Implementing a home modification to enable elderly's safe and independent living is the key plan to realize their aging in place. South Korea in which had entered an aged society is not yet vitalized in home modification for the elderly compared to that of welfare-developed countries, and South Korea provides support that is limited to the low-income elderly. Therefore, this purpose of this study was to analyze the laws related to the home modification, the present condition of home modification support, the standards and guidelines in home modification, the support in house modification cost, and supporting organization and working force in the home modification. Through the analyzing process, this study examined the current situation and problems of institutional support in the home modification for elderly and the proposed plan for institutional improvement. The suggestions based on the results are as in the following. 1) Home modification support law(act or regulation) is required to be improved 2) Home modification support system correspondent to aging process should be provided regardless of their income levels. 3) Delicate plan standard and guideline are necessary for a process of implementing the home modification for the elderly. 4) Information on life behavior is in need for the implementation of elderly-customized home modification. 5) Cost for the home modification should be considered to cover by the Act on Long-Term Care Insurance for the Aged. 6) Housing Welfare Center and Housing Welfare Professional should be actively utilized for the home modification support institution and work force.
The purpose of this research is to analyze the overall problems at the moment of October 2008, and then to find the improvements of home-help services of the Long-Term Care Insurance(LTCI), which has been revealed many problems since it was released in July 2008. The research uses the literature survey which analyzes 2nd-hand materials studied by other people already, and survey research was executed from active social workers in the area of LTCI. Based on the policy analysis framework of Gilbert and Specht, all the data are analyzed in the scopes of client·benefit(service)·finance·transferring system. This research has found the problems in each scope of home-help services of the LTCI. Firstly, the client system has some problems in mismatching between registered and service clients, estimating client number, and judging service levels. Secondly, the service system reveals deficiency in professionality of social workers, service quality lowering by loose qualification criteria on workers, non-reasonable limitation of service time available, and the same fare system applied to visiting-help service in spite of different levels. Thirdly, in financing system, clients need to pay additional money to get extra services such as meal, hair cutting, bathing etc., due to government financial support stopped, some organizations have to reduce services and replace full-time workers to part-time ones, which makes the service quality worse. Lastly, in the transferring system, the management system for service quality is not well prepared. There are too much competion because of allowing too many home-help service organizations and care worker academies. The suggestions that this research has found to improve the policy are as follows. ① It is desirable to make the registered clients the service ones as many as possible in the long term perspective. ② The LTCI organization requires more workers and higher professionality. ③ Many elderly people who are not eligible now require connection system to be more served. ④ Management system and service manual for care worker are to be developed. ⑤ Laws related to the service contents and process should be modified, the proportion of client charge needs to adjust. ⑥ Home-help service organization licensed by the LTCI needs to be financially supported publicly. ⑦ Monitoring system to home-help service organization needs to be strengthened. ⑧ Evaluation tools to home-help service organization and workers is required. ⑨ Specification to open the home-help service organization needs to be more strict.
Purpose: This study was done to investigate factors associated with depression and quality of life (QoL) among the community-dwelling elderly. Methods: This study used a descriptive correlational research design. The subjects were 730 elders aged over 65 living in D district of Daegu. Data were collected using questionnaires for 30 days in April, 2007. The research instruments utilized in this study were a physical function scale of long-term care insurance system, Geriatric Depression Scale Short Form Korea Version (GDSSF-K), and Korean Quality of Life Scale (KoQoLs). The collected data were analyzed by descriptive statistics, t-test, ANOVA, Duncan, stepwise multiple regression, and Spearman correlation. Results: The mean age of the subjects was 72.6, and 68.8% and 57.9% of subjects were, respectively, female and living alone. 12.3% of variance in depression was explained by age, education, economic status, subjective health, alcohol consumption, condition of teeth, and fall experience. 18.2% of variance in QoL was explained by economic status, number of diseases, condition of teeth, incontinence, paralysis, and IADL. Economic status and condition of teeth were contributing factors to depression and QoL of the elderly. Conclusion: Findings of this study may be useful in understanding the health status of the community-dwelling elderly and developing more regionally specific health promotion strategies.
The purpose of this study is to investigate the determinants of job satisfaction among care givers. Finding out the determinants of job satisfaction among care givers, the dependent variables were defined 3 groups with 9 sub-factors, such as personal factors of growth needs and self-efficacy, job factors of job diversity, job autonomy, job importance, role clarity and workload, organizational factors of comradeship, education & training. For the study, data were collected form 479 care givers working in 52 care facilities of Chung Nam Province and analyzed them by regression analysis technique using the SPSS 20.0 statistical package. The results are as followings, job diversity, job importance, comradeship, education & training have positive effects on job satisfaction respectively, workload effects on it negatively, but growth needs, self-efficacy, job autonomy, role clarity no effectiveness on job satisfaction. Based on the results of study, theoretical implications are discussed for care givers to promote the job satisfaction and suggested the directions for further study. Further more, the findings provide the basic sources for care giver human resource management in long term care facilities which are suffering from recruiting of care giver, and it also contribute to successful management of long term care insurance system as a meaningful suggestion.
