Primary Spinal Dumbbell-Shaped Mesenchymal Chondrosarcoma Located Intradurally and Extradurally

  • Mesenchymal chondrosarcomas are rare malignant tumors of the bone and soft tissue. Spinal mesenchymal chondrosarcomas are even rarer and, to the best of our knowledge those that are concomitantly located in the intradural and extradural regions, have never been reported. We report a case of a 25-year-old man with back pain and bilateral progressive weakness of the lower extremities. Magnetic resonance imaging revealed a markedly enhanced dumbbell-shaped mass at the T7 level. The lesion was intradurally located at the left side of the spinal cord, and extended extradurally to the extraforminal space through the T7-8 intervertebral foramen. The tumor was completely excised through a posterior approach. Microscopic examination and immunohistochemical studies confirmed mesenchymal chondrosarcoma. Postoperative radiation therapy and chemotherapy were also performed to prevent local recurrence and metastasis. The patient has been symptom-free for two years after surgery. Herein, we reviewed and discussed the clinical characteristics, treatments, and outcomes of primary intraspinal mesenchymal chondrosarcomas in the literature.

성대폴립 및 결절에서의 원섬유성 교원질 (Fibrillar Collagen)의 발현양상 (Fibrillar Collagens in Vocal Polyp and Nodule)

  • Benign vocal fold lesions such as polyps, nodules and edema are known to be caused by vocal trauma such as voice misuse and/or abuse. Even though these lesions are known to be caused by the same etiology, phonotrauma, they show widely different clinical features and different responses to voice therapy. Previous studies suggested that benign vocal fold lesions represent disturbance in the balance of the extracellular matrix(ECM) constituents of the vocal folds. Collagen is one of the major constituents of ECM. Among collagens, fibrillar collagens are most important ones for maintaining the structural integrity. On the basis of gross morphology, vocal polyps wert divided into angiomatous one and edematous one, and nodules were divided into conical one and sessile one. In these four groups, the pattern of distribution of various fibrillar collagens(type 1, 2, 3, 5) was studied by immunohistochemical staining using paraffin embedded tissues. Within each group, differences among collagen subtypes were insignificant. In edematous polyp, collagens were sparsely dispersed in lamina propria by diffuse edema. In angiomatous polyp, collagens were displaced into submucosal layer by hemorrhagic space. In nodules, collagens were stained compactly in lamina propria. Quite different distributions of fibrillar collagens between polyps and nodules are thought to suggest that vocal polyps and nodules are totally different disorders in their pathophysiology.

후두에 발생한 악성 섬유성 조직구종 (A Case of Malignant Fibrous Histiocytoma of the Larynx)

  • Malignant fibrous histiocytoma is one of the rare types of larynx tumor. The most common sites of the tumor are limbs, trunk, and retroperitoneal space, but tumor localization within head and neck are very rare. It is built of histiocytes, fibroblasts and multinuclear giant cells. A diagnosis of the tumor includes microscopic and immunohistologic examination with identification of specific tissue markers and intermediate filaments of proteins. This disease has been treated by several methods combining radical surgery, radiotherapy, and chemotherapy, but the prognosis is poor. We present 74-year-old Asian man with dysphonia for 2 years. The tumor of the larynx was examined on laryngoscopy. The radical surgery rendered the final pathological diagnosis, confirmed histologically and immunohistochemically as malignant fibrous histiocytoma. This tumor was treated with laser cordectomy followed by radiotherapy. 3.5 year's observation of the patient didn't either show any signs of recurrence or dysphonia.

