Review on Case Reports of Syndrome differentiation and Herbal Medicine Treatment for Tic Disorder (틱 장애의 한의학적 변증 및 한약치료 치험례에 대한 고찰)

  • Objectives : This study analyzed domestic case reports of syndrome differentiation and herbal medicine treatment for tic disorder published from 2000 to 2018 and aimed to seize the latest trend of Korean herbal medicine treatment according to syndrome differentiation. Methods : We searched herbal medicine treatment of tic disorder through five major Korean academic literature databases; NDSL, KTKP, KISS, RISS, DBPIA. Results : 12 articles were included in this study from 49 articles. These studies were analyzed by year, evaluation criteria, Korean treatments, treatment effects, syndrome differentiation, herbal medication, duration of medication, and type of herbal medicine. Treatment results were assessed by YGTSS. Most cases accompanied Korean integrative treatments and only one case report used herbal medicine alone. Most common type of syndrome differentiation for patients was spleen. Duration of medication varied according to each case and decoction was the most used type of herbal medication. Conclusions : It has been reported that herbal medicine treatment according to the syndrome differentiation could be effective in treating tic disorder. This study suggests that additional studies will be required to justify classification of syndrome differentiation and the sole effectiveness of herbal medicine on tic disorder.

Effects of one hour daily outdoor access on lying and sleeping postures, and immune traits of tethered cows

  • Objective: We investigated the effects of outdoor access for 1 h per day on the animal welfare (AW) of tethered cows, in terms of lying and sleeping postures, and immune function. Methods: A total of five dry cows were tethered all day indoors (tethering) for 30 days and then tethered indoors with 1 h daily outdoor access (ODA-1h) for 30 days. To analyze the effects of ODA-1h, we calculated the total duration and bout frequency per day, and bout duration of lying and sleeping postures during the last five days of each treatment period. We also analyzed the populations of T cells, B cells, and NK cells in peripheral blood mononuclear cells (PBMC) by fluorescence-activated cell sorting and determined the concanavalin A (Con A) -induced proliferation rate of T cells. Results: The mean total time per day of lying during the ODA-1h treatment was significantly shorter than that during the tethering treatment (p<0.001). The Con A-induced proliferation rate of T cells during the ODA-1h treatment was significantly higher than that during the tethering treatment (p = 0.007). The proportion of NK cells in PBMC during the ODA-1h treatment tended to be higher than that during the tethering treatment (p = 0.062). Conclusion: Although ODA-1h may decrease lying time, it increases the available space for tethered cows towards that typically found in grazing and free barn feeding systems. This increased available space promotes the expression of normal behaviors such as walking and social behaviors except lying and may also improve the immune function of tethered dry cows, thereby improving their overall welfare.

Related Factors of Exercise Injury from Hospital Patients (일부지역 병원 환자조사를 통한 운동상해 관련요인 연구)

  • Objective: This study was performed to identify the related factors of exercise injury throughout the actual cases among patients. Method: The subjects of the study consisted of 304 patients(209 male and 95 female) over 20's in seoul and kyonggi area, and data collected by using questionnaires were analyzed with SPSS/WIN 11.0 program. To prove the relative factors of exercise injury from hospital patients Results: 1) It was found that the general degree of exercise injury showed significant difference regarding gender, age and education level. In other words, if it was male, young and high school graduate, the portion of exercise injury for the person was higher than others. On the other hand, the duration of medical treatment for the patient, who is male, old and college graduate took longer than the others. 2) It demonstrated that the lower extremity was most intensively damaged due to the exercise injury, especially, knees and ankles had the most portion of exercise injury and a sprain was the most common form of exercise injury for these body parts. 3) The degree of exercise injury regarding of exercise maintains significant difference with the frequency of exercise. In fact, the more exercise was done by a person such as 3 to 4 days a week, the less duration of medical treatment for the person was taken. 4) It also showed that there was significant difference between the habit of exercise and the degree of exercise injury. In other words, a person had better exercise habit, it took the person less duration of medical treatment. As well, if someone had better warming up, he or she had lower degree of exercise injury after all. Conclusion: Consequently, according to the data, the research suggested that the male needed more carefulness than the female in exercise. Old age people needed to be more careful of getting sprain of knee and ankle in particular. As well, it concluded that people needed not only enough warming up before doing exercise but also positive habit of exercise in order to minimize the exercise injury.

