Detection of MecA Gene in Clinical Isolates of Staphylococcus aureus by Multiplex-PCR, and Antimicrobial Susceptibility of MRSA

  • Multiplex-PCR protocols were designed in order to make a rapid identification of MRSA. MecA, femB, and 165 rRNA genes were amplified for making a detection of MRSA. The incidence of MRSA in the clinical isolates of Staphylococcus aureus was examined by using a multiplex-PCR assay. The mecA gene was detected in 266 strains out of 336 clinical isolates of S. aureus, thus the incidence of MRSA was approximately 76.5%. The MRSAs of 247 strains (96.1%) showed resistance to more than eight species of the antimicrobial agents tested. The isolates of MRSA showed 27 different antimicrobial-resistant patterns. The results indicate that many different MRSA strains having high multidrug resistance are actually prevalent in Korea. Also, VISA was screened from the MRSA. Two strains were grown on the BHI agar plate supplemented with $8\;\mu\textrm{g}/ml$ of vancomycin at a frequency of $1/10^8$ colony forming units or higher.

A Comparative Study of Nurses' Recognition and Practice Level of General Nosocomial Infection, MRSA and VRE Infection Control (일반 병원감염, MRSA 및 VRE 감염관리에 대한 간호사의 인지도와 수행정도 비교연구)

  • Purpose: The purpose of this study was to describe nurses' recognition of, and practice level in management of general nosocomial infections, and methicillin resistant staphylococcus aureus (MRSA) and vancomycin resistant enterococci (VRE) infections. Method: A self-administered questionnaire was used to collect data. Data were collected on June, 2003 from 190 nurses in one university affiliated hospital located in Suwon. Result: The mean score for nurses' recognition of general nosocomial infection control was 3.57, MRSA control was 3.54, and VRE control was 3.86. The mean score on practice for control of general nosocomial infection was 3.19, for MRSA control, 3.20, and for VRE control, 3.63. There were statistically significant relationships between the recognition level and practice level for general nosocomial, MRSA, and VRE infection control. According to the general characteristics of the nurses, the mean scores for both recognition and practice were higher for those nurses who had had infection control education, for those who had worked longer in nursing, and for those who worked in the ICU. Conclusion: It is suggested that appropriate hospital infection control programs should be developed through continuous education and practice to improve nurses' level of the practice in general infection control, and especially in MRSA and VRE infection control.

Estimation of Dietary Exposure to Antimicrobial Resistant Staphylococcus aureus from Pork-based Food Dishes (돈육섭취에 의한 항생제 내성 황색포도상구균 및 독소의 식이노출평가)

  • Antimicrobial resistance of foodborne pathogens is an important food safety issue worldwide as well as in Korea. In this study, exposure to antimicrobial resistant (AMR) Stapylococcus aureus was assessed from the consumption of pork based food dishes prepared in food service operations using the Monte Carlo simulation. Thirty five isolates of S. aureus were obtained from 124 semi-processed pork products and their antibiotic resistance patterns were determined. The highest resistance was observed for penicillin (76.7%) followed by ampicillin (70.0%). Two isolates were resistant to oxacillin (6.7%) and no vancomycin resistance was observed. Dietary exposure to penicillin resistant S. aureus as the most frequently observed AMR S. aureus from pork-based dishes was estimated based on contamination data as well as compliance to guidelines for time and temperature controls during food service operations. The mean level of penicillin resistant S. aureus in pork dishes during preparation was below 1 Log CFU/g. As a conservative approach, 95th percentile estimated level of penicillin resistant S. aureus was below the level for toxin production. The estimated probability of staphylococcal intoxication by AMR S. aureus was very low using currently available data.

