Intensive Korean Medicine Treatments for Low Back Pain and Decreased Living Function due to Herniation Intervertebral Discs in the Lumbar Spine: A Case Study (요추 추간판 탈출증으로 인한 요통 및 생활기능저하에 대한 한방 집중치료 치험 1례)

  • Background: This case study suggests the potential use of Korean medicine treatments as a conservative management for low back pain and decreased living function due to herniation of intervertebral discs in the lumbar spine. Case Summary: The patient suffered low back pain and decreased living function due to herniation of intervertebral discs of the lumbar spine. Korean medicine treatments, including herbal medicine, acupuncture performed on BL23, BL40, SP6, and GB39, and pharmacupuncture containing Sinbaro, were applied for eight days. The numeric rating scale (NRS) results for low back pain decreased from 6 to 2 and the Oswestry Disability Index (ODI) low back pain scores decreased from 75.56 to 31.11. Walking time also increased from less than one minute to 20 minutes, with an increase in range of motion (ROM) from "Uncheckable" to about normal motion range. Conclusion: Korean medicine treatment can be considered to be an effective conservative management option for pain and decreased living function in patients with herniation of intervertebral discs in the lumbar spine.

  • Recently, the incidence of pressure sore has been increased, due to increased number of patients with central nervous system injuries after traffic and industrial accidents or with long term loss of consciousness due to drug intoxication. The management of sacral pressure sore has been improved through the development of myocutaneous flap. However, sacrifice of the deep muscle cause some problems such as intraoperative bleeding, functional disabilities of donor site and further limitation of reconstruction for recurrent pressure sores in paraplegic patients. The development of perforator-based island skin flap introduce a new treatment modality for sacral pressure sores. We used perforator-based island skin flap in 15 cases with reasonable result from January 1998 to February 2000. This flap has a many advantages such as no significant sacrifice of the gluteus maximus muscle, wide rotation arc, reliable blood flow of the perforator, short elevation time for the flap, and no post-operative hindrance to walking in patients who are not paraplegic. There was no significant complication without significant sequelae and donor sites could be repaired primarily.

Suggestions for Building Coast-Inland Linkage Transport System to Promote Island Tourism (도서관광 활성화를 위한 연안-내륙 연계교통체계 구축 방향)

  • Recently, there has been a noticeable increase in the number of tourists in islands that are well-known for their natural landscapes. It is expected that this number will go up steadily, due to the increase in per capita income and leisure time. Regarding tourism transportation, utilizing linkage transport between coastal passenger terminal and inland transport system is crucial. However, the existing system does not measure up to the expectations. This is attributable to the fact that the distances between coastal passenger terminals and inland transportation terminals are too far. Also, the current public transportation system lacks support and needs to take the role of connecting the two terminals. This study shows how coast-inland linkage transport system should be reformed in order to promote island tourism. It focuses on building linkage transport system and minimizing walking distance. Also, adjusting passenger ship schedule to the characteristics of island tourism is needed. For the sake of passengers, coastal passenger ship fare should be reduced, and integrated transport information should be provided.

  • The Asian ladybird, Harmonia axyridis, might be distinguished in biological control agents for the several aphid species in the greenhouses as well as in the fields. Therefore, feeding behavior and predacious capability characteristics were observed. When ladybirds are searching for aphids as their preys, they use the maxillary palpus as contact reception. Otherwise, most of aphids escape from walking or chewing ladybird to other place (mainly move to near-by leaf, or fall down to the under-leaf or ground) instead of plant sucking. Consumed time of ladybirds for searching and feeding prey were significantly different and dependent on their development stage. The ladybird adults ate about 120 green peach aphids and 257 cotton aphids per day. The 3rd instar of ladybirds ate 2,420 and 2,750 eggs of greenhouse whitefly on the cucumber and tomato leaf, respectively, as well. However, ladybird instars taken greenhouse whitefly eggs could not continuously develope to adult, on the other hand, ladybirds taken aphids were normal condition.

