The effect of reticuloendothelial hyperfunction on hypothermic preservation of lung was studied in dogs. In order to evaluate the viability after hemodynamic_ load in preserved isolated lung, observations were made on the rate of increase in weight, degree of edema,compliance and surface activity of lung. The results obtained as follows: l. In the group of activating of the reticuloendothelial system by injection of sodium thiosulfate intravenously before pneumonectomy and infusion of naphthionine through the pulmonary artery before hypothermic preservation of isolated lung the limit of preservation was eight hours whereas four hours in non-treated control group. 2.Therefore the method of activating of the reticuloendothelial system before and after pulmonary resection seems effective in preserving for prolonging the period of preservation of lung by means of inhibition of pulmonary edema. 3. Pulmonary surface activity is expected to be valuable as a method in evaluation of the viability of preserved lung along with compliance and rate of increase in weight of lung.
Objectives : The purpose of this study is to investigate that effect of Angelica gigas NAKAI pharmacopuncture(AGN-Ph) by concentration at $ST_{36}$(足三里) and $BL_{23}$(腎兪) in Freund's adjuvant rats. Methods : The experimental model of arthritis was induced by injection of Freund's adjuvant into Sprague Dawley(SD) rats. After arthritis was induced, AGN-Ph was injected by concentration at $ST_{36}$ and $BL_{23}$ of rats every other day for 6 times. Thereafter, edema rate, body weight, IFN-${\gamma}$, TNF-${\alpha}$, hematologic assay were measured. Results : The results were as follows, 1. After 3 times AGN-Ph treatment, the mean of edema rate was significantly decreased in AGN-Ph group 3 than control group. And after 6 times AGN-Ph treatment, the mean of edema rate was significantly decreased in AGN-Ph group 1, 2, 3 than control group. 2. The mean of body weight was significantly increased in AGN-Ph group 1, 3 than control group and saline group. 3. The mean of IFN-${\gamma}$ was significantly increased in AGN-Ph group 3 than control group. 4. The mean of TNF-${\alpha}$ was significantly increased in saline group than control group. But the mean of TNF-${\alpha}$ in AGN-Ph group 2, 3 showed no significance compared with control group's. 5. In hematologic assay, levels of WBC, RBC, Hemoglobin, Hematocrit showed no significance in all groups. Conclusions : These results are suggest that the Angelica gigas NAKAI pharmacopuncture(AGN-Ph) at $ST_{36}$ and $BL_{23}$ has a suppressing inflammation effect on Freund's adjuvant arthritis in rats.
Objectives This study was carried out to investigate the experimental effects of Lonicerae Caulis and Bee-venom Acupuncture in Yanglingquan (陽陵泉, GB34) that have clinical efficacy in the Rheumatoid Arthritis. Methods Materials of present study are Lonicerae Caulis Extracts (LCE), Bee-Venom Acupuncture (BVP), Sprague-dawley rats (250 g or so, ♂), and various kinds of needing experimental studies. We measured several experimental items of the rats with the arthritis induced by Freund's complete adjuvant (0.2 ml/kg), such as body weight, rate of paw edema, analgesic effect by hot plate method, WBC, TNF-${\alpha}$ cytokine and IL-10 cytokine. Rats were divided into four groups; Normal group that was treated with normal saline 1.0 ml (o.p) and $15{\mu}l/kg$ (GB34) to normal rats, Control group that was treated with normal saline 1.0 ml (o.p) and $15{\mu}l/kg$ (GB34) to pathologic model rats induced by Freund's complete adjuvant 0.2 ml/kg, Experimental group A that was treated with LCE 1.0 ml (o.p) and normal saline $15{\mu}l/kg$ (GB34) to pathologic model rats induced by Freund's complete adjuvant 0.2 ml/kg and Experimental group B that was treated with LCE 1.0 ml (o.p) and BVP $15{\mu}l/kg$ (GB34) to pathologic model rats induced by Freund's complete adjuvant 0.2 ml/kg, and the experiment took over after 28 days. The results were analysed using SPSS for windows 12.0. Results Experimental group A showed the increase in body weight, paw licking times and IL-10 cytokine compared to Control group. Also it was decreased in rate of paw edema, WBC, and TNF-${\alpha}$ cytokine compared with Control group. Experimental group B showed the increase in body weight, paw licking times, and IL-10 cytokine compared to Control group, and showed the decrease in rate of paw edema, WBC and TNF-${\alpha}$ cytokine compared to Control group. Especially TNF-${\alpha}$ cytokine and rate of paw edema were accepted statistical significance compared with Control group. Conclusions It is suggested that Lonicerae Caulis (o.p) and Bee-venom Acupuncture (GB34) can be used in the treatment of rheumatoid arthritis.
