Objective : This study was carried out to analyze the quality and quantity of Clinical Trials that have been published in the journal of korean medical ophthalmology, otolaryngology, dermatology(JKOOD). Methods : We analyzed 25 clinical trials that published in JKOOD from 1988 to 2014. We excluded case reports, protocol and retrospective studies and classified searched papers into three categories; Randomized Clinical Trials(RCT), Non Randomized Clinical Trials(NRCT), Before After Study(BAS) by using study Design Algorithm for Medical literature of Intervention(DMAI). All articles were analyzed according to diagnosis, statistics program and intervention period. The bias of RCTs were evaluated by Cochrane Risk of Bias(RoB). Result : 1. The number of searched journals is 25 papers; 13 RCT, 2 NRCT, 10 BAS 2. Distribution of clinical trial; 'Atopic dermatitis' ranked the highest(44%) in disease, 'External application' raked the highest(71%) in treatment method. 3. 'allocation sequence' and 'prevention of allocated intervent to patients and therapists' are graded 'Low' but 'incomplete outcome date' and 'selective outcome' are graded 'Uncertain'. Conclusions : It is necessary to study more RCT. It will be helpful to study systematic reviews and meta analysis in JKOOD.

Objective : To search the trend of studying thread-embedding therapy in Korean medicine academia up to now and to expand the range of using thread-embedding therapy in the future, this study was designed. Methods : Using search portal sites the following; http://oasis.kiom.re.kr, http://www.riss.re.kr, collected 37 articles and devided those three parts and classificated those into four subsection as follows; total analysis, original article, case report. Results : The number of articles are 8 review articles, 14 original articles, 15 case reports. In the year 2013, 14 articles were published and ranked highest(37.8%). The journal of acupuncture & moxibustion medicine society ranked highest(40.5%). 5 persons ranked highest(32.4%) in number of author. Facial beauty care ranked highest(21.6%) in clssification of disease. Remedy item ranked highest(50.0%) in classification of subjects of 8 review articles. No more than 25 persons ranked highest(66.7%) and 6~12months of treatment period ranked highest(33.3%) in 14 original articles. Conclusions : Thread-embedding therapy is considered very useful on various part of Korean medicine clinic. We need to expand application scope of thread-embedding therapy in the future.

Objective : This study aims to provide fundamental data for new directions(the improvement directions) from searching research papers of randomized controlled trials among thesis of Atopic Dermatitis related to oriental medicine in Korea from 2004 to 2014 and identifying the problem and level of a clinical study through an assessment of the quality, CONSORT statement. Methods : Data was collected through the Koreanstudies Information Service System(KISS). Two experts in Oriental Medicine reviewed the title and abstract in thesis, the results of searching the title, "Atopic dermatitis" and topic, "oriental medicine" at KISS, and then they handsearched Randomized Controlled Trials related to oriental medicine in Korea. According to guidelines defined by CONSORT statement, they assessed whether 37 items were followed by guidelines at yes or no answers in order to evaluate the quality as well. They assessed the definition of each item independently. After comparing, they made a decision on the item of different outcomes through an agreement with a third party. Results : Total number of randomized controlled trials in Atopic Dermatitis related to oriental medicine is eleven. A randomized controlled trial was first published in 2007. One or two papers associated it were released every year except 2013. 9 papers of them were approved by Institutional Review Board and were received written consent. The study included between 20 to 40 subjects for 2-arm parallel study design. The effect of treatment was observed for 4 to 8 weeks. On average, the number followed guidelines was 18.64 among those of 37 CONSORT statement and 50.38% of them was followed overall. Conclusion : As an interest about randomized controlled trials was increased, it was needed to care the standard providing information of CONSORT statement on randomized controlled trials related to oriental medicine in Korea. It is suggested to provide more clear information about it, so that it can be a motive for improving quality of the journal in oriental medicine.

