Analysis of the Causes of and Risk Factors for Mortality in the Surgical Repair of Interrupted Aortic Arch (대동맥궁 단절증 수술 사망 원인과 위험인자 분석)

  • Background: Interrupted aortic arch is a rare congenital heart anomaly which still shows high surgical mortality. In this study, we investigated the causes of and the risk factors for mortality to improve the surgical outcomes for this difficult disease entity. Material and Method: From 1984 to 2004, 42 patients diagnosed as IAA were reviewed retrospectively. Age, body weight at operation, preoperative diagnosis, preoperative PGE1 requirement, type of interrupted aortic arch, degree of left ventricular outflow stenosis, CPB time, and ACC time were the possible risk factors for mortality. Result: There were .14 hospital deaths. Preoperative use of PGE1, need for circulartory assist and aortic cross clamp time proved to be positive risk factors for mortality on univariate analysis. Preoperative left ventricular outflow stenosis was considered a risk factor for mortality but it did not show statistical significance (p-value=0.61). Causes of death included hypoxia due to pulmonary banding, left ventricular outtract stenosis, infection, mitral valve regurgitation, long cardiopulmonary bypass time and failure of coronary transfer failure in TGA patients. Conclusion: In this study, we demonstrated that surgical mortality is still high due to the risk factors including preoperative status and long operative time. However preoperative subaortic dimension was not related statistically to operative death statistically. Adequate preoperative management and short operation time are mandatory for better survival outcome.

Anti-inflammatory Effect of Stevia Rebaudiana as a Results of NF-κB and MAPK Inhibition (NF-κB와 MAPK 억제를 통한 스테비아 잎의 항염증효과)

  • Objectives : Stevia rebaudiana is a well-known herbal sweetener in the Korea, Japan and China, and its medical uses were originated from countries in South America. Although it has been shown the various medical effects of S. rebaudiana including contraception and treatment of human diseases such as hyperglycemia, it has almost not been studied about the efficacy of S. rebaudiana methanolic extract (SRE) on the acute inflammation and its action mechanism. Methods : To investigate the anti-inflammatory effects of SRE, we treated SRE and examined the level of inflammatory mediators in LPS-stimulated Raw264.7 cells. Results : Treatment of macrophage with LPS markedly induced the production of NO, $PGE_2$ and pro-inflammatory cytokines. Pretreatment of SRE blocked the induction of inflammatory mediators and the expression of iNOS protein. More importantly, LPS-induced phosphorylation of $I{\kappa}B-{\alpha}$ was suppressed by the treatment of SRE, suggesting SRE inhibition of NF-${\kappa}B$ activation. Furthermore, SRE blocked LPS-induced phosphorylation of MAPKs. Conclusions : SRE inhibited the induction of NO, PGE2 and pro-inflammatory cytokines in Raw264.7 cells. SRE's effect may be mediated with its inhibition of NF-${\kappa}B$ activation and MAPK phosphorylation, which suggests its uses as an anti-inflammatory agents.

The comparative study on physiological activity of White ginseng, Red ginseng and Black ginseng extract (백삼, 홍삼, 흑삼 추출물의 생리활성 비교 연구)

  • This study presents a comparative study for differences in efficacy and ingredient of white ginseng(WG), red ginseng(RG), and black ginseng(BG) using anti-oxidative activity test and anti-inflammatory activity test. In the results of cytotoxicity test for WG, RG, and BG, the survival rate of all cells was more than 95%. In the total polyphenol analysis, DPPH, ABTS radical scavenging test, and ROS production test, BG showed higher anti-oxidant activity than WG and RG. RG and WG showed higher inhibition activity of NO production and $PGE_2$ production, respectively. As results of the test for the effect on reduction of inflammatory cytokine production, WG and RG were effective on reduction of IL-$1{\beta}$ production, and BG was effective on reduction of IL-6. In the case of TNF-${\alpha}$ production, there was no difference among samples. This study could be useful basic data for the development of functional food and the fabrication of safe cosmetic.

The Inhibitory Effect of CW-501027 and CW-501029 on the Anti-inflammatory Action in Rats (흰쥐의 염증반응에 대한 CW-501027과 CW-501029의 억제효과)

  • The use of nonsteroidal anti-inflammatory drugs (NSAIDs) is limited by their ability to induce gastrointestinal injury. It has been shown that nitric oxide (NO), similar to pro staglandins (PGs), appears to play an important role in gastric mucosal defence. We hypothesized that NSAIDs contained NO group would be less acutely toxic to the gastric mucosa, but would not interfere with their ability to suppress inflammatory process in rats. We have compared the ulcerogenic and anti-inflammatory effect of CW-501029 (NO-NSAIDs), CW-501027 (NSAIDs) and indomethacin. Both did not change mean blood pressure and heart rates, indicating that they had no side effect on cardiovascular system. We found that CW-501029 increased nitrite/nitrate levels without changing of blood pressure and heart rates. We suggest that it may help gastric mucosal blood flow, the which helps reducing the discomfort in astrointestinal system. Carrageenan-induced PGE2 increase was reduced in a similar tendency when compared CW-501027 or CW-501027 with control in back exudate of rats, but CW-501029 less reduced PGE2 than CW-502027 or indomethacin in gastric tissues. CW-501027 or CW-501029 reduced platelet aggregation. From these results we suggest that CW-501029 may improve the side effect by reduction of short-term gastric injury and less inhibition of PGs synthesis.