Mid-Term Outcomes of Single-Port versus Conventional Three-Port Video-Assisted Thoracoscopic Surgery for Primary Spontaneous Pneumothorax

  • Background: The benefits of video-assisted thoracoscopic surgery (VATS) have been demonstrated over the past decades; as a result, VATS has become the gold-standard treatment for primary spontaneous pneumothorax (PSP). Due to improvements in surgical technique and equipment, single-port VATS (s-VATS) is emerging as an alternative approach to conventional three-port VATS (t-VATS). The aim of this study was to evaluate s-VATS as a treatment for PSP by comparing operative outcomes and recurrence rates for s-VATS versus t-VATS. Methods: Between March 2013 and December 2015, VATS for PSP was performed in 146 patients in Kyungpook National University Hospital. We retrospectively reviewed the medical records of these patients. Results: The mean follow-up duration was $13.4{\pm}6.5$ months in the s-VATS group and $28.7{\pm}3.9$ months in the t-VATS group. Operative time (p<0.001), the number of staples used for the operation (p=0.001), duration of drainage (p=0.001), and duration of the postoperative stay (p<0.001) were significantly lower in the s-VATS group than in the t-VATS group. There was no difference in the overall recurrence-free survival rate between the s-VATS and t-VATS groups. Conclusion: No significant differences in operative outcomes and recurrence rates were found between s-VATS and t-VATS for PSP. Therefore, we cautiously suggest that s-VATS may be an appropriate alternative to t-VATS in the treatment of PSP.

Long-term Outcome of Trigeminal Nerve Block with Alcohol for the Treatment of Trigeminal Neuralgia (삼차신경통 환자에서 알코올 신경차단의 장기 추적 결과)

  • Background: Recently trigeminal nerve block with alcohol (TnbA) for the treatment of trigeminal neuralgia (TN) has come to be known as a procedure with a short-term effect and high complications. There has been none of report about long-term outcome of TnbA for TN. The objective of this prospective study for the long-term results of TnbA was to analyse the pain free duration and complication after the administration of blocks and compare them in the first block and subsequent blocks. Methods: From March 1996 to May 2005, 304 consecutive patients with primary trigeminal neuralgia were treated with TnbA including supraorbital nerve block, infraorbital nerve block, maxillary nerve (V2) block, mandibular nerve (V3) block, and V2 and V3 at the same time and were prospectively followed up every two months for 10 years. Results: The mean value of pain free duation of 1st, 2nd and 3rd TnbA were 43, 38 and 48 months, respectively using Kaplan-Meier analysis. The probability of pain recurrence in 1 and 3 years after the 1st, 2nd and 3rd blocks were 25%, 25%, 20% and 53%, 54%, 34%, respectively. The pain free durations of first and subsequent blocks were not statistically different. Complications were reported at 36 (11.8%), 5 (4.2%), and 0 in 1st, 2nd and 3rd blocks. Conclusions: TnbA showed the relatively long duration of pain free and low incidence of complications. Repeated TnbA has pain free duration as long as the 1st block and less complications as well. TnbA is a valuable treatment of TN as a percutaneous procedure.

Efficacy and Safety of Bevacizumab in Chinese Patients with Metastatic Colorectal Cancer

  • Objective: To evaluate the efficacy and safety of bevacizumab in the treatment of patients with metastatic colorectal cancer (mCRC). Methods: In a single-center, observational study of 91 Chinese patients with mCRC who received bevacizumab in combination with chemotherapy was conducted. Objective response rates (ORRs), progression-free survival (PFS), overall survival (OS) and adverse events were recorded, and the relationships between various clinical factors and PFS or OS were evaluated by Cox proportional hazards models. Results: Treatment with bevacizumab and chemotherapy was effective and tolerable. Univariate analysis showed that PFS and OS were significantly associated with the Eastern Cooperative Oncology Group performance status (ECOG-PS) score, duration of bevacizumab exposure, and whether chemotherapy was continued after discontinuation of bevacizumab treatment. A multivariate analysis showed that the duration of bevacizumab exposure and whether chemotherapy was continued after discontinuation of bevacizumab were independent prognostic factors for PFS and OS. Conclusion: In Chinese mCRC population, the shorter the duration of exposure to bevacizumab and chemotherapy, the worse the prognosis is.

Impact on Respiratory Diseases of Acupoint Sticking in Dog-Days (삼복첩(三伏貼)의 호흡기 질환에 대한 영향)

  • Objectives Acupoint sticking in Dog-Days is one of Oriental medical prophylaxis to prevent winter diseases over the summer. The research has been conducted to evaluate the efficacy of Acupoint sticking in Dog-Days. Methods We visited one kindergarten in Dog-Days of 2011, and the subjects were 72 children (38 boys, 34 girls, $58.24{\pm}12.28$ months, 37-105 months). 72 children were attached Socheongo to BL-13, BL-15, BL-17 for 4-6 hours. Exacerbating effect or non-improvement in respiratory symptoms before and after the treatment were nullified. Results 31 children (43.1%) have shown positive effect in Socheongo, with no significant differences among the groups. There were significant improvements in Socheongo group in frequency of having cold, duration and visits; duration under tonsillitis, frequency of having otitis media, duration and visits (P<0.05). Lung weak score also had significant decrease in the three times Socheongo group, from $11.27{\pm}5.61$ to $9.90{\pm}4.66$ (P=0.030). Side effects has been reported in Socheongo group; 7 erythema (9.7%), 2 heating (2.8%), 9 pruritis (12.5%), 2 vesicle (2.8%), 13 scar (18.1%). Conclusions Acupoint sticking in Dog-Days improves lung weak symptoms such as common cold, tonsilitis, and otitis media in children older than three years old, throughout the three times of the treatment. However, the ways to reduce the side effects are needed.

