항로표지사고 분류체계의 재정립에 관한 연구 (A Study on the Re-establishment of the Accident Classification for Aids to Navigation)

  • 항로표지가 이용자에게 지속 가능한 서비스를 제공하기 위해서는 항로표지사고가 발생하지 않을 때 가능하다. 만약 항로표지사고가 발생한다면 관리자는 이를 효율적으로 관리하여 유사한 사고를 예방해야 한다. 하지만 현재 항로표지사고 관리는 사고의 원인과 종류만 명시할 뿐 별도의 지침이 없어 관리자에 따라 다르게 기록되고 있는 실정이다. 이에 본 연구에서는 항로표지사고를 재정립하고자 하였다. 이를 위해 지난 23년(00-22년)간 발생한 항로표지사고, IALA의 항로표지 정보 표준인 S-201, 사고의 범주인 교통사고와 해양사고를 분석하였다. 이를 토대로 항로표지사고의 요인은 내부적 원인과 외부적 원인으로 구분하였고, 사고유형은 등탑사고, 부표사고, 장비사고의 3종으로 구분하였다. 1차 항목을 좀더 세분화하여 사고 원인은 계류구, 기상악화 등 7개 항목과 사고 현상(종류)은 등탑파손, 부표유실, 장비고장 등 11개 항목으로 재정립하였다. 이러한 연구결과는 차후 항로표지사고 통계를 제공하기 위한 기초자료로 활용될 수 있을 것이다.

비골골절의 Stranc 분류에 따른 임상적 연구 (A Clinical Study of The Nasal Bone Fracture According to Stranc Classification)

  • Purpose: The nasal bone fracture is known as the most common facial fracture. Several authors reported the classification of nasal bone fracture. Stranc classified the type of nasal fractures based on a pattern of impaction and level of injuries. Stranc classification proposed here is based on careful clinical observation and relates to both treatment and prognosis. The aim of this study is to determine any predictive value to the preoperative classification of nasal fracture, using the description by Stranc and Robertson. Methods: We reviewed 310 patients with nasal bone fracture treated at our hospital for last two years. Results: Lateral impact type of nasal bone fracture predominated more than frontal impact in the ratio of 2.3:1. The most common type of Stranc classification was lateral impact plane I(48.4%). Male predominated more than female in the ratio of 3:1. The most frequent age group was first decade(27.1%), second decade in frontal impact(30.5%), first decade in lateral impact (30.7%). The most common etiology was violence (31.3%) followed by slip down(21.3%), and traffic accident(18.1 %).The most common associated fracture was orbital(22.9%) followed by zygoma(10%), and maxilla (6.1%). The most common complication was septal deviation(20.0%) in frontal impact, and nasal deformity (26.0%) in lateral impact. The incidence of nasal deformity in lateral impact(26.0%) was more higher than frontal impact(15.8%) Conclusion: By assessing the pathomechanics and resultant degree of injury to the nasal skeleton, a better understanding of the treatment plan and prognosis was obtained. Using this information, satisfactory informed patient consent can be obtained.

전로사고 예방을 위한 인적오류 분석 (A Case Study on the Human Error Analysis for the Prevention of Converter Furnace Accidents)

  • Occupational fatal injury rate per 10,000 population of Korea is still higher among the OECD member countries. To prevent fatal injuries, the causes of accidents including human error should be analyzed and then appropriate countermeasures should be established. There was an severe converter furnace accident resulting in five people death by chocking in 2013. Although the accident type of the furnace accident was suffocation, many safety problems were included before reaching the death of suffocation. If the safety problems are reviewed throughly, the alternative measures based on the review would be very useful in preventing similar accidents. In this study, we investigated the converter furnace accident by using human error analysis and accident scenario analysis. As a result, it was found that the accident was caused by some human errors, inappropriate task sequence and lack of control in coordinating work by several subordinating companies. From the review of this case, the followings are suggested: First, systematic human error analysis should be included in the investigation of fatal injury accidents. Second, multi man-machine accident scenario analyis is useful in most of coordinating work. Third, the more provision of information on system state will lessen human errors. Fourth, the coordinating control in safety should be performed in the work conducting by several different companies.

