A SURVEY OF GENERAL ANESTHESIA IN PEDIATRIC CLINIC AT SEOUL NATIONAL UNIVERSITY DENTAL HOSPITAL (서울대학교병원 소아치과에서 시행한 전신마취에 대한 연구)

  • We tried to find out the information which can be drawn from the survey of general anesthesia in pediatric clinic at Seoul National University dental hospital from 1997 to 1999. The subjects were 37 patients Collected data were primary reasons for general anesthesia, preoperative physical status, age distribution, peformed dental treatment, complications, follow-up, and etc. Following were the results: (1) Most(97%) of patients treated under general anesthesia were handicapped and preoperative physical status was ASA Class I or Class II. (2) Majority(41%) of patients were between 11 and 15. (3) Average of 8.1teeth were restored and 3.3teeth were extracted. (4) In 32% of patients, there were mild complications. (5) 12(32%) of the patients responded to the six-month recall check.

The Effect of Ipidural Pain Block After Thoracotomy (개흉술후 지속적 경막외마취가 통증감소에 미치는 영향)

  • Continuous epidural pain block with a local anesthetic agents is a commonly employed technique for pain relief after thoracotomy. In this study, we evaluated the effectiveness of the continuous epidural pain block in 19 patients undergoing elective lateral or posterolatrral thoracotomy with control group(n=19) from November 1994 to July 1995, Epidural lidocaine and morphine mixtures were injected via an epidural catheter as a bolus after operation, and then bupivacaine and morphine mixtures were injected continuously following 5 or 6 days. The pain score, upper arm elevation(ROM score), and respiratory rate were significantly changed(P<0.05) from 30min after injection. The CO2 tension of arterial blood was decreased significantly(P<0.05) from 2hr after injection. The postoperative hospital days were decreased significantly(P<0.05). Side effects of the epidural pain block were urinary retention(n= 10), urticaria(n=2) and a case of headache. There was no postoperative lung atelectasis. We conclude that the continuous epidural pain block is good for prevention of the postoperative lung complication and early recovery after thoracotomy.

Video-assisted Thoracioscopic Surgery under Epidural Anesthesia in the High-Risk Patients with Secondary Spontaneous Pneumothorax (고위험인자를 가진 이차성 자연 기흉환자의 경막외마취를 이용한 흉강경 수술)

  • Video-assisted thoracoscopic surgery is good indication of secondary spontaneous pneumothorax. This method usually required general anesthesia and single-lung ventilation with collapse of other lung. But, risks of general anesthesia and single-lung ventilation must be considered in high-risk patients. Material and Method: Between September f999 and August 2001, 15 high-risk patients were treated by video-assisted thoracoscopic surgery under epidural anesthesia. Result: Video assisted thoracoscopic surgery was successfully performed in 15 patients. Duration of postoperative air-leakage was 4.3days, Significance of complication was none, No recurrence of pneumothorax was encountered. Conclusion: Video-assisted thoracoscopic surgery can be performed safely under epidural anesthesia for treatment of secondary spontaneous pneumothorax in high-risk patients.

  • Pediatric caudal anesthesia was done in 50 infants and children under 10 years of age, who were to undergo surgery of inguinal region. All cases were given 10mg/kg body weight of 1% lidocaine solution with epinephrine 1:200,000. The results were as follows : 1) Pediatric caudal anesthesia was simple, easy and reliable in technique. 2) Additional intravenous administration of Ketamine or pentothal sodium was needed. ie, to provide a more cooperative state. 3) Anesthetic effect was judged very Excellent. 4) Cardiovascular and respiratory changes were minimal. Author's came to conclusion that caudal anesthesia for pediatric inguinal region surgery is reliable, simple in technique, favorable to surgeon, and is considered to be a good technique for pediatric anesthesia.

Low Dose Spinal Anesthesia for Ambulatory Surgery of Varicose Vein (하지정맥류의 외래수술을 위한 저용량 척추마취)

  • Background: Bupivacaine with fentanyl might be suitable as the spinal anesthesia for performing ambulatory surgery to treat varicose vein. Material and Method: Thirty patients who underwent spinal anesthesia for a varicose vein operation were enrolled in this study. They were classified into 2 groups of either fentanyl 25ug mixed with bupivacaine 4mg (group FB4) or bupivacaine 8mg (group B8). We compared the groups for the success of the analgesia, the recovery time from sensory and motor block, the side effects and the postoperative complications. Result: The groups did not differ significantly regarding the success of analgesia (13 of 15 [group FB4], 15 of 15 [group B8]). None of the patients were converted to general anesthesia due to surgical pain. None of the patients required medication for hypotension and/or bradycardia. The operative and nonoperative side effects of motor block (tested for by using a modified Bromage scale) was significantly lower in group FB4 than that in group B8, as checked at 2 hours after spinal anesthesia (p<0.05). Recovery from spinal block was significantly quicker in group FB4 than that in group B8 (p<0.05). The first voluntary micturition time did not differ significantly (6.5 hours v 4.5 hours [p=0.143]) between the groups, but a nelatone catheter was inserted into 2 of the group B8 patients due to dysuria. Conclusion: Adequate intraoperative analgesia and hemodynamic stability and faster mobilization were achieved using bupivacaine 4mg with fentanyl 25ug. Low dose spinal anesthesia with fentanyl is suitable for performing ambulatory surgery to treat varicose vein.