Evaluation of Ecotoxicity and Characteristics on Indirect Effluents and Related Wastewater (간접방류사업장 관련 폐수의 생태독성수준 및 특성 평가)

  • This paper aims to evaluate the characteristics of direct and indirect effluents'toxicity level and difference between two test durations (24 h and 48 h) of test method. The proportion of the indirect effluent samples which exceeded the ecotoxicity permit limitations (TU 1 or TU 2) showed more than 2 times higher than that of direct effluent samples. However, effluent toxicity of Wastewater Treatment Plants (WTPs) indicated less than TU 1 regardless of influent toxicity. From this results, treatment process was thought to have a good efficiency. WTP Salinity was very similar between influent and effluent. This trend could be reconfirmed by the component ratio of ion concentration between them and $Na^+$, $SO_4^{2-}$, $C^-$ lions which have a greater percentage than other ions. In addition, in case of high salinity, toxicity value also showed high level. To judge from above results, indirect effluents which were exempted from application of ecotoxicity standards, may need a new effluent limitations regardless of the treatment efficiency of WTP. According to circumstances, effective countermeasure may need to restrain the discharge of salinity-contained effluents which came from indirect-effluent factories. Test duration comparative study indicated that 48 h results were higher toxicity (exceeding rate of ecotoxicity criteria) than 24 h by the 5 to 5.4 percentage. 24 h test duration seemed to be useful in case of rapid detection, whereas 48 h test method could be applied for reinforcement of ecotoxicity regulatory system.

Optimal Timing and Duration of Cold Application for Breaking Diapause in Queens of the Bumblebee Bombus ignitus

  • Bumblebees are important pollinators of crops and wildflowers. The Korean native bumblebee, Bombus ignitus, undergoes one generation per year, and induction of artificial hibernation is essential for year-round rearing of the bumblebee. Keeping queens under cold treatment conditions for several mo is an effective method for terminating their diapause and promoting colony development. In the present study, we investigated how the timing and duration of chilling affect the artificial hibernation of B. ignitus queens. In the timing assessment, cold treatment was instituted at 12 d, 40 d, or 100 d after eclosion under a constant temperature of $5^{\circ}C$ and 80% humidity. The queens that entered cold treatment at 12 d after emergence evidenced the highest survival rates: 86.7% at two mo, 73.3% at three mo, and 46.4% at 4 mo. Survival rates were reduced under storage conditions at 12 d, 40 d, and 100 d after emergence. When queens were subjected to chilling at 8 d, 12 d, or 16 d after eclosion with constant 80% humidity, the queens stored at 12 d after eclosion exhibited the highest survival rates, which were 84.6 at one mo, 25.0% at two mo, and 7.9% at three mo. In regards to the duration of the cold period, the queens that hibernated for at least two mo evidenced optimal colony development rates. The rates of oviposition, colony foundation, and progeny-queen production of queens hibernated for two mo were 60.0%, 30.0%, and 13.3%, respectively. These values were 6.0 to 13.3 times higher than those in the queens that hibernated for 15 d. Therefore, a cold period of at least 2 mo applied 12 d after emergence were found to be the most favorable conditions for diapause break in B. ignitus queens.

Changing trends in clinico-pathologic characteristics and treatment outcomes in patients with gastric cancer: A single-center, public hospital, retrospective study

  • Purpose: We analyzed our 10-year experience in a single-center, public hospital and thereby evaluated the changing trends of clinico-pathologic and surgical characteristics as well as treatment outcomes in patients with gastric cancer. Methods: The current single-center, retrospective study was conducted with patients who had been treated at department of our medical institution during a period ranging from March 1, 2007 to June 16, 2018. The eligible patients were divided into two groups: group I (March 2007-April 2012) and II (May 2012-June 2018). Then, we compared time-dependent changes in clinico-pathologic characteristics between the two groups. Results: The mean age was $63.0{\pm}11.3$ years in group I and $65.8{\pm}10.5$ years in group II, respectively (P=0.017). The American Society of Anesthesiologist (ASA) score was 34.9% for 1 point, 38.3% for 2 points, and 26.9% for 3 points or more in group I, and 31.1% for 1 point, 52.5% for 2 points, and 16.4% for 3 points or more in group II, which was statistically significant (P=0.012). The average follow-up duration was significantly different between the two group ($39.8{\pm}39.7$ vs. $23.4{\pm}20.6$) (P<0.001). The duration of postoperative hospital stay was 1.8 days longer in group II than group I (P=0.047). Tumor depth, node metastasis and distant metastasis were significantly different between the two groups (P<0.001, P=0.009, and P=0.019, respectively). Conclusion: There were significant differences in the age, ASA score, average follow-up duration, postoperative hospital stay, tumor depth, node metastasis and distant metastasis between the two groups.