교통사고 위험그룹 및 사고유형별 심각도 결정 연구 - 서울시 중심 - (The Determination of Risk Group and Severity by Traffic Accidents Types - Focusing on Seoul City -)

  • 본 연구는 교통사고의 발생 유형과 교통사고 심각도(Severity)와의 관계를 규명함으로써 위험유형을 제시하고, 운전자 특성과 교통사고의 관계를 규명하고자 하였다. 교통사고 유형을 여덟 가지로 세분하고, 결과의 객관성 확보를 위해 안전벨트 착용여부를 추가하여 상해정도와의 관계를 분석하였으며, 위험그룹의 분류를 위한 운전자의 특성은 성별, 차종, 연령 등을 대상으로 하였다. 카테고리 자료의 분석을 위하여 로그-선형 모형 및 로짓 모형을 사용하였다. 분석결과 사고유형과 심각도와의 관계에서는 정면충돌 사고와 앞지르기시, 우회전시 사고가 부상 또는 사망사고에 연루될 가능성이 높았다. 위험그룹 분석에서는 20세 미만의 이륜차 운전자, 41세에서 50세까지의 택시 운전자가 가장 위험한 집단으로 분석되었으며 또한 남자보다는 여자가 승용차와 중형화물 등에 관계되었을 때 더 위험한 것으로 분석되었다. 따라서 교통사고 발생시 인명 피해를 줄이기 위해서는 정면충돌 사고와 앞지르기시, 우회전시 발생하는 사고를 줄일 수 있는 방안이 연구되어야 하고, 교통사고 취약계층으로 분석된 위험그룹에 대한 교통안전 교육 및 단속이 강화되어야 할 것이다.

  • 사고 원인에 대한 철저한 분석은 사고 재발 방지를 위한 필수적인 과정이다. 해체공사 사고의 원인을 살펴보면 작업자의 불안전한 행동, 불안전한 상태, 심리적·신체적 상태, 현장관리 원인 등 매우 다양하다. 현재 해체공사 사고통계는 지속적으로 조사·보고되고 있으나 사고 유형에 따른 보다 근본적인 원인 분류 정보가 필요하다. 본 연구에서는 하인리히의 도미노 이론을 바탕으로 해체공사 사고의 유형에 따라 사고원인(불안전한 행동, 불안전한 조건)과 휴먼에러(인적요인)를 분류하였다. 본 연구에서는 해체공사시 사고유형에 따라 사고원인을 체계적으로 분류하기 위해 QFD-FMEA(Quality Function Deployment - Failure Mode Effect Analysis) 3단계 모델을 사용하였다. 사고원인 분류 결과는 사고예방을 위한 안전지식 및 체크리스트로 활용할 수 있다.

LightGBM 알고리즘을 활용한 고속도로 교통사고심각도 예측모델 구축 (Predicting of the Severity of Car Traffic Accidents on a Highway Using Light Gradient Boosting Model)

  • 본 연구는 고속도로 교통사고 심각도 예측모델을 구축하기 위해 다섯가지 머신러닝 기반의 분류모형 적용하였다. 2015년~2017년 동안 전국 고속도로에서 발생한 사고 데이터 21,013건을 5가지의 분류 모형을 적용한 결과 LightGBM(Light Gradient Boosting Model)이 가장 좋은 성능을 나타내는 것으로 나타났다. LightGBM에서는 교통사고심각도 추정에 있어 우선순위 요인으로 사고차량 수, 사고유형, 사고지점, 사고차로유형, 사고차량 유형 순으로 나타났다. 이러한 모형의 결과를 기반으로 일관적인 사고심각도 예측 과정을 통하여 교통사고대응관리 전략 수립에 활용할 수 있다. 본 연구는 국내 기계학습을 활용한 사례가 적은 여건에서 향후 빅데이터 기반의 다양한 기계학습 기법을 활용이 가능함을 제시하고 있다.

12인승 밴 전복사고의 상해 분석 (Injury Analysis of a 12-passenger Van Rollover Accident)

  • The fatality of rollover accidents in motor vehicle crashes is high despite their low incidence. Through the investigation of a 12-passenger van rollover accident in which 10 passengers were involved, we intend to analyze the correlation between the severity of the injury and the position of the occupants. We collected accident information from medical records, interviews, photo-images of the damaged van, field surveys, and the results of the Korean New Car Assessment Program (KNCAP). Based on the occupants' position, we classified injury sites and estimated injury severity. Passenger injury severity was evaluated by trauma score calculation. The initiation type of the rollover accident was passenger side 'fall-over' and the Collision Deformation Classification (CDC) code for the damaged van was 00TDZO3. The crash of the van involved 10 passengers, with an average age of $16.3{\pm}4.2years$. Few of the occupants had fastened seat belts at the time of the incident, and there was no airbag installed. One patient sustained severe liver injury and another was diagnosed with a fracture of the right humerus. The most common injuries were at the upper extremities and the neck. The average of Injury Severity Score (ISS) was $4.8{\pm}5.9$, and the average ISS of right-seated, mid-seated and left-seated occupants was $7.5{\pm}9.3$, $1.5{\pm}0.7$, and $3.3{\pm}2.1$ respectively (p>0.05). In the rollover (to-passenger side) accident of occupant unfastened, the average ISS of right-seated occupants (near side) was higher, but there was no statistically significant difference.

