Full mouth rehabilitation of iatrogenic attrition patient (의원성 마모 환자의 전악 수복 증례)

  • Physiologic tooth attrition is accepted as a natural change. But when pathologic changes, such as loss of occlusal vertical dimension, masticatory pain, loss of masticatory function, and loss of esthetics occur, restorative measures should be taken by dentist. A patient visited the clinic with the chief complaint of 'My lower teeth bite the palate and it hurts'. It was diagnosed as iatrogenic attrition of lower natural dentition caused by inappropriate upper restorations, resulting in traumatic overbite. Through model analysis and diagnostic wax up, increase in vertical dimension was decided. Upper and lower dentitions were restored with new prostheses. After restoration, satisfactory outcomes were achieved both in functional and esthetic aspects.

5 Neuropsychiatric Disorder Cases Diagnosed and Treated as Greater Yin Disease Based on Disease Pattern Identification Diagnostic System by Shanghanlun Provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 태음병(太陰病)으로 진단하고 치료한 신경정신과 질환 증례 5례)

  • Objective : The purpose of this study is to explore the therapeutic potential of Shanghanlun for neuropsychiatric disorders through a comparative review of five neuropsychiatric disorder cases diagnosed with Greater yin disease pattern/syndrome (太陰病). Methods : We collected and compared five neuropsychiatric cases diagnosed and treated with Greater yin disease pattern/syndrome (太陰病) from four korean medicine clinics. Clinical symptoms common to the five cases were analyzed by connecting them with Shanghanlun provision 273 and 279. Results : In all five cases, the improvement of chief complaints and decrease in general anxiety were observed after administration of Gyejigajakyak-tang (桂枝加芍薬湯) and Gyejigadaehwang-tang (圭支加大黃湯). In addition, neuropsychiatric symptoms and physical symptoms that are mentioned in Shanghanlun provision 273 and 279 were confirmed in all five cases. Conclusions : We not only reconfirmed that the core features of Greater yin disease pattern/syndrome(太陰病) proposed in previous studies; zi-tong (自痛), yin-er (因爾), jie-ying (結硬), but also that the physical symptoms associated with fu-man (腹満), tu (吐) and zi-li (自利) were common in the presenting symptoms and past histories of the five patients.

Perception, Attitude, Performance on the Hand Washing by Dental Hygienist (치과위생사의 손씻기 인지도와 태도 및 수행도)

  • The purpose of this study was to examine perception, attitude, performance on the hand washing by dental hygienist. A self-reported questionnaire was surveyed by 165 dental hygienist in G area. The data were analyzed for frequency analysis, average, standard deviation, independent t-test, one-way ANOVA, pearson's correlation coefficient by using SPSS 21.0 program. The perception on the hand washing, age and work place, career, Number of hand washing, Frequency of hand washing education were higher significantly. The attitude, performance on the hand washing by dental hygienist, age, career, work place were higher significantly. There were positive correlations among the perception, attitude, performance of dental hygienists on the hand washing by dental hygienist. The result of this study indicates a necessity for developing education programs consistently to improve perception, attitude, performance of hand washing by dental hygienist.

Case Series Reporting 7 Somatic Symptom Disorder Cases Diagnosed as Greater Yin Disease by Shanghanlun Provisions (『상한론(傷寒論)』 태음병(太陰病)으로 진단된 신체증상장애 환자 7례 증례 보고)

  • Objectives: This study was conducted to confirm the reproducibility of the diagnosis of greater yin disease (太陰病) and the efficacy of herbal treatment using Gyejigajakyak-tang or Gyejigadaehwang-tang in somatic symptom disorder (SSD). We also further refined the characteristics of greater yin disease in SSD. Methods: We analyzed the demographic and treatment data of seven patients with SSD from four Korean medical clinics. The reliability of diagnosis and the efficacy of the treatments were evaluated by the Beck Anxiety Inventory (BAI), a numerical rating scale (NRS), and the patients' statements. Finally, we analyzed patients according to the diagnostic points of Shanghanlun provision 273. Results: Seven SSD patients took Gyejigajakyak-tang or Gyejigadaehwang-tang. The mean age of the patients was 52.1±15.8 years old, and the herbal medication period was 82.2±38.2 days. At the first visit, the patients showed hypochondriasis, obvious stress, and a severe BAI score (40.4±10.9). The NRS significantly decreased from 8.9±1.6 to 0.7±0.7 after the treatments. There were no side effects reported. As a result of analyzing these 7 cases, hypochondriasis, obvious stress, and anxiety were identified, which had been previously suggested as 自痛, 因爾, and 結硬. Conclusions: We confirmed the reproducibility of greater yin disease in 7 SSD cases from 4 medical institutions. These results imply that there could be a considerable relation between SSD and greater yin disease as defined by the Shanghanlun provisions.

