수술실 간호과오 측정도구 개발 (The development of Scale of nursing malpractice in operating room)

  • Purpose : The purpose of this study was to develop the Operating Room Nursing Malpractice Scale and to test reliability and validity of the instrument and to describe frequency of malpractice. Method : The subjects used to verify the Scale's reliability and validity were 179 nurses who working at 3 university hospital and 6 general hospital in Busan and Kyoung nam province from september 1 to October 10, 2002. The data was analyzed by the SPSS/ WIN 10.0 program. Result : The factor analysis classified a total 5 factors statistically, it's communality was 44.18%. Item content are as follows. The factor were 'malpractice in the patient's preparation', 'malpractice in nursing technique', 'malpractice in the management of the environment', 'malpractice of the patient's security', 'malpractice in the supervision of the patient'. The most frequent malpractice was 'The patient complained of waiting for a long time to go back from the operating room because of inadequate communication with another department'. Conclusion : The scale of operating room nursing malpractice was to prevent the severe nursing accident and to provide the basis of needs of educational program.

수술실 간호사의 간호과오경험 후 대처가 업무 스트레스에 미치는 영향 (Effect on Coping Behavior on the Job Stress after Nursing Error Experience in the Operation Room)

  • Purpose: The purpose of this study was to identify the relationship between nursing error experience, coping behavior and job stress in operating room. Methods: A descriptive research design was used in this study. The participants were 228 operating room nurses in G city who surveyed between October 25 and November 25, 2017 using self-report questionnaires. The data were analyzed using IBM SPSS/WIN 24.0/AMOS WIN 24.0 Program, which determined frequency, percentage, mean, standard deviation, Pearson correlation coefficient, and structural equation model. Results: There were significant positive correlations between six sub-categories of nursing errors and job stress. We found negative correlations between coping behavior and job stress. There was a mediating effect of active coping between knowledge of nursing error and job stress. We found passive coping between inspection & monitoring related error and job stress. Conclusion: Study findings suggest that adequate education and the improvement in hospital environment and system should be required to reduce the nurses' job stress related to the patients' safety in operating room.

간호과오책임 예방을 위한 웹기반 진단평가 프로그램 개발 (Development of a Web-based Diagnostic Evaluation Program for Prevention of Nurse Malpractice Liability)

  • Purpose: This study was done to develop a web-based diagnostic evaluation program for nurses to prevent malpractice liability. Methods: A comprehensive review of the literature and 9 specialist interviews were used to search for learning goals and content for protection for nurses from malpractice. Data on needs for learning goals were collected from 56 hospital nurses who agreed to complete a self-report questionnaire. The diagnostic program was evaluated between September 2008 and August 2009 by 35 new hospital nurses using an application of the web-based program evaluation tools by Chung (2000). Results: A comprehensive review of the literature and interviews were used to search for learning goals and content. The evaluation program was composed of the 73 questions for diagnostic evaluation under 23 learning goals and 6 grand learning goals which included the principles of law, patient's rights, legal responsibility, patient's safety, regulation on nursing practice and patient's rights protection. Evaluation of the program showed that the mean for program evaluation was 3.43 (SD=.37). Conclusion: This diagnostic evaluation program could be an efficient method for teachers and learners to improve nurses' behavior in protecting the patient's rights and preventing malpractice claims.

수술실의 간호오류 및 과오 예방을 위한 E-learning 실무교육 프로그램의 개발 및 평가 (Development of an E-learning Education Program for Preventing Nursing Errors and Adverse Events of Operating Room Nurses)

