Various Pathologic Conditions of Sinus Tarsi Syndrome Assessed by Imaging and Arthroscopic Findings (영상학적 및 관절경적 소견으로 평가한 족근동 증후군의 다양한 병적 상태)

  • Purpose: Sinus tarsi syndrome (STS) is caused by various pathologies. However, the exact etiology of STS remains controversial. This study evaluated the imaging and arthroscopic findings of patients who underwent surgical treatment after conservative treatment for STS failed. Materials and Methods: Between December 2014 and August 2018, 20 patients (21 cases) who underwent surgical treatment for STS were included in the study. The clinical results were analyzed using the visual analog scale (VAS) and the American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot functional scale. The radiographic results were analyzed using Meary's angle, calcaneal pitch angle, and hindfoot alignment angle. The pathologic conditions of sinus tarsi were confirmed by magnetic resonance imaging (MRI) and subtalar arthroscopy. Synovitis, bone edema, and accessory anterolateral talar facet (AALTF) were evaluated on MRI. Synovial thickening, cartilage damage, interosseous talocalcaneal ligament (ITCL) and cervical ligament rupture, soft tissue impingement, AALTF, and accessory talar facet impingement (ATFI) were evaluated by subtalar arthroscopy. Results: The mean duration of symptoms was 28.7 months (4~120). All patients showed significant improvement in the VAS and AOFAS ankle-hindfoot scale. Significant improvements in hindfoot alignment angle and Meary's angle postoperatively were noted in patients who underwent medial displacement calcaneal osteotomy. MRI confirmed synovitis in all patients, AALTF in 19 cases (90.5%), and ATFI with bone edema in seven cases (33.3%). In subtalar arthroscopy, pathologic conditions were observed in the following order: synovitis in 21 cases (100%), AALTF in 20 cases (95.2%), ITCL partial rupture in nine cases (42.9%), and soft tissue impingement in seven cases (33.3%). All cases had two or more pathological conditions, and 15 (71.4%) had three or more. Conclusion: In cases of STS that do not respond to conservative treatment, a comprehensive examination of the lesions of the tarsal sinus and lesions around the subtalar joint is essential.

Results of Arthroscopic-assisted Minimally Invasive Removal of a Lateral Periarticular Plate used for the Treatment of AO Type-C Distal Femoral Fractures (AO C-형 원위 대퇴골 골절의 치료로 삽입된 관외측 금속판의 절경 보조하 최소 침습적 제거의 결과)

  • Purpose: To evaluate the usefulness of minimally invasive arthroscopy-assisted plate removal of a laterally inserted periarticular distal femur plate used for the treatment of AO type-C distal femur fractures. Materials and Methods: From October 2002 to November 2005, we evaluated 17 patients whose plates were removed through minimally invasive arthroscopy-assisted plate-removal technique and 15 patients who got their plates removed through conventional method without using arthroscopy, 32 patients in total. All these patients included in this study initially underwent open reduction and internal fixation of the distal femoral fractures with a lateral plate, and complained of continued pain over the lateral femoral condyle after the fracture fixation. The average age was 42.6 (ranges: 20~66) and initial fracture types included 16 cases of C1, 11 cases of C2, and 5 cases of C3 following AO/ASIF classification guidelines. Measured outcomes included: associated intra-articular pathologies, time needed to return to activities of daily living, patients' overall satisfaction, complications following the removal of hardware, and pain before and 6 months after the operation. Results: The distal-most end of the plate was placed in the knee joint in all cases and damage of the lateral articular capsule was found in 23 cases. Continuous wound discharge after surgery was found in one case who underwent arthroscopy-assisted plate removal, and it was treated by irrigation and re-suture. Average time needed to return to activities of daily living was 7 days in arthroscopy assisted group and 7.6 days in conventionally removed group. Fourteen patients (82.4%) who underwent arthroscopyassisted plate-removal reported above 'fair' satisfaction and the Visual analog scale pain score decreased from 4.9 to 1.9, six months after the plate removal. Thirteen patients(86.7%) who underwent conventional plate removal reported above 'fair' satisfaction and the Visual analog scale pain score decreased from 5.2 to 2.5, six months after the operation. Conclusion: Through minimally invasive arthroscopic-assisted plate removal, intrarticular pathology of the knee joint was able to be simultaneously identified and treated at the time of hardware removal. Damage of lateral capsule of the knee joint caused by the inserted plate for the treatment of type C distal femoral fracture was very frequently found and following the plate removal, patients experienced an improvement in pain score. We therefore recommend routine lateral distal femoral plate removal if the bony union is attained in such cases as type C distal femoral fractures whose distal most end of the plates are located in the joint.

