Factors Associated with the Diabetes Self-care Activities of the Elderly with Type 2 Diabetes (노인 당뇨환자의 당뇨 자가관리와 관련요인)

  • The aims of this study are to identify levels of diabetes self-care activities and find associated factors of the elderly with Type 2 diabetes. The participants for this study were 264 elderly Koreans over 65 years old who visited one general hospital in S city in South Korea for their medical treatment. In the results, there are few statistically significant differences in general characteristics and disease related variables according with diabetes self-care activities (DSCA). DSCA showed negative correlation with depression (r=-.278, p=.033) and positive correlation with ADLs(r=.310, p=.013)and IADLs(r=.415, p=.001). Therefore, the findings of this study suggest implementing self-care activities to the elderly with diabetes based on their psychological factors (depression) and functional capacities (ADL and IADL). In addition, the advanced research will be needed for developing tailored nursing educations and interventions to improve diabetes self-care activities of elderly patients.

Perception about Shared Decision Making of Family Caregivers of Early Dementia Patients: A Qualitative Content Analysis Study (초기 치매환자 가족 돌봄제공자의 공유 의사결정에 대한 인식: 질적 내용분석 연구)

  • The purpose of this study was to explore perception about shared decision making of family caregivers of patients with early dementia (PWED). This study was conducted with a sample of 12 family caregivers (mean age = $71.4{\pm}10.4$) of PWED from three dementia safety centers in Seoul. In-depth interviews were done for each participant about shared decision making and data were analyzed using qualitative content analysis. Six categories and 17 sub-categories identified for participants' perception about shared decision making: means to facilitate communication with patients with dementia, means to secure autonomy of patients, opportunity to facilitate treatment, cause of increasing family caregivers' burden, cause of worsening relationship with patients, and option for choices depending on priority change. The findings of this study can provide a knowledge basis for health care professionals and policy makers to understand how family caregivers of PWED think about shared decision making. It would be of great value to develop educational programs and practical guidelines about shared decision making for PWED and their family, which may contribute to respecting PWED's self-determination right as well as reducing burden of their family.

Nursing Needs for Elderly Patients with Regional Anesthesia during Operation (부위마취 수술 노인환자의 수술 중 간호요구)

  • Purpose: The purpose of this study was to identify the nursing needs of elderly patients who got a surgical operation under regional anesthesia(spinal, epidural, nerve block). Methods: The participants were 126 elderly patients who completed a questionnaire. The data were collected October 1 and October 31, 2012, and analyzed using descriptive statistics, t-test and ANOVA, $Scheff{\acute{e}}$ test. Results: The mean score for the nursing needs of elderly patients administered regional anesthesia during surgery was $3.08{\pm}0.38$, The scores for specific nursing needs were as follows: educational needs ($3.47{\pm}0.50$), spiritual needs ($3.37{\pm}0.78$), physical needs ($3.31{\pm}0.46$), emotional needs ($2.72{\pm}0.50$), and environmental needs ($2.51{\pm}0.47$). There were significant differences in nursing needs relative to gender, religion, and spouse status. Additionally, there were significant differences in nursing needs according to surgery department, the length of time the surgery, and the ASA(American Society of Anesthesiologists) physical classification. of the operation-related characteristics. Conclusion: When caring for elderly patients during the surgery, nurses must provide adequate information about the surgery and anesthesia. Further studies are needed to develop and evaluate nursing interventions to provide quality surgical care for the elderly patients.

Comparison of Effects of Perceived Stress and Coping Patterns on Depression between Cancer Patients and Healthy Adults (암환자와 건강한 성인의 스트레스 지각과 대응양상이 우울에 미치는 효과 비교)

  • Purpose: This study was undertaken to compare perceived stress and coping patterns, and their effects on depression between cancer patients and healthy adults. Methods: A descriptive design was used with 278 subjects, consisting of 139 cancer patients and 139 health adults, living in an urban area. All participants completed the following prerequisites; Stress Visual Analog Scale, Ways of Coping Checklist (W.C.C.L), Depression Index (CES-D), and Demography and Disease Data Questionnaire. Results: Perceived stress and depression were significantly higher, while wishful thinking was lower among cancer patients than healthy adults, after adjusting for education and family income which were not homogeneous between the groups. Among cancer patients, 20.2% of depression was mainly due to seeking support (10.7%), perceived stress and education, while 30.7% of depression in healthy adults was due to perceived stress, problem solving, and seeking support. Conclusion: These findings suggest that interventions to manage depression in cancer patients should include strategies to best foster positive coping patterns and reduce perceived stress.

Comparison of Safety Perception between Patients and Nurses and Factors Affecting Nurses Safety Management Activities in Tertiary Hospitals (상급종합병원 환자와 간호사의 안전 인식 비교와 간호사의 환자안전관리 직무수행 영향요인)

  • This study is a descriptive research study to compare the safety perception between patients and nurses and to identify the factors affecting the nurses' safety management activities in tertiary hospitals. Data were collected with structured questionnaires from 147 patients and 147 nurses from a university hospital in J city, Gyeongnam, from July 24 to September 23, 2019, and analyzed using SPSS 23.0. The result showed that the patient's safety perception was significantly higher than the nurse's perception. The significant factors affecting patient management activity were nurse's educational level, position, effective communication, patient safety perception, and perception of importance on patient safety management. In the multiple regression analysis, knowledge about 'effective communication', and 'concerned about patient safety management', explained 54.8% of the nurses' patient management activity. Based on the results of this study, it is emphasized that nurses need to develop strategies to improve the communication and attention between patients and hospital members to improve patient safety management activities in tertiary hospitals.