Effects of Kagammokbanggi-Tang on the Adjuvant-Induced Arthritis in Rats (가감목방기탕(加減木防己湯)이 흰쥐의 Adjuvant 유발 관절염에 미치는 영향)

  • Objectives : This study was to investigate the effect of Kagammokbanggi-tang on the Freund's Complete Adjuvant(FCA)-induced arthritis in rats. Methods : Arthritis was induced by intradermal injection of FCA into base of tail. Arthritic rats were divided into control(n=10) and sample(n=10) group. Control group was taken normal saline for twenty days and sample group was taken extracts of Kagammokbanggi-tang for same duration. Normal group(n=10), non-arthritic group, was injected with mineral oil and was taken normal saline for twenty days. Body weight, paw edema volume and ankle joint thickness were measured at 0, 10, 15, 20 days after treatment. $TNF-{\alpha}$, $IL-1{\beta}$, IL-6, $PGE_2$ in synovia were analysed by ELISA at 20 days after treatment. Histochemical investigation of NADPH-d in the PAG and histopathological study on the ankle joint were performed at 20 days after treatment. Results : Paw edema volume, ankle joint thickness, $TNF-{\alpha}$, $IL-1{\beta}$, IL-6, $PGE_2$, NADPH-d of sample group were significantly decreased compared with control group. Conclusions : These results indicated that Kagammokbanggi-tang has antiarthritic and analgesic effects, and inhibited expression of NOS on the progression of FCA- induced arthritis in rats.

Biological Effects Of Flurbiprofen Loaded Chitosan To Gingival Fibroblast (Flurbiprofen 함유 키토산 제제가 치은 섬유아세포에 미치는 영향)

  • The main goal of periodontal regeneration is to be achieved by epithelial exclusion, periodontal ligament cell activation or alveolar bone regeneration. The purpose of this study was to investigate on the physico- chemical and biological characteristics of biodegradable chitosan beads. Chitosan beads were fabricated by ionic gelation with sodium tripolyphosphate and they had the size in 300um diameter. As therapeutic agent, flurbiprofen was incorporated into the beads by 10, 20% loading contents. The release of drugs from the chitosan beads was measured in vitro. Also, biological activity tests of flurbiprofen loaded chitosan beads including cytotoxicity test, ihhibition of $IL-1{\beta}$ production, suppression to $PGE_2$ production, collagenase inhibition test, the ability of total protein synthesis, and tissue response were evaluated. The amount of flurbiprofen released from chitosan was 33-50% during 7 days. Minimal cytotoxicity was observed in chitosan beads. Flurbiprofen released from chitosan beads significantly suppressed the $IL-1{\beta}$ production of monocyte, $PGE_2$ production and markedly inhibited collagenase activity. Meanwhile, flurbiprofen released from this system showed increased ability for protein synthesis. Throughout 4 -week implantation period, no significant inflammatory cell infiltrated around chitosan bead and also fibroblast like cell types at the beads - tissue interface were revealed with gradual degradation of implanted chitosan beads. From these results, it was suggested that flurbiprofen loaded chitosan beads can be effectively useful for biocompatible local delivery system in periodontal regeneration.

Studise on the Effects of Gami-Shengmnayuipung-tang on Acnes (가미승마위풍탕이 면포(Acne)에 미치는 실험적 연구)

