BDI. QOL and depression showed a negative correlation(r= -.598), whereas QOL and perceived health status was correlated positively(r=.370). Depression, perceived health status, and time since diagnosis accounted for 47.4% of variance of the QOL. Depression was the most significant predictor of QOL in breast cancer survivors. Conclusion: Breast cancer survivors may experience depression and alteration in QOL. Future nursing research should be directed toward the implementation and evaluation of interventions that promote QOL and decrease depression in breast cancer survivors.

Evaluation of Image Quality for Diagnostic Digital Chest Image Using Ion Chamber in the Total Mastectomy (변형근치유방절제술 환자의 Ion chamber 변화에 따른 디지털 흉부 영상의 화질 평가)

  • The patients who had been operated total mastectomy are different from general women in their breasts thickness due to breast surgery. As a result, digital chest image from total mastectomy patients will be different attenuation. The main objective for this study is to show that a proper Ion chamber standard combination measuring MTF which is objective basis for Digital image, when be x-ray for total mastectomy patient. We have designed the unique number that shown Left is 1, Right is 2, Center is 3 and have put the edge phantom on detector ion chamber. Lastly, we have obtained experiment images. The evaluations of all image quality have measured by 50% MTF of spatial resolution and absorption dose using Matlab(R2007a). The result showed that average exposure condition, MTF value, absorption dose for 1+3 and 2+3 combinations were 2.745 mAs, 1.925 lp/mm, 0.688 mGy. Consequently, that showed high MTF, DQE and low dose than other combinations. Therefore, a proper changes of ion chambers are able to improve image quality and to reduce radiation exposure when be X-ray for total mastectomy patients. Also, it will be possible to standard for application chamber combination and utilization on clinical detection.

A study on the adjustment of daily life ana body image of the mastectomy patients (유방 절제술 환자의 퇴원후 신체상과 생활적응에 관한 연구)

  • The purpose of this study was to identfy the relationship between the body image adjustment of daily life. 58 subjects who had undergone radical mastectomy or modified radical mastectomy from 1979 to April 1984 at Seoul National University Hospital were selected as a sample and used for a final analysis. Finding of this study were as follows; 1) The relationship between the body image and the adjustment of daily life were significant differences. The body image is more positive, the adjustment of daily life is bet-ter. However, the cause-effect relationship was not found in this study. (p<0.05) 2) There were no significant differences in the body image between the right side mastectomy and left side mastectomy, between the radical mastectomy and modified radical mastectomy between those who had occupation and those who did not have occupation, and between the level of education. 3) There was no significant correlation between the body image and age, between the body image and elapsed time after surgery.

Evaluation of superficial dose for Postmastectomy using several treatment techniques (유방전절제술을 시행한 환자에서 치료기법에 따른 피부선량 평가)

  • Purpose : The purpose of this study was to evaluate the surface and superficial dose for patients requiring postmastectomy radiation therapy(PMRT) with different treatment techniques. Materials and Methods : Computed tomography images were acquired for the phantom(I'mRT, IBA) consisting of tissue equivalent material. Hypothetical chestwall and lung were outlined and modified. Five treatment techniques(Wedged Tangential; WT, 4-field IMRT, 7-field IMRT, TOMO DIRECT, TOMO HELICAL) were evaluated using only 6MV photon beam. GafChromic EBT3 film was used for dose measurements at the surface and superficial dose. Surface dose profiles around the phantom were obtained for each treatment technique. For superficial dose measurements, film were used inside the phantom and analyzed superficial region for depth from 1-6mm. Results : TOMO DIRECT showed the highest surface dose by 47~70% of prescribed dose, while 7-field IMRT showed the lowest by 35~46% of prescribed dose. For the WT, 4-field IMRT and 7-field IMRT, superficial dose were measured over 60%, 70%, and 80% for 1mm, 2mm, and 5mm depth, respectively. In case of TOMO DIRECT and TOMO HELICAL, over 75%, 80%, and 90% of prescribed dose was measured, respectively. Surface and superficial dose range were uniform in overall chestwall for the 7-field IMRT and TOMO HELICAL. In contrast, Because of the dose enhancement effect with oblique incidence, The dose was gradually increased toward the obliquely tangential angle for the WT and TOMO DIRECT. Conclusion : For PMRT, TOMO DIRECT and TOMO HELICAL deliver the higher surface and superficial doses than treatment techniques based linear accelerator. It showed adequate dose(over 75% of prescribed dose) at 1mm depth in skin region.