Nursing Homes do not have a defined standard in the space area nor does it have a detailed standard facility requirement by law. This can possibly lead to the deterioration of the facility and the system. This directly affects the medical treatment space area within the nursing home. The medical treatment area provides medical treatment to seniors and this is where the seniors get most of their daily services. Therefore, this is research is about the study of the space area of the medical center and the ratio trend of the space area for the medical treatment facility located in senior nursing homes. Ten facilities have been selected in this study to analyze the correlation factors between space area and its trends. The analysis performed includes the conditions relating to the area and what affects the center. We have followed up with a proposal for improvement of the facility and area configuration for the medical treatment facility. Based on the analysis, the following conclusion can be made: First, the senior welfare centers are mostly used as a residence purpose followed a temporary stay of residence facility for the seniors. Second, research indicates that the bigger the facility, nursing and public functions took a larger portion of the space area compared to other services within the senior welfare centers. Third, the study shows the management space area took up about 1%~6% of the entire medical center within the nursing home which is a narrow space area because of the integrated management. Fourth, analysis based on the trend in the time-series indicate after the adoption of the system, there is a continued decline in the space area of nursing, management and public areas. Lastly, since before and after 2008, the space area composition of the nursing facility shows a continuous decline in our study. We can safely conclude that the revised senior welfare act's construction plan has an effect in the facility and is effectively working to meet its requirement. Therefore, the revision of the law is required to reflect the social needs of the residents.
Today, home health care services needs a linkage plan of the customized home visiting health service in public health center, the medical institute home health care service, and home visiting nursing service based on long term care insurance for the elderly program which acknowledges the independence and professionalism of the home health care services while minimizing overlap through linking the projects. So, this study was performed by applying the Delphi technique, which draws agreement from professional opinion, to determine a method to link home health care services in Korea. The results of this study are as follows. Specialists agreed on 24 important items within the two domains of institutional linkage and medical linkage. And the significance of this study is as follows. The 24 items deduced for the approved nursing service linkage plan are expected to improve the home health care service business system, enhance the quality of home health care service, and bring increased satisfaction for service recipients. Also, seeking ways to minimize overlap in service can increase the effectiveness of health care and public health management at a national level. In addition, it is considered that this will ultimately reduce public medical costs as well as improve home health care service.
The purpose of this study is to identify a potential user's need for use and operational of the elderly day care center and to identify the problems and suggest development directions. The data collection was conducted on 320 ordinary people living in J-do through a questionnaire, and sought to understand the operational status of in-depth interview institutions for the heads and employees of six center. As a result of the analysis, a potential user's ware not aware of the functions and specific roles of day care center, the cost of facilities was covered by the government, and that facilities and the environment were important factors when using the center. In-depth interviews show that the current policy or system is far from reality and does not fit the status or role of the workers and the operational status of the agency, requiring the re-establishment of the on-site system. Based on the research results, the development direction of day care facilities requires active promotion and strengthening of family support for improving and enhancing awareness of day care facilities to citizens at the national level. Second, the quality of nursing services should be enhanced through improvements in facilities and the environment that reflect users' needs. Third, The government should improve the quality of nursing services by improving the poor employment conditions and treatment of workers to fit the reality.

In Korea, welfare needs brought on by the rapidly growing of elderly population. Also, the senior group with spare time, wealth and health is increasing. Currently, the people have known about needs welfare system for improving quality of life during senescence personally and socially. In addition to the senior friendly industry, having distinction with simple approach medical services, will be great expanding after 2010. And there will be a new welfare services plan supporting long term care insurance for senior citizens. But we don't have prepared systematically organized senior friendly industry and products yet. So, the purpose of the study is to analyze systematically the classification characteristics of the senior friendly products according to the difficulty of daily living for senior citizens. Above all, the study finds out the senior friendly products characteristics according to the consumer's various situations about health and housing space. Therefore the result of the study reveals that the simulation guidelines for choosing the products depend on the customer's needs are essential and useful to improve senior friendly industry. And the post evaluation data of senior friendly products would be put to practical use in the industry.
This paper convergent examined caregiver burden, depressive symptoms and health related quality of life of 226 caregivers of stroke patients, who had been hospitalized in 7 general hospitals located in Cheongju and Daejeon Metropolitan city. The quality of life was measured using SF-8, which is a short version of HRQoL. According to Multiple regression Analysis, dependence of a patient were important factors in the physical related quality of life. These results suggest that in order to improve the quality of life related to the physical health of the stroke patient's caregiver, the elderly long-term care insurance system and the caregiver system and the ward of the Ministry of Health and Welfare are expanded ward without a caregiver. There is a need to raise expectations for recovery through education and family support groups.
The purpose of this study is to investigate the relationship between public care and family care. Public care for older adults began in 2008 with the implementation of the Long-Term Care insurance in South Korea. Although the expansion of public care has the purpose of reducing the care burden for the family, it is not easy to say whether the developments of public care system reduce the amount of family care for older family members. Theoretically, public care and family care are expected to have various relationships depending on the degree of the role and function(substitution, hierarchical compensatory, task specific, supplementation, complementarity). And literatures have showed inconsistent results depending on the country, data, and methods. In this study, we analyzed the relationship between two care types focusing on home care services for older persons. Analyses were based on data from the second(2008) to sixth(2016) waves of Korean Longitudinal Study of Ageing(KLoSA). To investigate elderly care dynamics in the households, we pooled the data for four changes between two periods(2008-2010, 2010-2012, 2012-2014, and 2014-2016). This study used an analytic sample of 262 older adults, who are aged 55 over and experienced public care at least one point of time. We used Fixed-Effects(FE) model to analyze the differences within the same individuals under the condition that time-invariant unobserved factors are controlled. This study distinguished the cases of entry into public care and other cases of exiting public care. The results showed that older people who are dependent on public care are less dependent on family care than before. In both entry and exit groups, negative relations were maintained, but in the entering stage of public care, the degree of negative relations was relatively small, whereas in the stage of maintaining or departing from public care, relatively negative relations were strong. At the beginning periods, even though public care increased, family care did not decrease significantly. On the other hand, at the time of ending public care and relying on family care, family care increased significantly. The results of this study show that the relationship between public care and family care is close to hierarchical compensatory model and varies according to the stage of caring transition. Also, it was found that the cases of transition from public care to family care have the biggest burden of elderly care than other groups.
