Heart Rate Variability in Patients with Coronary Artery Disease (관상동맥질환 환자의 심박동변이도)

  • This study is based on previous information regarding reduced cardiac vagal activity in patients with coronary artery disease(CAD), on reduced variance(SDNN : standard deviation of all normal RR intervals), low-frequency power(LF), and the complexity of heart rate variability(HRV) in patients with chronic heart failure(CHF), and on the normalized high-frequency power of HRV is the highest in the right lateral decubitus position among 3 recumbent postures in patients with CAD, However, nothing is known about the nonlinear dynamics of HRV for the 3 recumbent postures in patients with CAD. To investigate the linear and non-linear characteristics of HRV in patients with CAD, 29 patients as CAD group and 23 patients as control group were studied. Electrocardiogram(ECG) with lead II channel was measured on these patients for 3 recumbent postures in random order. The HRV from ECG was analyzed with linear method(for time and frequency domains) and nonlinear method. The lower the high-frequency power in normalized unit(nHF) in the supine or left lateral decubitous position, the higher the increase in nHF when the position was changed from supine or left lateral decubitous to right lateral decubitous. Among the 3 recumbent postures in patients with severe CAD, the right lateral decubitus position was observed to induce the highest vagal modulation, the lowest sympathetic modulation, and the highest complexity of human physiology system.

Dipyridamole Induced Transient Left Ventricular Dysfunction in the Tl-201 Gated Myocardial SPECT (Tl-201 게이트 심근 SPECT에서 디피리다몰 부하로 유도된 일시적 좌심실 기능이상)

  • Purpose: Recently the occurrence of dipyridamole stress-induced short term stunning was proven and it is reported that Bland Altman analysis by repeated acquisition Tl-201 gated myocardial SPECT (gSPECT) revealed the 95% limit of agreement for LVEF was 10.3 %. The purpose of this study was to investigate the clinical value of dipyridamole induced transient LV dysfunction on Tl-201 gSPECT. Materials and Methods: Total 93 patients were included and coronary angiography was peformed in all patients less than 2 month from gSPECT. The patients with myocardial infarction were excluded. All patients underwent both dipyridamole stress and 4-h redistribution Tl-201 gSPECT. Forty nine patients of total 93 showed normal coronary arteries (Group 1) and the remaining 44 patients had coronary artery disease (Group 2). We compared LV EF, EDV and ESV during post-stress and 4-h redistribution period calculated by gSPECT using quantitative gated SPECT software and the incidence of dipyridamole induced transient LV dysfunction between group 1 and 2. The criteria for transient LV dysfunction was defined more decrease ${\geq}11%$ of LVEF during post-stress than 4-h redistribution according to previous reported Bland Altman analysis. Results: During post-stress and 4-h redistribution average of 3.1% increment in LVEF, 6.6% increment in LVEDV and 0.7% decrement in LVESV were shown after stress in Group 1, whereas 4.1% decrement, 9.7% increment and 7.2% increment in Group 2 respectively. Dipyridamole induced transient LV dysfunction was only detected in group 2 (18.2%) and not in group 1. It was more frequently observed in triple vessel disease and left main disease (31.8%, N=22) than one and two vessel disease (4.5%, N=22). Conclusion: As with Tc-99m myocardial agent post-stress LV dysfunction was observed in dipyridamole Tl-201 gSPECT. It was only detected in CAD and more frequently occurred in multivessel disease. Thus this finding seems to provide additional information in the diagnosis of coronary artery disease and prediction of prognosis.