Objectives : The purpose of this study was to investigate the clinical manifestations and influences of occupation, concomitant allergic diseases, various aggravating factors and seasonal changes for patients with dyshidrotic eczema. Methods : A total of 219 patients who showed clinical manifestations of dyshidrotic eczema were included in this study. We retrospectively reviewed the clinical characteristics of all the patients. Results : 1. Among 219 patients, there were 76 male patients (34.7%), 143 female patients (65.3%), male to female ratio was 1: 1.88. The average age of the patients at admission was $31.9{\pm}9.8$ years, had a lot of 20s and 30s the distribution. 2. The average of duration of the admission to the clinic after the onset was $4.01{\pm}4.42$ years. 3. Common accompaning diseases dyshidrotic eczema patients have were allergic rhinitis(87 cases), and atopic dermatitis(55 cases), sweating(45 cases), and asthma(10 cases) in order. 4. The most commonly occurring sites for dyshidrotic eczema patients are fingers(60.3%), palm(53.4%), dorsum of hand(19.2%) and nails(5.5%) in order. 5. The most commonly contacted materials for dyshidrotic eczema patients are water(130 cases), cosmetics(40 cases) and rubber gloves(34 cases) in order. The aggravating factors of dyshidrotic eczema patients are emotional stress(110 cases), fatty food(22 cases) and alcohol(22 cases) in order. 6. The number of patients showing seasonal changes in symptoms was 111. The season with the most severe symptom was summer with 76 cases, which was statistically significant. Conclusions : This study demonstrated the various clinical characteristics of Korean patients with dyshidrotic eczema.

Objectives : The aim of present study was to investigate the skin characteristics of twenty-five healthy Korean subjects. These results can be used as basic data for the development of future herbal applications. Methods : Skin type, hair and scalp condition, water content, sebum content were investigated through the measurement of skin conditions and subjective questionnaire survey to know the skin characteristics of subjects. Results : Most of subject has 'Dry Skin' and they have trouble about their keratin(dandruff), scalp itching, alopecia, hair texture, scalp moisture. Conclusions : More studies are needed to investigate the skin characteristics of healthy Korean subjects.

Objective : This study was performed to find out the differential points of the pattern identification for Acne vulgaris and suggest them to the clinic setting Methods : We searched the papers that suggest patterns identification for Acne vulgaris through China National Knowledge Infrastructure(CNKI), Oriental Medicine Advanced Searching Integrated System(OASIS), Korean Traditional Knowledge Portal(KTKP), Korean Studies Information Service System(KISS). We looked through all the papers, excluded unintended papers, and finally chose some papers that are suitable for inclusion. Then, We extracted all the patterns of Acne vulgaris and chose frequently suggested patterns. Finally we extracted symptoms from selected patterns and arranged them in order of frequency. Results and Conclusions : 1. We found 215 papers and chose 17 papers (Korean : 6, Chinese : 11). 2. We extracted total 67 demonstrations and chose Patterns of Wind-Heat, Damp-Heat, Phlegm and Static blood, Disharmony of the Thoroughfare and Controlling vessels. 3. In Acne vulgaris, the pattern of Wind-Heat tends to have a red papule, sometimes pustule, pain with itching, burning sensation, tidal reddening of the cheeks, white head or black head. The pattern of Damp-Heat tends to have a pustule, node, redness and swelling, pain and greasy skin. The pattern of Phlegm and Static blood has a tendency to be a nodule or cystoma that is hard, dark, painful, long lasting and hard to recover and easy to recur with a scar, pimple, pigmentation. The pattern of Disharmony of the Thoroughfare and Controlling vessels tends to be exacerbated before menstruation, have a small papule that is not easily treated under the chin or around the mouth.

Objectives : This study is aimed to analyse immunohistochemical research trend on psoriasis and consider the results in terms of Korean medicine. Methods : We analyse both immunohistochemical research papers and Korean medical papers on psoriasis, which I searched using KISS(Korean studies Information Service System) and OASIS(Oriental Medicine Advanced Searching Integrated System). Results : Psoriasis is characterized by hyperporliferation of the epidermis, inflammatory cell accumulation and increased tortuosity and dilatation of dermal papillary blood vessels. Recently, angiogenesis is considered to play a major role in pathogenesis of psoriasis. Conclusion : In Korean medicine, blood-tonifying, blood-cooling and blood-activating medicinal herbs have generally been used to treat psoriasis. I consider these herbs which also treat vessel problem can have an effect on angiogenesis of psoriasis.

Objective : The purpose of this study is to summarize the differential diagnosis between allergic and nonallergic rhinitis and suggest management. Methods : We reviewed the current update of medical history and physical examination for allergic and nonallergic rhinitis. Then we analyzed clinical characteristics according to onset age, gender incidence and state of nasal symptoms, etc. Results : 1. Patients with nonallergic rhinitis tend to develop symptoms at a later age(>35 years of age), and there is a female-to-male incidence ratio for nonallergic rhinitis of 2:1 to 3:1. 2. Patients with nonallergic rhinitis report nasal congestion, nasal rhinorrhea and more often report postnasal drip rather than sneezing and itching, which are predominant symptoms of allergic rhinitis. And the nasal mucosa in nonallergic rhinitis usually looks normal. 3. Patients with nonallergic rhinitis have few complaints of concomitant symptoms of allergic symptoms and the absence of other atopic diseases in the patient or in the family supports the diagnosis of nonallergic rhinitis. 4. Common triggers of nonallergic rhinitis are nonspecific irritant exposures and many patients with nonallergic rhinitis find that antihistamines have no benefit. Conclusions : The differential diagnosis between allergic and nonallergic rhinitis is clinical and relies on a detailed medical history and physical examination.