Vaginal Reconstruction with Laparoscopic-perineal Rectosigmoid Colpopoiesis in Mayer-Rokitansky-Kuster-Hauser Syndrome: A Case Report (Mayer-Rokitansky-Kuster-Hauser 증후군 환자에서 회음부 복강경하 직결장질성형술을 이용한 질의 재건: 증례보고)

  • Purpose: Various operations have been proposed to compensate for congenital absence of the vagina using ileal or colonic interposition. These methods involve laparotomy, which shows postoperative complications such as long scar and delayed recovery. One case of neovagina reconstruction with laparoscopic rectosigmoid colpopoiesis in Mayer-Rokitansky-Kuster-Hauser syndrome is presented to avoid laparotomic complications. Methods: Laparoscopic surgery was performed in a 27-year-old MRKH syndrome patient. After a cruciate incision, blunt dissection through two-finger wide space was created between the bladder and the rectum. A 14-cm rectosigmoid segment vascularized by a branch of sigmoid artery was isolated by laparoscopy. The distal end was sutured with vaginal vestibule mucosa. A continuity of intestine was restored by circular end-to-end proximate curved intraluminal stapler CDH29$^{(R)}$ through perineal opening. Results: Total operation time was 4 hr 15 min. Normal walking and ingestion were possible within 3 days and 4 days after surgery. The hospital stay was 7 days and the patient was followed up for 6 months. The neovaginal introitus was wide enough for inserting two fingers, and there has been no narrowing of the neovagina on palpation as confirmed by vaginogram. The patient had functional self-lubricating neovagina without excessive mucous production or the need for routine dilation or unnoticeable scar. Conclusion: The successful result of this laparoscopic vaginal reconstruction technique with rectosigmoid segment suggests that this technique can be considered for the option of vaginal reconstruction in girls with the MRKH syndrome.

The characteristics of farmer's dermal exposure during pesticide spraying and dilution in cut rose greenhouse (절화장미 시설하우스에서 방제 작업시 농약의 피부노출 특성)

  • This study was conducted in cut rose cultivation field in Goyang to evaluate pesticide exposure of farmers and the relationship between the exposure and work environment/method for cut rose farmers. Dermal exposure was assessed with patch (thin chromatography layer paper), cotton glove by body parts during mixing and spraying works in which pyrethroid, organophosphate, carbamate pesticide were used in 4 cut rose greenhouses located in Goyang province, checking characteristics of environment/working method at the same time. Body parts assessed were as follows ; glove : hand, patch : head, neck(front/back), chest, back, shoulder(right/left), upperarm(right/left), forearm(right/left), thigh(right/left), shin(right/left). Pesticides were analyzed using gas chromatography(NPD/FID) after extracting with ethyl acetate. Exposure amount differed according to pesticide type. But after standardizing with total net weight of pesticide sprayed, there were no significant difference among pesticides. There were significant difference in exposure amount among body parts(especially exposure amount of thigh, shin were more than the others), which means exposure happen not by pesticide dispersion in air but by contact with cut rose(leaf, branch) indirectly. Walking forward during spraying made farmers more exposed than waling backward, these results means contacts with leaves/branches made exposure happen in cut rose greenhouse also. As a result, pesticide exposure in cut rose greenhouse was related with contact of leaf/branch which pesticide remained in, which made exposure pattern (especially exposure amount) differ by body parts

Comparative Analysis on Gait Patterns of the Elderly and the Young Regarding to Foot Pressure (고령자와 청장년층의 발바닥 압력분포에 따른 보행패턴 비교 분석)

  • The purpose of this study was to find the difference in gait patterns when elderly and young people walk by analyzing COP, Gait Line, Foot pressure pattern, and ensuring the original biomechanics technology of developing high performance footwear for the elderly. The subjects who took part in the test consist of 20 elderly people and 20 young people. The physical features of the elderly people that were recruited for the study are as below: 20 healthy male subjects(elderly people) with an average age of 75.43 yrs(S.D 6.46 yrs), weight of 68.10 kg(S.D 0.94 kg) and a height of 168.65 cm(S.D 1.47 cm). Foot pressure pattern data was collected using a EMED-AT system(Novel Gmbh, Germany) operating at the 50 Hz during walking. The results are as follow : COP route of the elderly leans to lateral compared to the young, and Gait Line from heel to toe is not clear and laterally curved. At the same time, a contact are aonthe midfoot is high compared to the young, and maximum force of the forefoot is low. As a result of analysis, in order to develop high performance footwear for the elderly, it is necessary to develop lasts and soles reflecting the elderly's gait patterns.

