Purpose: A eutectic mixture of local anesthetics (EMLA$^{(R)}$) cream has been used as a topical anesthetic to reduce the pain of procedures penetrating the skin. It is generally applied for 40 to 60 minutes before the painful procedure. Because of the long application period, EMLA$^{(R)}$ is not useful in the emergency department (ED). The purpose of this study was to determine whether a 20-minute application of 9.6% lidocaine would be useful in reducing the pain of routine peripheral intravenous cannulation in the ED. Methods: We examined 27 male and 19 female patients ages over 18 years of age who required intravenous cannula insertion. Intravenous insertion was performed on 46 patients: 24 patients in the placebo group (mean age: 40.0 years) and 22 in the 9.6% lidocaine group (mean age: 37.6 years). The 9.6% lidocaine or placebo gel was applied and covered with an occlusive dressing for 20 minutes. Pain was scored by the patients using a 0- to 10-cm visual analogue scale. Results: The patients in the 9.6% lidocaine group (mean pain score: 3.4) experienced less pain than those in the placebo group (mean: 5.3), and the difference was statistically significant (p=0.029). Conclusion: We concluded that a 20-minute application of 9.6% lidocaine is safe and effective for reducing pain associated with venipuncture.
The effect of pH of chitosan solution (w/v) on antifungal activity against Malassezia pachydermatis isolated from a dog with otitis externa was evaluated. This study was investigated to find out the optimal pH of chitosan solution that could be able to eliminate yeast cells. For comparison of antifungal activity of chitosan solution 2% chlorhexidine that have been already proved the antifungal effect against M. pachydermatis was used as positive control. The chitosan solution at different pH (2.5, 3.0, 3.5, 4.0, 4.5, 5.0) were involved this experiment. The chitosan solutions at each pH could eliminate a number of yeast cells when they were compared to other antifungal agents. In chitosan solution's antifungal effect of varying degrees according to the pH M. pachydermatis was inhibited at pH of < 5.0. Especially it has been proved an effective antifungal effect at pH 3.5. Therefore, the pH of chitosan solution had a effect on antifungal activity and the optimal pH was 3.5 in vitro. The possibility of topical therapy with acidific solution of chitosan has a potential in skin infection against M. pachydermatis.
This study was designed to examine the effect of tetracycline application to the planed periodontal surgery of the experimentally induced periodontal disease in dogs. Modified Widman flap surgery was done and aqueous tetracycline (1%) was applied to the root surface for 5min, after which the wound was rinsed with saline, and flaps were coronally repositioned. Root surface ntoches were used as reference points. The animals were sacrificed 1 week, 2 weeks, 4 weeks, and 8 weeks after surgery, and block sections of tooth and surround tissue were processed for conventional light and electron microscopy. The results were as follows : 1. A more coronal position of junctional epithelium was observed in the area treated with tetracycline. 2. In the most of the tetracycline - treated teeth, the new collagen fibrils of connective tissue were oriented vertical/or oblique and parallel to the root surface. The vertical or oblique fibers were inserted into the denuded dentin matrix and contacted with exposed dentin collagen fibrils. 3. In the tetracycline - treated root, new cementum apposition, most of acellular extrinsic fiber cementum, was seen with bundles of oriented collagen fibrils incoporating into the cementum. 4. In the control and tetracyclin - treated teeth, bone resorption was observed at the alveolar crest in the 1 week groups.

Purpose: The aim of this study was to investigate the therapeutic effects of a herbal formula, PerioH-035, containing Angelica sinensis, steamed Rehmannia glutinosa, Angelica dahurica, Cimicifuga heracleifolia, and Zanthoxylum piperitum on the periodontal breakdown in a well-established ligature-induced periodontitis model in rats. Methods: Sprague-Dawley rats were randomly assigned to 1 of 4 groups: NL (non-ligatured), L (ligatured), P1 (ligatured and treated with 1 mg/mL PerioH-035), P100 (ligatured and treated with 100 mg/mL PerioH-035). Periodontitis was induced by placing a ligature around the mandibular first molars. PerioH-035 was topically applied to both sides of the first molar for 2 weeks. The right side of the mandibles was retrieved for micro-computed tomography (CT) and methylene blue staining to analyze alveolar bone loss. The left side of the mandibles was histologically analyzed by TRAP and H&E staining. The MMP-9 mRNA level in gingival tissue was investigated by RT-PCR. Results: Alveolar bone resorption was significantly reduced in the PerioH-035-treated groups. The number of dense multi-nucleated cells found to be TRAP-positive by staining in the ligatured rats was markedly decreased by PerioH-035 application. In addition, periodontal tissue destruction, especially cementum demineralization, was ameliorated in the P1 and P100 groups. Moreover, gingival tissue from the PerioH-035-treated group showed a decrease in the MMP-9 mRNA level, resulting in recovery of collagen degradation. Conclusions: These results suggest that PerioH-035 has therapeutic effects on periodontitis, and thus, PerioH-035 shows promise as a treatment for periodontitis.

