Photodynamic Therapy Combined with 1064-nm Nd:YAG Laser Therapy and Topical Efinaconazole for Refractory Onychomycosis: Case Series

  • Onychomycosis is the most common disease of the nail, with a worldwide prevalence of 5.5%. It causes local pain, paresthesia, and cosmetic problems and decreases the patients' quality of life. Conventional treatments include the administration of topical and systemic agents. However, factors such as subungual hyperkeratosis and biofilm formation may affect the efficacy of these treatments. Moreover, side effects associated with systemic agents are a major concern for patients. Therefore, various novel treatments are being developed; among them, photodynamic therapy (PDT) and Nd:YAG laser are promising, and several studies have demonstrated the efficacy and safety of PDT and laser therapy. Here, we report the efficacy and tolerability of PDT combined with 1064-nm Nd:YAG laser therapy and topical efinaconazole for the treatment of three patients with refractory onychomycosis.

Single Dose Topical Application of Clotrimazole for the Treatment of Otomycosis: Is This Enough?

  • Background and Objectives: To determine the efficacy of filling the external auditory meatus with 1% clotrimazole at a single visit for the treatment of otomycosis. Subjects and Methods: This prospective study included 40 patients who were referred to our clinic with complaints of ear itching, pain, and fullness, and were diagnosed with unilateral otomycosis. After cleaning the mycotic hyphae from the external auditory meatus, the ear canal was filled with 1% clotrimazole, using an intravenous catheter and syringe. The patients received follow-up examinations on post-treatment days 7, 15, and 45. Results: The follow-up otomicroscopic examinations revealed that 95% of the ear canals were entirely clean and that all symptoms had resolved. The post-treatment scores of pain, aural fullness and itching were significantly lower than the pre-treatment scores (p<0.01). Conclusions: Filling the external auditory meatus with 1% clotrimazole at a single visit is an easy, efficient, and cost-effective treatment for otomycosis. Additionally, high patient compliance makes this treatment superior to long-term topical therapy.

의약분업 예외지역 약국의 스테로이드 외용제 복약지도 실태조사 (A Survey on Pharmacists' Prescription Behaviors for Topical Steroids in Regions without Separation of Dispensary from Medical Practice in Korea)

  • The purpose of this study is to survey on the pharmacists' medication teaching service for topical steroids in regions without separation of dispensary from medical practice. The questionnaire was sent to 199 pharmacists who work inregions without separation of dispensary from medical practice via mail, e-mail or fax. 112 out of 199 pharmacists requested have answered the survey (response rate 56.28%). The questionnaire was composed of 4 categories with demographic characteristics, generally explained contents, adverse effects and precaution in pregnancy, breastfeeding and children. We analyzed the questionnaire by the 6 items with reference to the practical guidelines for medication teaching service. The medication teaching service for topical steroids in terms of generic name, physical characteristics, efficacy, usage and dosage, warning and precaution has been well provided by pharmacists in regions without separation of dispensary except for means and duration of optimal storage. The majority of pharmacists provided their patients with 4 and more kinds of items (69.64%), but the generally recommended 4 kinds of items such as generic name, efficacy, usage and dosage, warning and precaution were provided in as low as 48.22% of the respondents. These results show that the medication teaching service for topical steroids in terms of means and duration of optimal storage (45.54%) and the generally recommended 4 kinds of items have not been enough provided by pharmacists in regions without separation of dispensary. Taken together pharmacists in these regions should make a proper guideline for medication teaching service that can prevent the adverse effects of topical steroids.

미세혈관문합시 헤파린의 국소 및 전신 투여가 혈전 형성에 미치는 영향 (EEFFECTS OF TOPICAL AND INTRAVENOUS HEPARIN ON THROMBOSIS OF MICROVASCULAR ANASTOMOSES)

  • This study was performed to evaluate the effect of topical and intravenous heparin on thrombosis and patency in the microvascular anastomosis of the traumatized veins. Nine white rabbits weighing about 2 kg were used. After exposure of both femoral veins, the veins were crushed by the jaws of smooth needle holder in order to create a thrombosis model. Transectional incision was made in the vein. The animals were then divided into 3 groups based on the administration method of heparin: 1) Experimental Group I, topical irrigation of lumen with heparin saline solution (n=6); 2) Experimental Group 2, topical irrigation of lumen with heparin saline solution and intravenous injection of heparin (0.75 mg/kg) via the marginal ear vein for 3 days; 3) Control Group, topical irrigation of lumen with saline solution (n=6). The patency was evaluated with empty-and-refill test and thrombus formation was judged by surgical microscope. The results were as follows: 1. Thirty minutes after microvascular anastomosis, the patency of all Experimental Groups was better than Control group. However, there was no significant difference among groups. 2. Three days after anastomosis, the patency of all Experimental Groups was much more improved than that of Control Group (P<0.05). There was no significant difference between Experimental Group 1 and 2. 3. Three days after anastomosis, the amount of thrombus in all Experimental Groups was much less than that of Control Group (P<0.05). 4. In histologic findings a lot of luminal thrombus were observed around sutured area in Control Groups. Few luminal thrombus was observed in all Experimental Groups. Mild necrosis in the vessel wall was observed around sutured area in all specimens. These results indicate that topical irrigation of heparin may improve the patency and inhibit the formation thrombus in the microvascular anastomosis of the traumatized veins.

