Job Satisfaction, Job Stress, Burnout, and Turnover Intention of Comprehensive Nursing Care Service Ward Nurses and General Ward Nurses (간호·간병통합서비스병동 간호사와 일반병동 간호사의 직무만족, 직무스트레스, 직무소진 및 이직의도)

  • This study was carried out to analyze and compare job satisfaction, job stress, burnout, and turnover intention of comprehensive nursing care service ward nurses and general ward nurses. The participants were 181 nurses from six hospitals providing comprehensive nursing care service in G metropolitan city. The data were collected using structured questionnaires in May 16-June 30, 2017. The data were analyzed with SPSS program, which was used for t-test, one-way ANOVA, and Pearson's correlation. As a result, there were no significant differences in the variables between comprehensive nursing care service ward nurses and general ward nurses. However, comprehensive nursing care service ward nurses had higher job satisfaction, lower job stress, lower burnout, and lower turnover intention than general ward nurses. There was a significant relationship between job stress (r=.39, p=<.001), burnout (r=.43, p=<.001), job satisfaction (r=-.66, p=<.001), and turnover intention. Comprehensive nursing care service ward nurses were more positive about their nursing work than general ward nurses. The findings of this study can be used as basic data for effective nursing resource management in a comprehensive nursing care service ward.

Institutionalization of a Patient-Sitter Program in Acute Care Hospitals (보호자 없는 병원 제도화 방안)

  • In an effort to resolve the burden of patients hiring patient sitters, this study sought to review the Ministry-of-Health-and-Welfare-initiated pilot program of running hospitals without patient-sitter to identify its background, operation method, performance results, and limitations. Based on the review, the study derived the necessity of introducing a hospital system without patient-sitter as well as its operation and systemization methods. The ministry-initiated pilot programs were conducted twice: in 2007, and in 2010. A review of the 2007 pilot program revealed that the patients and families' satisfaction score with nursing services was 9.1 points (on a 10-point scale), their intention to reuse the service was 97.8%, and their intention to recommend the service was 98.0%, all high scores. Appropriate nursing manpower, derived from the 2007 pilot project, indicated 2.3 patients per nurse and 4.0 patients per nurse aid. The 2010 pilot project results indicated that the patients and families' satisfaction was high at 8.0-9.1 points (on a 10-point scale), and that the intention to reuse and recommend the service was also high. Compared with the 2007 pilot project, however, the types of medical institutions and the nurse to patient ratios were diverse, offering limitations. In conclusion, to systemize hospitals without patient-sitter, it is necessary to develop policies designed to establish criteria for the appropriate nurse to patient ratio and skill-mix, to standardize the work, to prepare finances for securing nursing staff, to evaluate the nursing demands, and to monitor the quality management.

Optimal Nursing Workforce and Financial Cost to Provide Comprehensive Nursing Service in the National Health Insurance System (국민건강보험 간호·간병통합서비스의 전면 도입을 위한 간호인력 및 재정비용 추계)

  • This study estimated the optimal nursing workforce and financial costs of providing comprehensive nursing services at hospitals under the national health insurance system. Data on registered nurses, nursing aids, medical institutions, and number of patients were obtained from the Health Insurance Review and Assessment Service. The optimal size of the nursing workforce was calculated using the workload model. A bottom-up approach was used to estimate the annual total financial cost of comprehensive nursing services. The number of registered nurses and nursing aids would need to be increased by 81.75% and 83.23%, respectively, in order to fully apply comprehensive nursing care on a national scale. The additional financial costs for comprehensive nursing services at all hospitals was estimated to be as much as 110.39% of the current cost. For the comprehensive nursing service, nurses with a career and newcomers need to be retained at their hospitals, and the validity of the nurse-patient ratio should be continuously checked. The financial shock to the national health insurance system could be minimized by gradually extending the system to all hospitals.

Factors Influencing Care workers' Intention of Implementing Hand Hygiene in Long-term Care Hospitals (요양병원 간병 인력의 손 위생 이행의도에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to investigate factors influencing care workers' intention of hand hygiene implementation in long-term care hospitals. Methods: A total of 180 care workers working at long-term care hospitals were recruited. Data collection was done from July 22 to September 7, 2018. Results: The significant TPB variables influencing the intention of hand hygiene implementation were perceived behavior control (β=.41, p<.001), normative belief (β=.28, p<.001) and attitude toward behavior (β=.15, p=.014). These factors explain 39% of care workers' intension of implementing hand hygiene in long-term care hospitals. Conclusion: In order to strengthen the commitment of hand hygiene, it is necessary to have a positive attitude toward hand hygiene by eliminating the obstacles to hand hygiene.

Research on Factors Affecting Intention to Re-Use Hospitals for Patients Who Have Used Integrated Nursing and Care Services (간호·간병통합서비스를 이용한 입원환자의 병원재이용의도에 영향을 미치는 요인)

  • The purpose of this research was to investigate the effects of the communication ability, communication type and satisfaction of nursing service on the re-use intention of nursing and care integration services of the hospital. The research subjects were 291 patients who were hospitalized for more than 3 days in the integrated nursing and care service center. Data were analyzed using SPSS 21.0. As a result of the study, the variables that significantly affect the reuse of integrated nursing services in hospitals were satisfaction with nursing services and integrated services. The explanatory power of these variables was 63.4%. Therefore, to increase the intention to re-use integrated nursing and care services of the hospitals, efforts are needed to increase the satisfaction of nursing services. And research on other variables that affect the guarantee is highly recommended.