Development of Image Quality Measurement Method of Coronary Angiography Using Image Analysis Program (영상 분석 프로그램을 이용한 관상동맥 혈관 조영상의 화질 측정 방법 고안)

  • Research should be actively conducted for the ability of X-ray equipment and Retrospective image analysis of X-ray equipment used in hospitals. Retrospective image analysis of X-ray machines, CT and MRI of radiology and medical equipment has been actively conducted. However, image quality measurement using angiography equipment of angiography room is mostly measured with phantom, and image quality measurement on image after being taken by actual patient is insufficient and researches on accurate image quality measurement method are remarkable. It is in short supply. Therefore, through this study, the researcher devised a method to measure the image quality of the acquired image after coronary angiography, and to provide a high quality image to the operator. The equipment and programs used were angiographic examination equipment (Axiom Artis Zee Ceiling) and Image J program. Subjects were images automatically saved in PACS program after coronary angiography.For image quality measurement, selected the AP Caudal 30° image that show the LCA vessel well and the LAO 30° image that show the RCA vessel well during the coronary angiography. In order to measure the background and ROI of the selected image by selecting an image, a criterion on how to find and measure a section where the overlap of the shadow, such as blood vessel, liver and lung is minimized, is presented. In conclusion, there is no exact standard for analyzing an image quality measurement method of angiography image. Therefore, in order to provide quality images to the practistioners, not only the technicians of the equipment but also the users who actually use them should become researchers and conduct research on image quality measurement in various ways. Thus, it is expected to provide excellent images to patients.

Relationship between Uncertainty, Sick Role Behaviors, and Quality of Life of Rehospitalized Patients underwent Percutaneous Coronary Intervention (관상동맥중재술을 받고 재입원한 환자의 불확실성, 환자역할행위 및 삶의 질의 관계)

  • Purpose: This study was conducted to investigate the relationship between uncertainty in illness and the future, sick role behavior with what diet, weight control, no smoking, abstinence, doctor visits, medications, etc, and quality of life of rehospitalized patients after percutaneous coronary intervention in a cardiology ward. Methods: A total of 120 patients participated in the study. Data were collected using a questionnaire and analyzed using t-test, ANOVA, $Scheff{\grave{e}}$ test, and Pearson's Correlation Coefficient. Results: The mean score for uncertainty was $3.45{\pm}1.08$. Sick role behavior of the patients showed a moderate value with a mean of $3.68{\pm}0.79$. The mean score for quality of life was $3.52{\pm}0.64$. Uncertainty in illness and the future was significantly correlated to sick role behavior with that diet, weight control, no smoking, abstinence, doctor visits, medications, etc (r=-.27, p=.002), and quality of life (r=-.35, p<.001), and sick role behaviors were significantly correlated to quality of life (r=.62, p<.001). Conclusion: The results implicate that there is a need to decrease the levels of uncertainty and reinforce positive behaviors by patients in order to improve their quality of life.

Effect of Education and Counselling-based Cardiac Rehabilitation Program on Cardiovascular Risk, Health Behavior and Quality of Life in Elderly with Coronary Artery Disease (교육상담 기반 심장재활 프로그램이 노인 관상동맥질환자의 심혈관 위험도, 건강행위 및 삶의 질에 미치는 효과)

  • Purpose: The purpose of this study was to investigate the effect of education and counselling-based cardiac rehabilitation program on cardiovascular risk, health behavior and quality of life in elderly with coronary artery disease. Methods: A quasi-experimental study used a non-equivalent control group pre-post test design. A five week education and counselling-based cardiac rehabilitation program for the elderly with coronary artery disease was developed and offered to the experimental group. Participants were drawn from hospital in Busan. Twenty two were selected for the experimental group while 22 were assigned to a control group. Cardiovascular risk, health behavior and quality of life were measured. Results: There was a statistically significant difference in the cardiovascular risk(U=118.5, p=.002), health behaviors(t=5.200, p=<.001) and quality of life(t=2.431, p=.001) between the experimental group and the control group. Conclusion: Education and counselling-based cardiac rehabilitation program can be not only an effective nursing intervention for old patients having coronary artery disease, but also the very basis of further research on aged people who have the same symptom.

  • Purpose: This study investigated the mediating effect of perceived stress in the relationship between subjective health and depression in patients with coronary artery disease. Methods: This secondary analysis study used data from the 2016-2018 Korea National Health and Nutrition Examination Survey (KNHANES). Study participants included 466 patients over 20 years of age with coronary artery disease. Measures included questions about general characteristics, subjective health, perceived stress, and depression. The data were analyzed using complex sample design, descriptive analysis, Rao-Scott ��2 statistic, and multivariate logistic regression analysis with the SPSS 24.0 program. Results: Subjective health influenced depression, and perceived stress partially mediated it. Conclusion: To prevent depression in patients with coronary artery disease, it is necessary to develop effective stress management strategies.