Objectives : We performed a clinical study to investigate autonomic conditions in persistent allergic rhinitis depending on various pattern identifications and the availability of heart rate variability (HRV) as a pattern identification diagnostic tool. Methods : 32 patients with persistent allergic rhinitis were asked to interview with doctor of Korean Medicine and perform the four pattern questionnaires (Cold-Heat Pattern, Phlegm Pattern, Yin Deficiency pattern, bloodstasis pattern). Then, they were examined their autonomic conditions with heart rate variability test. Results : Patients were classified as three pattern groups (Lung-stomach heat, Lung qi deficiency cold, Lung-spleen qi deficiency) by doctor. In the Lung qi deficiency cold group, Total power of the HRV (TP) and the power of the low frequency component (LF) significantly higher than in the Lung-stomach heat or Lung-spleen qi deficiency group (P < 0.05). Also, Patients were classified as 8 pattern groups (Cold/Heat, Phlegm/Non-phlegm, Yin deficiency/Non-yin deficiency, Bloodstasis/Non-bloodstasis) by four pattern questionnaires. Only in the Yin deficiency group, the power of the low frequency component (LF) significantly lower than in the Non-yin deficiency group (P < 0.05). There were not any significant differences in the rest groups. Conclusions : The result may provide that HRV doesn't reflect well the differences in the various pattern groups, and the HRV's availability is low. Continuous studies are needed to develop the objective and standardized pattern identification diagnostic tool for allergic rhinitis.

Objectives : 본 연구를 통하여 한방안이비인후과 임상실습에서 Objective Structured Clinical Examination (OSCE) 수업의 효용성을 알아보고자 한다. Methods : 2014학년도 한의과대학 본과 4학년 학생을 대상으로 안이비인후과 임상실습의 한 방법으로 OSCE를 시행한 후, 학생들은 OSCE 수업 만족도에 관한 설문지를 작성하고 레지던트는 학생들을 평가하였다. OSCE 평가 성적과 기타 임상실습 및 필기시험 성적과의 연관성을 분석하여 OSCE 성적의 학점 부여 타당도를 살펴보았다. Results : 만족도 관련 설문 결과 총 평균점수가 4.34점이 나왔다. OSCE성적과 기타 임상실습 및 필기시험 성적과의 상관성을 pearson correlation을 통하여 살펴본 결과, OSCE성적은 임상실습 성적 및 (r=0.210, p=0.056), 필기시험 성적과 통계적으로 유의한 상관성이 없었다 (r=0.064, p=0.567). Conclusions : 위의 결과로부터 임상실습에서 OSCE 시행은 학생들의 학습 만족도를 높인다는 것을 알 수 있었다. 임상실습에서 효율적으로 OSCE를 활용하는 방안을 모색하기 위해 더 많은 연구가 필요할 것으로 생각된다.

Objective : This study was performed to investigate the effect of herbal steam therapy on peripheral facial paralysis. Method : 33 patients with peripheral facial paralysis were treated with Korean Medicine therapy including herbal steam therapy. We evaluated the effect of herbal steam therapy by Yanagihara grading system and satisfaction measurement. Results : 1. 33 patients were treated with korean medicine therapy and $27.85{\pm}16.44$ times of herbal steam therapy. 2. After treatment, Y-system score was significantly increased(p-value<0.001) from $18.45{\pm}6.5196$ to $35.42{\pm}5.2501$. 3. 87.88% of patients was satisfied with herbal steam therapy. Conclusions : Herbal stream therapy seem to be effective to improve symptoms of peripheral facial paralysis. Further studies will be needed to identify the beneficial of herbal steam therapy on peripheral facial paralysis.