A Study on Relations among the Job Stress, Job Satisfaction, Emotional Labor, and Turnover Intention of Nurses at the Comprehensive Nursing Care Service and General Wards

  • In this paper is a descriptive survey research to compare correlations among the job stress, job satisfaction, emotional labor, and turnover intention of nurses between the comprehensive nursing care service and general wards. The subjects include total 185 nurses that consisted of 93 nurses at the comprehensive nursing care service ward and 92 at the General Ward. Results of this study are as follows; The study analyzed correlations among the job stress, job satisfaction, emotional labor, and turnover intention of nurses at the comprehensive nursing care service and general wards and found that both the groups showed significant positive correlations between job stress and emotional labor, between job stress and turnover intention, and between emotional labor and turnover intention, which indicates that higher job stress led to higher emotional labor and that higher job stress and emotional labor led to higher turnover intention. These findings raise a need to increase the manpower standard, offer proper rewards, and improve the work environment and treatment for nurses, who play the pivotal roles at the hospital organization, to solve their increased job stress and emotional labor issues and ultimately ensure the successful expansion of the comprehensive nursing care service system. There should be multifaceted efforts to reduce the turnover of skilled nurses that are capable of providing quality nursing service.

Job stress and job satisfaction of the nurses in comprehensive nursing service units and the nurses in general units (간호간병통합서비스병동 간호사와 일반병동 간호사의 직무스트레스와 직무만족)

  • This study was conducted to compare and analyze occupational stress and satisfaction levels of the nurses in two different units to provide data for effective nursing resource management. Survey was conducted in four hospitals in June 2019 and data was obtained from 62 nurses in comprehensive nursing service units (CNSU) and 62 nurses in general units (GU). CNSU nurses had higher level of job satisfaction than GU nurses. Contributing factors included pay, professional status, administration, autonomy, task requirements, and interaction. Job stress level was not significantly different between groups. GU nurses had higher stress levels from nursing practice, conflict with doctors, and patients and caregivers, while CNSU nurses had higher stress levels in working condition. Negative correlation was found between job satisfaction and stress in GU nurses, whereas not in CNSU nurses. Clear definition of task, smaller work load, appropriate reward, and educational support is suggested to enhance job satisfaction.

  • The purpose of this study was to investigate and compare job satisfaction, job stress, and nursing task performance for nursing and nursing integrated service wards and general ward nurses to provide basic data for efficient nursing manpower management in nursing and nursing integrated service wards. For comparative research. The subjects were 136 nurses in the nursing and nursing integrated service ward and general ward nurses at a general hospital in D metropolitan area. There was no significant difference between the groups in the job satisfaction, job stress, and nursing task performance of the nursing and nursing integrated service ward nurses and general ward nurses. The performance was high. Factors influencing the performance of nursing tasks were job satisfaction and job stress, with 20.6% of nurses in nursing and nursing integrated service wards and 47.5% of nurses in general wards. Therefore, there is a need for support to improve the ability to perform nursing tasks that can increase job satisfaction and maintain job stress.

  • The purpose of this study was to examine job stress, job satisfaction, and turnover intention of the nurses who work at the comprehensive nursing service units in two academic medical institutions that provide healthcare service at the tertiary care level in Korea. A descriptive study was conducted with the nurses by using self-reported questionnaires. We used descriptive statistics to summarize the data and Mann-Whitney U test for group comparison. The mean scores of job stress, job satisfaction, and turnover intention were $89.17{\pm}12.56$, $107.25{\pm}6.85$, $37.19{\pm}4.86$, respectively. Nurses with religion showed more job stress than those without. Nurses with their age below 30 years old had significantly less job satisfaction than those older than 30 years old. Monthly income was significantly associated with turnover intention in our data. The top three sub-category of the job stress were conflict with physicians, patients and their caregivers, and conflicts with others; and those of the job satisfaction were pay and promotion and task requirement. Job stress, job satisfaction, and turnover intention were significantly correlated. In order to reduce turnover intention of nursing professionals in the comprehensive nursing service, job stress related to issues on relationships with patients, their family, health care providers need to be taken into consideration. Policy and administrative support are needed to improve their work environment.

A Comparative study on Perceived Importance and Frequency of Basic Nursing Performance ability of Comprehensive Nursing Care Service Wards versus General Wards (간호·간병 통합서비스 병동과 일반병동 간호사의 기본간호술 수행 빈도 및 중요성 인식 비교)

  • The purpose of this study was to analyze the differences in the frequency and importance of basic nursing practice between comprehensive nursing care service wards and general wards and to provide basic data for the efficient of services. The subjects of this study were 99 comprehensive nursing care service and 90 general wards. Data were collected from February 1 to 28, 2018 and analyzed using a t-test, one-way ANOVA, Pearson's correlation coefficient, Scheffe test. The results showed that the frequency of basic nursing practice was higher by 2.43, and there were significant differences in personal hygiene, nutrition and excretion, exercise & activity. The recognition of the importance of basic nursing was high in both groups, but comprehensive nursing care service wards were slightly higher at 4.24 points. However, as the frequency of practice is lower than the recognition of the importance of basic nursing, a method to increase the frequency of practice is necessary. These results suggest that comprehensive nursing care service wards should provide appropriate nursing staff and system development so the nurses can provide care service as professionals.