Research on the Actual Condition of Dental Outpatient Prescriptions (치과 외래처방의 실태 조사)

  • This study investigated the medicines prescribed in dental services in order to provide information and materials on dental clinics and hospitals, for the intention of leading the dental circle to make efforts for voluntary improvement, do adequate prescription, and attempt for bench marking through this unfolding of their tendency in continuous prescription behaviors, and leading the people to have a right recognition on adequate medicine use through this information on their medicine-taking behaviors including antibiotics and injections. From the records of outpatient prescriptions of medicines under health insurance over the period of June 1st to September 31th in 2003 in the 34,226 recuperation institutions, antibiotics and injections were analyzed into administration days, prescription frequency, medicine cost per administration day, the number of medicines per prescription, and the number weight of high priced medicines. The findings were as below: 1. Adminstration days of antibiotics was 90.11% in the dental clinics, which was a decrease than the same quarter and the previous quarter of the previous year. While the prescription frequency of antibiotics was 15.5%, higher than the same quarter and the previous quarter of the previous year. In dental hospitals, administration days and prescription frequency of antibiotics were 71.57% and 21.05%, respectively, a little higher than the previous quarter. Compared to other kind of recuperation institutions, dental clinics and hospitals had higher administration days and lower prescription frequency. 2. For injections, adminstration days and prescription frequency in dental clinics were 0.13% and 0.05%, respectively, which were decreases than the same quarter of the previous year. In dental hospitals, adminstration days and prescription frequency were 1.03% and 0.88%, respectively, a little lower than those of the previous quarter. Compared to other kind of recuperation institutions, dental clinics and hospitals were very lower in injection administration days and injection prescription frequency. 3. The number of prescribed medicines was 2.79 in the dental clinics, which was lower than the same quarter of the previous year but higher than the previous quarter. Dental hospitals put 2.67 numbers of medicines per prescription, an increase than the same quarter of the previous year and the previous quarter. Compared to other kind of recuperation institutions, dental clinics and hospitals put smaller number of medicines per prescription. 4. Medicine cost per administration day was 863 won in the dental clinics, which was an increase than the same quarter of the previous year and in the previous quarter. Compared to other kind of recuperation institutions, dental clinics and hospitals had lower medicine cost per administration day. 5. The number weight of high priced medicines was 46.43% in dental clinics, which was an increase than the previous quarter. In dental hospitals, it was 54.05%, so remarkable an increase than the previous quarter. Compared to other kind of hospitals and clinics, dental clinics and hospitals prescribed larger number of high priced medicines. 6. By districts, the frequency of antibiotics prescriptions was the highest in Kwanju and the lowest in Daejeon. The frequency of injection prescriptions was high in all Youngnam districts as was in the second quarter, while low in all the Metropolitan districts. There was a large variation in the prescription frequency to the districts, as the district of the highest prescription frequency had more than 2 times larger frequency than the district of the lowest frequency. Medicine cost per administration day was the highest in Ulsan but the lowest in the north part of Cholla province. The number of medicines per prescription was the largest in Kyonggi province while the smallest in Cheju-do.

The Distribution of Pediatric Patients According to Accessibility to Dental Facilities in Pusan National University Dental Hospital (치과의료기관 접근성에 따른 부산대학교 치과병원 소아치과의 신환 분포)

  • The purpose of this study was to analyze the accessibility to dental facilities and the differences in chief complaints of new patients who visited Pusan National University Dental Hospital. We collected information from electronic medical records for 1,820 new patients. The accessibility was analyzed by measuring the distance from the patient's house to the dental facilities. The distance was categorized into 10 km sections, and the presence or absence of a local pediatric dental clinic within 10 km of the patient's house was also categorized. As the distance between the house and Pusan National University Dental Hospital increased, the proportion of patients who visited for dental caries decreased, and orthodontic treatment increased. Dental caries accounts for 27.9% of less than 10 km and 20.5% over 30 km. Orthodontic treatment accounts for 25.4% within 10 km and 27.3% more than 30 km away. The presence or absence of a local pediatric dental clinic within 10 km did not significantly affect the distribution of chief complaints. This study can be used as basic research data to establish effective treatment measures that can improve the physical and geographical accessibility of patients visiting pediatric dentistry.

An Institutionalization and Legislation Productivity of Korean Metropolitan Councils: Panel Data Analysis (광역의회제도화와 입법생산성: 패널데이터 분석)

  • This study analyzes the effect of institutionalization of Korean metropolitan councils on legislation productivity. Based on the theory of institutionalization of legislatures, three independent variables (stability, complexity and adaptability) were selected to measure the level of institutionalization of a metropolitan council and nine sub-analysis indicators. The main results of the analysis of the effect of the institutionalization of the metropolitan council on legislation productivity are as follows: First, the factors that determine the number of reported bills were the ratio of first-term lawmakers, average number of elected of the chairmen, number of special committees, number of legislative experts, actual age of metropolitan councils, and number of voters per lawmaker. Second, the factors that determine the rate of reported bills were the average number of elected of the chairmen, the number of special committees, the number of legislative experts, the actual age of metropolitan councils, and the number of voters per lawmaker. Third, the factors that determine the number of reported bills per lawmaker is the average number of elected of the chairmen, the actual age of metropolitan councils, and the number of voters per lawmaker. The above result points out that legislation productivity differences of past metropolitan councils can be understood as differences arising from legislative institutionalization levels and several policy considerations can be made to enhance legislation productivity of metropolitan councils.
