Effect of Allergy Related Disease on Suicide Ideation among Adolescents in Korea (청소년 알레르기성 질환의 복합성과 중증도가 자살 생각에 미치는 영향)

  • Background & Objectives: There were increasing evidence about the relationship between allergy related disease such as asthma, atopic dermatitis and allergic rhinitis and suicide ideation. However little was known about the concrete relatedness between severity and comorbidity of allergy related disease with suicide ideation. The objective of this study was to investigate the cases of the prevalence of suicide ideation among adolescents with allergy related disease such as asthma, atopic dermatitis and allergic rhinitis, and examine the association between allergy related disease and suicidal ideation among adolescents in South Korea. Methods: Data was based on Korean Youth Risk Behavior Web-based Survey(2014) which was a cross-sectional study containing 34,874 Korean middle and high school students who diagnosed with allergy related disease. We used the weights, strata and primary sampling unit information provided by the public use dataset to compute descriptive statistics and logistic regressions. Computations were done with SPSS version 20.0. Results: 19.9%, 15.6%, 13.8% of adolescents who suffered from one, two and three of allergy related diseases respectively reported having been thought of suicide ideation. Socio-demographic factors were adjusted as control variables. Students with greater severity of disease were more likely to have suicide ideation. Odds ratio for students who were absent one to three days from school because of allergies was 1.96(95% CI 1.51-2.46), and odds ratio for those who were absent more than four days from school was 3.60(95% CI 2.46-5.28). Conclusions: Given that adolescents' severity and comorbidity of allergy related disease were clearly associated with suicide ideation, suicide prevention programs for adolescents with allergy related disease should be improved by strategic approaches towards the severity and comorbidity of disease.

The Relationship between Possibility of Bipolar Disorder and Suicidal Attempt in Emergency Room (자살시도로 응급실에 내원한 환자의 자살 위험성과 양극성 장애 가능성의 연관성)

  • Objectives : The aim of this study was to investigate the factors including possibility of bipolar disorder that are related to intensity of suicidal idea. Methods : The study subjects consisted of 50 patients who did suicidal attempt and treated at the Inje University Haeundae Paik Hospital Emergency Room. All participants underwent psychiatric interview and underwent Columbia Suicide Severity Rating Scale(C-SSRS), Beck Depression Inventory(BDI), Korean Version of Mood Disorder Questionnaire(MDQ) and Bipolar Spectrum Diagnostic Scale(BSDS) to evaluate patient's suicide attempt, severity of depression and possibility of bipolar disorder. Results : Compared to non-high risk group, suicide high risk group showed significantly higher BDI(p<0.001) and intensity of ideation(IOI) in C-SSRS(p<0.001). Also intensity of ideation(IOI) was correlated with Mood Disorder Questionnaire(MDQ) bipolarity positive(p=0.033). Conclusions : The present study indicated that possibility of bipolar disorder plays a significant role in suicide attempters. Assessment of suicide ideation severity and possibility of bipolar disorder should be considered when suicide attempters come to emergency room.

  • Objectives This study aimed to review the Korean Constitution articles 14 and 20 of the "Law on suicide prevention" and investigate public perceptions of specific improvements to suicide prevention policies using results from the Korean 2018 National Survey on Suicide. Methods The questionnaire was designed to analyzing the act restricts sharing of patient information between hospitals, making it difficult to track suicide attempts. The questionnaire was also designed to suggest further medical and normative criteria for objective judgment of continuous follow-up utilizing suicide risk evaluations and proportional principle review that consider patients' and medical staff's basic rights. Results This study identified the result of the 1500 respondents, 79.1% believed that Korea should allow suicide prevention management to be implemented without requiring individual consent to protect suicide attempters. Conclusions According the results, I propose the following criteria for policy improvement: use of anonymized information and non-profit research for technical and ethical considerations, access to medical information only for therapeutic purposes, and use of surgical severity assessment criteria appropriate for Korea.