Paradigm shift in management of infected immature permanent teeth has occurred. The new concept of the treatment includes minimal or no intracanal instrumentation, disinfection with triple antibiotic paste and sealing with mineral trioxide aggregate. This regenerative endodontic treatment promotes differentiation of periradicular stem cells that induce regeneration of vital tissue and continuation of root formation. Thorough disinfection and three-dimensional scaffold are important in this new concept of the treatment. Platelet-rich fibrin has been reported as 'new scaffold' instead of blood clot, which had been used in the past. Triple antibiotics can be used to disinfect the tooth but may lead to complications including discoloration. Three cases of infected immature permanent tooth caused by dens evaginatus fracture are presented. After removal of necrotic pulp and thorough intracanal irrigation, only platelet-rich fibrin was applied to the root canal in the first case. In the other cases, topical antibiotics was used for disinfection and platelet-rich fibrin for scaffold. In all the cases, the opening was sealed with mineral trioxide aggregate. All the cases showed proper healing of inrabony lesion and some lengthening of root. According to these cases, regenerating vital tissue of the infected immature permanent tooth can be achieved with disinfection and application of platelet-rich fibrin.
Curcumin and curcumin containing polyherbal preparations have demonstrated anti-microbial and antiviral properties in pre-clinical studies. Till date no therapeutic intervention has been proved to be effective and safe in clearing established cervical human papillomavirus (HPV) infection. The present study evaluated the efficacy of Basant polyherbal vaginal cream (containing extracts of curcumin, reetha, amla and aloe vera) and of curcumin vaginal capsules to eliminate HPV infection from cervix. Women were screened by Pap smear and HPV DNA test by PCR. HPV positive women without high grade cervical neoplasias (N=287) were randomized to four intervention arms to be treated with vaginal Basant cream, vaginal placebo cream, curcumin vaginal capsules and placebo vaginal capsules respectively. All subjects were instructed to use one application of the assigned formulation daily for 30 consecutive days except during menstruation and recalled within seven days of the last application for repeat HPV test, cytology and colposcopy. HPV clearance rate in Basant arm (87.7%) was significantly higher than the combined placebo arms (73.3%). Curcumin caused higher rate of clearance (81.3%) than placebo though the difference was not statistically significant. Vaginal irritation and itching, mostly mild to moderate, was significantly higher after Basant application. No serious adverse events were noted.
Background: The purpose of this study was to assess the effectiveness of a recombinant human epidermal growth factor (EGF)-based cream for the prevention of acute radiation dermatitis in breast cancer patients receiving radiotherapy (RT). Materials and Methods: Between December 2012 and April 2013, 40 breast cancer patients who received postoperative RT were prospectively enrolled in this study and randomly assigned to receive human recombinant EGF-based cream (intervention group) or general supportive skin care (control group). The grade of radiation dermatitis and pain score were examined at weekly intervals during RT and 6 weeks after RT completion. Results: All patients completed the planned RT and complied well with instructions for applying the study cream and general supportive skin care. In the intervention group, radiation dermatitis of maximum grade 3, 2, and 1 developed in 3 (15%), 11 (55%), and 6 patients (30%), respectively. In comparison, in the control group, radiation dermatitis of maximum grade 3, 2, and 1 developed in 8 (40%), 10 (50%), and 2 patients (10%), respectively. The intervention group showed lower incidence of grade 3 radiation dermatitis than the control group (p=0.068 in univariate analysis and p=0.035 in multivariate analysis). There was no statistically significant difference in the maximal pain score between the two groups (p=0.934). Conclusions: This single-blind randomized preliminary study showed that recombinant human EGF-based cream can have a beneficial role in preventing or minimizing radiation dermatitis in breast cancer patients. To confirm the results of our study, additional studies with a large sample size are required.