This study was designed to check of Scolopendra subspinipes multilans Herb-Acupucture on effect of anti-inflammtory function, anti-febrile function and analgestic function in carrageenan-induced arthritic rat. Each of 8 rat were classified into control, sample group. Control group were inject by normal saline and Sample group were injected by Scolopendra subspinipes multilans. Herb-Acupuncture. After elicitating edema and inflammation of Sprgue Dawely(SD) rats by injection of 1% Carrageenan 0.1ml, then the rate of increase of paw edema, CRP, ESR, WBC were checked and rectal-temperature, topical temperature was checked about anti-febrile. writhing syndrome was checked about analgesic. The results were as follows 1. In the sample group, the rate of increase of paw edema was significantly decreased as compared with thatof control group(P<0.05) 2. In the sample group, the rectal-temperature was non-significantly decreased as compared with that of control group (P<0.05) 3. In the sample group, CRP and WBC was significantly decreased as compared with that of control group, but ESR was not.(P<0.05) 4. In the sample group, opical-temperature was significantly decreased as compared with that of control group (P<0.05) 5. In the sample group, analgestic function was not sigiflcntly effected as compared with that of control group(P<0.05) According to the above result, it can be concluded Scolopendra subspinipes multilans Herb-Acupucture showed the treatment effects on the artifical arthritis resulted from carrageenan in rat and it is suggested that more interest and study in the mechanism and clinical use were needed.
Objective : This research carried out to find out the effective efficiency of acupuncture at ST36 GB39 and herbal-acupuncture using different concentrational Uris Fel Bovis Calculus on adjuvant arthritis in rats. Method : In this study, I do acupuncture treatment and herbal-acupuncture in ST36 GB39 by solution that diluted to fivefold and tenfold with combined solution of Bovis Calcuclus and Ursi Fel at 10:1. And then I observed the change of inhibitory rate of paw edema, body weight, hematologic assay, histologic assay. Result : 1. In Adjuvant Arthritis in rats, the herbal-acupuncture group 1 is more efficienct then acupuncture group. 2. In histologic assay of knee joint, synovial cell and fibroblast markedly activated in herbal-acupuncture group. 3. The albumin level in serum more markedly increased in herbal-acupuncture group 2 than the control group. 4. The paw edema significantly decreased in acupuncture group and herbal-acupuncture group 2 compared with the control group. 5. The body weight significantly increased in acupuncture group and herbal-acupuncture group 2 compared with the control group. Conclusion : In these results, The herbal-acupuncture using Ursi Fel, Bovis Calculus, suppress inflammation of Adjuvant Arthritis, and decrease markedly the count of WBC, and increase the albumin level in serum, and have an influence on Adjuvant Arthritis with activating of synovial cell, fibroblast. And these were more effective than the others, and then needed consistent study of its mechanism.
Although re-expansion pulmonary edema (RPE) is rare (incidence rate <1%), it is associated with a mortality rate of >20%; therefore, early diagnosis and treatment are important. We report a case of RPE following chest tube insertion in a patient with spontaneous pneumothorax. We have specifically focused on the mechanism underlying RPE and the possible etiology. An 82-year-old man with a history of chronic anemia, chronic obstructive pulmonary disease, diabetes mellitus, and hypertension was referred to the emergency department for management of recurrent right-sided pneumothorax. We performed emergency closed thoracostomy for suspected tension pneumothorax, which led to stabilization of the patient's vital signs; however, he coughed up frothy pink sputum accompanied by severe right-sided chest pain 30 min postoperatively. The patient showed new-onset right pulmonary consolidation on chest radiography, as well as desaturation, tachycardia, and tachypnea and was diagnosed with RPE. He was transferred to the intensive care unit for mechanical ventilation and supportive treatment using diuretics, ionotropic agents, and prophylactic antibiotics. RPE gradually resolved, and the patient was extubated 3 days after admission. He has not experienced recurrent pneumothorax or pulmonary disease for 4 months. We emphasize the importance of RPE prevention and that aggressive ventilator care and supportive treatment can effectively treat RPE following an accurate understanding of the underlying pathogenetic mechanisms and risk factors.