Breast Conserving Operation and Radiation Therapy in Early Breast Cancer : Interim Analysis (초기유방암에서 유방보존수술 후 방사선치료 : 중간분석)

  • Purpose : To evaluate interim results in terms of failure, cosmetic results and survival after breast conserving operation and radiation therapy in early breast cancer. Material and Methods : From January 1992 through December 1997, seventy two patients with early stage 0, I and II breast cancer were treated with conservative surgery plus radiotherapy at Keimyung University Dongsan Medical Center. Age distribution was 25 to 77 years old with median age of 43. According to TNM stage, five patients had stage 0, thirty three were stage I, twenty five were IIa, and nine were IIb. Most patients underwent excision of all gross tumor and ipsilateral axillary dissection. Breast was irradiated through medial and lateral tangential fields of 6 MV photons to 50.4 Gy in 28 fractions over 5.5 weeks. We delivered a boost irradiation dose of 10 to 16 Gy in 1 to 2 weeks to excision site. Adjuvant chemotherapy was administered in forty one patients with CMF (cyclophosphamide, methotrexate, 5-fluorouracil) regimens of 6 cycles concurrently or before radiation. Cosmetic results were assessed by questionnaire to patients grading of excellent, good, fair, poor. Follow-up periods were 22 to 91 months with median 40 months. Results : Five year disease free survival rate (5YDFS) was $95.8\%$. According to stage, 5YDFS was $100\%,\;96.9\%,\;96\%\;and\;88.9\%$ in stage 0, I, IIa and IIb, respectively. Two patients had distant metastasis and one had local and distant failure. One patient with distant failure had bone and liver metastasis at 14 months after treatment and the other had lung and both supraclavicular metastasis at 21 months after treatment. Patient with local and distant failure had local recurrence on other quadrant in same breast and then salvaged with total mastectomy and chemotherapy but she died due to brain metastasis at 55 months. Complications were radiation pneumonitis in five patients (four patients of asymptomatic, one patients of symptomatic) and hand or arm edema(4 patients). Fifty nine patients answered our cosmetic result questionnaire and cosmetic results were good to excellent in fifty one patients $(86\%)$. Conclusion : We considered that conservative surgery and radiation for the treatment of early stage invasive breast cancer was safe and had excellent survival and cosmetic results. We need to assess about prognostic factors with longer follow up and with large number of patients.

Disease Related Stress Experienced by women with Mastectomy : Q Methodology (유방 절제술 후의 질병관련 스트레스 : Q-방법)

  • Purpose: To identify the type of disease-related stress experienced by women who received mastectomy for breast cancer using Q methodology. Method: Q sample included 30 statements obtained from literature and interviews with women with mastectomy. P sample consisted of 22 patients with mastectomy. The data were collected from November to December 2002 and analyzed using QUANL program. Result : Four types of disease-related stress experienced by women with mastectomy were found. Type 1 was manifested for a short duration following surgery and characterized by lowered self-esteem, feelings of emptiness and depression because of impaired body image. Type 2 was characterized by declined physical strength, resulting in frequent fatigue in the daily life and less intimate relationship with spouse. Type 3 was manifested by perfectionists with strong sense of self-pride who received or considered breast reconstruction surgery. Type 4 was reported by those who had long period of post-surgery. This type reported a strong sense of discomfort for wearing an artificial breast but didn't show any intention of trying breast reconstruction surgery. Conclusion: Women with mastectomy were found to experience different type of disease-related distress. Therefore, nurses should assess the type of stress the patient experiences following a mastectomy to provide appropriate nursing care.