Analysis of social factors influencing authenticity of suicide for patient who attempt to suicide in emergency department: Retrospective study based Post-suicidal Care Program data (응급실로 내원한 약물중독 자살시도자의 자살 진정성에 미치는 사회적 요인 분석: 응급실 기반 자살 시도자 관리시스템 자료를 이용한 후향적 연구)

  • Purpose: This study was conducted to analyze the social factors influencing the 'authenticity of suicidal ideation' based post-suicidal care programs in emergency departments (EDs). Methods: This retrospective study was an analysis using the data of patients who had attempted suicide and visited the ED in tertiary urban hospitals from January 1, 2016, to December 31, 2018. The variables examined included gender, age, history of previous psychiatric disease, suicide method, and the number of previous attempts. Univariate and multivariate logistic regression analyses were conducted to identify factors influencing the registration rate for the community-based post-suicide care program. Results: Overall,1,460 suicides were analyzed, 177 (1.16%) showed a high authenticity of suicide. The social factors influencing the authenticity of suicide intent were the unmarried status of men and women, joblessness, history of mental illnesses, more than two previous suicide attempts, the influence of alcohol, and an attempt to commit suicide after midnight more specifically between 24:00 to 6:00 hours in the morning (p<.05). The factors influencing the severity of the condition of high authenticity suicide patients were low Glasgow coma scores (12 points or less), lactate levels, and oxygen saturation observed in the patients who were admitted to the intensive care unit for treatment and died (p<.05). Conclusion: The need for evidence-based preventive measures and early assessment tools at the emergency medicine level is emphasized to reduce the rate of suicide attempts. If the results of this study are used in the management of suicide prevention, the evaluation of the authenticity of suicide intent will be more likely to be made at the emergency medicine level, allowing the severity to be assessed earlier.

  • Korea has the highest suicide rate among Organization for Economic Cooperation and Development countries, with increasing rates observed among children and adolescents with low levels of life satisfaction. Patients in these age groups exhibit particularly turbulent and impulsive behaviors, which make suicide highly contagious and fatal. The loss of meaningful work or activities increases the risk of suicide, especially in young people. Following revisions to the Mental Health Welfare Act (2016), which has included occupational therapists among mental health professionals, a multidisciplinary approach including occupational therapy has been emphasized upon in mental health services. Screening tools for preventing suicide in children and adolescents include the Suicidal Ideation Scale, Beck's Suicidal Ideation Scale, Columbia-Suicide Severity Rating Scale, Reasons for Living Scale for Adolescents, and the Student Emotional and Behavioral Characteristics Test. The Canadian Occupational Performance Measure, which is an occupational therapy evaluation tool, is also used. Various suicide prevention programs have been proposed across academic disciplines; however, due to the urgent nature of high-risk groups, there is a need for timely services. Currently, most existing programs focus primarily on the cognitive-behavioral aspects. In this study, we aimed to introduce diverse suicide prevention programs for mental health professionals working with high-risk children and adolescents in order to equip them with the relevant information and help apply their learnings effectively in different situations.

A Study on Policy Improvement for Ensuring the Effectiveness of Suicide Prevention Law (「자살예방 및 생명존중 문화 조성을 위한 법률」의 실효성 확보를 위한 정책적 개선 방안 - 「개인정보보호법」과의 충돌문제 해결을 중심으로 -)

  • The essential policy of suicide prevention is to continuously manage and treat suicide attempted people through data base related to suicide retry rate and follow-up study report. In Korea, only few people are allowed to follow-up by the Personal Information Protection Act. As a result, the research participation rate and the service participation rate are rather low, so that the research participants is limited to a part of the suicide attempted people. Therefore, the policy proposals to be improved in the Ministry of Health and Welfare Act were examined comparatively in order to increase the practical utilization of the suicide prevention about Article 14 and Article 20 of the Suicide Prevention Act. As a criterion for policy improvement, measures for non-discrimination of information to be considered in terms of technical and ethical dimensions and non-profit research and medical information for medical purposes were suggested. In addition to the severity of the suicide, the suicide risk was assessed and the criteria for the objective assessment of the follow-up observation were considered in consideration of the severity of the suicide.

Association between Perceived Parenting Style and Adolescents' Attitudes toward Suicide

  • Objectives: Adolescent suicide, a major cause of adolescent death, is affected by various factors, including attitudes toward suicide. This study investigated the association between parenting style and adolescents' attitudes toward suicide and the mediating role of attitude toward suicide between parenting style and suicidal ideation. Methods: We surveyed 1,071 adolescents from eight middle schools in Incheon, Korea. The survey included sociodemographic information, attitudes toward suicide, perception of parenting style, depression severity, and suicidality. Results: Students in the authoritarian parenting group had a more permissive attitude toward suicide compared with the democratic and permissive parenting groups. These students considered that suicide is justified in certain situations and that choosing suicide is an individual's right. They also had a negative attitude toward talking about suicide or intervening in others' suicide. This association remained statistically significant after adjusting for the impact of confounding factors that could affect attitudes toward suicide, except for suicidal processes and preparedness to prevent suicide. In the mediation analysis, we observed that some factors of the attitudes toward suicide mediated between authoritarian parenting attitudes and suicidal ideation, namely, suicide as a right, preventability, suicide as normal/common, preparedness to prevent suicide, and resignation. Conclusion: This study revealed the significant impact of parenting style on children's attitudes toward suicide. Educating parents about the appropriate parenting attitudes-sympathetic and rational-can help prevent youth suicide.

