A Case Report on the Use of Korean Medicine Treatment for a Patient with Dysarthria Caused by Progressive Bulbar Palsy (PBP)

  • Objectives : To introduce a rare case of a patient with Progressive Bulbar Palsy (PBP) and suggest the possibility of treatment using electroacupuncture and Korean Medicine. Methods : A 61-year-old man with PBP, complaining of dysarthria, was treated with electroacupuncture and Korean Medicine, from June 16 to July 10, 2015. Improvements in symptoms were measured using the speech mechanism screening test (SMST), measurement of tongue and orbicularis oris motility, and speech handicap index (SHI). Results : The scores of SMST, motility of tongue and orbicularis oris showed a tendency for gradual improvement with 25 days of Korean Medicine Treatment after admission, but conversation was still impossible. In SHI scores, one point increased in the speech domain and one point decreased in the psycho-social domain, and three points increased in the other domain. Conclusion : Electroacupuncture and Korean Medicine Treatment improved dysarthria caused by PBP, but not completely. Korean Medicine Treatment seems effective in the management of accompanying symptoms such as black hairy tongue, dry mouth, and general condition. The symptoms of PBP are similar to those of amyotrophic lateral sclerosis and there is controversy regarding the classification of PBP. The most important aspect of treating a patient with PBP is an early diagnosis and devising appropriate rehabilitation strategies.

Effects of Frozen Gauze with Normal Saline and Ice on Thirst and Oral Condition of Laparoscopic Cholecystectomy Patients: Pilot Study (생리식염수를 이용한 냉동거즈와 얼음 제공이 복강경 담낭절제술 환자의 갈증 및 구강상태에 미치는 효과-Pilot study)

  • Purpose: The purpose of this study was to examine the effects of using gauze frozen with normal saline or ice on thirstrelief and oral condition of laparoscopic cholecystectomy patients. Methods: A quasi-experimental nonequivalent control group, pretest-posttest design was used. Participants (n=53) received either gauze frozen with normal saline (n=17), ice (n=18) or wet gauze (n=18) for thirst-relief. The subjective thirst level and oral condition of the participants were assessed before the intervention, 15 min after the first intervention and 15 min after the second intervention. Results: After oral care was provided twice, there were significant differences in thirst level among the groups. When oral care was provided twice, the oral condition of tongue, saliva, mucosal membrane, and gingiva was improved in patients receiving gauze frozen with normal saline or ice. Conclusion: Gauze frozen with normal saline and ice can be effective for oral care in reducing the thirst level and improving the condition of the oral cavity.

  • Angioedema is defined as self-limited, localized swelling. The swelling is asymmetric, non-pitting, and non-tender. Common locations of swelling include periorbital area, lips, tongue, extremities, and bowel wall. A 54-year-old woman visited our hospital with the clinical complaint of left maxillary swelling. Swelling of the left maxillary area was diffuse and temperature of the involved area was normal. No infectious source was found on panoramic radiography and cone-beam computed tomography. After considering the patient's medical history and assessing clinical examination, we suspected angioedema and administered corticosteroid only. In the oral and maxillofacial area, proper diagnosis and prompt treatment of angioedema is important because angioedema of the tongue or larynx may lead to airway obstruction or a life-threatening condition.

Two Case Reports of Taeumin Delirium Patients (태음인(太陰人) 담망 환자 치험 2례)

  • 1. Objectives The purpose of this case report is to evaluate the effects of Sasang constitutional diagnosis and treatment of two delirium patients who were diagnosed Taeumin Dry-Heat Syndrome(燥熱證). 2. Methods Two delirium patients were diagnosed Taeumin Dry-Heat Syndrome(燥熱證) based on their Nature & Emotion (性情), physical characteristics, symptoms. They were medicated Chungpyesagan-tang(淸肺瀉肝湯) through of sleep, complexion, thirst, stool, coating of the tongue, and MMSE(Mini-Mental State Exanmination) score were used of measure improvement. 3. Results and Conclusions Two delirium patients who were treated with Chungpyesagan-tang(淸肺瀉肝湯) showed improvement in delirium symptoms, thirst, constipation, redness of complexion, white coating of tongue, and general condition. These case reports describe the effects on delirium and symptoms of Sasang Constitutional Medication. The necessity of managing the psychological aspects of the pathology through Nature & Emation(性情) is also mentioned.

  • It is used the important method that Oriental doctor determines patient's disease status observing patient's state of tongue in Oriental medicine clinic. In this paper, it developed the how to use the pulse diagnosis and tongue diagnosis based on s mart based electronic acupuncture. It will do objective judgment without wrong diagnosis. In this paper, we developed the algorithm that it automatically determines patient health condition and smart electronic acupuncture kit using fuzzy logic and fuzzy reasoning system were completed. In this paper, Simulation results proved that acupuncture is effective than the traditional method of using electronic intelligence.

  • Macroglossia is a relatively uncommon condition that occurs in pediatric patients for several reasons and contributes to variety of functional problems. Most of macroglossia arises from tissue overgrowth and tongue muscle hypertrophy. There are no definite guideline in prenatal management or diagnosis in this conditions. However, macroglossia is often associated with syndrome or congenital disease, prenatal diagnosis is important in early detection. There are difficulty in measurement of tongue size, and standardization. Macroglossia can be risky in some aspects, such as airway obstruction. In this review, the author suggest prenatal ultrasonographic findings of macroglossia, investigate differential diagnosis of conditions associated with macroglossia, and management in clinical situation. Macroglossia, when present, can cause a number of functional and aesthetic problems for individuals. Treatment of this problem is challenging and controversial.