Objectives : The purpose of this study is to investigate the effects of DOGO phreatic water containing germanium on Atopic Dermatitis in NC/Nga mouse. Methods : We made DOGO phreatic water added germanium. After making atopic dermatitis caused by sensitizing NC/Nga mouse to DNCB(dinitrochlorobenzene), we made mouse swim in tanks each filled with distilled water, tap water, DOGO phreatic water, DOGO phreatic water(added germanium) for 30minutes everyday. 3weeks later, we analyzed skin clinical score, total IgE levels(by ELISA), WBC differential counting(Neutrophils, Monocytes), absolute cell number of $Neutrophil^+Gr-1^+$, CCR3 mRNA expressions(by Real-time PCR), IL-4, IFN-${\gamma}$ production levels(by ELISA), histologic test(by H&E staining, toluidine blue staining). Results : The results of making NC/Nga mouse induced atopic dermatitis swim in tanks filled with DOGO phreatic water(contain germanium) are as follows. 1. Skin clinical scores were decreased significantly in comparison to control group. 2. Total IgG levels were decreased significantly in comparison to control group. 3. WBC differential counting(Neutrophils, Monocytes) were decreased significantly in comparison to control group. 4. Absolute cell number of $Neutrophil^+Gr-1^+$ were decreased significantly in comparison to control group. 5. CCR3 mRNA expressions were decreased significantly in comparison to control group. 6. IL-4, IFN-${\gamma}$ production levels were decreased significantly in comparison to control group. 7. The epithelial tissue thickness, leucocytes infiltration, erythema, edema, excoriation, scaling, mast cells infiltrations in dorsal skin were decreased in comparison to control group. Conclusions : These results indicate that DOGO phreatic water(contain germanium) can be used for helping treat atopic dermatitis.

Purpose : The purpose of this study is to investigate the effect of ARTEMISIAE CAPILLARIS HERBA and COPTIDIS RHIZOMA Extracts on cell death in pancreatic cancer cells. Method : Human-derived pancreatic cancer cell line, MIA PaCa-2 cells were treated by Prescription A with various concentrations and the cytotoxicity was determined by MTT assay. To investigate the effects of Prescription A on pancreatic cancer cells, the staining of Annexin V/PI, cell cycle arrest, nuclear chromatin condensation and the production of reactive oxygen species (ROS) were examined. The effect of Prescription A's effective components, ARTEMISIAE CAPILLARIS HERBA and COPTIDIS RHIZOMA Extracts on cell death were also observed. Results : The viability of MIA PaCa-2 cells treated with Prescription A were decreased in a dose dependent manner. Prescription A induced cell death in MIA PaCa-2 cells as shown by result of Annexin V/PI double staining, chromatin condensation and cell cycle arrest. In addition, production of ROS was increased by Prescription A treatment, suggesting that ROS induced by Prescription A mediated cell death. Furthermore, Prescription A's effective components, ARTEMISIAE CAPILLARIS HERBA and COPTIDIS RHIZOMA Extracts were also induced apoptosis of MIA PaCa-2 cells through ROS production. Conclusion : These results suggest that Prescription A's effective components, ARTEMISIAE CAPILLARIS HERBA and COPTIDIS RHIZOMA Extracts induced death of MIA PaCa-2 through ROS production.

목적 : 본 연구에서는 백개자 열수추출물이 활성화된 대식세포 및 사람 비만세포주, HMC-1에서 염증 반응을 효과적으로 억제하는가를 관찰하고자 하였다. 방법 : 대식세포에 여러 농도의 백개자 열수추출물을 가한 뒤 LPS로 염증을 유도하여 NO 생산, iNOS와 COX-2 단백질 발현을 관찰하였으며 HMC-1에도 여러 농도의 백개자 열수추출물을 가한 후 PMACI로 염증을 유도하여 histamine 분비와 NF-${\kappa}B$ 활성 및 $I{\kappa}B$-${\alpha}$의 인산화, MAPKs pathway에 대한 저해효과를 관찰하였다. 결과 : 백개자 열수추출물은 대식세포에서 LPS로 유도된 NO 생성 및 INOS, COX-2 단백질 발현을 농도 의존적으로 저해하였으며 HMC-1에서 PMACI로 유도된 histamine의 분비와 p38 MAPK, ERK, JNK의 인산화 반응 및 $I{\kappa}B$-${\alpha}$의 인산화와 NF-${\kappa}B$의 활성을 저해하였다. 결론 : 백개자 열수추출물은 대식세포 및 비만세포의 활성을 저해함으로써 알레르기 질환의 치료에 사용될 잠재성이 크다고 사료된다.