Analysis of Antimicrobial Resistance Pattern and Distribution of Multi-drug Efflux Pump Genes and Virulence Genes in Enterococcus faecalis Isolated from Retail Meat in Seoul (서울시내 시판 식육에서 분리한 Enterococcus faecalis의 항생제 내성 유형, 다중약물 유출 펌프 유전자 및 병독성 유전자의 분포도 분석)

  • The aim of this study was to investigate the distribution of genes that encode multi-drug efflux pumps and virulence factors in Enterococcus faecalis isolated from retail meat and antibiotic resistance patterns of these strains. Of the 277 retail meat samples, 93 Enterococcus faecalis were isolated. The strains exhibited resistance to ampicillin (35.5%), chloramphenicol (6.4%), ciprofloxacin (4.3%), erythromycin (18.3%), levofloxacin (0%), quinupristin-dalfopristin (76.3%), tetracycline (45.2%), teicoplanin (0%) and vancomycin (0%). The strains were positive for MFS type eme(A) (100%), ABC type efr(A) (100%), ABC type efr(B) (98.9%) and ABC type lsa (91.4%) efflux pump gene. The strains were positive for gelE (68.8%), ace (90.3%), asa1 (47.3%), efaA (91.4%) and esp (12.9%) virulence gene. This research will help to assess the hazards associated with the occurrence of drug resistance among enterococci from retail meat. Therefore, it is necessary to monitor enterococcus spp. isolated from retail meat continuously.

Antibiotic Resistance of Food-Borne Pathogens Isolated from an Indoor Environment of a Lunchroom in a Child Care Center (보육시설 급식실 실내 환경에서 분리된 식중독 미생물의 항생제 내성 특성)

  • Objectives: This study was performed in order to evaluate antibiotic resistance and analyze the multiple antibiotic resistance of food-borne pathogens isolated from indoor air and an air cleaner at a lunch room in a child care center. Methods: An antibiotic test of food-borne pathogens, including four Staphylococcus aureus and 23 Bacillus cereus was conducted through the disk diffusion method from Clinical and Laboratory Standard Institute. Results: All Staph. aureus was resistant to Ampicillin and Penicillin, while B. cereus was also resistant to Ampicillin, Cefepime and Penicillin. All isolates showed Vancomycin susceptibility but three out of four Staph. aureus and all B. cereus were resistant to Oxacillin. Staph. aureus and B. cereus presented two or more multiple antibiotic resistances. Conclusions: The results indicated that food-borne pathogens isolated from indoor air and an air cleaner at a lunch room in a child care center showed multiple antibiotic resistances. The repeated control of indoor environment quality is required and continuous surveillance of antibiotic resistant strains is demanded.

Early-onset sepsis in a neonatal intensive care unit in Beni Suef, Egypt: bacterial isolates and antibiotic resistance pattern

  • Purpose: To identify the frequency of bacterial isolates in early-onset neonatal sepsis (EONS) and their antimicrobial resistance pattern. Methods: A retrospective study of EONS was conducted at the Beni Suef University Hospital from September 2008 to September 2012. A case of EONS was defined as an infant who had clinical signs of infection or who was born to a mother with risk factors for infection, and in whom blood culture obtained within 72 hours of life grew a bacterial pathogen. Results: Of 673 neonates screened, there were 138 positive blood cultures (20.5%) (confirmed EONS). Of the recovered isolates, 86.2% were gram-negative pathogens. Klebsiella pneumoniae (42.8%), Enterobacter cloacae (22.5%), and Escherichia coli (13.8%) were the commonest isolated organisms. The most common gram-positive microorganism was Staphylococcus aureus accounting for only 12 isolates (8.7%). All Klebsiella isolates and 93% of Enterobacter isolates were resistant to ampicillin. Gram-negative pathogens had the maximum overall sensitivity to imipenem, cefepime, and ciprofloxacin; whereas, gram-positive isolates were most susceptible to vancomycin, imipenem, and piperacillin. Conclusion: K. pneumoniae was the predominant causative bacteria of EONS followed by E. cloacae and E. coli. There was a high resistance to ampicillin. Imipenem had the maximum overall activity against the causative bacteria. Continuous surveillance is needed to monitor the changing epidemiology of pathogens and antibiotic sensitivity.

