The mandibular buccal frenum is defined as a fold of mucous membrane at the posterior labial vestibule and attaches the lips and the cheeks to the alveolar mucosa, gingiva, and underlying periosteum. The buccal frenum becomes a problem when its attachment is too close to the marginal gingiva. It may then pull on healthy gingiva, encourage plaque formation and interfere with tooth brushing. Especially, heavy buccal frenum mucogingivally results in insufficent attached gingiva, inadequate vestibular depth and high frenum attachment and also difficulty in eruption of mandibular premolar. Frenotomy, frenectomy and mucogingival surgery are used in treating heavy buccal frenum. Frenotomy with autogenous free gingival graft has been used popularly because of its stable result. But, it is difficult in younger children because of inadequate donor site, difficulty in making recipient site and behavior management. Frenotomy with apically positioned flap is considered as more efficient way for a very young child with heavy buccal frenum. Additionally, modified deep sedation with $N_2O-O_2$ can be used as an adjunct for the effective treatment outcome. Decrease in muscle pull, adequate width of attached gingiva and increased vestibular depth can be expected from this treatment approach.
Objectives : Postural instability and gait difficulty(PIGD) can develop in all Parkinson's disease patients, especially late in the disease course, but does not respond well to conventional dopamine treatment. This study aimed to report three cases of PIGD in Parkinson's disease patients treated with Korean and Western medicine treatment. Methods : We used acupuncture, bee venom acupuncture, herbal medicine and moxibustion to treat patients during hospitalization. They continued Western medication and received rehabilitation treatment. We observed the changes of PIGD using the Unified Parkinson's Disease Rating Scale(UPDRS). Results : After treatment, PIGD symptoms improved and UPDRS scores were decreased. Especially, scores of falling, walking, gait and postural stability subsections related with PIGD were decreased. In addition, general conditions of patients were improved. Conclusions : This study suggests that Korean medicine treatment could be effective in the treatment of PIGD in Parkinson's disease patients.
Many patients with mental retardation need extensive dental treatment because they have much difficulty in maintaining their oral hygiene. However, because they are not cooperative and not manageable, they require physical restraints, drug induced sedation or general anesthesia. General anesthesia is useful in control of the patients who cannot be treated in other ways. Additionally, general anesthesia provides more safe environment for medically compromised patients. And medical treatment can be provided simultaneously under general anesthesia. Furthermore, almost all treatment can be provided without visiting several times. This case reports of periodontal, restorative and ophthalmological treatment of patient with mental retardation under general anesthesia.
Dental treatment for the disabled should be a customized that considers the characteristics and degree of cooperation of each disabled patient. There are additional considerations during implant treatment and tooth extraction in disabled patients. Since some brain lesion disorder or cardiac disease patients may be taking antiplatelet or anticoagulant medications, it is necessary to evaluate whether these medications should be discontinued before an invasive procedure. Precautions should be taken for patients with heart valve disease considering the risk of infective endocarditis, especially during invasive dental procedures. Moreover, disabled patients may have difficulty in following instructions and cautions. There are specific considerations for each stage of implant treatment in disabled patients. In the case of patients who are in the pre- or post-transplant state, it is necessary to assess their general condition and oral disease due to the risk of infection. Since disabled patients with various systemic diseases may visit the dental clinic, it is important to understand their characteristics and treatment process in order to flexibly adjust the dental treatment plan accordingly.
Patients with disabilities have difficulties tolerating in-office dental treatment due to limitations relating to cooperation and/or physical problems. Therefore, they often require general anesthesia or sedation to facilitate safe treatment. When deciding on dental treatment under general anesthesia, the plan should be carefully determined because compared to general patients, patients with disabilities are more likely to experience anesthetic complications because of their underlying medical conditions and potential drug interactions. Clinicians prefer simpler and more aggressive dental treatment procedures, such as extraction, since patients with impairment have difficulty maintaining oral hygiene, resulting in a high incidence of recurrent caries or restorative failures. This study aimed to review the available literature and discuss what dentists and anesthesiologists should consider when providing dental treatment to patients with severe disability under general anesthesia.
