Calcifying Aponeurotic Fibroma - A Case Report - (석회화 건막 섬유종 - 증례 보고 -)

  • Calcifying aponeurotic fibroma is a benign, aggressive fibrous tumor seen in childhood or adolescence that most commonly affects the palm. We report our experience in treating calcifying aponeurotic fibroma in the hand of fifteen-year-old boy. He was presented with a recurrent mass in hypothenar area of his left hand. He had a history of local excision of the mass 7 years ago in local clinic. The palpable mass was hard, tender and movable. The simple X-ray revealed multifocally scattered microcalcifications in the mass. The excised mass showed the features of dense fibrotic soft tissue tumor with multiple small whitish calcifications. The microscopic sections showed a lobulated and a poorly circumscribed proliferation of dense fibrous tissue, containing islands of metaplastic chondroid differentiation with prominent calcification. The recommended treatment of this lesion is wide excision allowing for sparing of functional or vital structures, but high recurrence rate as this case has been reported to be more than 50 percent.

PU.1 Is Identified as a Novel Metastasis Suppressor in Hepatocellular Carcinoma Regulating the miR-615-5p/IGF2 Axis

  • Invasion and metastasis is the major cause of tumor recurrence, difficulty for cure and low survival rate. Excavating key transcription factors, which can regulate tumor invasion and metastasis, are crucial to the development of therapeutic strategies for cancers. PU.1 is a master hematopoietic transcription factor and a vital regulator in life. Here, we report that, compared to adjacent non-cancerous tissues, expression of PU.1 mRNA in metastatic hepatocellular carcinoma (HCC), but not primary HCC, was significantly down-regulated. In addition, levels of PU.1 mRNA in metastatic hepatoma cell lines MHCC97L and MHCC97H were much lower than in non-metastatic Hep3B cells. Transwell invasion assays after PU.1 siRNA transfection showed that the invasion of hepatoma cell lines was increased markedly by PU.1 knockdown. Oppositely, overexpression of PU.1 suppressed the invasion of these cells. However, knockdown and overexpression of PU.1 did not influence proliferation. Finally, we tried to explore the potential mechanism of PU.1 suppressing hepatoma cell invasion. ChIP-qPCR analysis showed that PU.1 exhibited a high binding capacity with miR-615-5p promoter sequence. Overexpression of PU.1 caused a dramatic increase of pri-, pre- and mature miR-615-5p, as well as a marked decrease of miR-615-5p target gene IGF2. These data indicate that PU.1 inhibits invasion of human HCC through promoting miR-615-5p and suppressing IGF2. These findings improve our understanding of PU.1 regulatory roles and provided a potential target for metastatic HCC diagnosis and therapy.

  • The composting is a biological process that converts organic matter into useful resources such as fertilizers. It is a continuous transition of microbial communities to adapt changes in organic matter and environmental conditions (carbonation rate, temperature, humidity, oxygen supply, pH, etc.). Most of the composting plants are located in the proximity of the residential areas. It is a general scenario where government authorities receive complaints from the local residents due to release of odor from the composting, and has become a social problem in Korea. Identification of dominant microorganisms, understanding change in microbial communities and augmentation of specific microorganism for composting is vital to enhance the efficiency of composting, quality of the compost produced, and reduction of odor. In this paper, we suggest the optimum operation conditions and methods for compost depot to reduce odor generation. The selection of the appropriate microorganisms and their rapid increase in population are effective to promote composting. The optimal growth conditions of bacteria such as aeration (oxygen), temperature, and humidity were standardized to maximize composting through microbial degradation. The use of porous minerals and moisture control has significantly improved odor removal. Recent technologies to reduce odor from the composting environment and improved composting processes are also presented.

  • This study is a research based on the non-equivalent control group pretest-posttest design whose purpose is to examine the effects of music therapy on the anxiety of patients who take the Intravenous Urography test and their feeling of discomfort during the test. "The first hypothesis that the experimental group who receive music therapy will be lower in score for anxiety during the Intravenous UrographyI test than the control group who do not was supported because after the therapy, the experimental group was found significantly decreased in that score in comparison to the control group. "The second hypothesis that the experimental group who receive music therapy will be fewer in vital signs after the Intravenous Urography test than the control group who do not" was rejected in terms of both systolic and diastolic blood pressure. But the same hypothesis was partially supported because the two groups showed a significant difference in pulse rate after the test. "The third hypothesis that the experimental group who receive music therapy will be less in the feeling of subjective discomfort during the Intravenous Urography test than the control group who do not" was verified to find that the two groups were significantly different from each other in the feeling. Specifically, there was a significant difference between the two groups in only one sub-area of that These findings suggest that music therapy could be an alternative method which can effectively reduce the state anxiety of patients during the Intravenous Urography test.