산소중독에 대한 Glutathione과 Chlorpromazine의 보호효과에 관한 실험적 연구 (A STUDY ON POTENTIAL PROTECTIVE ACTIVITIES OF GLUTATHIONE AND CHLORPROMAZINE AGAINST OXYGEN TOXICITY)

  • Effective measure to prevent oxygen toxicity is greatly required as there increase chances to be exposed to high oxygen pressure, for example, space travel, deep sea diving and hyperbaric oxygen therapy. In the present study, in an attempt to evaluate glutathione and chlorpromazine as protective agents against oxygen toxicity, effects of the agents were tested on various toxicities (death rate, convulsion rate, time to convulsion, increase in weight of lung and brain and pathological changes in the organs) observed in rats exposed to 5 Absolute Atmosphere (ATA) of 100% oxygen for 120 minute. Glutathione reduced mortality rate and convulsion rate and also markedly suppressed the increase in lung and brain weight. The pathological changes observed in these organs were ameliorated by administration of glutathione. Chlorpromazine also reduced mortality rate but its effects appeared to be limited mainly to pulmonary toxicities. Thus glutathione seems to be more effective than chlorpromazine as a protective agent. The results obtained may support that oxygen toxicity is mediated by oxygen free radicals.

  • The outer-most electrons of metal atoms and the remining valence electrons of any molecular atoms make three dimensional crystallizing $\pi$-bondings. The electrons on the $\pi$-bonding orbital rotate clockwise or counter-clockwise and they then make electro-magnetic waves between atoms on the orbital because electron move between plus charged ions. The three dimensional crystallizing $\pi$-bonding orbitals are quantum-mechanically modeled by a cyclic Kronig-Penny Model and energy band structures are analyzed with their potential barrier thickness. The waves generated between plus charged ions are the particular $\pi$-far infrared rays, which have dual properties between material and electro-magnetic waves and can be measured not by modern electro-magnetic tester but biosensor such as finger's force tester. Because the $\pi$-rays can be modulated with electro-magnetic waves it can be applied for harmful electro-magnetic wave killers. Because the $\pi$-rays make new three dimensional crystallizing $\pi$-bonding orbitals in the material the food and drink can be transformed into a helpful physical constitutional property for human health. Distinction between crystalline and amorphous metals is possible because very strong crystalline $\pi$-bonding orbitals can not easily be transformed into another. The $\pi$-rays can also be applied for biofunctional diagnostics and therapy. Gravitational field is one of the electro-magnetic fields. 곁인대가 파열되고 주관절이 탈구된 개에서 이중 Locking-loop 봉합법의 치료효과 (Therapeutic Effect of a Double Locking-loop Suture Pattern on the Elbow Luxation with Rupture of Collateral Ligament in a Dog)

  • A nine-month-old male Jindo with non weight-bearing on the right forelimb in flexed position, pain and edema of the elbow, and resist elbow extension was brought to the Veterinary Teaching Hospital, Chungbuk National University. Elbow radiographs showed loss of humeroradial joint space and lateral displacement of the radius and ulna. Closed reduction was reported the best therapy in most cases of luxation of the elbow but conservative reduction was impossible. Open reduction of the luxated elbow was performed and ruptured collateral ligaments were identified. Displaced elbow was required bloody surgical operation and gentle reduction to restore elbow joint. Internal reduction of choice for elbow luxation with rupture of collateral ligament in the dog was a double locking-loop suture pattern. To ensure secure grasping of parallel bundles of ligament fibers to transverse bites of each suture were placed superficial to the longitudinal bites. All ligaments were repaired with 3-metric (size 2 USP) monofilament polypropylene suture. No complications have been noted during a five-month follow up.

Revitalization of necrotic mature permanent incisors with apical periodontitis: a case report

