Diagnosis of Acetabular Labral Injury (고관절 비구순 손상의 진단)

  • As the instruments as well as techniques for hip arthroscopy have developed, the interests and understandings of acetabular labral tear have also increased. As a consequence, the diagnosis itself was increased. However, it is still difficult to be diagnosed accurately in an ordinary clinic. In this brief review, the clinical and radiological characteristics and diagnostic implications of acetabular labral lesions were described to help the surgeon to make a right decision.

  • Foreign body in the air passage is not uncommon in the field of otolaryngology. The majority of bronchial foreign bodies can be removed by bronchoscopy, but some cases may require surgical procedure. Recently, we experienced an impacted foreign body (ball-pointed pen cap) in the left main bronchus with pneumothorax on the right side. Authors tried bronchoscopic removal under the general anesthesia but failed. Lateral thoracotomy was performed by chest surgeon and foreign body was successfully removed.

  • Screw insertion surgery is frequently required among surgical procedures. Especially, very careful attention should be paid to the insertion of screw in the operation of the cervical vertebra. Therefore, there is a need for a guide that allows the surgeon to reliably and promptly perform treatment by calculating the desired insertion angle and length for screw insertion. In this study, the center and direction of the pedicle were calculated through 3D modeling and 3D vector numerical analysis using the CT or MRI image of the patient for the safe operation of the guide, and based on this, After that, we will implement surgical guide based on this.

  • Laryngeal framework surgery (LFS) is a unique phonosurgical concept that enables us to influence the laryngeal biomechanics by changing the shape/position of the laryngeal cartilages. LFS procedures can be favorably combined with one another but also with other phonosurgical methods, and they are usually reversible and correctable. Type I thyroplasty and arytenoid adduction are still useful in spite of the recent popularity of injection laryngoplasty. Basic surgical principles have seldom been changed since Isshiki's development, but a number of modifications have been tried and are still going on. These delicate surgeries require exhaustive training, but the reward is great to both the surgeon and the patient.

  • Purpose Displaced lateral end fracture of clavicle and acromioclavicular dislocation type Ⅲ∼Ⅵ may be required surgical treatment. Material and Methods : From May 1998 to March 2001, we operated with Welter plate with minimal incision by one surgeon. Immediately, pendulum and passive exercise was initiated after surgery. The shoulder function was evaluated using UCLA score. Average follow up was on 28(12∼45) months. Results : All 11 patients were regained satisfactory function. Average UCLA score was 31.9(29~35) at last follow up. Conclusion . The merit of Welter plate fixation with minimal incision is simple technique, reduced surgical time, smaller scar than large plate, strong fixation, early exercise, reduced implant failure. The disadvantage is expensive, skin irritatatation by long hook. But Welter plate fixation with minimal incision is a good method of internal fixation and excellent clinical result in surgical treatment of type H displaced lateral end fracture of the clavicle and type Ⅲ∼Ⅵ acromioclavicular dislocation.

  • Microsurgery training in Singapore began in 1980 with the opening of the Experimental Surgical Unit. Since then, the unit has continued to grow and have held microsurgical training courses biannually. The road to becoming a full-fledged reconstructive surgeon requires the mastering of both microvascular as well as flap raising techniques and requires time, patience and good training facilities. In Singapore, over the past 2 decades, we have had the opportunity to develop good training facilities and to refine our surgical education programmes in reconstructive microsurgery. In this article, we share our experience with training in reconstructive microsurgery.

  • The increased application of the da Vinci robotic platform (Intuitive Surgical Inc.) for microsurgery has led to the development of new adjunctive surgical instrumentation. In microsurgery, the robotic platform can provide high definition $12{\times}-15{\times}$ digital magnification, broader range of motion, fine instrument handling with decreased tremor, reduced surgeon fatigue, and improved surgical productivity. This paper presents novel adjunctive tools that provide enhanced optical magnification, micro-Doppler sensing of vessels down to a 1-mm size, vein mapping capabilities, hydro-dissection, micro-ablation technology (with minimal thermal spread-$CO_2$ laser technology), and confocal microscopy to provide imaging at a cellular level. Microsurgical outcomes from the use of these tools in the management of patients with infertility and chronic groin and testicular pain are reviewed. All these instruments have been adapted for the robotic console and enhance the robot-assisted microsurgery experience. As the popularity of robot-assisted microsurgery grows, so will its breadth of instrumentation.

  • The late Prof. Kyeok Boo Han (1913-2005) was one of the pioneers in the early stages of the establishment of thoracic surgery in Korea. He was in charge of thoracic surgery at Seoul National University Hospital from 1948 to the outbreak of the Korean War in 1950. He presented a thoracic surgical case entitled "Adhesive (constrictive) pericarditis: one surgical case" at the first academic meeting of the Chosun (an old name for Korea) Medical Association, held in 1947. This presentation is considered to be the first thoracic surgical case presented by a Korean surgeon at a domestic medical meeting after the National Liberation from Japanese colonial rule in 1945. In this regard, this study was intended to analyze the content and the meaning of the case, published in a journal in 1948.

  • 흡연 당사자의 직접흡연이 이와 관련한 발병률과 사망률의 주요 원인이 되어 왔다는 것은 여러 연구조사자료에 보고되어 왔다. 그러나 최근 들어 담배연기에 대한 비흡연자의 비자의적인 노출 즉, 환경담배연기(Environmental Tobacco Smoke, 이하 ETS)로 인한 보건학적 위해성에 대한 논란이 제기되면서 간접흡연은 새로운 사회문제로 대두되고 있다(Surgeon General, 1986). 1986년에 미국 National Research Council은 미국 내에서 연간 3000명에 달하는 폐암의 발생이 ETS에 대한 노출에서 기인한다고 발표한 바 있으며(NRC, 1986), 이를 근거로 1990년대 초에 미국 EPA와 NIOSH에서는 세계최초로 ETS를 인체발암성물질로 규정한 바 있다(NIOSH, 1991; USEPA, 1992). Currarino Triad

  • The authors report a case of Currarino triad which had a congenital anorectal stenosis associated with a sacral defect and a presacral mass. A 1-year-old female presented with constipation since birth. Neurological deficits were not found on admission. She had had a diverting colostomy due to anorectal stenosis at another hospital before admission. Lumbar X-ray films showed bony defect caudal to the third sacral vertebra. Magnetic resonance image demonstrated a round cystic pelvic mass which was connected with a dural sac via anterior sacral defect. Posterior approach with Intradural removal of the presacral cystic mass was performed and followed by anoplasty by a pediatric surgeon. The cystic mass was verified histologically as mature teratoma with cystic change. Postoperatively, the urinary function and bowel movement remained intact. Currarino triad should be suspected and evaluated physically and radiographically in a case of congenital anorectal stenosis. Prompt recognition and close cooperation between pediatric surgeons and neurosurgeons is advisable to ensure adequate surgical treatment.