  • Background: Bronchiectasis is a irreversible disease, a lot of cases of which are associated with chronic bronchitis, pulmonary emphysema and bronchial asthma due to chronic recurrent pulmonary infection. Therefore, pulmonary functions in bronchiectasis may also vary with associated diseases or involved segments. Methods: For the evaluation of ventilatory dynamics in bronchiectasis with respect to the pathoanatomic types of bronchiectasis and the degree of dyspnea, a total of 93 cases comprising 45 cases of tubular, 30 saccular and 18 mixed type of bronchiectasis whose clinical diagnosis was confirmed by bronchography were analyzed retrospectively. They were also divided into two groups: those with Hugh-Jones dyspnea grade 1 & 2 (group I) and those with Hugh-Jones dyspnea grade 3 & 4 (group II). Pulmonary functions tested in this study were analyses of curves of forced expiratory volume and flow-volume, and determinations of maximal voluntary ventilation and closing volumes. Results: The results were as follows; 1) The vital capacity and parameters reflecting expiratory flow rate except PEF were significantly reduced in saccular and mixed type than that in tubular type of bronchiectasis. 2) In saccular and mixed type, the maximal voluntary ventilation tended to decrease while CV/VC tended to increase. 3) As the degree of dyspnea became serious, the involved segments were progressively increased. In contrast, ventilatory functions were significantly reduced in proportion to the severity of dyspnea. Conclusion: These findings suggest that in bronchiectasis, there be obstructive ventilatory impairment combined with mild restrictive ventilatory impairment, which becomes more prominent in saccular and mixed type and also as the degree of dyspnea progresses.

  • Background : An assessment of the presence and the degree of reversibility of airflow obstruction is clinically important in patients with asthma or chronic obstructive pulmonary disease. However, the time responses of spirometric parameters in response to bronchodilator have not been well investigated. Methods: We studied 15 patients with asthma. Spirometric and mini-Wright peak expiratory flow measurements were performed at 15, 30, 45, and 60 minutes after using single dose($200{\mu}g$) of inhaled bronchodilator, salbutamol. Results : The mean values of forced expiratory volume in one second($FEV_1$) and forced vital capicaty(FVC) were significantly increased at 60 minutes after using bronchodilator in comparison to 15 minutes. And peak expiratory flow rate measured by either mass flow sensor or mini-Wright peak flow meter were significantly increased at 45 minutes after using bronchodilator in comparison to 15 minutes. Conclusions : To appropriate evaluation of the bronchodilator response in patients with reversible airflow limitation, it would be useful measuring either $FEV_1$ or PEF at the later time point 60 or 45 minutes in comparison to 15 minutes after using bronchodilator.

  • CATCH22 syndrome or DiGeorge syndrome is a medical acronym of cardiac defects, abnormal facial appearances, thymic hypoplasia, cleft palate, and hypocalcemia. Patients with CATCH22 syndrome are susceptible to infection due to an absent or hypoplastic thymus and often have difficulties in maintaining good oral hygiene, which may require dental treatment. We present a case of dental treatment for the uncooperative child with CATCH22 syndrome under general anesthesia. A 4-year-old, 14.8 kg boy with CATCH22 syndrome visited Seoul National University Dental Hospital for dental check up. The patient had multiple caries requiring dental treatment. He experienced the corrective heart surgery due to Tetralogy of Fallot a few years ago. General anesthesia was planned because his heart rate and vital sign had shown unstable during the previous conscious sedation procedure. Dental restorative treatments were successfully performed and no complications were observed during and after the procedure. Safe and effective dental management of the patients with CATCH22 syndrome could be performed with the help of general anesthesia and careful monitoring.