A Case of Vocal Cord Paralysis Following Endotracheal General Anesthesia (삽관마취후의 성대마비 1례)

  • Endotracheal intubation is common practice for either general anesthesia or keeping artificial airway in various conditions. Despite of its great usefulness, however, laryngeal edema, ulceration, and granuloma are occasionally developed following endotracheal intubation as posttraumatic complications. Vocal cord paralysis is also rarely developed. Recently we experienced a case of left vocal cord paralysis with accompanying hoarseness following endotracheal general anesthesia for appendectomy. The symptoms of hoarseness and sore throat developed immediately after the general anesthesia. Periodic check revealed gradual return to normal function of the paralyzed cord with the complete function about 3 months after the anesthesia.

Epidural Space Identification Device Using Air-filled Catheter (공기도관을 사용하는 경막외강 자동탐지기구)

  • An assist device was developed to identify the epidural space by continously monitoring the air-filled catheter pressure. The pressure signal appropriately amplified and filtered enabled to alarm the needle introduction into the epidural space by thresholding detection. Ten LEDs provided a visual change of catheter pressure before alarming for user convenience. Clinical trials were performed in 30 patients with 83% success rate at the first trial. When failed, the second trial was enough for successful anesthesia. The air volume introduced during each anesthesia was less than 1ml, causing side effects. Air filling of the catheter could also minimize infection possibility. Therefore, the present device guarantees safe anesthesia with user convenience.

  • 구강저를 통한 기관내 삽관은 1986년 Altemir에 의해 처음 소개된 바 있는데 그 후 1993년 Hoenig와 Braun 및 같은 해 Stoll 등에 의해서나, 또는 1996년 Prochno 등에 의해 계속 변형된 방법으로 문헌에서 기술되어 왔다. 이 방법은 치과 영역 특히 구강악안면외과의 외상수술시 이상적인 교합을 얻기 위해 임시 상하악간 고정(intermaxillary fixation)을 할 수 있고 중안모 골절(midface fracture)의 회복을 위한 비관 삽관의 불편함을 피할 수 있는 유리한 점이 있으며, 또한, 정복 및 고정술이 필요한 비골 골절(nasal bone fracture)에서나 두개기저골 골절(skull base fracture)에서 여러 감염 등 합병증을 피하기 위해 추천될 수 있다. 또한, 목 부위에 비심미적인 반흔을 만들게 되며 여러 합병증을 초래할 수 있는 기관절개술(tracheostomy)보다 유용할 수 있다. 본 증례보고에서는 교통사고로 두개기저부 골절을 동반한 심한 중안모 골절 환자에서 적용시킨 경우를 알아보고, 아울러 이러한 악하부 삽관의 장, 단점 및 시술 과정 등에 대해 고찰해보고자 한다.

Mortality rate undergoing anesthesia in Thoroughbred racehorses at Busan Race Park (부산경남경마공원 Thoroughbred 경주마의 마취중 치사율)

  • The report about equine anesthesias in Korea are very rare. This paper aimed at the mortality rate during anesthesia in Thoroughbred horses at Equine Hospital of Busan Race Park, KRA in South Korea from 2005 to 2010. Drugs used in anesthesia was IV injection of detomidine hydrochloride (0.01 mg/kg) or xylazine(0.5mg/kg) for sedation and premedication, Guaifenesin(50-100 mg/kg) for muscle relaxation, ketamine hydrochloride(2 mg/kg) for induction of anaesthesia and Inhalational isoflurane(1.3-1.5 %) to maintain anesthesia. Total number of anesthetic cases was 190, 150 of inhalational anesthesia and 40 of general anesthesia, repectively. The purpose of anesthesia was highest in the disorder of musculoskeletal system, followed by urogenital system and respiratory system Mortality case due to anesthesia was one during arthroscopic surgery for removal of osteochondral chip fragments. The time of anesthesia was 150 min, fatal sign was hypoxemia and the reason was improper machine operation of the anesthetist. In conclusion, the perianesthetic mortality rate during anesthesia in Thoroughbred horses at Busan Race Park was 0.52%(1 death per 190 anesthetics).

Airway Management Using the I-gel Supraglottic Airway Device in Patients with Grisel's Syndrome -Case Report- (그리셀증후군 환자에서 I-gel 성문상기도유지기를 사용한 기도관리 -증례보고-)

  • Grisel's syndrome is a non-traumatic subluxation of the atlantoaxial joint with an inflammatory condition in the adjacent soft tissues. Due to the instability of the cervical spine, careful airway management is crucial to prevent potential cervical spinal cord injury following airway manipulation. We successfully secured the patient airway using a supraglottic airway device (I-gel) in a patient who had previously diagnosed with Grisel's syndrome. The operation was successfully completed, and the patient recovered without any neurological complications. I-gel can be a good option for airway management during general anesthesia in a patient diagnosed with Grisel's syndrome.