A Study on Actual Condition of Damaged Korean Fir(Abies koreana) Caused by Rodents and Identifying Suspected Species in Seseokpyeongjeon Area of Jirisan National Park (지리산국립공원 세석평전 일대 설치류에 의한 구상나무 피해실태 및 가해 의심종 구명)

  • The purpose of this study was to investigate the damage caused by rodents in the area around Seseokpyeongjeon in Jirisan National Park and identify suspected perpetrating species by analyzing rodent excrement found near the damaged trees. Damaged trees were surveyed regardless of species of trees in the exploration trail in the target site where visual observation was possible. Sequencing analysis of mitochondrial DNA Cyt-b was performed on rodent excrement around the damaged trees to identify the perpetrating species. A total of 52 trees were damaged by rodents, and they were all identified to be Korean Fir (Abies koreana) and were concentrated in places where Korean Fir formed communities with high density. A total of 40 Korean Fir over 1.5 meters high were surveyed, and only the affected branches, not the whole tree, were found to be withered in 39 of them. A total of 12 Korean Fir under 1.5 meters high were surveyed, and 7 of which were found to have been completely withered, indicating that relatively young trees were more vulnerable to withering. The genetic sequencing analysis of excrement confirmed that the species that perpetrated the Korean Fir was Craseomys regulus.

Intake of fruits and vegetables may modify the risk of cataract in Korean males: data from Korean National Health and Nutrition Examination Survey 2012 (채소 및 과일의 섭취가 백내장 유병율에 미치는 영향 : 2012년 국민건강영양조사 자료를 이용하여)

  • Purpose: Cataract is a major cause of a loss of eye sight, and is a critical health issue in an aging society. The oxidative stresses to the lens due to various exterior/interior stimuli leads to degenerative changes of the visual system, resulting in cataract. Therefore, reducing the level of oxidative damage is critical in the prevention of the disease. This study examined the association between the risk of cataract and intake of fruits and vegetables, the major dietary source of antioxidants. Methods: Using the data of Korean National Health and Nutrition Examination Survey 2012, the intake of fruits and vegetables of 1,332 males (222 cases) and 2,012 females (377 cases) were analyzed, taking into account the cataract phenotype. Their dietary intake was categorized in 6 groups: TV (total vegetables), F (fruits), TVF (TV+F), SV (salted vegetables), NSV (Non-SV), and NSVF (NSV+F). The level of intakes were evaluated referring to the recommendation level of various dietary guidelines. Results: The presence of cataract was associated with some type of fruit and vegetable intake. In males, the controls had more TVF (p < 0.001), TV (p = 0.001), SV (p = 0.012), NSV (p = 0.007), and NSVF (p < 0.001) intake than the cases, and the lower intake of TVF (< 500g), TV (300g), NSV (< 100g), and NSVF (< 400g) increased the risk of cataract by up to 1.7 fold [95% confidence interval: 1.06-2.71]. In females, the controls also had a higher intake of TVF (p < 0.001), TV (p = 0.042), and NSVF (p < 0.001), but the intake of such fruits and vegetables did not predict the meaningful risk of cataract. The intake of SV and F was not associated with the cataract phenotype in either males or females. Conclusion: The intake of fruits and vegetables can modify the risk of cataract in Korean males and a sufficient intake of those could be effective in the prevention of cataract.

Effects of Gammakdaejo-tang(GMD) on DNCB induced Atopic Dermatitis in Mice (감맥대조탕이 DNCB로 유발된 생쥐의 아토피피부염에 미치는 영향)

  • Objectives : Atopic dermatitis is a chronic inflammatory skin disease with frequent relapses. This study was to investigate the effects of Gammakdaejo-tang(GMD) in DNCB induced atopic dermatitis mice. Methods : The study was divided into five comparion groups. 2,4-dinitrochlorobenzene(DNCB) solution was applied to Nc/Nga mice to induce atopic dermatitis, followed by normal group, negative control group with distilled water, positive control group with Dexamethasione and GMD 200mg/kg or 400mg/kg. The control group was orally administered 200㎕ once daily for 4 weeks. Visual skin condition, Immunoglobulin E, Histamine, Cytokine, Immune cells, Tissue biomarkers were observed. Results : As a result of the dermatitis score evaluation, it was confirmed that the GMD-administered group improved symptoms compared to the negative control group. As a result of measuring IgE, the GMD-administered group significantly decreased compared to the negative control group. As a result of measuring Histamine, GMD group except 200mg/kg of GMD significantly decreased compared to negative control group. As a result of measuring cytokine, GMD 200mg/kg significantly reduced IL-1β, IL-6 and TNF-α compared to the negative control. 400mg/kg significantly reduced IL-1β, IL-4, IL-5, IL-6, IL-10, TNF-α and significantly increased IL-2, IFNγ. As a result of confirming the immune cells, all experimental groups showed no difference in basophil, GMD group significantly reduced monocyte and eosinophil compared to negative control group, and GMD 400mg/kg group significantly reduced white blood cell and neutrophil. And significantly increased lymphocytes. As a result of measuring the gene expression level, all GMD group significantly increased TGF-β1 compared with the negative control group, and filaggrin, VEGF and EGF were significantly increased in GMD 400mg/kg group. Epidermis, dermis thickness, and eosinophil infiltration were found to be decreased in all GMD groups compared with the negative control group. Conclusions : GMD is effective in atopic dermatitis by reducing imbalance of immune response of T cells (Th1 / Th2) and reducing skin tissue damage and inflammatory response.

