지역사회 주민의 정기적 암검진 수검율 향상을 위한 정보시스템 구축 및 효과평가 (Developing a Computerized Reminder System and Evaluating the Effects on the Improvemtent of Cancer Screening)

  • Purpose: This study was aimed to develop a computerized reminder system and evaluate it's effect in terms of percent age change of screening, and satisfaction. Method: It was conducted through 6 phases: Analyzing the job and defining the basic input data, developing the information system, collecting and inputting data, testing the system, working with the system, and evaluating it's effect. Participants were 787 people (female 30-69 years, and males 40-49 years) in 2 dong of Suyoung gu, Busan, who haven't had cancer screening for the stomach, breast, or cervix since Dec. 2000. There were three experimental groups: a letter; calling and calling after the letter reminder, and a non-equivalent control group. To determine whether services were obtained, a telephone survey was done after two months of follow-up. Result: A cancer screening information system with five DB modules was developed. Overall compliance with screening was not statistically significantly changedbefore and after applying computerized reminders for all three screening sites. Only 16% were satisfied with the reminder. Conclusion: This data didn't show that a reminder effort was effective of screening. However, because the evaluation interval was too short to find a difference in screening rate, we recommend additional longer prospective follow up studies.

U-Healthcare 서비스를 위한 통합의료정보시스템의 구축방안 (Implementation Plan of Integrated Medical Information System for Ubiquitous Healthcare Service)

  • 현대사회는 정보화의 개념을 넘어서 유비쿼터스(Ubiquitous) 컴퓨팅 사회라고 말한다. 그리고 정보기술(IT)은 관련기술과의 융합으로 시스템들을 서로 통합하는 형태로 발전하고 있으며, 특히 유비쿼터스 환경에서의 의료정보 산업은 U-Healthcare 서비스 분야에 많은 관심을 가지고 있다. 본 연구에서는 첫째, U-Healthcare 서비스 환경과 통합의료정보시스템의 구성내용을 살펴보고, 둘째, 의료정보시스템의 통합을 위한 기본적 기술요소인 데이터웨어하우스, 네트워크, 통신 표준화 및 U-healthcare 서비스 관련기술 등을 검토한다. 마지막으로 이러한 기술적 요소들의 관점에서 U-Healthcare 서비스를 위한 새로운 통합의료정보시스템의 구축방안과 운영과제를 제안하였다. 이것은 고객들을 실시간으로 근접간호(POC : Point of Care)하고, 고객 개개인의 다양한 진료 데이터를 이용하여 정확한 진단을 하고, 그 정보가 다시 고객에게 전달됨으로써 고객만족이 향상될 것으로 기대된다.

한국형 Nursing Minimum Data Set(NMDS)구축을 통한 환자결과에 대한 연구 (Study on Patient Outcomes through the Construction of Korean Nursing Minimum Data Set (NMDS))

  • Purpose: The purpose of this study is developing the nursing information system which contains the core elements of nursing practice, the Nursing Minimum Data Set (NMDS) that should be collected and documented all the settings in which nursing care is provided. Method: The program was developed under the hospital information system by TCP/IP protocol and used NANDA, Nursing Interventions Classification (NIC), and Nursing Outcomes Classification (NOC) to fill out the elements of NMDS. The Oracle was used as DBMS under the Windows 98 environment and Power Builder 5.0 was used as a program language. Results: This study developed linkage among the NANDA-NOC-NIC to facilitate choosing correct nursing diagnosis, interventions, and outcomes and stimulate nurses' critical thinking. Also the system developed includes nursing care sensitive patient outcomes, so nurses can actively involve in nursing effectiveness research by analyzing the data stored in the database or by making relational databases with other health care related databases. Conclusion: The program developed in this study ultimately can be used for the nursing research, policy development, reimbursement of nursing care, and calculating staffing and nursing skill mix by providing tool to describe and organize nursing practice and measure the nursing care effectiveness.

인공지능을 도입한 간호정보시스템개발 (Development of a Nursing Diagnosis System Using a Neural Network Model)

  • Neural networks have recently attracted considerable attention in the field of classification and other areas. The purpose of this study was to demonstrate an experiment using back-propagation neural network model applied to nursing diagnosis. The network's structure has three layers ; one input layer for representing signs and symptoms and one output layer for nursing diagnosis as well as one hidden layer. The first prototype of a nursing diagnosis system for patients with stomach cancer was developed with 254 nodes for the input layer and 20 nodes for the output layer of 20 nursing diagnoses, by utilizing learning data set collected from 118 patients with stomach cancer. It showed a hitting ratio of .93 when the model was developed with 20,000 times of learning, 6 nodes of hidden layer, 0.5 of momentum and 0.5 of learning coefficient. The system was primarily designed to be an aid in the clinical reasoning process. It was intended to simplify the use of nursing diagnoses for clinical practitioners. In order to validate the developed model, a set of test data from 20 patients with stomach cancer was applied to the diagnosis system. The data for 17 patients were concurrent with the result produced from the nursing diagnosis system which shows the hitting ratio of 85%. Future research is needed to develop a system with more nursing diagnoses and an evaluation process, and to expand the system to be applicable to other groups of patients.

