Quantitative evaluation of hepatic function for pre- and post-surgery patients using deconvolution technique in Tc-99m DISIDA SCAN (Tc-99m DISIDA SCAN에서 deconvolution 방법을 이용한 외과적 수술 전후의 간의 정량적 평가)

  • In this study, we measured upper right, lower right, upper left region to find the optimal region which represent liver function. We found that the upper right region is optimal because of high accuracy and low standard deviation. For normal subjects all the calculated HEFs were greater than 100% and all those of abnormal ones were less than 80%. Thus HEF can be assumed to discriminate abnormal livers from normal ones. It was found that the patients with surgical operation would survive if both the pre-and the post-operative HEFs are greater than 50%. Therefore HEF method can be a good estimator for surgeon to determine the surgical operation and to evaluate remaining hepatic function after surgery as well as it is reliable and simple.

  • Several distinct pathologic entities are encompassed in benign vocal fold lesions, including intracordal cysts, vascular ectasia, as well as vocal fold nodules and vocal fold polyps. Treatment options for theses lesions include both medical/conservative and surgical techniques. First approaches should be focus on correcting the underlying causative factors, largely through voice therapy and education. There are several laryngomicrosurgery techniques for removal of benign lesions. Much debate continues regarding the relative merits of cold instruments versus carbon dioxide laser removal of benign vocal fold lesions. Both techniques have the merits and the demerits each other. Therefore the surgeon should well comprehend the merits of each techniques and choose the proper procedure for patient's lesions.

  • The repair of esophageal atresia with a long gap continues to pose difficulties for the surgeon. There is a general agreement that the child's own esophagus is the best, however, primary repair is not always possible. Foker JE et al. (1997) developed a technique of esophageal lengthening using external traction sutures. We successfully treated one patient with a 4.5cm gap esophageal atresia (4 vertebral spaces) using the external traction suture technique.

  • 지난 5월25일 미국의 보건복지부는 구강건강에 대한 의정백서를 발표했다. 이는 미국의 복지부장관인 샬랄라(Donna.E.Shalala)장관의 지시에 따라 의정국장인 스케처(David Scatcher)박사의 발표한 것으로서 미국 역사상 처음으로 구강건강에 대한 의정백서를 완료한 것이다. 샬라라 장관은 "처음으로 이러한 보고서를 작성한 목적은 국민들에게 구강건강의 진정한 의미를 확실히 인식하게 하고 구강건강이 전신의 건강과 복지게 얼마나 중요한 역활을 하고 있는지에 대해 주지시키기 위함"이라고 밝혔다. 보고서에서는 구강건강이 건강한 치아만을 말하는 것이 아니라는 사실을 상기시키고 구강건강없이 전체적으로 건강하다고 말할 수 없다고 지적하고 있다. 또한 보고서에서는 모든 사람이 구강건강을 향상시키고 구강질환을 예방하는 과정에서 활용할 수 있는 안전하고 효과가 확실한 질병예방법에 대해 요약했다. 미국의 구강백서는 모두 305쪽에 달하는데 이중 실무요약한 25쪽을 한글로 번역해 게재한다. 지난 10월호에는 상편이 게재됐으며 이번 11월호에는 하편을 게재한다.

  • Local pain management is the most critical aspect of patient care in dentistry. Local anesthesia is a reversible blockade of nerve conduction in an applied area that produces loss of sensation. The chemical agents used to produce local anesthesia stabilize neuronal membranes by inhibiting the ionic fluxes required for the propagation of neural impulses. Proper local anesthesia permits the dental surgeon to perform the necessary surgical procedure in a careful, gentle fashion that will be less stressful for both the operator and the patient. The improvements in agents for local anesthesia are probably the most significant advances that have occurred in dental science. Today's anesthetics are safe, effective, and can be administered with insignificant soft tissue damage and minimal concerns for allergic reactions. This article reviews the widely used local anesthetic agents for obtaining local anesthesia, and also discusses some frequently seen complications.

  • The zygoma is protruded on both sides of the midface and plays an important part in determining the impression of face. There are much different esthetic consideration of zygomatic prominence between the oriental and western. Because slight prominent zygoma is thought to be esthetic in the western, there has been done many augmentation therapy. But, in the oriental, prominent zygoma, especially in the female, is thought to be aggresive and manlike image, there has been done many zygoma reduction therapy. There are two methods to reduce the prominent zygoma, one is shaving method and the other is zygoma repositioning with ostectomy. The approach to zygoma and zygomatic arch without surgical morbidity is very difficult, so this problem has been difficult subject to oral & maxillofacial surgeon in a long period. We report the treatment case of facial asymmetry with the unilateral zygoma reduction & genioplasty using only intraoral approach with the retrospective study.

  • After making the surgical treatment plan, the surgical movements are duplicated in the model surgery. During this procedure, reference points and lines are drawn on the base of the models over the dental arch, and sawcuts are made according to these marked osteotomy lines. This method requires more accuracy for better postsurgical results in that the surgical splint which enables the surgeon to position the jaws intraoperatively is made from the casts as repositioned by the model surgery, and finally it will define the postsurgical results. This technique, however, has been found to be inexact, especially when the jaws are moved in several dimensions simultaneously. To overcome this, different methods have been developed for an accurate repositioning of the jaws as planned. A new appliance, Surgical Jaw Relator, was devised by the author for the simple 3-dimensional relocation of the upper and lower models, resulting in the easy construction of the splints such as centric relation splint, intermediate and final splint. This article describes an introduction and a clinical application of this appliance.

  • Malocclusion can be often found after improper reduction of facial bone fractures, especially by the plastic surgeon or other medical doctor. This causes lots of problem in esthetics, mastication, or facial symmetry. We present four cases which are related above problems. These were well treated by orthognathic surgery or orthodontic treatment.

  • This paper represents the design of 3D endoscopic video system in order to improve visualization and enhance the ability of the surgeon to perform delicate endoscopic surgery. In comparison of the polarized and electric shutter-type stereo imaging system, The former is superior in terms of accuracy and performance speed for knot-tying and loop pass test. The result of experiments show that the proposed 3D endoscopy system has a wide viewing angle and zone which is necessary for multi-view and it has better image quality and stability of the optical performances than the electric shutter-type does.

  • Conventional clonoscope is semiflexible tube that should be manipulated by operator for inspection and treatment. Therefore, it sometimes causes pain for the patient and even perforation to the wall of colon if a surgeon is not well trained. For safe colonoscopy, self-propelling robots with inchworm locomotion are studied. But, inchworm locomotion has some problems in adaptiveness to the variation of colon diameter. In this paper, we propose novel locomotive mechanism which can move in the colon without any external assistance. It has several cams that have constant phase difference each other and located along the centerline of the mechanism. This mechanism could realize the robotic endoscope which has continuous and smooth thrust force.
