추락 및 투신자살 방지시스템의 조사 및 Rollinder System 적용기술 (Evaluation of Prevention System of Falls and Committing Suicide with Application Technology of Rollinder System)

  • 대한민국은 심각한 자살시도에 휘말려 2003년 이후 지금까지 OECD 회원국 가운데 자살률 통계에 있어 상위권을 차지하고 있으며, 전년 대비 -5.0% 소폭하락 했지만 여전히 사망 요인 중 10세 이상 전 연령에서 자살이 가장 높으며, 이것은 반드시 해결해야 할 매우 중요한 사안들이다. 자살 수단별 사망률의 변화추이를 살펴보면 추락 및 투신을 통한 자살이 큰 비중을 차지하고 있으며, 국내뿐만 아니라 해외에서도 이에 대한 근본적인 대책마련이 없어 쉽게 해결할 수 없는 심각한 문제로 대두되고 있다. 대표적인 예로 미국의 금문교에 최근 2억 달러를 들여 자살방지 그물망을 설치하고 있으며, 뉴질랜드의 경우 교량에 철망을 철거했다가 치솟은 자살률로 인해 디자인을 바꿔 다시 설치했으며, 캐나다와 호주의 경우도 교량에 철책을 둘러쌓아 교량의 미관을 포기한 채 투신자살만을 방지하고 있다. 따라서 본 논문은 국내 외의 자살방지 시스템과 관련된 특허기술에 대한 비교평가를 제안하고, 보안시설 및 제한구역에서의 접근차단, 특수시설에서의 월담방지와 추락 및 투신자살을 효과적으로 방지하기 위한 기계적 기술적 시스템인 롤린더 시스템을 제시하고자 한다. 마창대교는 롤린더 시스템 설치 전인 2016년 까지 8년간 33명이 자살시도를 하였으나, 2017년 롤린더 시스템 설치 후에는 자살시도의 획기적인 감축효과를 거두었다.

  • 테러리즘의 핵심적 목표는 계획적 사회혼란과 심리적 공포의 확산을 통해 정치적, 이념적, 종교적 등의 이해에 유리한 여건을 조성하는 것이다. 따라서 테러집단은 폭력적 충격을 통해 대중적인 심리적 공포를 확산시킬 수 있는 수단과 방법을 최대한 동원한다. 또한 테러리즘은 현대문명의 발전과 국제사회구조를 반영하면서 개념과 수단 방법 등이 유기체적으로 진화하게 된다. 현대 테러리즘의 특징은 테러리즘이 비국가 무장단체의 국가를 상대로 한 전쟁의 수단으로 진화하였으며, 자살폭탄테러는 테러를 통한 국제정치의 일환으로 이슬람 극단 테러조직의 전략에 부합됨으로써 유행처럼 확산되고 있다는 점이다. 인류의 테러리즘의 위협으로부터 해방되기 노력은 무력에 의한 조급한 정책을 십 수년간 실행하여 왔으나 실효성 있는 결과를 얻지 못했으며, 자살폭탄테러라는 극단적인 위협이 확산되는 상황에 직면했다. 자살폭탄테러의 예방은 자살폭탄테러범의 심리적 범행동기를 고려하여 병리현상의 치료와 같은 개념으로 종교적 이질감과 문명에 대한 적대감을 해소함으로써 자살공격의 기반을 줄여나가는 노력을 지속적으로 진행하여 나가야 한다.

광공업지역과 비광공업지역 주민의 자살충동에 영향을 미치는 요인: 한국의 한 중소 도시를 대상으로 (Determinants of Suicide Impulse of Residents Living in Mining Region and Other Areas in One City)

  • Objectives: The purpose of this study is to find the determinants of suicide impulse of residents living in mining region and other areas in one city. The past studies did not examine the suicide related attitudes or behaviors in mining region. This study also examines how coping resources and behaviors moderate the suicide impulse. Methods: For this purpose, hierarchical logistic regression method was used to predict the likelihood of suicide impulse. The personal characteristics, depression, coping resources and behaviors were considered as the independent variables. The data collected in this study was gathered through questionnaire survey with 502 residents in other areas as well as mining area in one city. Results and Conclusion: The results and conclusions are as follows: 1. The chi-square test revealed that residents living mining region showed higher percentage of suicide impulse compared to other areas. 2. The t-test revealed that those with suicide impulse had higher level of depression compared to those without it. This pattern was consistent in other areas as well as mining region. 3. The hierarchical logistic regression revealed that age, education, depression showed positive effect on suicide impulse in mining region. However, in other areas, education, illness, and depression showed positive effect on suicide impulse. Also, this result implies that suicide prevention efforts should be actively made in mining region.