  • In this paper by using the image taken from the rear of the vehicle to effectively extract the license plate and how to recognize the characters appearing in the offer. How to existing research on the entire video by following the pre-edge (edge) images to obtain yijinhwa. Qualified heopeu in a binary image (Hough) to convert the horizontal and vertical lines to obtain, using the characteristics of the plates to extract the license plate area. The problem with this method, the processing time is so difficult to handle real-time status of irregular points, and visual contrast with yagangwan border does not appear in the plates to extract the license plate area is that it is not. In addition, the rear of the vehicle license plate area from images taken using the characteristics of the plates myeongamgap changes sutjapok in the area, background area and the number number area of the region confirmed the contrast of the car and identified the number and the number of 42 of distance to extract the license plate area. How to research, the existing damage to the border of the plate to fail to extract the license plate area, a matter of hours to resolve problems in real-time, practical application is processed. Chapter 100 as the results of the experiment the sample video image in a car that far experiment results automatically read license plates have been able to extract the license plate and failing to represent 13% of images, character recognition result of failing to represent the image was 0.4%

Intra-tracheal Administration of the Disinfectant Chloromethylisothiazolinone/methylisothiazolinone (CMIT/MIT) in a Mouse Model to Evaluate a Causal Association with Death (마우스의 기도 내 점적을 통한 가습기살균제 CMIT/MIT와 사망 간의 원인적 연관성에 관한 연구)

  • Objectives: The deaths of Korean victims exposed to the disinfectant CMIT/MIT have remained unresolved. This is mainly due to a lack of concordance between the few available toxicity tests and the abundant epidemiological data, making it difficult to establish a cause-and-effect relationship. Therefore, this study was carried out to investigate any potential associations between CMIT/MIT exposure and death. Methods: Groups of experimental and control C57BL/6 mice were instilled (in the trachea) with chloromethylisothiazolinone/methylisothiazolinone (CMIT/MIT) using a visual instillobot. CMIT/MIT was instilled over a period of three days and eight weeks, respectively, to achieve acute and chronic exposures. A threshold dose-response model was applied for estimating the threshold level as one line of evidence for a causal association between CMIT/MIT and death. Results: An acute exposure of 1.2 mg ai/kg/day of CMIT/MIT was estimated to reflect the threshold for death. The dose-response curve with this threshold showed a very steep slope and a narrow range of CMIT/MIT exposures. The narrow range of CMIT/MIT exposures, in particular, indicated an evident boundary between survival and death, thus implicating a strong causal association. A similar threshold dose-response relationship observed following acute exposure was also seen following chronic exposure to CMIT/MIT. Airborne disinfectant exposure was visible as minimal or mild lung damage with no fibrosis, as shown by histopathological tests. However, many observations are considered to be functional respiratory tract or lung failure due to death, as observed in necropsies of the mice that died due to CMIT/MIT exposures. Conclusions: There are two strong lines of evidence for a causal association between death and CMIT/MIT exposure: 1) The threshold dose-response curve, with a very steep slope and a narrow range of CMIT/MIT exposures showing a visible boundary between survival and death; and 2) many cases of functional respiratory or lung failure.