  • Objective: This study was carried out to investigate the effects of Gami-Shengmayuipung-tang on acne. Methods : The effects of Gami-Shengmayuipung-tang (SYTSRG) on acne were measured by the $5\alpha-reductase$ inhibition, the sterilizing power for Propionibactrium acnes, the cytotoxicity of human monocyte, the inhibition for prostaglandins$(PGE_2)$, interleukins $(IL-l\beta)$ and $TNF-\alpha$ in inflammation, and the size of the hamtster ear sebaceous gland related to P. acnes. Results: On the $5\alpha-reductase$ inhibition of SYTSRG in vitro, an undiluted solution of SYTSRG showed $80\%$ inhibition on $5\alpha-reductase$ and 1/10 diluted solution of SYTSRG showed $40\%$ inhibition on $5\alpha-reductase$. On the sterilizing power for Propionibactrium acnes related acne, an undiluted solution and 1/10 diluted solution of SYTSRG formed $12\beta{\AE}$ clear zone diameters. SYTSRG did not show cytotoxicity of human monocyte. Concentrations of $0.01\%\;and\;0.04\%\;and\;0.08\%$ of SYTSRG inhibited the production of prostaglandins $(PGE_2)$ in the human monocyte stimulated with P. acnes LPS. $0.08\%$ and less of SYTSRG inhibited the production of interleukins $(IL-l\beta)$ in the human monocyte stimulated with P. acnes LPS. Concentrations of $0.04\%,\;0.08\%\;and\;0.12\%$ of SYTSRG inhibited the production of $TNF-\alpha$ in the human monocyte stimulated with P. acnes LPS. As the antiandrogenic compound, SYTSRG was used in hamster ears with topical application. SYTSRG diminished the size of the hamster ear sebaceous gland in males, but not in females. Conclusion: The present data suggest that SYTSRG may affect the primary stage of inflammation of acne.

Growth of Intestinal Bacteria and Intestinal Inflammation of Sprout Extract from Common Buckwheat and Tartary Buckwheat (일반메밀과 쓴메밀의 새싹 추출물의 장내 유익균 증식 및 염증조절 효능 평가)

  • We aimed to assess the potential growth-promoting effects of buckwheat sprout on intestinal bacteria and their anti-inflammation effects in a cellular model of intestinal inflammation. The growth of Bifidobacterium longum ssp. infantis BT1 was enhanced with the addition of the sprout extract of tartary buckwheat. Further, in the inflammatory model cells cultured with Raw 264.7 cells were treated with buckwheat sprout including each 10 probiotics before the addition of lipopolysaccharide (LPS) to induce inflammation in Raw 264.7 cells. Buckwheat sprout in both Bifidobacterium longum ssp. infantis BT1 and Lacticaseibacillus paracasei LPC5 significantly reduced the production of NO and PGE2. The above results indicate that buckwheat sprout extract which contains with various physiologically active substances such as rutin, quercetin, and choline is effective in suppressing NO and PGE2 production, which are inflammation-related indicators. The present study suggests that buckwheat sprout could induce positive effects on the intestinal beneficial bacteria and in anti-inflammation.

Paclitaxel Stimulates Cyclooxygenase-2 Expression via MAP Kinase Pathway in Rabbit Articular Chondrocytes

  • Paclitaxel, an antimicrotubule agent, binds to beta-tubulin in the microtubule and stabilizes the polymer, thereby repressing dynamic instability. Here, we have demonstrated that microtubule cytoskeletal architecture involved in regulation of the COX-2 expression in chondrocyte treated with paclitaxel. Paclitaxel enhanced COX-2 expression and prostaglandin E2 production, as indicated by the Western blot analysis, reverse transcriptase PCR(RT-PCR) and immunofluorescence staining, and $PGE_2$ assay, respectively. In our previous data have shown that paclitaxel treatment stimulated activation of ERK-1/2 and p38 kinase(Im et al., 2009). SB203580, an inhibitor of p38 kinase, blocked the induction of COX-2 expression by paclitaxel. Also PD98059, an inhibitor of ERK-1/2 kinase was blocked the induced COX-2 expression. These results indicate that activation of ERK-1/2 and p38 kinase is required for COX-2 expression induced by paclitaxel in rabbit articular chondrocytes.

4'-O-β-D-Glucosyl-5-O-Methylvisamminol Attenuates Pro-Inflammatory Responses and Protects against Oxidative Damages

  • We attempted to examine anti-inflammatory and anti-oxidant effects of 4'-O-${\beta}$-D-glucosyl-5-O-methylvisamminol (GOMV), the first epigenetic inhibitor of histone phosphorylation at Ser10. While GOMV did not affect the viability of murine macrophage RAW 264.7 cells, it significantly suppressed lipopolysaccharide (LPS)-induced generation of prostaglandin $E_2$ ($PGE_2$) and nitric oxide (NO) through transcriptional inhibition of cyclooxygenase-2 (COX-2) and inducible nitric oxide synthase (iNOS). GOMV also scavenged free radicals in vitro, increased NF-E2-related factor 2 (NRF2), and activated antioxidant response element (ARE), thereby resulting in the induction of phase II cytoprotective enzymes in human keratinocyte HaCaT cells. Finally, GOMV significantly protected HaCaT cells against 12-O-tetradecanoylphorbol-13-acetate (TPA)-induced oxidative intracellular damages. Together, our results illustrate that GOMV possesses anti-inflammatory and anti-oxidant activity.