독성 화학물질 누출사고 대응 기술연구 - 불산 및 암모니아 누출을 중심으로 - (A Study on the Response Technique for Toxic Chemicals Release Accidents - Hydrogen Fluoride and Ammonia -)

  • Since the unprecedented hydrogen fluoride leak accident in 2012, there has been growing demand for customized technical information for rapid response and chemical accident management agencies including the Ministry of Environment, the National Emergency Management Agency, and the National Police Agency need more information on chemicals and accident management. In this regard, this study aims to provide reliable technical data and guidelines to initial response agencies, similar to accident management technical reports of the US and Canada. In this study, we conducted a questionnaire survey and interviews on initial response agencies like fire stations, police stations, and local governments to identify new information items for appropriate initial response and improvements of current guidelines. We also collected and reviewed the Canada's TIPS, US EPA's hydrogen fluoride documents, domestic and foreign literature on applicability tests of control chemicals, and interview data, and then produced items to be listed in the technical guidelines. In addition, to establish database of on-site technical information, we carried out applicability tests for accident control data including ① emergency shut down devide, safety guard, shut down valve, ground connection, dyke, transfer pipe, scrubber, and sensor; ② literature and field survey on distribution type and transportation/storage characteristics (container identification, valve, ground connection, etc.); ③ classification and identification of storage/transportation facilities and emergency management methodslike leak prevention, chemicals control, and cutoff or bypass of rain drainage; ④ domestic/foreign analysis methods and environmental standards including portable detection methods, test standards, and exposure limits; and ⑤ comparison/evaluation of neutralization efficiency of control chemicals on toxic substances.

  • Through so that accident of semiconductor industry deduces unsafe factor of the person center on unsafe behaviour that incident history and questionnaire and I made starting point that extract very important factor. It served as a momentum that make up base that analyzes factors that happen based on factor that extract factor cause classification for the first factor, the second factor and the third factor and presents model of human error. Factor for whole defines factor component for human factor and to cause analysis 1 stage in human factor and step that wish to do access of problem and it do analysis cause of data of 1 step. Also, see significant difference that analyzes interrelation between leading persons about human mistake in semiconductor industry and connect interrelation of mistake by this. Continuously, dictionary road map to human error theoretical background to basis traditional accidental cause model and modern accident cause model and leading persons. I wish to present model and new model in semiconductor industry by backbone that leading persons of existing scholars who present model of existent human error deduce relation. Finally, I wish to deduce backbone of model of pre-suppression about accident leading person of the person center.

조사 대상 부지 신규 분류 체계 제안 및 개황조사 강화를 통한 토양정밀조사 방법 개선 연구 (Improvement of Detailed Soil Survey Guidance through the New Site Classification System and Reinforcement of Exploratory Soil Survey)

  • This study suggested the new site classification system according to land use, type of contamination and contaminants. Because the present site classification system can not cover all the areas, we changed the concept of land use to more detail one and enlarged the concept of other areas to cover all the areas not defined as certain land use. In case of the present industrial area, it was merged as other areas to avoid the confusion with oil and toxic material storage tank farm area. Accident area was separated from other areas and defined as only accident area caused by the mobile storage facility. In addition to classify the sites according to the basic land use, we classify the sites again in lower level according to the type of contamination and contaminants. With this classification system, we proposed different soil sampling strategy with the consideration of the origin of contamination and the interactions between soil and contaminants. We removed the surface soil sample (0~15 cm depth) around above storage tank because it was not a effective sample to assess whether that area contaminated or not. We also proposed to take the deeper soil samples at minimum three sampling points to confirm the depth of contamination in exploratory soil survey. We also proposed to remove the one point of 15 m depth sampling because it is not effective to confirm contaminated soil depth and needs the exhausted labor and cost. Instead of doing this, we added the continuous sampling to uncontaminated subsoil. Soil sampling points and depth in detailed soil survey is determined based on the results of exploratory soil survey. Therefore, effectiveness and reinforcements of exploratory soil survey would play an important role in improving the reliability of detailed soil survey.