  • Purpose: This study was to analyze the trends and content of all the research (171 research papers) published in the Journal of Fundamentals of Nursing from 1994, when it began to 2000 using an objective tool developed by the researches, and to identify the direction for the future research in Fundamentals of Nursing. Method: Research published in the Journal of Fundamentals of Nursing from 1994 were reviewed. Result: Most of research (94.2%) was non-degree based and funded research was very limited. The average number of researchers per research study was 2.2 and collaborative research has been gradually increasing. The total number of key words was 331 and the average was 2.6 per research. The major key words were not different from other nursing departments and included home care nursing (10 times), health promotion (8 times), self-care (7 times). self-efficacy (7 times), and homo-dialysis (6 times). Eighty seven percent of the research was quantitative research. and there were very few qualitative studies. Considering theory level, it was found that 40% were factor related research For study design, non-experimental studies were most frequent (66%), and the rate, especially of surveys, has remarkably decreased. Selection of subjects by convenience sampling, was most frequent and there were very few studies that provided the rationale for the calculation of sample size. The major subjects of study were patients (44.8%) with various diagnoses. The subjects usually gave oral consent to take part in that study. Giving information (46.9%) and exercise (26.5%) were common nursing interventions, and physiological indices (16.5%). vital signs (10.3%), physical functioning (8.2%), level of knowledge/skill (7.2%) and level of activity (6.2%) were frequently measured as outcome. Variables questionnaires were the major approach used to collect data, and 57.8% of the research provided the Cronbach alpha to guarantee internal consistency of study instruments. Data were analyzed with computerized statistical packages using, ANOVA (42.0%), T test(39.5%), and chi-square test. For the last seven years, nursing research in Fundamentals of Nursing has gradually improved in both quantify and quality. Conclusion: It was difficult to find any uniqueness or difference compared to other departments of nursing. In fact, because the history of the Journal of Fundamentals of Nursing is rather short, we can expect that there will be further improvement in qualify and content in the future.

  • High-frequency induction heating equipment can heat the metal by applying a High-Frequency power to the resonant circuit. The resonance circuit is composed of the work coil and the conduction-cooled power capacitor, it influences the performance of the heat treatment equipment according to the characteristics of the capacitor. However, dependence on conduction-cooled power capacitor's import is high due to lack of core technology research and development. Minimizing the generation of internal heat transmitted inside during LC resonance, reduce the reactive power loss, there is a need for a capacitor within the voltage characteristic outstanding. To implement localization it is vital that prior study of the analysis on the frequency response characteristic for the finished capacitor advanced manufacturer be implemented. Studying the interpolation method to read the value at any point of the characteristic curve for a given log-log scale was applied to the analysis tool of the capacitor by my proposed algorithm. The simulation for reproducing frequency response curves was attempted by assuming a capacitor in a simplified series equivalent RC circuit to obtain the equivalent series resistance value. It was confirmed that the reproduction rate was the result value above 83% as compared to the simulation of the properties and characteristics on the actual reactive power for Peak value, and that the algorithm can be applicable when analyzing and predicting the characteristic curves of a simpled model capacitor.

XRCC1 Gene Polymorphism, Clinicopathological Characteristics and Stomach Cancer Survival in Thailand

  • Background: Stomach cancer is one of leading causes of death worldwide. In Thailand, the incidence and mortality of stomach cancer are in the top ten for cancers. Effects of DNA repair gene X-ray repair cross complementary protein 1 (XRCC1) polymorphisms and clinicopathological characteristics on survival of stomach cancer in Thailand have not been previously reported. The aim of this study was to investigate the effects of XRCC1 gene and clinicopathological characteristics on survival of stomach cancer patients in Thailand. Materials and Methods: Data and blood samples were collected from 101 newly diagnosed stomach cancer cases pathologically confirmed and recruited during 2002 to 2006 and followed-up for vital status until 31 October 2012. Genotype analysis was performed using real-time PCR-HRM. The data were analyzed using the Kaplan-Meier method to yield cumulative survival curve, log-rank test to assess statistical difference of survival and Cox proportional hazard models to estimate adjusted hazard ratio. Results: The total followed-up times were 2,070 person-months, and the mortality rate was 4.3 per 100 person-months. The median survival time after diagnosis was 8.07 months. The cumulative 1-, 3-, 5-years survival rates were 40.4%, 15.2 % and 10.1 % respectively. After adjustment, tumour stage were associated with an increased risk of death (p= 0.036). The XRCC1 Gln339Arg, Arg/Arg homozygote was also associated with increased risk but statistically this was non-significant. Conclusions: In addition to tumour stage, which is an important prognostic factor affecting to the survival of stomach cancer patients, the genetic variant Gln339Arg in XRCC1 may non-significantly contribute to risk of stomach cancer death among Thai people. Larger studies with different populations are need to verify ours findings.