Prognostic Factors of Acute Poisoning in Elderly Patients (65세 이상 노인 약물 중독 환자의 예후 인자)

  • Purpose: Incidents of suicide attempt and acute poisoning in the elderly population is rising. This study investigated the clinical nature of acute poisoning and differences between the survival and mortality groups in the elderly. Methods: We retrospectively investigated 325 patients with acute poisoning who visited the emergency department. Patients were divided into two groups, one survival and one death group. Information regarding patient sex, age, root cause of poisoning, time of year of poisoning, type of intoxicant, duration of time between intoxicant ingestion and arrival at the emergency department, total length of hospital stay, and any previous suicide attempts and subsequent hospitalization were collected. An initial Glasgow Coma Scale (GCS) and an initial and final Poison Severity Score (PSS) for each patient was calculated. Results: The survival rate for men was higher than for women in this study. The most common reason of drug intoxication was suicide, with accidental ingestion and substance abuse occurring in descending frequency. Seasonal factors were reflected in the data with the highest number of incidents occurring in spring and the lowest number in winter. Compared with the mortality group, the survival group had a lower initial PSS with a higher GCS. Conclusion: We conclude that being female, having suicide as a root cause, agrochemicals as an intoxicant, low initial Glasgow Coma Scale and high initial Poisoning Severity Score, are all associated with poor prognosis.

The Relationship Between the Severity of Drinking and Suicidal Ideation in The General Korean Population-Analysis by Gender and Age Groups

  • Objective : The suicide mortality rate among Koreans was ranked first among the OECD countries in 2018. Alcohol consumption is a significant risk factor for suicidal behavior. This study examined the relationship between the severity of subjects' drinking habits and the suicidal ideation among Koreans according to gender and age. Methods : The data of 113,602 people who visited the Kangbuk Samsung Hospital Health Screening Center from January 2013 to December 2014 were used. The severity of subjects' harmful drinking habits was assessed using the Korean version of the Alcohol Use Disorders Identification Test (AUDIT-K). The relationship between the severity of harmful drinking and suicidal ideation was compared using logistic regression analyses. We also examined the tendency through p for trend to identify a trend. Results : Of the 113,602 study subjects, 6,586 (5.8%) responded that they had thought of committing suicide. After controlling for gender, marriage, education, body mass index (BMI), depression and anxiety, the results showed that the higher the level of harmful drinking, the higher the suicidal ideation. Conclusion : This study found that, even after controlling depression and anxiety, the severity of drinking could be a significant risk factor for suicidal ideation.

Comparison of the Suicide Attempt Characteristics Associated with Mixed and Non-Mixed Depression in Koreans

  • Objectives : This study aimed to compare the characteristics of suicide attempts among Korean patients with mixed and non-mixed depression. Methods : Patients who visited the emergency room due to a suicide attempt and participated in the Korean Cohort for the Model Predicting a Suicide and Suicide-related Behavior study were included. Using the Montgomery-Asberg Depression Rating Scale (MADRS) and Young Mania Rating Scale (YMRS), 111 patients were classified into the mixed depression (n=46) and non-mixed depression groups (n=65). The Koukopoulos Mixed Depression Rating Scale (KMDRS) score was calculated using the MADRS and YMRS scores. Suicide attempt characteristics were evaluated using the Columbia Suicidal Severity Rating Scale (C-SSRS) and Suicide Intent Scale (SIS). Results : In the mixed depression group, the reason item among the ideation intensity score of the C-SSRS was higher, and the deterrent item score was lower. Scores on the timing and suicide note items of the SIS were higher, and scores for overt communication items were lower in the mixed depression group. The KMDRS score was positively correlated with the C-SSRS ideation intensity and total SIS score. After adjusting for additional variables, the KMDRS scores had a significant effect on the C-SSRS ideation intensity and total SIS scores. Conclusions : The mixed depression group showed a difference in the intensity of suicidal ideation and suicidal intention compared to those in the non-mixed depression group. The overall suicidal ideation intensity and suicidal intention increased according to the degree of mixed depression.
