PSEUDOHYPOPARATHYROIDISM : CASE REPORT (가성부갑상선 기능저하증 환아의 구강 증상)

  • The parathyroid hormone plays a major role in the metabolism of calcium and phosphorus. In hypoparathyroidism the parathyroid glands are atrophied or absent associated with autoantibodies against parathyroid tissue. Pseudohypoparathyroidism is a metabolic disease caused by the disturbance in peripheral action of parathormone, but parathormone level is normal. In general, patients with pseudohypoparathyroidism have short stature, round face, brachydactylia, obesity, mental retardation, cataracts & ectopic calcifications on soft tissues. Dental manifestations are enamel hypoplasia, delayed eruption, blunting of root apex, hypodontia, pulp calcification, thickened lamina dura, excessive caries & malocclusion. In this case, intraoral examination showed enamel hypoplasia on the erupted permanent teeth & hypermobility on the remaining deciduous teeth. From the radiographic view severe dental anomalies were observed on canines and shortening and blunting of root apex was observed on mandibular incisors. Pathologic root resorption was also observed on deciduous teeth.

  • Sleep Apnea Syndrome is characterized by hypersomnolence, snoring, and sleep apnea. The symptoms of hypothyroidism include apathy, somnolence, lethargy, personality change, and intellectual deterioration and may be related to hypothyroid-related sleep disorders. Central, obstructive, and mixed patterns of sleep apnea may be observed in hypothyroidism. A 60-yr-old man was admitted due to hypersomnolence, snoring and generalized edema. Polysomnogram showed obstructive pattern of sleep apnea syndrome. Neck CT scan revealed narrowing of upper airway which is consistent with obstructive sleep apnea syndrome. Physical examination and hormonal study also disclosed the evidence of hypothyroidism. We report a case of sleep apnea syndrome due to primary hypothyroidism with review of literatures.

  • The hallmarks of sleep apnea syndrome include daytime hypersomnolence, snoring, obesity, disturbed nocturnal sleep, and apneic episodes. Thyroid deficiency states are now a well recognized cause of sleep apnea. Central, obstructive, and mixed patterns of sleep apnea may be observed in hypothyroidism. A-33-yr-old man was admitted complaining of daytime hypersomnolence, snoring, sleep apnea and obesity. A polysomnogram showed obstructive pattern of sleep apnea syndrome. Physical examination and hormonal study revealed a narrowing of the upper airway and primary hypothyroidism. Dilated cardiomyopathy was diagnosed by echocardiogram. The poctients was treated for obstructive sleep apnea syndrome with nasal continuous positive airway pressure(CPAP) for 2 months and the underlying hypothyroidism with thyroid hormone therapy. This successfully controlled the snoring, sleep apnea, and other apnea-related symptoms. Here, we describe our experience of nasal CP AP and thyroid hormone therapy in a patient with primary hypothyroidism associated with obstructive sleep apnea syndrome and dilated cardiomyopathy with review of the relevant literature.

Korean Medicine Treatment (Banhabakchulchunma-tang) for an Obese Patient With Hypothyroidism: Two Case Reports (갑상선 기능 저하증을 동반한 비만 환자에게 반하백출천마탕 투여 후 호전된 증례 2예)

  • Obesity often develops to hypothyroidism patients because thyroid hormone is associated with changes in body weight, body temperature, and energy expenditure. Obesity is not only pointed out as the cause of various other diseases, but also can worsen thyroid function, so obesity treatment is essential for hypothyroidism patients. But there is lack of studies about obesity treatment for hypothyroidism patients in Korea. In this study, two obese patients who diagnosed hypothyroidism were treated by Korean medicine. We prescribed herbal medicine (Banhabakchulchunma-tang) for about 2 months. A total of 6.5 kg and 4.5 kg of weight loss were observed respectively, and there were no serious side effects. Further large-scale clinical trials are needed to evaluate the anti-obesity effect and the effect of thyroid function improvement.

  • We measured the flow at peripheral blood vessel of finger dorsum surface using noninvasive LDF(Laser Doppler Flowmeter). We investigated the flow properties between the normal and the diabetic with the Amplitude Spectral Densities of FFT analysis. We classified the diabetic as four groups of negative neuropathy, positive neuropathy, hyperthyroid and hypothyroid. And tested the neuropathic and thyroid effects of peripheral blood flow. As the results, the diabetic, the positive neuropathic and the hypothyroid respectively showed the abnormal blood flow properties on the contrary of the normal flow of negative neuropathic and hyperthyroid.

Primary Hypoparathyroidism in a Miniature Schnauzer Dog (미니어처 슈나우저에서 발생한 원발성 부갑상선기능저하증)

  • A 7-year-old intact female, Miniature Schnauzer dog was presented with an acute seizure episode and tremors. Fever and panting were noted when presented. Physical examination revealed apparent forelimb muscle rigidity, fasciculations and stiff gait. Characteristic laboratory findings in this dog were severe hypocalcemia with hyperphosphatemia. The low serum concentration of intact parathyroid hormone and ionized calcium levels were consistent with the diagnosis of primary hypoparathyroidism. In addition, there were increased urinary excretion of calcium and decreased urinary excretion of phosphorus in this case. Urgent treatment was initiated with 10% calcium gluconate. The dog remained healthy with vitamin D analogue and oral calcium supplementation. A transient hyperphosphatemia was controlled well with sevelamer hydrochloride. To the author's knowledge, this is the first case report describing the clinical and laboratory characteristic features of canine primary hypoparathyroidism and its clinical outcome in Korea.