  • Purpose: This study was to develop, implement, and evaluate an e-learning education program for improving practical knowledge and preventing nursing errors and adverse events of nurses working in the operating room (OR). Method: The e-learning program was developed and evaluated according to the following processes: 1) preparation phase 2) implementation phase 3) evaluation phase. In evaluation phase, the effectiveness was analyzed based on the Kirkpatrick's model. Results: The e-learning program consisted of OR basic nursing skills and techniques and nursing activities' manual based on the categories of nursing errors: surgical operation preparation, nursing skills and techniques, environment management, patient safety and comfort, and patient monitoring. The program was provided through on-line, http://cafe.daum.net/pnuhorn, for 4 weeks. The mean score(percent) of participants' satisfaction was $21.24{\pm}1.71$(68.2%). Their total knowledge level was significantly improved(Z=-3.00, p=.003) and specifically in the category of environment management(Z=-3.77, p<.001) and patient monitoring(Z=-2.46, p=.014). The occurrence of nursing errors or adverse events was a little decreased, but not statistically significant(Z=-3.10, p=.756). Conclusion: E-learning for nurses is one way of effective and efficient teaching-learning strategies. For better e-learning, it is important to develop the vital content of the education and objective measures for detecting nursing errors and adverse events.

판례를 이용한 환자안전관련 간호과오 예방교육이 간호사의 안전지각, 안전통제감, 자율성 및 책임수용성에 미치는 효과 (Effects of an Education Program on Prevention of Malpractice using Precedent Cases related to Patient Safety in Safety Perception, Safety Control, Autonomy and Accountability in Clinical Nurses)

  • Purpose: The purpose of this study was to identify the effects of an education program on safety perception, safety control, autonomy and accountability in clinical nurses. Precedent cases related to patient safety were used in the education program. Methods: A quasi-experimental design with pretest and posttest measures was used. Participants in the study, 72 nurses in the experimental group, 71 nurses in the control group, were enrolled for 3 months. The education program was composed of the 20 precedent cases related to patient safety from home and foreign countries. Results: The major findings of this study were as follows: Safety perception (p=.000), Safety control (p=.000), attitude toward autonomy (p=.000), and attitude toward accountability (p=.000) improved after the education program. Conclusion: The findings from this study indicate that an education program using precedent cases is an efficient method to improve behavior and change attitudes towards protecting patients' safety and preventing malpractice claims against nurses.

일개 종합병원 의료종사자들의 환자안전과 관련된 의료과오보고 인식에 영향을 미치는 융합요인 (Convergence Factors Influencing on Perception of Medical Errors Report Related to Patient Safety of Healthcare Workers in a General Hospital)

  • 본 연구의 목적은 종합병원 의료종사자들의 환자안전과 관련된 병원환경, 조직문화, 의료과오보고에 대한 인식을 파악하고, 의료과오보고에 영향을 미치는 요인을 확인하는 것이다. 연구방법은 B광역시 종합병원 의료종사자 244명을 대상으로 2017년 2월 13일부터 2월 28일까지 설문조사 후 SPSS 22.0 프로그램을 이용하여 분석하였다. 연구결과 병원환경 평균 점수는 5점 만점에 3.26(${\pm}0.31$)점, 조직문화 평균점수는 3.74(${\pm}0.54$)점, 의료과오보고 평균점수는 3.64(${\pm}0.57$)점이었고, 의료과오보고에 영향을 미치는 요인은 성별(${\beta}=.137$, p=.023), 직종(${\beta}=289$, p=.001), 간호직의 근무부서(${\beta}=-.196$, p=.023), 병원환경(${\beta}=.327$, p=<.001), 조직문화(${\beta}=.288$, p=<.001)로 나타났다. 그러므로 의료종사자들이 의료과오보고를 두려워하지 않고 보고할 수 있는 시스템을 구축하고 적절한 인력의 확보 및 배치와 개방된 의사소통이 이루어지도록 관리자와 병원경영진의 노력이 이루어져야 할 것이다.