Purpose: The purpose of this study was to investigate the effects of SP-6 Seoam pellet therapy given to female university students in terms of menstrual pain, menstrual distress, and ADL difficulties. Methods: This study used a pre-test, post-test design with a nonequivalent control group. The participants were 68 female students in two universities. The experimental group(n=34) received Seoam pellet therapy at Sanyinjiao while the control group(n=34) received sham therapy. Data were collected from Sep. 16 to Nov. 30, 2012. Severity of menstrual pain, menstrual distress, and difficulty of ADL were assessed prior to and following treatment. Independent t-test, $x^2$ test, and repeated measures ANOVA were used for examination of group differences using the SPSS/WIN program. Results: Severity of menstrual pain (F=41.09, p<.001), menstrual distress (F=56.31, p<.001), and ADL difficulties (F=46.64, p<.001) showed a significant decrease in the experimental group compared to the control group. Conclusion: Results indicate that this Seoam pellet therapy is effective in reducing menstrual pain, menstrual distress, and difficulty of ADL. Therefore, use of this method is recommended for relief of menstrual pain and menstrual distress through self-care.
1. Objectives The primary purpose of this case study is to report that a Soeumin Mang-yang Syndrome with redness of urine and urination difficulty was treated with 'Soeumin Seungyangikgibuja-tang(升陽益氣附子湯)(SIBT)' and then his symptoms, laboratory test(urine) and urine color improved. 2. Methods The patient has the symptoms including redness of urine, discomfort after urinating, general body sweating, constipation and thirst. We diagnosed him as Soeumin Mang-yang Syndrome. So we treated him with 'SIBT(升陽益氣附子湯)'. 3. Results and Conclusions After the constitutional treatment with SIBT(升陽益氣附子湯) on Soeumin Mang-yang Syndrome patient, his symptoms and laboratory test(urine) improved and urine color was clear. 'SBIT(升陽益氣附子湯)' is considered to be effective in Soeumin Mang-yang Syndrome. The change of urine color can be good evidence for the symtoms of Mang-yang Syndrome is improved.
Adrenoleukodystrophy (ALD) is a rare X-linked genetic disorder associated with various central nervous system problems and adrenal insufficiency. It is common in childhood and symptoms include loss of previously acquired neurological abilities, seizures, ataxia, Addison's disease as well as degeneration of visual and auditory function. These children frequently require anesthesia during imagining procedure such as MRI or during some surgical procedures like gastrostomy. There is special need for careful management because of numerous anesthetic challenges like difficulty in cooperation, seizure disorders, life-threatening airway obstruction, copious oral secretion and possibility of aspiration. In addition, adrenal involvement and hypofunction must be considered for safe anesthesia management. We report a successful anesthetic management in a patient with ALD for dental procedures.
Purpose: The purpose of this study was to identify the effects of Koryo Hand-Acupuncture on health status(pain, trunk flexion, IADL, depression) of patients with chronic low back pain. Method: This study used a quasi experimental pre-test and post-test design. Data were collected from December 1st, 2000 to December 20th, 2001. 63 chronic low back pain patients(35 experimental group, 28 control group) admitted to the Back-School and consented to this study. The experimental group participated in treatment: Koryo Hand-Acupuncture and AB-Bong. Two groups was homogeneity. After 4 weeks the effects of treatment on the health status was measured between experimental and control group. Data were analyzed using SPSSWIN 10.0 with crosstab, t-test, and paired t-test. Result: In the experimental group, pain(t=4.85, p=.000) and IADL difficulty(t=2.05, p=.045) was significantly lower than those in the control group. It makes no difference trunk flexion(t=-1.60, p=.114) and depression(t=1.50, p=.138) between experimental and control group. Conclusion: These findings indicate that Koryo-Hand Acupuncture is an effective method for reducing pain and IADL difficulty in patients with chronic low back pain, and is considered as a independent nursing intervention for chronic low back pain.
The purpose of this study is to observe the effects of text modification by comparing differences in Korean reading comprehension levels that arise from differences in vocabulary levels in texts. This study intends to use simplified texts with the vocabulary difficulty adjusted differently from the original text to measure reading comprehension levels of Korean learners and analyze the result. To measure reading comprehension, the researcher divided 55 Korean learners of intermediate to advanced level of fluency into two groups; the control group read the original text and the treatment group read a simplified text in which complex vocabulary were substituted with easier words of medium difficulty. Then the two groups were tested with the same questionnaire to measure comprehension levels of each group. The result showed that the groups that read simplified texts scored higher than the control group; this suggests that the reading comprehension level was increased in the treatment group. The experiment confirmed that unknown vocabulary density has direct impact on Korean reading comprehension. The result shows that the proportion of unknown vocabulary should be reduced for meaning-focused reading. It also demonstrates that comprehension of the learner was enhanced with lexical simplification rather than structural simplification i.e. simplification of grammar or sentences. Thus, diverse reading materials adjusted to the learners' level of fluency should be developed to enable reading for learning Korean. By reducing the burden of understanding the meaning of each vocabulary, learners will be able to achieve the initial goal of reading.