The purpose of this study was to determine the effects On the lymphedema patients of sponge padding and the correlation between the rate of edema reduction and downward shifting of bandages. Fifty secondary lymphedema patients with no infection, active cancer, limitation of movement, pain or drugs were randomly assigned to two groups: a sponge padding group, and a cotton padding group. Complex decongestive therapy was applied for 2 weeks. In each group the volume of the ankle, mid-calf, knee, and mid-thigh and downward shifting of bandages at the thigh was measured with measuring tape. The reduction of volume and downward shifting of bandages were analyzed by the unrelated t-test and the Pearson Correlation Test. The results were as follows: 1. In comparison between the sponge padding group and the cotton padding group, there was no significant difference in volume except in the thigh area. The edema volume was reduced significantly to $37.77{\pm}31.15%$ at the thigh with sponge padding after treatment (p<.05). 2. Bandage shifted 10.12 cm with cotton padding, but only shifted 2.85 cm when sponge padding was used. Sponge padding had an excellent effect in preventing the shifting of bandages. There was a significant difference in the amount of shifting that occurred with each type of padding (p<.05). 3. There was also a strong correlation between the reduction of edema and the shifting of bandage. These results suggest that short-stretch bandaging with sponge padding is more effective than bandaging with cotton padding. Further study on skin irritation and the comfort of sponge padding is needed to use padding materials safely and effectively.
Purpose: The purpose of this descriptive study was to investigate the application of physical restraints in ICUs of a university hospital. Method: Data were collected from August 1 to October 1 2009, using a recording tool from Electric Nursing Records and an observation tool for physical restraints and related factors. Frequency, percentage, and paired t-test with the SPSS/WIN 16.0 program were used to analyze the data. Results: The application rate for physical restraints in 5 ICUs was 34.3%, the highest application rate was 48.4% in the neurosurgical ICU. The mean frequency for physical restraint application per patient was 1.14, and the mean hours of restraint application per incident was 113.01 hours. The most common complications of physical restraints were bruising and edema. Conclusion: Many ICU nurses use physical restraints when caring for patients who are at high risk for falls, including patients have high levels of acuity, decreased level of consciousness or increased irritability. When physical restraints are applied, patients show resistance against the use resulting in side effects of bruising and edema. Therefore, nurses should observe changes in restrain sites, perform nursing interventions to prevent complications, and minimize the use of physical restraints.
Sangphisan, sangphisan with Cortex Mori Cortex Lycii, Semen Lepidh were administrated into rats and then they were injected by Paraquat. The results are as follows 1. After 72 hours following the injection of paraquat, the survival rate of rats increased Sangphisan, Sangphisan with Cortex Mori Cortex Lycii Group. 2. Respirayory rates of rats witch survived 72 hours later significantly decreased in sangphisan, sangphisan with Semen Lepidh. 3. Lung weights of rats which survived for 72 hours later significantly decreased in Sangphisan, Sangphisan with Cortex Mori Cortex Lycii and mild decreased in Sangphisan with Semen Lepidh. 4. After the injection of paraquat, a lung tissue showed severe hemorrhage, edema and some broken aibeoli and Sangphisan, sangphisan with Cortex Mori Cortex Lycii, Sangphisan with Semen Lepidh recovered significantly.
Tocolytics 임신부의 조기진통시 치료제로 많이 사용되는 약제지만 드물게는 폐부종을 일으켜 치명적일 수 있다. Tocolytics가 폐부종을 일으키는 기전은 용적증가, 교질삼투압의 감소, 심근 손상, 모세혈관 내막의 손상등에 의해 발생한다고 설명되고 있으나 아직 불확실한 상태이며 폐부종 발생의 양대 기전인 정수압적 폐부종과 투과성 폐부종의 양상이 모두 관여하는 것으로 알려져 있다. 또한 임산부가 다산부이거나 다태아 임신인 경우, 임신중 수액공급을 많이 받은 경우, 심장 질환이 동반된 경우, 감염이 동반된 경우, 동시에 steroid나 MgSO4로 치료 받은 경우등이 tocolytics 사용시 폐부종이 쉽게 발생될 수 있는 선행요인으로 작용함이 알려져왔다. 저자들은 조기진통이 있어 tocolytics인 ritodrine 치료후 정상분만하였으나 분만후 급속히 진행하는 폐부종이 발생한 30세 여자 환자를 경험하였기에 문헌고찰과 함께 보고하는 바 이다.