The Effect of Types of Initial Drive-in Steps on Technical Factors in Basketball

  • Objective: The purpose of this study was to investigate the effect of types of drive-in initial steps in basketball on technical factors, to provide basic information for the enhancement of basketball skill. Method: Ten men (age: $24.70{\pm}2.26years$; height: $181.00{\pm}5.72cm$; weight: $75.70{\pm}8.23kg$; career length: $10.00{\pm}3.59years$), each with a career length of over five years and no history of injury to the lower extremities within the prior six months, participated in this study. They were asked to perform four types of drive-in movements at $35{\sim}60^{\circ}$, wearing their own shoes, after running from a start line 5 m away and catching a basketball passed by an expert passer. The drive-in movements were measured by eight infrared cameras (Oqus 300, Qualisys, Sweden). Collected raw data were used to calculate total initial step time, displacement, velocity, center of mass (COM) height, and COM velocity. Results: Total initial step displacement and velocity of cross drive-ins (JC, SC) were greater than that of direct drive-ins (JD, SD; p < .05). COM velocity of cross drive-ins (JC, SC) was also greater than that of direct drive-ins (JD, SD; p < .05). Conclusion: Our results indicated that cross drive-ins, regardless of stop step type, are more effective than direct drive-ins. This is because cross drive-ins are technically bold due to less influence from walking violations and double dribble rules in basketball. However, using one-sided movement is too difficult to play in competitive game; therefore, basketball players should develop the ability to choose appropriate movement frequency.

Correlations Among the Berg Balance Scale, Gait Parameters, and Falling in the Elderly (노인에서 Berg 균형 척도, 보행 변수, 그리고 넘어짐과의 관계)

  • This study examined the correlations among the Berg Balance Scale, which is a clinical tool used to evaluate balance ability, spatiotemporal parameters of gait, and falling; determined the parameters most closely related to falling; and identified a discriminatory parameter and its predictability. Thirty-four subjects aged 72 to 92 years participated in this study. Following a questionnaire survey about falling, the Berg Balance Scale and spatiotemporal parameters of gait were measured. The results revealed that the incidence of falls increased with aging and an accompanying reduction in the flexion range of motion of the hip joint. The gait characteristics of elderly people who fell easily included a slower walking speed, shorter stride, and longer stance time than other elderly. When the cutoff score was set at 45, the Berg Balance Scale was able to identify correctly those individuals who truly have experience of falling than when the cutoff score was set at 39. But when the cutoff score was set at 39, the scale's specificity identifying correctly those individuals who truly have not experience of falling was higher than at the cutoff score of 45. Therefore, the Berg Balance Scale is an appropriate screening method in a clinical setting for the early detection of elderly people at risk of falling. In conclusion, elderly people with a Berg Balance Scale score. below 45 are the most likely to fall owing to their decreased balance ability.

  • In this paper, we introduce a mobile-type tiny sensor device that can classify the activities of daily living based on the state-dependent motion analysis using a 3-axial accelerometer in real-time. The device consists of an accelerometer, GPS module, 32bit micro-controller for sensor data processing and activity classification, and a bluetooth module for wireless data communication. The size of device is 50*47*14(mm) and lasts about 10 hours in operation-mode and 160 hours in stand-by mode. Up to now, the device can recognize three user activities ("Upright", "Running", "Walking") based on the decision tree. This tree is constructed by the pre-learning process to activities of subjects. The accuracy rate of recognizing activities is over 90% for various subjects.