Clinical, hematological, and pathohistological findings of cattle with bovine leukocyte adhesion deficiency (BLAD) (우백혈구유착결손증(牛白血球癒着缺損症)의 임상(臨床), 혈액(血液) 및 병리조직소견(病理組織所見))

  • During the period from April 1991 to July 1992 clinical, hematological, and pathohistological findings of Holstein-friesian calves 47 with bovine leukocyte adhesion deficiency(BLAD, immunologically ascertained), which were referzed to the clinic for diseases of cattle, veterinary school, Hannover, were described. Most cases show poor body condition, rough and dry in haircoat, salivation, gingivitis, reduction of gingiva and alveolar bone, exposing the incisors' necks, loss of teeth, phlegmonous subcutaneous swellings, ulcerated tongue, recurang fever, coughing, dyspnea, pharyngeal and laryngeal stertor, periodical diarrhea, impaired swallowing, placid and less painsensitive. Relevant laboratory findings are persistent leucocytosis(with more than 30,000 up to 150,000 cells per $mm^3$ of blood), marked neutrophilia(without "shift to the left"), hyperproteinemia, and hypergammaglobulinemia. At post-mortem the carcass of BLAD-affected calves is usually emaciated. All lymphnodes of the respiratory and gastrointestinal tract appear markedly activated(swollen). Lesions in the mouth(gingivitis, defective dentition, pulpitis/alveolar paraodontitis, ulcerated tongue), throat and larynx(inflammation/ulceration), and lungs(pneumonic foci) correspond to the clinical symptoms seen on the living animal. There may be ulcers on the prestomachal mucosa, hyperemia of the intestinal mucosa with hyperplasia of Peyer's patches, ulceration and/or intramural abscesses. The spleen shows follicular hyperplasia. Microscopically, both myeloand erythropoesis are markedly activated in the bone marrow ; capillaries in many organs show leucocytostasis.

Implementation of Intelligent Electronic Acupuncture Needles Based on Bluetooth

  • In this paper, we present electronic acupuncture needles we have developed using intelligence technology based on Bluetooth in order to allow anyone to simply receive customized remote diagnosis and treatment by clicking on the menu of the smartphone regardless of time and place. In order to determine the health condition and disease of patients, we have developed a software and a hardware of electronic acupuncture needles, operating on Bluetooth which transmits biometric data to oriental medical doctors using the functions of automatically determining pulse diagnosis, tongue diagnosis, and oxygen saturation; the functions are most commonly used in herbal treatment. In addition, using fuzzy logic and reasoning based on smartphones, we present in this paper an algorithm and the results of completion of hardware implementation for electronic acupuncture needles, appropriate for the body conditions of patients; the algorithm and the hardware implementation are for a treatment time duration by electronic acupuncture needles, an automatic determinations of pulse diagnosis, tongue diagnosis, and oxygen saturation, a function implementation for automatic display of acupuncture point, and a strength adjustment of electronic acupuncture needles. As a result of our simulation, we have shown that the treatment of patients, performed using an Electronic Acupuncture Needles based on intelligence, is more efficient compared to the treatment that was performed before.

  • Sialolithiasis is a condition in which the salivary gland excretory duct is obstructed due to the formation of calcareous deposits and is uncommon in children compared to adults. The treatment modalities range from a conservative approach involving hydration to a surgical approach. Though several studies have analyzed the sialolith micromorphology structures, studies on pediatric sialoliths remain scarce. This brief communication aims to describe the sialolith micromorphology to understand the mechanism of mineralization and growth of pediatric sialoliths. A 6-year-old Korean female presented with swelling under her tongue. The intraoral examination revealed a painless yellowish hard mass beneath the tongue near the Wharton's duct which was suspected as a sialolith. After receiving the informed consent, the sialolithotomy was performed under local anesthesia. The obtained stone was analyzed through histopathology and transmission electron microscope examinations to understand the mechanism of mineralization and growth of pediatric sialolith. The micromorphology and growth processes of pediatric sialolith remain undescribed. More comprehensive microscopic studies are needed regarding their distinctive characteristics. By expanding knowledge about sialoliths micromorphology, development of new preventive, diagnostic and patient-tailored treatment methods of pediatric sialolithiasis will be enhanced.

  • Anterior open bite is a condition in which maxillary and mandibular incisors do not occlude at central occlusion. It is a vertical discrepancy of the jaws and dental arches that has many etiologic factors making it difficult in diagnosis, treatment and prediction of prognosis. One of the causes of open bite is abnormal size and shape of the tongue. Macroglossia, a condition in which tongue is oversized, is caused by several factors which are not clearly identifiable, and it may be a major factor of anterior and posterior open bite. Macroglossia is subdivided into true, functional and pseudomacroglossia depending on its relative size in the oral cavity. In this case report, a patient was diagnosed as skeletal Class II with pseudomacroglossia, and was treated with SARPE in order to expand the narrowed maxillary arch and Quad helix for the mandibular arch. As a result the transverse deficiency was treated. In the adult patients where no skeletal growth is expected, SARPE has shown to be effective in treating maxillomandibular transverse discrepancies in which macroglossia was accompanied as in this case.
