A twenty six year old man was admitted to our clinic for bleeding from a venous ulcer of the left leg. His left femoral vein had been ligated for bleeding control after a traffic accident that happened when he was 5 years old. He had a varicose vein, stasis dermatitis, and a venous ulcer on his left leg. To shunt left femoral venous return to the right femoral vein, we did a crossover bypass with arteriovenous fistula using the right greater saphenous vein. To get good patency of the arteriovenous fistula, the bypass graft was closed 6 weeks after the initial operation. He maintained his left leg circumference relatively well for 6 months after the operation. We report the case of a patient who had a vein crossover bypass for a chronic femoral vein ligation.
The main purpose of this study was to identify the effects of integrated stress management program on the stress symptoms of psychophysiological patients. especially patients with peptic ulcer. The study employed a quasi-experimental design using two different experimental groups. The samples in the integrated stress management program participated in autogenic training with biofeedback. discussions on effective coping method. cognitive. behavioral, and emotional management. They were also provided with an educational booklet on stress management and an tape on progressive muscie relaxation. Each session lasted one hour and the program consisted of seven sessions over four weeks. The other group was only given an tape on progressive muscle relaxation. The data were collected from May 20 to september 25, 1996. A total 47 patients from one university hospital located in Seoul participated, experiment group 1(integrated stress management training) had 23 subjects and experiment group 2(progressive muscle relaxation training) had 24 subjects. The effects of these programs were measured by the stress symptom scale developed by Kogan(1991) which was translated by Lee(1992) and the healing status of the ulcer evaluated by a physician. The data were analyzed using Chi-square test, t- test, ANOVA, repeated measure ANOVA. The result are as follows : 1. The integrated stress management group reported a significantly lower stress symptom score than the group given the progressive muscle relaxation only. 2. The integrated stress management group showed a significantly improved ulcer status as compared to the group given a progressive muscle relaxation only. In conclusion, it was found that the integrated stress management program was more effective in decreasing self-reported stress and physiological symptoms among patients with peptic ulcer as compared to the progressive muscle relaxation group. Based on this finding, the following suggestions can be made. 1. It is necessary to broaden the scope of nursing practice for psychophysiological patients so nurses can include stress management as part of patient care. 2. It is necessary to develop stress management program for other patients whose symptoms are known to be related to stress. 3. It is necessary to replicated this study with a larger sample in different settings.

Mesalazine(5-Aminosalicylic acid,5-ASA), a component of Sulfasalazine (sulfapyridine bound to 5-ASA), is used to treat inflammatory bowel disease. Most adverse pulmonary effects caused by sulfasalazine have been attributed to sulfapyridine. However, lung toxicity associated with mesalazine(5-ASA) is rare. Here we report a case of eosinophilic pneumonia in a 44-year-old woman with ulcerative colitis, who was treated with mesalazine. She developed symptoms of a dry cough, mild night fever, and exertional dyspnea. Bilateral peripheral pulmonary infiltrates, peripheral blood eosinophilia, and histologic features were consistent with eosinophilic pneumonia. The symptoms improved quite rapidly after the discontinuation of mesalazine and initiation of steroid therapy. Therefore, adverse pulmonary reactions to mesalazine must be considered in a differential diagnosis of pulmonary involvement in patients with inflammatory bowel disease who are receiving with mesalazine therapy.
In the southern sea of Korea, the culturing Rock bream, Oplegnathus fasciatus, ranging from 16.3 to 20.1cm in body length were sampled to examine how serious parasitic infection is. As a result, they were infected with 10 to 31 individuals of Monogenea, Benedenia sp. The parasitic sites of fish body was trunk (81.8%), caudal peduncle (16.5%) and the other portion (1.6%), and where ulceration and bleeding were observed with a large amount of muscus. When the treatment with formalin and freshwater were performed to get rid of the parasite, they were exterminated after 8 and 20 minutes in 100% and 70% freshwater, respectively and were killed after 10, 15 and 20 mimutes in the seawater added with formalin to be 250 ppm, 200 ppm and 150 ppm, 200 ppm and 150 ppm, respectively. During the treatment for parasite extermination, the fish showing a serious illness were dead in the 100% freshwater, whereas toxicity caused by formalin treatment was not detected in this study. The former is probably due to failure in the osmotic regulation of Rock bream.
Purpose: The purpose of this study was to investigate treatment types and the status of referral to home care services for patients with diabetic foot. Methods: A retrospective survey was conducted by reviewing medical records from January to December in 2008 at a university hospital. The subjects were 76 patients at the age of 20 years or older who were admitted, had home care services, or received outpatient care for diabetic foot. The data were analyzed using descriptive statistics. Results: Among the total of 9,317 patients diagnosed with diabetes, 5.03% (n=469) had diabetic foot. Admission (81.6%) was the most frequently used treatment type followed by outpatient care only (7.9%) and hemodialysis only (10.5%). Of the 76 admission cases, 44.9% received post-discharge care at outpatient clinics, 20.5% had both outpatient and home care services, and 16.7% were transferred to other hospitals. Readmission rate after discharge was 15.6% for one year. Conclusion: This study suggests referral to home care services should be encouraged to provide effective follow-up care to patients with diabetic foot after discharge from a hospital.
This study was to survey the relationship between employment stress and self-rated oral health status in university students from Sep. 2 to 27, 2019. Data were analyzed using the SPSS 22.0 program. In case of high employment stress, the comparative risk of self-rated oral health status was 1.7 times in Gingiva swelling, 1.46 times in ulcer and 1.53 times in hypersensitivity. After correcting the general characteristics (gender, age, grade, major), the comparative risk was 1.56 times for Gingiva swelling, 1.42 times for ulcer and 1.44 times for hypersensitivity. Therefore employment stress was found to be related to oral health and it is considered necessary to find ways to prevent and manage oral health by seeking various methods such as counseling support or stress control to reduce employment stress.
Xanthomonas arboricola pv. pruni is the causal agent of bacterial black spot disease on some stone fruits, e.g. peach, plum and apricot. To evaluate pathogenicity of Xanthomonas arboricola pv. pruni strain from plum, inoculum of the isolated strain was spray inoculated to fruits and leaves of apricot, Japanese apricot and plum. Apricot and Japanese apricot showed severe black spot symptoms on fruits and shot hole symptoms on leaves. In case of apricot, about fifty percent of fruits did not grow and dropped by hypersensitive reaction to spray infection. Plum, cv. Formosa was very susceptible, showing severe black injury lesions on fruits and cankers on leaves and new twigs. On the other hand, plum cv. Daesukjosaeng, was highly resistant. Fruits of several plum cultivars such as Formosa and Chuhee were severely infected at natural infected orchards by X. arboricola pv. pruni. Where as those of Daesukjosaeng, Taeyang, Soldam and Hongrogen were moderately infected.