The aim of this study was to investigate the alterations in meningeal blood flow by stimulation of trigeminovascular system. An open cranial window was prepared on the right parietal bone of male Sprague-Dawley rats. Trigeminovascular system was stimulated by electrical stimulation of trigeminal ganglion (ETS), somatosensory (whisker) stimulation, or topical applications of capsaicin and neuropeptides including substance P and calcitonin gene-related peptide (CGRP). Neonatal capsaicin pretreatment was performed with subcutaneous administration of capsaicin (50 mg/kg) within the first 24 hours after birth. Changes in regional blood flow of dural artery (rDBF) and pial artery (rPBF) were continuously measured through the cranial window by laser-Doppler flowmetry. Both ETS and capsaicin caused a chain of alterations in rPBF and rDBF responses, i.e., an immediate transient decrease followed by rapid and marked increase in rPBF, which were significantly attenuated not only by pretreatments with L-733,060, a $NK_1$ receptor blocker, $CGRP_{8-37}$, a $CGRP_1$ receptor blocker, and 7-nitroindazole monosodium salt (7-NINA), a neuronal nitric oxide synthase inhibitor but also by neonatal capsaicin treatment. Exogenous neuropeptides including substance P and CGRP increased the meningeal blood flow, which was significantly attenuated not only by pretreatment with L-733,060 and $CGRP_{8-37}$, respectively, but also by pretreatment with 7-NINA. The rPBF response to whisker stimulation was significantly attenuated not only by trigeminovascular system injuries including nasociliary nerve denervation and neonatal capsaicin treatment but also by pretreatments with L-733,060, $CGRP_{8-37}$ and 7-NINA. These results suggest that the stimulation of trigeminovascular system causes prominent alterations in meningeal blood flow, and that neuropeptides as well as nitric oxide in the trigeminovascular system are importantly implicated in the regulation of meningeal blood flow.
Objective : To investigate the effect between Fructus Immaturus Ponciri (FIP, the immature fruit of Poncirus trifoliata) and Fructus Ponciri (FP, the ripe fruit of Poncirus trifoliata) on mast cell-mediated immediate-type allergic reactions. Methods : We performed anaphylactic reaction, histamine release, cAMP, $TNF-{\alpha}$, IgE. Results : The aqueous extract of FIP dose-dependently inhibited systemic and local allergic reaction was induced by compound 48/80 or anti-dinitrophenyl (DNP) IgE in a murine model. FIP also significantly inhibited mast cell-dependent ear swelling response induced by topical application of compound 48/80. When mice were orally pretreated with FIP, the plasma histamine levels were reduced in a dose-dependent manner. FIP dose-dependently inhibited histamine release from the rat peritoneal mast cells (RPMCs) was activated by compound 48/80 or anti-DNP IgE. The level of cAMP in RPMCs, when FIP was added, increased compared with that of a normal or control. In addition, FIP had a significant inhibitory effect on anti-DNP IgE-induced tumor necrosis factor-a ($TNF-{\alpha}$) production from the RPMCs and IgE produced by lipopolysaccharide-stimulated murine whole spleen cells or U266B1 as human IgE-bearing B cells. However, FP showed the lower inhibition rate than those of FIP in above all allergic reactions. Conclusion : These data have important implications for our understanding of the clinical effects of FIP and FP on allergic diseases, and FIP is more effective than FP on the allergic reaction.
Backgrounds : Androgenetic alopecia is a relatively common disorder, but its precise mechanism is not elucidated. There are two commercial drugs approved by FDA. One(finasteride) has an inhibition activity of 5$\alpha$-reductase(type 2) and the other(minoxidil) has a vasodilation activity. Objectives : A verified herbal remedy for baldness is needed for medicinal treatment or preventing alopecia, which could be demonstrated by alopecia-related in vitro & in vivo tests Methods : On the basis of oriental pharmacognosy, we classified many herbal medicines into four groups (malnutrition, aging, alopecia and gray hair) according to its effect. The mitosis induction of hairy dermal papillae cell and the metabolic inhibition for type 2 $5{\alpha}-reductase$ were tested with five herbal extracts. Also, five herbal extracts were added to the normal essence formulation (HHRHG0202-80) in ranges of 0.1~0.3%, which was applied two-mouse models to validate each hair growing activity in vivo. Results : Stimulation of follicular papillae cell proliferation was observed in treatment of three herbal extracts (Glycyrrhizae radix:159.7%, Corni fructus : 144.7%, and Coicis semen : 136.6%) at a dose of $10\mu\textrm{g}/ml$. Three herbal extracts (Biotae semen. Glycyrrhizae radix and Coicis semen) showed inhibitory activity for $5{\alpha}-reductase$(type 2) at 93.18%, 73.36% and 47.6%, respectively at the same dose. We observed the enhancement of hair growth activity in C57b1/6 mouse and the inhibition of alopecia in AGA mouse after topical administration of the hair essence. Conclusions : Hair essence product, which contains five medicinal plants, would be used for the remedy for male pattern baldness (MPB) and the other alopecia diseases.
