A new protocol of the sliding mechanics with Micro-Implant Anchorage(M.I.A.) (Micro-Implant Anchorage(MIA)를 이용한 Sliding mechancis)

  • Anchorage plays an important role in orthodontic treatment. Because of limited anchorage Potential and acceptance problems of intra- or extraoral anchorage aids, endosseous implants have been suggested and used. However, clinicians have hesitated to use endosseous implants as orthodontic anchorage because of limited implantation space, high cost, and long waiting period for osseointegration. Titanium miniscrews and microscrews were introduced as orthodontic anchorage due to their many advantages such as ease of insertion and removal, low cost, immediate loading, and their ability to be placed in any area of the alveolar bone. In this study, a skeletal Class II Patient was treated with sliding mechanics using M.I.A.(micro-implant anchorage). The maxillary micro-implants provide anchorage for retraction of the upper anterior teeth. The mandibular micro-implants induced uprighting and intrusion of the lower molars. The upward and forward movement of the chin followed. This resulted in an increase of the SNB angle, and a decrease of the ANB angle. The micro-implants remained firm and stable throughout treatment. This new approach to the treatment of skeletal Class II malocclusion has the following characteristics . Independent of Patient cooperation. . Shorter treatment time due to the simultaneous retraction of the six anterior teeth . Early change of facial Profile motivating greater cooperation from patients These results indicate that the M.I.A. can be used as anchorage for orthodontic treatment. The use of M.I.A. with sliding mechanics in the treatment of skeletal Class II malocclusion increases the treatment simplicity and efficiency.

  • Treatment of adult patients with Class III malocclusion frequently requires a combined orthodontic and surgical approach. However, if for various reasons, nonsurgical orthodontic treatment is chosen, a stable outcome requires careful consideration of the patient's biologic limitation. This case presents the orthodontic treatment of an adult with a Class III malocclusion, which was treated nonsurgically using indirect skeletal anchorage.

  • The maxillary protraction headgear has been widely used in the treatment of skeletal Class III children with maxillary deficiency. A variety of treatment objectives which allow dentoalveolar movements may be established, but when only maxillary protraction without dentoalveolar movement is needed, one of the limitations in maxillary protraction with conventional tooth-borne anchorage is the loss of dental anchorage. This is because a bone remodeling occurs not only at circummaxillary sutures but also within the periodontal tissues. During protraction treatment in the mixed dentition phase, in older children or for the patient with multiple congenitally missing teeth, it is not uncommon to observe undesirable mesial movement of maxillary teeth. Such a side effect can be eliminated or minimized using absolute anchorage such as skeletal anchorage. The purpose of this case report is to introduce a new technique of the maxillary protraction headgear treatment using surgical miniplates.

  • Decalcification of the buccal surface of the teeth often occurs during fixed orthodontic treatment. This case report describes two cases in which cervical decalcificated teeth that occurred during orthodontic treatment were treated with direct resin veneer restoration. Early lesions without caries can be remineralized through periodic fluoride application, diet control, and oral hygiene improvement. As it progresses, appropriate repair treatment is required, and it is more preferable to focus on prevention rather than treatment after the occurrence of the lesion.

  • The purpose of this study was to observe the tissue response to the orthodontic force applied immediately after buccal horizontal subapical osteotomy of maxilla. Five mongrel cats weighing about 2.5 Kg were used for this experiment. The left upper canine and premolar of each cat served as a experimental side and the right ones as a control side. On the experimental side, a 100gm orthodontic force was applied between the upper canine and premolar immediately after the osteotomy. On the control side, the same orthodontic force was applied without the osteotomy. After 7days, the experimental animals were sacrificed. The results were as follows: 1. In the pressure zones of the experimental group, significant increase of osteoclasts and direct resolution along the alveolar bone surface was observed as compared with those of the control group. 2. In the pressure 2ones of the experimental group, a less extensive hyalinized zone was observed than those of the control group. 3. In the pressure zones of both group, no root resorption was found. 4. Tn the tension zones of the experimental group, new bone deposition increased along the alveolar bone surface as compared with those of the control group. In conclusion, the results suggest the possibility that early orthodontic treatment after orthognathic surgery may have some benefits if the stability of the repositioned segment at surgery is secured.