오마하 시스템 가이드라인의 스마트폰 애플리케이션 개발 (Developing a Smart Phone Application for the OMAHA System Guidelines)

  • Purpose: The remarkable progress in information and communication technology has had a great effect on the healthcare delivery system. The development of smart phone applications is a new field. The aim of our research was to provide assistance in developing smart phone applications for community health nursing. Methods: Based on an informative approach, this study developed persona and site maps, followed by a storyboard as a way of analyzing users' requirements and designing responses in the context of smart phone application development methodology. Results: We developed persona, user interface and database design successfully, and then seven nurses selected four nursing problems (income, residence, pain, and digestion hydration). The search time in seconds for the 2005 English OMAHA guidelines to find three nursing interventions for these problems was used to evaluate the effectiveness of the smart phone application. The results showed that smart phone applications' search was 21 times faster on the average than book guidelines. Conclusion: An English version of the OMAHA system application was developed for the Android smart phone market. It is hoped that smart phone applications such as this will be used internationally for nursing education.

해외 이민 한국인의 정신건강관리를 위한 웹기반 지능형 전문가시스템 개발 및 적용 (Development and Application of a Web-based Expert System using Artificial Intelligence for Management of Mental Health by Korean Emigrants)

  • Purpose: The purpose of this project was to develop an international web-based expert system using principals of artificial intelligence and user-centered design for management of mental health by Korean emigrants. Using this system, anyone can access the system via computer access to the web. Methods: Our design process utilized principles of user-centered design with 4 phases: needs assessment, analysis, design/development/testing, and application release. A survey was done with 3,235 Korean emigrants. Focus group interviews were also conducted. Survey and analysis results guided the design of the web-based expert system. Results: With this system, anyone can check their mental health status by themselves using a personal computer. The system analyzes facts based on answers to automated questions, and suggests solutions accordingly. A history tracking mechanism enables monitoring and future analysis. In addition, this system will include intervention programs to promote mental health status. Conclusion: This system is interactive and accessible to anyone in the world. It is expected that this management system will contribute to Korean emigrants' mental health promotion and allow researchers and professionals to share information on mental health.

  • 의료분야에서의 품질 관리의 중요성이 강화되면서 임상의사결정지원 시스템의 중요성이 높아지고 있다. 이로 인하여, 대형병원 뿐만 아니라 규모가 작은 지역 병원에서도 임상의사결정지원 시스템을 구축하는 것이 필요한 실정이다. 그러나 임상의사결정지원 시스템 구축은 지식저작과 정보시스템 구축, 기존 시스템과의 연동 등 다양한 활동이 필요한 작업으로 비용과 복잡도가 크다. 본 논문에서는 재사용 및 상호운영성을 보장할 수 있는 의료 지식과 재사용할 수 있는 임상의사결정 지원시스템 구축에 필요한 아키텍처를 구현한다. 3개 병원에 제안한 아키텍처 적용 결과를 바탕으로 아키텍처의 실무 적용 가능성 및 유용성을 검증한다.

  • 기존의 너스 콜 시스템은 유선 방식으로 병실 외의 장소에서는 사용할 수 없는 단점을 가지고 있다. 기존의 너스 콜 시스템이 가지고 있는 이동성 등의 단점을 보안하기 위하여 무선 방식의 너스 콜 시스템을 제안하였다. 제안한 무선 너스콜 시스템은 환자용 단말기, 간호사용 단말기, 서버부로 구성된다. 제안한 무선 너스 콜 시스템이 기존 너스 콜 시스템이 가지는 단점을 보완하는 것을 실험을 통하여 확인하였다.

The Factors affecting self-directed learning ability of nursing students who experienced online lectures

  • 본 연구는 온라인 강의를 수강한 간호 대학생의 자기 주도적 학습능력에 영향을 미치는 요인을 알아보기 위해 시도하였다. 연구방법은 2~4학년 간호 대학생을 대상으로 구조화된 설문지로 조사하였으며, 분석방법은 t-test와 ANOVA, 다중회귀분석으로 분석하였다. 연구결과 간호 대학생의 자기 주도적 학습능력은 5점 만점에 평균 3.48점이었고, 학습몰입이 자기 주도적 학습능력에 가장 큰 영향을 미치는 것으로 나타났다. 본 연구결과를 토대로 학습자의 자기 주도적 학습능력 향상을 위해 학습자가 학습에 몰입하도록 다양한 강의방식 또는 맞춤형 강의 프로그램을 적용하는 교수자의 노력과 양질의 교육시스템 구축이 필요할 것이다.

국내 전자간호기록 개발 및 실무적용 현황 조사 (The Adoptions and Use of Electronic Nursing Records in Korean Hospitals: Findings of a Nationwide Survey)

  • Purpose: To provide clear estimates of the adoption and use of electronic nursing records (ENRs) with standard terminology in Korea and identification of the scope and use as well as perceived or potential benefits of ENRs. Methods: A survey was done of 733 hospitals at three levels: tertiary advanced hospitals, general hospitals, and community hospitals. After performing a literature review a modified version of an existing survey tool was used for 2 months in 2012. The collected information related to EHR functionality and coverage of nursing documentation and nursing process, application of standard terminology, and perceived satisfaction and benefits of ENRs. Results: The response rate was 39.4% (289/733), and 202 hospitals (70.1%, 95% CI64.8~75.5%) of the respondents had ENR systems (82.5% of tertiary hospitals, 66.7% of general hospitals, and 70.1% of community hospitals). Out of these hospitals less than 10% had ENRs fully covering nursing documentation. The adoption rate of standard terminology was 55%, and hospital satisfaction with ENRs was 70%. But personalized care was identified as needing improvement in ENRs. Conclusion: The ENR adoption rate was high but there are many potential opportunities for improving ENR systems in terms of the data standardization and personalized care.