  • Despite considerable focus on the regenerative endodontic treatment of immature teeth with necrotic infected pulps and apical periodontitis, little data exist with regard to its possible implementation in necrotic permanent teeth with complete apical and radicular development. The present report describes the procedures and outcome of a regenerative endodontic treatment approach in 2 previously-traumatized incisors with closed apex with apical periodontitis. A 2-visit treatment procedure was employed. At initial visit, the root canals were copiously irrigated, followed by placement of a triple antibiotic paste containing ciprofloxacin, metronidazole, and clindamycin into the root canals. After 4 weeks, the antibiotic paste was removed, and apical bleeding was initiated with size 10 hand files beyond the apices. The root canals were coronally sealed with mineral trioxide aggregate, and the access cavities were restored with bonded resin composite. At post-operative 60 months, both teeth were remained asymptomatic, with the recall radiographs showing complete resolution of apical radiolucency and reestablishment of periradicular tissues. In both teeth, the dimensions of root space remained unchanged as verified by image analysis. The revitalization protocol utilizing root canal disinfection and induced apical bleeding in necrotic, closed-apex incisors may offer a clinically acceptable alternative to conventional root canal treatment.

Full Mouth Rehabilitation of a Patient with Bite Collapse in the Molar Area Using Removable Partial Denture and Dental Implant Prosthetics

  • Dental clinicians often encounter cases wherein the patient's lost molar area was neglected and left untreated for an extended period of time, thereby causing the extrusion of opposite molars and occlusal disharmony as well as occlusion in the anterior teeth and consequently resulting in anterior displacement in the area. Clinicians normally carry out prosthetic treatment via occlusal plane lifting when such becomes absolutely necessary due to the lack of sufficient space needed for prosthetic therapy aimed at proper anterior and lateral induction. In this case report, we examined occlusal disharmony and VDO loss in a patient who had lost his molars and had not received prosthetic treatment for an extended period of time. We treated the maxillary area with dental implant prosthetics and Kennedy Class I RPD and the mandibular area with residual natural tooth-based implant placement and dental implant prosthetics. The patient reported treatment outcomes that were deemed satisfactory both functionally and aesthetically.

하악대구치의 치성감염으로 유발된 안와골막하농양의 치험례 (ORBITAL SUBPERIOSTEAL ABSCESS SECONDARY TO ODONTOGENIC INFECTION OF LOWER MOLAR : A CASE REPORT)

  • A 61-years old man with diabetes mellitus(DM) was admitted to our hospital, complaining of progressive right periorbital swelling, headache and toothache on the right lower second molar. On the first visit, moderate swelling was noticed from the right periorbital region with exophthalmos and subconjunctival effusion. Intraorally, right lower second molar had a severe periodontal disease and fistular formation on its distal area. From 3 days after hospitalization, the visual acuity of his right eye was gradually worsen and we performed CT scan. CT scan demonstrated an inflammatory change at the right orbit with subperiosteal abscess at the inferior orbital wall, which was extended from the right infratemporal, parapharyngeal and internal pterygoid space. Patient was treated by mean of intraoral(right upper vestibular and retromolar) and extraoral(infraorbital) incision and drainage, massive anti-therapy and DM control. The patient improved gradually and finally was discharged from the hospital, but his visual loss of right side was not recovered.

협부에 발생한 점액양 지방육종의 치험례 (A Case of Myxoid Liposarcoma of the Cheek)

  • Purpose: Liposarcoma is the most common soft tissue sarcoma, and usually occurs on the thigh or in the retroperitoneal space, but rarely in the oral region. This report presents a case of liposarcoma of the cheek and includes a review of the literatures. Methods: A 21-year-old woman was admitted with a palpable mass in her cheek of about two years duration, which increased in size gradually initially, but had increased rapidly over the three months. There was no particular pain or tenderness. MRI showed a well-enhanced, well-defined mass, which suspected to be hemangioma. Results: The spherical, well-encapsulated mass was surgically excised. Biopsy results revealed myxoid liposarcoma. FDG PET-CT on the seventh postoperative day, revealed a minimal to mild FDG-uptake soft tissue lesion around the mass defect area without evidence of distant metastasis. The patient is being observed and undergoing radiation therapy. Conclusion: Liposarcoma in the head and neck region is a rare disease, and can be overlooked as a benign tumor without a pathologic diagnosis. Therefore, proper treatment and follow-up are required based on an understanding of this disease.