The Efficacy of SHI-1909 was investigated in comparision with predinisolone in acetic acid and Picrylsulfonic acid solution (TNBS)-induced rat inflammatory bowel disease (IBD) for 5 days. 7% Acetic acid and 5% TNBS solution were administered with polyethylene (P.E) tube inserted to rats intracolon, which causing colitis to the rats. The acetic acid and TNBS control group (the saline treated colitic rat) exhibited ulceration and inflammation of the distal colon with formation of granuloma and pathologic connections. We checked the inflammatory parameters like rat's weight, food intake quantity change during administration. After 5 days, we sacrificed the rats and checked the colon's length, ulcer and pathologic condition. Oral treatment with SHI-1909 resulted in significant recovery of macroscopic parameters like weight and diet intake change. Especially, SHI-1909 had a more potent effect than prednisolone on macroscopic colonic damage score. We can suggest that SHI-1909 could be a promising drug in the treatment of IBD.

Crohn's disease, which is recently increasing in Korea, may be seen throughout the gastrointestinal tract and cause various symptoms. In particular, Crohn's disease is especially difficult to diagnose with several symptoms similar to other ulcerative colonic diseases. Thus, some studies are underway to distinguish two or more similar diseases. However, the previous studies have not described the procedural diagnosis process of it, which may lead to over-examination in the process. Therefore, we propose a diagnosis process of Crohn's disease based on the analysis of redundancy, sequential linkage and decision point in the diagnosis of Crohn's disease, so that it enables to identify ulcerative colonic diseases with symptoms similar to Crohn's disease. Finally, we can distinguish the colon diseases that have symptoms similar to Crohn's disease and help diagnose Crohn's disease effectively by defining the proposed process-oriented association as an ontology. Applying the proposed ontology to 5 cases showed that more accurate diagnosis was possible and in one case it could be diagnosed even with fewer tests.
Purpose: This study aimed to identify the risk factors for diabetic foot ulceration (DFU) to develop and evaluate the performance of a DFU prediction model and nomogram among people with diabetes mellitus (DM). Methods: This unmatched case-control study was conducted with 379 adult patients (118 patients with DM and 261 controls) from four general hospitals in South Korea. Data were collected through a structured questionnaire, foot examination, and review of patients' electronic health records. Multiple logistic regression analysis was performed to build the DFU prediction model and nomogram. Further, their performance was analyzed using the Lemeshow-Hosmer test, concordance statistic (C-statistic), and sensitivity/specificity analyses in training and test samples. Results: The prediction model was based on risk factors including previous foot ulcer or amputation, peripheral vascular disease, peripheral neuropathy, current smoking, and chronic kidney disease. The calibration of the DFU nomogram was appropriate (χ2 = 5.85, p = .321). The C-statistic of the DFU nomogram was .95 (95% confidence interval .93~.97) for both the training and test samples. For clinical usefulness, the sensitivity and specificity obtained were 88.5% and 85.7%, respectively at 110 points in the training sample. The performance of the nomogram was better in male patients or those having DM for more than 10 years. Conclusion: The nomogram of the DFU prediction model shows good performance, and is thereby recommended for monitoring the risk of DFU and preventing the occurrence of DFU in people with DM.