Objective : The purpose of this study is to report the efficacy of using oriental medical treatment(herb-medicine, acupuncture) with Melonix Calyx Vomiting Therapy for atypical facial pain. Methods : We experienced two cases of atypical facial pain treated with oriental medical treatment(herb-medicine, acupuncture) and Melonix Calyx Vomiting Therapy(MCVT). MCVT is a traditional treatment, which induces vomiting by eating Melonix Calyx powder with water. To evaluate the results of this treatment, we used the Visual Analogue Scale(VAS) and Pain Rating Scale(PRS). Result : After using MCVT with oriental medical treatment, VAS, PRS and clinical symptoms of 2 patients were improved remarkably. Conclusion : This study suggests that using MCVT with oriental medical treatment is effective on atypical facial pain.

Objective : The purpose of this report is to know the effect of Sopung-san and Cheonggisamseup-tang(消風散 合 淸肌滲濕湯) on seborrheic dermatitis. Methods : We experienced one case of seborrheic dermatitis patient with Sopung-san and Cheonggisamseup-tang. To evaluate the effectiveness of this treatment, we used the Visual Analogue Scale(VAS) about symptoms of flare, itching, and observe scales. Results : After the treatment, the grade of flare and itching VAS was decreased, and clinical symptoms were improved including scales. Conclusion : The result indicate that Sopung-san and Cheonggisamseup-tang is helpful in the treatment of seborrheic dermatitis. And an author consider that additional clinical studies are needed to verify the effectiveness of herbal medicine with objective assessment tools.

홍반과 소양감을 주증으로 하는 모르타르로 인한 직업성 알레르기성 접촉성 피부염 환자 1례에서 한약치료, 침 치료, 약침치료 및 외치요법 등의 한방치료만으로 양호한 치료효과를 확인하여 이를 보고하는 바이다.

손발에 수포를 형성하고 스트레스에 민감하며 수족다한증에서 호발하는 한포진을 한의학적으로 비주사말(脾主四末)이 되지 않은 상태와 스트레스에 의한 심화(心火)의 범주에 속한다고 가정하고 사상의학(四象醫學)의 각 체질별 표병(表病)과 이병(裏病)에서 비주사말(脾主四末)의 개선과 심화(心火)를 해결할 수 있는 처방들을 투여하여 환자 3례에서 유효한 치료효과를 얻었기에 치험례로 보고하는 바이다. 향 후 다른 처방들에 대한 사상의학적 연구도 필요할 것으로 사료된다.

Objective : The purpose of this study is to report the Korean medicine treatment on Pustulosis Palmaris et Plataris with Extra-Palmoplantar lesions. Method : We adopted herbal medicine(Seungchung-tang-1, Eunjin-1), acupuncture and vapor treatment to treat Pustulosis Palmaris et Plantaris with Extra-Palmoplantar lesions. Steroids were administered for preventing aggravation. We observed patient's hot sensation, itching and pain of both hands and patient's itching and pain of both feet with the Visual Analogue scale(VAS). Also photographs were taken for judging inflammatory skin. Result : After the treatment, the grade of VAS was decreased from 10 to 0 and clinical symptoms were resolved. By judging photographs patient's inflammatory skin was cured. Administered steroid were also reduced from 15 mg per day to 0. Conclusion : This study shows that the Korean medicine treatment was effective to treat Pustulosis Palmaris et Plataris with Extra-Palmoplantar lesions and enable to stop steroid therapy.

Objective : The purpose of this report is to know the effect of oriental medical treatments on acute allergic contact dermatitis with multiple nummular eczema. Methods : We treated a 34-years-old woman patient with acute allergic contact dermatitis on face and multiple nummular eczema on both arm, right ankle and left inguinal region by Korean medicine. After Korean medical treatment for 42 days in hospital, we measured the decrease of exudation and pruritis as well as recovery of skin damages. We recorded pictures of changes on symptoms. Results & Conclusion : The symptoms of ACD and nummular eczema were significantly improved except scars. Exudation and pruritis were disappeared and skin damages were almost recovered until discharge from hospital. After 20 days we could confirm the symptoms were clearly disappeared. Thus Korean medical treatments are effective on ACD and nummular eczema.

Objectives : The purpose of this study is to investigate the effect of eating Saengshik and applying Hwangryunhaedok-tang on skin for pregnant woman diagnosed as Pruritus Gravidarum. Methods : This case study was done on 5 Pruritus Gravidarum patients who had visited Korean Medicine Clinic. All the patients are treated with Saengshik and Hwangryunhaedok-tang was applied on their skin. VAS(Visualanalogue scale) and photographs were used to evaluate the symptom change. Results & Conclusion : Most symptoms of patients were improved in this study. Saengshik and Hwangryunhaedok-tang can be effective in patients who were diagnosed as Pruritus Gravidarum.