  • We know that humans are able to live comfortably when there is a balance between the living environment that surrounds us and us. Clothing, one of the necessities of life, serves a dual purpose of functioning as protection for the body as well as providing visual satisfaction, and so is the most effective bonding mechanism. A different type of comfort that has been provided through industrialization and advancements in technology has resulted in pollution problems in the environment, and it has reached a point of seriously thinking about its effects. The biggest problems in environmental pollution are air pollution and waste disposal, and something that cannot be ignored is that environmental hormones and harmful heavy metals have been discovered in the synthetic dyes used in the clothing product, and not only are they harmful to the body, but they also have adverse effects on skin allergies such as atopic dermatitis. Its effects on clothing, especially underwear and baby products, can become a serious issue. The use of natural vegetable dye can be considered the basis for the traditional Korean dyeing process, however not all natural dyeing process can be thought of as traditional Korean dyeing. This is because natural dyeing used to be the most common method of dyeing before synthetic dyes came along and became the dye of choice for most clothing. Natural dyes are beneficial to the body, and the nature colors, achieved from the natural materials, relieve eye strains as well as have a healing effect for people who are suffering from psychological instability such as stress. However, the use of mordant in order to increase the fastness and get better color effects can lead to even more damage to the environment, and it cannot be a dyeing method that is good for the body. Traditional natural dyeing is different from regular natural dyeing because it uses colors that the Korean people can identify with and applies specific methods in order to achieve those colors. Even though experimental study with dyestuff and practical use of the dyeing methods for traditional Korean dyeing has been started(Soh, H., 1983) and has been ongoing, it is still not easy for regular people to use the method on their own. At the present time, natural dye materials are getting made and sold more than ever, but there is a lot of confusion among consumers regarding the difference between traditional Korean dyeing and natural dyeing, as companies are using the two terms interchangeably to market their natural dye products. So getting a better understanding of the characteristics of traditional Korean dyeing and traditional colors should be considered first priority at this time. The purpose of this thesis is to study the traditional Korean view of color, which developed the culture of traditional dyeing in Korea, and the characteristics of traditional Korean dyeing, a method that pursued achieving Korea's color through natural dyeing.

A Object-Based Image Retrieval Using Feature Analysis and Fractal Dimension (특징 분석과 프랙탈 차원을 이용한 객체 기반 영상검색)

  • This paper proposed the content-based retrieval system as a method for performing image retrieval through the effective feature extraction of the object of significant meaning based on the characteristics of man's visual system. To allow the object region of interest to be primarily detected, the region, being comparatively large size, greatly different from the background color and located in the middle of the image, was judged as the major object with a meaning. To get the original features of the image, the cumulative sum of tile declination difference vector the segment of the object contour had and the signature of the bipartite object were extracted and used in the form of being applied to the rotation of the object and the change of the size after partition of the total length of the object contour of the image into the normalized segment. Starting with this form feature, it was possible to make a retrieval robust to any change in translation, rotation and scaling by combining information on the texture sample, color and eccentricity and measuring the degree of similarity. It responded less sensitively to the phenomenon of distortion of the object feature due to the partial change or damage of the region. Also, the method of imposing a different weight of similarity on the image feature based on the relationship of complexity between measured objects using the fractal dimension by the Boxing-Counting Dimension minimized the wrong retrieval and showed more efficient retrieval rate.

Study on the oral health awareness and behavior in the special school for students of disability (일부 특수학교 장애인의 구강보건의식행태에 관한 조사연구)

  • Objectives : This study of oral health problems was conducted to 311 students in Special Schools in order to inquire into the state of their oral health behaviors, to find out the main obstacles against dental services, to secure dental reasonable basis for oral health promotion. Methods : A questionnaire survey was conducted in the Special School of the 311 students in the area of the metropolitan Seoul. By means of Chi-squared test and Fisher's exact test, oral hygiene habits for each type of the subjects, the contents of dental care services, the prerequisites to improve oral health, were inquired. To evaluate the effects to the current oral health status of types of disability, 2-way ANOVA was practiced. Results : The handicapped with Visual impairment, mental retardation, multiplicity with disabilities, answered negatively in their own oral health status. 47.4% of the deaf can do brush for theirselves without the inconvenience, but in other types of disability they showed that they were helped by others. Subjects did not use the secondary oral hygiene necessaries because, except brain damage, almost of types of disability impeded the convenient use. 60 to 88.2 percent of the total respondents were the recent visitors to dental clinique within 1 year, and the most common motives of the visiting dentist, is a routine medical examination, their movements to the dentist in all types of disabilities, needed helps of others. Most of them received medical treatment at a private dentist, the handicapped preferred the private care and place(49.8%). Only in the case of the brain disorders, extremely much of the disabled answered that they were needed the preventive treatment, and the another cases of disability were largely needed the treatment of the decayed teeth. To improve the oral health of the people with disabilities, at the opening of the clinics and hospitals over a certain size, the mandatory medical facilities for the care of the disabled should be preceded and followed by the improvement of dental insurance system so as to reduce the burdens of the cost of dental care. Conclusions : The improvement of oral health policy for the disabled are needed inevitably: Development of secondary oral hygienic easy to use for the disabled, the building systems of medical dental hospital with the disabled facilities, by the improvement of the insurance system, the reduction of the burden of payments.