Non-surgical treatment and retention of open bite in adult patients with orthodontic mini-implants (성인 환자에서 구치부 압하를 통한 개방교합의 치료와 유지)

  • Successful treatment of the non-growing patient with an open bite of either dental skeletal pattern often presents a difficult challenge. The morphologic pattern in anterior open bite is characterized by longer vertical dimensions, an increase in development of the maxillary posterior dento-alveolar structure and a steep mandibular plane. In such cases, molar intrusion would be a good remedy for treatment. This article reports the successful treatment and retention of two anterior open-bite cases. We used orthodontic mini-implants for treatment and a circumferential retainer with posterior bite block or skeletal fixed retainer for retention. The diagnostic criteria and mechanics for appropriate treatment are discussed. Our results suggest that open bite can be reduced successfully with intrusion of molars using orthodontic mini-implants (OMI) without orthognathic surgery, and that circumferential retainer with posterior bite block and skeletal fixed retainer are effective for retention.

A STUDY ON THE CHANCE OF STREPTOCOCCUS MUTANS IN SALIVA BY CARIESCREEN AFTER USE OF $0.05\%$ NAF IN ORTHODONTIC PATIENTS (고정성 교정장치 장착 환자에서의 NaF 양치액 사용시 Cariescreen을 이용한 Streptococcus mutans 변화에 관한 연구)

  • The purpose of this study was to evaluate the effectiveness of gargling solution with 0.05% NaF and 10% Xylitol in orthodontic patients with fixed appliance. The sample consisted of 30 adolescent patients who were classified into an experimental group and a control group, 15 patients each. Experimental group was used experimental gargling solution and the control group was used with placebo solution. The change of S. mutans in saliva was observed by $Cariescreen^R$ SM kit at pre and post 2, 4, 6, 8 weeks. The results were as follows 1. There were significant reduction in the number of S. mutans in saliva between pre and post 2 weeks(p<0.01), 4 weeks(p<0.05), 6 weeks(p<0.001), and 8 weeks(p<0.001) in experimental group. And significant reduction also were observed in the number of S. mutans in saliva between post 2 weeks, 6 weeks(p<0.05), and 8 weeks(p<0.05), but no significant reduction were showed in control group. 2. There were significant correlation in the number of S. mutans between each measurement time(pre and post 2, 4, 6, 8 weeks) in control group. 3. There were no correlation between pre and post 2 weeks, but significant correlation were observed between 2, 4, 6, 8 weeks in experimental group.

  • Delayed eruption disorders caused by systemic or local conditions are mostly found during childhood and can be treated with orthodontic forced eruption. When the disorder is not found nor treated during childhood, however, orthodontic eruption might become a difficult option while prosthodontic restoration can be considered as an another option. Considerations for the prosthodontic treatment plan include the extent of tooth loss, interdental mesio-distal space and interarch space, and age of the patient. In this case report, oral rehabilitation of the patient with delayed eruption disorder through zirconia partial fixed prostheses for both maxilla and mandible was performed.

  • Anchorage control in orthodontic treatment is an important factor affecting treatment results. In the conventional approach, intra-oral anchorage such as application of differential force and moment, Nance holding arch and lingual arch, as well as extra-oral anchorage such as head gear were used for anchorage reinforcement. However, these anchorages may result in undesired tooth movement and require patient cooperation. To overcome these disadvantages, skeletal anchorage system was introduced as orthodontic anchorage. Types of skeletal anchorage include implant, onplant, miniplate and miniscrew. Especially, miniscrew has many advantages such as reduced patient cooperation, low cost and easy placement. Recently, it is successfully used in orthodontic treatment. This cases were treated using orthodontic miniscrews for retraction of ectopically erupting maxillary canine and impacted mandibular canine and intrusion of maxillary incisors.