Hemodialysis Patients Experience in Food Craving (혈액투석 환자의 음식갈망 경험)

  • This study was attempted to understand the phenomenon in depth by exploring the food craving of hemodialysis patients experienced during hemodialysis. Participants selected seven adult patients diagnosed with end-stage renal failure and receiving hemodialysis treatment from medical institutions. he collected data were analyzed using a phenomenological analysis method. As a result of the analysis the food craving of hemodialysis patients was derived into two categories included craving experience in the cognitive domain craving experience in the emotional domain and five themes: 'Looking for foods that are water and watery as a result of thirst','Food thoughts come to mind all day as a result of a limited diet', 'Always hungry', 'Faced with appetizing situations every hour', 'experiencing negative emotions due to failure to control meals'. The results of this study which sheds light on food craving from the perspective of hemodialysis patients should improve the understanding of hemodialysis patients appetite control water restrictions and dietary compliance and allow them to understand the food craving attributes of hemodialysis patients and provide customized education optimized for that extent when applying dietary education and nursing interventions suitable for them.

Nurses' Perception on Fluid Therapy for Terminally Ill Patients (말기 환자의 수액요법에 대한 간호사의 인식)

  • Purpose: This study was conducted to investigate how nurses who take care of terminal patients perceive fluid therapy and how this therapy is currently being used in hospitals. Methods: This survey included 200 nurses, 87 of whom were working in the oncology units of 3 university hospitals in Seoul, Korea, and 113 were working in 18 hospice centers. The data for this study were collected by means of structured questionnaires and analyzed by using the Statistical Analysis System software. The differences in perception towards fluid therapy between nurses working in oncology units and those working in hospice centers were examined using the $x^2$ test and analysis of covariance. Results: Fluid therapy was perceived more negatively by the nurses from hospice centers than by those from oncology units. Continuous subcutaneous infusion was used in hospice centers, but not in oncology units. In addition, the average amount of fluid infused daily differed significantly between the oncology units and hospice centers. Conclusion: Our results show that there were differences in the perception towards fluid therapy between nurses in different clinical settings. Nurses caring for terminal and palliative care patients should not simply provide or withhold fluid therapy, but rather develop a wider range of views on fluid therapy, focusing on effective alternative interventions.

The Dementia Knowledge, Attitude and Preventive behaviors of Hemodialysis patients (혈액투석 환자의 치매에 대한 지식, 태도 및 예방행위)

  • The purpose of this study was to identify the dementia knowledge, attitudes and preventive behaviors of hemodialysis patients. The participants were 128 hemodialysis patients at the Artificial Kidney Center in G hospital of P city. Data were collected by self-report questionnaires from September 22 to October 10, 2017. Data were analyzed with descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient and Hierarchical regression analysis with SPSS/WIN 23.0. Dementia knowledge was significantly different by education level (F=4.58, p<.01). The attitude toward dementia was different by age (F=3.08, p<.05) and education level (F=6.64, p<.01). The preventive behaviors were different by gender (t=-2.30, p<.05). Dementia knowledge was positively correlated with attitude toward to dementia (r=.189, p<.05) and attitude toward dementia was positively correlated with preventive behaviors (r=.280, p<.01). The influencing factor for preventive behaviors was attitude toward dementia (${\beta}=.319$, p<.01). Therefore, in order to promote dementia preventive behaviors in hemodialysis patients, an education program should be developed to improve dementia knowledge and positive attitude toward dementia.

  • Purpose : To assess parental knowledge and understanding of epilepsy including social stigma and evaluate the effects of educational programs on parents in an epilepsy camp. Methods : We conducted an epilepsy camp on August 23-24, 2008, at Gyeongsan. Twenty families with an epileptic child each participated in the camp. We performed a survey before and after the camp to obtain data concerning parental understanding of epilepsy and the effects of the camp-based educational programs on the parents. The data were analyzed with SPSS 14.0 using frequency analysis. Results : The parents were sufficiently knowledgeable about epilepsy before the camp, and their knowledge and understanding of epilepsy improved further after the camp. Both perceived stigma and enacted stigma against epilepsy were found in 30.0% of the parents. The perceived stigma decreased to 15.0% after the camp. Conclusion : This study suggests that social approaches such as epilepsy camps are effective in improving parental knowledge and understanding about epilepsy as well as decreasing their perceived social stigma.

Relationship between Job Stress and Mental Health of Caretakers (간병인의 직무스트레스와 정신건강과의 상관관계)

  • The purpose of this study is to explore the relationship between job stress and mental health of caretakers by SCL-90-R. This study are to offer basic information for patients the quality of service interventions. The sample for the study consisted of 247 caretakers who work in Busan. The results of this study are as follows: 1) Caretakers' mental health and job stress was not serious level. 2) There was an important factor in job stress, which was suitable pay. 3) Caretakers' job stress was positively related to mental health. There is a statistical correlation between the relationship with mental health and job stress. 4) Caretakers' mental health was affected by job stress level. Therefore, We need training and program that focuses on management job stress in caretakers.