Sleep Quality and Factors affecting Sleep Disturbance among Percutaneous Coronary Angiography Patients in a General Hospital (종합병원의 경피적 관상동맥조영술 후 환자의 수면의 질과 수면방해요인)

  • Purpose: The purpose of this study was to investigate the variance of sleep quality and factors affecting sleep disturbance among patients with percutaneous coronary angiography in a general hospital. Methods: The subjects of this study was comprised of 101 patients with percutaneous coronary angiography in a general hospital located in B city. Data were collected by a structured questionnaire from November to December 2013. The data were analyzed with descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient and stepwise multiple regression. Results: The levels of the participants' sleep quality and sleep disturbance were 4.35 and 5.91 in order of each out of the total score, 10. There was no significant difference in the quality of sleep between intensive care unit and internal medicine unit. Factors influencing sleep quality were 'uncomfortable bed, gown and pillow'(${\beta}=-.279$, p=.003), 'living alone'(${\beta}=-.273$, p=.003), and 'toilet problem' (${\beta}=-.222$, p=.016), which explained 21.5% of the variance (F=10.03, p<.001). Conclusion: The results of this study will provide a basis for improving the sleep quality of patients with percutaneous coronary angiography.

A Comparison of Quality of Life between Patients with 12 Months after Coronary Artery Bypass Graft Surgery and General Population (관상동맥 우회술 후 12개월 환자와 일반인의 삶의 질 비교)

  • Purpose: The purpose of this study was to examine quality of life with patients in 12 months after coronary artery bypass graft surgery, compared with general population. Method: A cross-sectional comparative study was designed. Study participants were 90 adults who had undergone CABG surgery (n=45) in experimental group and general population (n=45) in control group. The subjects were asked the questionnaire (SF-36, Short Form-36) regarding the quality of life. The data were analyzed by the SPSS 13.0 program including frequency, percentage, $x^2$ test, t-test and One-Way ANOVA. Results: There were no significant differences in the quality of life between two groups including physical functioning (t=-.938, p=.351), role-physical limitation (t=-.322, p=.748), bodily pain (t=-.938, p=.351), general health (t=-.1418, p=.888), vitality (t=-.816, p=.417), social functioning (t=.720, p=.474), role-emotional limitation (t=-.710, p=.479), mental health (t=-.431, p=.667). Conclusion: Even though patients in experimental group had operation experience, the quality of life examined on the time of 1 year after bypass surgery is similar in that of general population. We suggest the study change the design as examination of quality of life before and after coronary artery bypass graft surgery.

Predictors of Intention to Quit Smoking among Patient with Coronary Heart Disease (관상동맥질환자의 금연의도에 영향을 미치는 요인)

  • The purpose of this study was to examine the level of intention to quit smoking and to identify factors influencing intention to quit among patients with coronary heart disease. Method: The subjects consisted of 80 male patients with coronary heart disease (angina pectoris, myocardial infarction) at three hospitals in Seoul. The data were collected with self reporting in a structured questionnaire. Stepwise multiple regression was used to identify predictors of intention to quit. Included variables were attitudes toward smoking cessation, subjective norms, perceived behavioral control, usefulness of smoking cessation, and previous attempts to quit. Result: 1. The mean score for intention to quit was 11.1($\pm$6.1) which was lower than median score of the scale. 2. There were significant correlations between the all predictive variables and the intention to quit(r=.24-.48, p<.05). 3. usefulness of smoking cessation, perceived behavioral control, and previous attempts to quit explained 34.6% of the variance for intention to quit. Conclusion: usefulness of smoking cessation, perceived behavioral control, and previous attempts to quit were identified as important variables in explaining the intention to quit smoking among patients with coronary heart disease. Thus, it is necessary to try to enhance this factors for increasing intention to quit among patients with coronary heart disease.