산성 불소가 복합레진 표면에 미치는 영향 (EFFECT OF ACIDULATED PHOSPHATE FLUORIDE ON THE SURFACE OF COMPOSITE RESIN)

  • 복합레진 수복은 치아우식 및 치아우식 예방치료를 위한 대표적인 술식으로 소아치과 영역에서 널리 시행되고 있으며 불소도포는 치아우식 예방을 위한 대표적인 술식으로 1년에 2번 장도 실시되고 있다. 하지만 이러한 이점에도 불구하고 Acidulated phosphate fluoride(APF)가 복합 레진의 표면을 거칠게 한다는 부작용에 대한 논란이 있다. 이에 본 연구에서는 불소도포가 복합레진의 표면에 미치는 영향을 평가하기 위해 시판용 레진인 Solitaire $2^{(R)}$(Heraeus Kulzer, Germany), Metafil $CX^{(R)}$(Sun medical, Japan), Composan $LCM^{(R)}$(Promedica, Germany), $Charmseal^{(R)}$(DenKist, Korea)을 이용하여 시편을 제작하고, 불소젤은 $Topex^{(R)}$(Sultan Dental Products, Canada)를 각각 1분과 4분동안 도포시킨 후 변화를 관찰하여 다음과 같은 결과를 얻었다. 1. 불소젤을 도포시키지 않은 대조군에서 가장 평활한 표면을 보였으며 1분군, 4분군 순으로 표면 거칠기가 증가하였다. 그리고 Composan $LCM^{(R)}$을 제외한 모든 제품에서 유의한 차이를 보였다(p<0.05). 2. 국소도포용 불소젤을 도포한 각 레진 시편의 무게 차이를 비교한 결과 대조군, 1분군, 4분군 간에 유의한 차이는 없었다(p>0.05). 3. 주사전자현미경 관찰시 불소젤을 도포시킨 실험군은 대조군에 비해 거친 표면을 나타내었다.

경구용 생약 복합 조성물과 경피용 육모 조성물의 복합 사용에 따른 육모 상승 효과 (Synergistic Effects of the Combination of Oral Herbal Composition and Topical Hair Tonic on Hair Growth in C57BL6 Mice)

  • 본 연구에서는 경구용과 경피용 육모 조성물의 병용 사용에 따른 육모 상승효과를 평가하기 위하여 C57BL/6 model을 이용하여 검은콩을 포함한 경구용 생약 복합 조성물과 탈모 방지 의약외품의 주요 효능 조성물로 구성된 경피용 육모 조성물의 복합사용에 따른 육모 상승 효과를 평가하였다. 그 결과 경구용 생약복합물을 섭취하거나 경피용 외용제외 도포하였을 때 성장기 모낭을 유도하여 모발의 성장을 촉진시키는 효과가 유사하게 나타나는 것을 확인하였고, 이 두 가지 방법을 병용하였을 때 그 효과가 더욱 상승하는 것을 관찰 할 수 있었다. 이러한 상승 효과는 외용제에서 나타나지 않은 경구제의 혈액 지표 개선 효과가 두 방법을 복합 사용할 경우 더해지기 때문인 것으로 추정된다. 경구제와 외용제의 병용에 따른 상승 효과를 확신하고 그 작용기전을 규명하기 위해서는 $5{\alpha}$-reductase를 비롯하여 탈모의 원인이 되는 다른 지표들의 변화를 평가하고, 이러한 효과가 인체를 대상으로 하였을 때도 나타나는지 확인하는 연구가 후속되어야 할 것이다. 또한 본 연구에서 사용된 조성물의 특이적인 효과가 아님을 확인하기 위하여 다양한 물질을 이용한 평가가 검증 연구가 필요할 것이다. 그러나 본 연구는 그간 막연하게 사용하였던 경구제와 외용제의 복합적인 사용이 기전적으로 구분되어 상승 효과를 가지는 것을 실험적으로 확인하였다는 것에 의의가 있으며 본 연구 결과를 바탕으로 탈모 치료법의 적절한 병용을 통하여 보다 나은 탈모 치료 효과를 기대할 수 있을 것이다.

  • An insecticidal isoflavon glycoside was isolated from the roots of Maakia ammurensis. Its structure was shown to be formononetin-7-O-$\beta$ glucosy [1-6] glucoside [1] by chemical and spectroscpic methods and to have insecticidal activities against Brown planthopper female adults by spray and topical applications.

A Rheological Study of Topical Vehicles

  • To find out the rheological characteristics of several pharamaceutical semisolid products, experiments were carriedout over the temperature range of 20 to 30 .deg.C . From continuous shear rheograms obtained with a Ferranti-Shirley cone and plate viscometer, loop area, shear rates, ryild values, limiting viscosities and activation energies were calculated. The systems were metastable and their viscosity decreased with time. As the temperature was raised, products studied showed a decrease in viscosity and yield value.

  • Twenty-nine dogs with chronic keratoconjunctivitis sicca (KCS) were treated by topical administration of a petrolatum based 2% cyclosporine ointment twice daily. Causes and durations of KCS varied. Tear production increased in 25 of 32 eyes and clinical improvement in corneal health was noticed in 35 of 47 eyes. No serious adverse reactions were observed and only mild irritation was noticed by the owners immediately after the application of the ointment in some cases .