간호사의 경과관찰업무에 대한 판례분석 및 관리지침 개발 (Legal Analysis and Development of managerial guideline to nursing activities related to observation)

  • Purpose. The study was done to develop nursing guidelines to prevent nursing negligence in the clinical setting that would lead to malpractice. Method. A comprehensive review of the literature and focus groups interviews were used to search for malpractice cases related to "observation", one of the most basic nursing activities. The cases were analyzed with respect to nursing practice and a legal expert was consulted on the content. Results. 4 malpractice cases related to observation negligence were selected for this study; 1) a case of failing to observe dyspnea after a thyroidectomy, 2) a case of failing to observe a depressed patient for suicide attempts, 3) a case of failing to observe a VP shunt obstruction, 4) a case of failing to observe a cardiac ischemia after a bronchoscopy. 11 guidelines were developed for clinical nurses and 13 guidelines for nursing managers. Conclusion. These guidelines are useful in preventing malpractice due to nursing negligence. Therefore nurses need to apply them to their practices and continue to make an effort to develop guidelines for other malpractice situations.

수술실 간호오류 예방을 위한 사례중심 멀티미디어 학습콘텐츠 개발 (Development of Case-based Multimedia Learning Contents for Preventing Malpractice in Operating Room)

  • 본 연구에서는 수술실에서의 간호오류 예방을 위한 간호사 대상의 교육을 목적으로 문헌고찰과 실제 경험한 오류사례와 요구도 조사 및 수술실 사건보고서 자료 분석을 통하여 사진, 음성녹음, 플래시 애니메이션, 동영상 자료를 활용한 사례중심 멀티미디어 학습콘텐츠를 개발하였다. 1시간 분량의 학습콘텐츠는 4가지 학습영역으로 구성되었으며, 학습영역별로 성우의 음성과 함께 플래시애니메이션을 활용한 실제 간호오류 사례를 이야기 형식으로 제공함으로써 학습자로부터 흥미와 현장감을 높였다. 또한 각 영역별 사례를 통해 학습자에게 비판적 사고를 유도하였고 표준화된 수술실 간호업무프로토콜을 대처방안으로 제시함으로써 교육의 효과성을 높이고자 하였다. 본 연구에서 개발된 학습콘텐츠는 신규간호사 뿐만 아니라 경력 간호사의 수술실 간호오류 예방을 위한 교육용 자료로써 실무에서 활용될 수 있기를 바라며, 콘텐츠의 지속적인 수정 보완을 위한 시스템 마련과 지원이 필요할 것으로 본다.

간호관리학실습 교과내용에 대한 중요도와 실습 경험에 대한 간호대학생과 임상현장지도자의 인식 비교 연구 (A Comparative Study on the Perceptions of Nursing Students and Clinical Instructors on the Importance of Learning Contents of Nursing Management and Practical Experience)

  • 본 연구의 목적은 간호관리학실습 교과내용의 중요도와 실습 참여 경험에 대한 학생과 임상현장지도자의 인식을 비교·분석해 간호관리학실습 개선의 기초자료를 제공하고자 하였다. 3개 대학 4학년 학생과 임상현장지도자를 대상으로 Mann-Whitney test와 교차분석 등으로 최종 343부 분석하였다. 연구 결과 간호대학생과 임상현장지도자 모두 간호관리학실습의 평가항목 중 '의사소통', '대인관계', '책임', '신뢰', '도덕성'을 중요 요소로 인식하였고 간호관리학실습 교과내용 중 가장 중요하게 인식한 상위 10개 항목 중 6개 항목('감염관리', '간호기록관리', '환자간호관리', '약품관리', '환자안전', '간호과오')은 두 그룹이 일치했다. 또한 임상현장지도자는 대인관계 및 관계윤리를 중요하다고 인식한 반면 간호대학생은 간호사의 '법적책임'을 중요하다고 인식했다. 두 그룹간의 실습경험 분석 결과, 간호대학생은 모두 간접 참여 경험이 높았으나 현장지도자는 직접 지도 경험이 높았다. 이와 같은 인식 차이가 확인된 교과내용은 학교와 실습기관의 유기적 파트너쉽 관계를 구축해 차이를 개선하고 간호대학생의 직접 실습 경험을 증대시키는 방안 마련이 필요하다.