Genetic Diversity and Antibiotic Resistance of Enterococcus faecalis Isolates from Traditional Korean Fermented Soybean Foods

  • Eighty-five Enterococcus faecalis isolates collected from animals (40 isolates), meju (a Korean fermented soybean product; 27 isolates), humans (10 isolates), and various environmental samples (8 isolates) were subjected to multilocus sequence typing (MLST) to identify genetic differences between samples of different origins. MLST analysis resulted in 44 sequence types (STs), and the eBURST algorithm clustered the STs into 21 clonal complexes (CCs) and 17 singletons. The predominant STs, ST695 (21.1%, 18/85) and ST694 (9.4%, 8/85), were singletons, and only contained isolates originating from meju. None of the STs in the current study belonged to CC2 or CC9, which comprise clinical isolates with high levels of antibiotic resistance. The E. faecalis isolates showed the highest rates of resistance to tetracycline (32.9%), followed by erythromycin (9.4%) and vancomycin (2.4%). All isolates from meju were sensitive to these three antibiotics. Hence, MLST uncovered genetic diversity within E. faecalis, and clustering of the STs using eBURST revealed a correlation between the genotypes and origins of the isolates.

Antimicrobial susceptibility of Staphylococcus aureus and S. intermedius isolated from dogs (개에서 분리한 Staphylococcus aureus와 S. intermedius의 항생제 감수성)

  • Staphylococci are Gram-positive, facultatively anaerobic cocci, normally found on the skin andmucosal surfaces of most warm-blooded animals and often involved in a wide variety of diseases in animals.Staphylococcal infections are treated with antibiotics and, consequently, antibiotic resistance and/or acquiredresistance have developed. Staphylococcus (S.) intermedius and Staphylococcus (S.) aureus are 2 comonveterinary isolates that are frequently associated with suppurative infections. This study was undertaken toexamine antimicrobial susceptibility of S. aureus (23 isolates) and S. intermedius (160 isolates) isolatedfrom dogs in Gwangju, Korea and investigate whether the antibiotic resistance of S. aureus and S.intermedius is efected by the site of isolation, age, and sex of dogs. More isolates were isolated fromadult dogs (71.3%) than juveniles (20.5%). Antimicrobial resistance was commonly found for Penicillin,Tetracycline, Trimethoprim-Sulphamethoxazole in both Staphylococcus species. All of the isolates weresusceptible to Amoxicillin/Clavulanic Acid, Cephalothin, Oxacilin, Neomycin, and Vancomycin. Appropriateprotocol for antibiotic use and strategies to reduce antimicrobial resistance rate will be needed. Periodicsubstitution of antimicrobial agents and limitation of antibiotic use should also be considered.

  • MRSI is the staphylococcal infection having resistance to the methicillin which is semisynthetic penicillinase-resistant agents against penicillinase. These infections are very difficult to treat because they have resistance to almost every antibiotics except for vancomycin. We experienced MRSE(methicilline-resistant staphylococcal epidermis) infected 56 years old man who developed 2 months after arthroplasty for TMJ ankylosis and MRSA(methicilline-resistant staphylococcal aureus) infected 59 years old man who was performed arthroplasty far traumatic TMJ disc displacement.

Characterization of Azomonas agilis PY101, a Cadmium-Resistant Strain Isolated from Anyang Stream

  • A cadimium-resistant strain isolated from Anyang stream, Azomonas agilis PY101 exhibited strong resistance to 1000 ppm of cadmium ion $(Cd^{2+})$. A agilis PY101 also exhibited resistance to various antibiotics, such as amoxicillin, amplicillin, bacitracin, cefazolin, erythromycin, penicillin, tetracycline, and vancomycin. In the presence of $Cd^{2+}$, the growth of A. agilis PY101 started after an extended lag phase and produced a green-fluorescent pigment induced by cadmium. The dramatic decrease (approximately 400ppm) of concentration of $cd^{2+}$ in the culture medium during the growth phase of A. agilis PY101 was confirmed by the inductively coupled plasma-atomic emission spectrophotometer. Transmission electron microscopic analysis revealed that A. agilis PY 101 actively accumulated $Cd^{2+}$ in the cytoplasm.