The characteristics of elderly suicidal attempters in the emergency department in Korea: a retrospective study

  • Background: Although Korea ranks first in the suicide rate of elderly individuals, there is limited research on those who attempt suicide, with preventive measures largely based on population-based studies. We compared the demographic and clinical characteristics of elderly individuals who attempted suicide with those of younger adults who visited the emergency department after suicide attempts and identified the factors associated with lethality in the former group. Methods: Individuals who visited the emergency department after a suicide attempt from April 1, 2017, to January 31, 2020, were included. Participants were classified into two groups according to age (elderly, ≥65 years; adult, 18-64 years). Among the 779 adult patients, 123 were elderly. We conducted a chi-square test to compare the demographic and clinical features between these groups and a logistic regression analysis to identify the risk factors for lethality in the elderly group. Results: Most elderly participants were men, with no prior psychiatric history or suicide attempts, and had a higher prevalence of underlying medical conditions and attributed their attempts to physical illnesses. Being sober and planning suicide occurred more frequently in this group. In the elderly group, factors that increased the mortality rate were biological male sex (p<0.05), being accompanied by family members (p<0.05), and poisoning as a suicide method (p<0.01). Conclusion: Suicide attempts in elderly individuals have different characteristics from those in younger adults and are associated with physical illness. Suicides in the former group are unpredictable, deliberate, and fatal. Therefore, tailored prevention and intervention strategies addressing the characteristics of those who are elderly and attempt suicide are required.

Medical Care Expenditure in Suicides From Non-illness-related Causes

  • Objectives: Several epidemiological studies on medical care utilization prior to suicide have considered the motivation of suicide, but focused on the influence of physical illnesses. Medical care expenditure in suicide completers with non-illness-related causes has not been investigated. Methods: Suicides motivated by non-illness-related factors were identified using the investigator's note from the National Police Agency, which was then linked to the Health Insurance Review and Assessment data. We investigated the medical care expenditures of cases one year prior to committing suicide and conducted a case-control study using conditional logistic regression analysis after adjusting for age, gender, area of residence, and socioeconomic status. Results: Among the 4515 suicides motivated by non-illness-related causes, medical care expenditures increased in only the last 3 months prior to suicide in the adolescent group. In the younger group, the proportion of total medical expenditure for external injuries was higher than that in the older groups. Conditional logistic regression analysis showed significant associations with being a suicide completer and having a rural residence, low socioeconomic status, and high medical care expenditure. After stratification into the four age groups, a significant positive association with medical care expenditures and being a suicide completer was found in the adolescent and young adult groups, but no significant results were found in the elderly groups for both men and women. Conclusions: Younger adults who committed suicide motivated by non-illness-related causes had a higher proportion of external injuries and more medical care expenditures than their controls did. This reinforces the notion that suicide prevention strategies for young people with suicidal risk factors are needed.

한국 성인의 자살태도측정도구의 요인구조 : 2013년 자살실태조사 자료를 바탕으로 (Factor Structure of Attitudes Toward Suicide (ATTS) of Adults : Based on a Nationwide Survey in South Korea)

  • Objectives Suicide is a complex phenomenon not only caused by the individual's mental illness and economic situation, but also occurred in sociocultural contexts and relationships around the individual. Therefore, the attitudes, which include collective norms, thoughts and feelings of a population, toward suicide play a fundamental role in the prevention of suicide. Factor structure of Attitudes Toward Suicide (ATTS), developed by Renberg, is also various in different sociocultural contexts. This is the first study to investigate factor structure of ATTS along with nationwide sample of Korean adults. Methods This study analyzed the factor structure and reliability of ATTS in 1500 regionally stratified Korean adults. Exploratory factor analysis was performed by using principal axis factoring and varimax rotation. Results The selection of 33 ATTS items based on internal consistency and communality was conducted. Five factors were analyzed, which were named 'Good reason,' 'Negative gaze,' 'Predictability,' 'Means of escape,' and 'Common secret,' respectively. Explained total variance was 41.94%, and the internal consistency ranged from 0.516 to 0.740. Conclusions Each of the five factors contains different semantic dimensions. Consistent with previous studies, factors containing permissive attitude toward suicide was high in those with history of suicidal thought and of suicide attempt. It is notable that the 'Good reason' showed higher score in elderly. This may be related to socioeconomic or physical adversities which have been considered the major cause of suicide in elderly.

자살 위험성 및 자살 시도 방지에 대한 전기경련치료의 역할 (The Electroconvulsive Therapy in the Prevention of Suicide Risks and Attempts)

  • Suicidality is the most serious complication of mood disorders and psychosis; effective treatment should reduce suicide rates. The Organization for Economic Cooperation and Development age-standardized suicide rate in Korea was 22.6 in 2018, much higher compared to other countries worldwide. As mental and psychiatric problems are the main reasons for suicide attempts, accounting for 31.6% in 2018, targeting such problems should be the focus of efforts to reduce suicide rates. However, the ability of current pharmacotherapeutic and psychotherapeutic interventions to reduce suicide rates is limited due to their delayed effects. Therefore, electroconvulsive therapy (ECT) has been proposed as an alternative treatment. This approach is effective for treating most mental disorders associated with high suicide rates, including severe depression, bipolar disorder, and intractable psychotic disorders; ECT is also effective for Parkinson's disease, which has the highest suicide rate among all disorders in Korea. The acute, long-term, and prophylactic effects of ECT on suicidality have been reported in the literature, and treatment guidelines outside of Korea recommend that ECT be used at an early stage for rapid reduction of suicide rates, as opposed to being applied as a treatment of last resort. However, only ~0.092 of every 10000 members of the Korean general population received ECT in 2018; this is much lower than the average rate worldwide, of 2.2 per 10000. Elimination of obstacles to the use of ECT, early crisis intervention involving administration of ECT for rapid stabilization, and maintenance ECT to prevent recurrence should reduce suicide rates.

Spatio-temporal Distribution of Suicide Risk in Iran: A Bayesian Hierarchical Analysis of Repeated Cross-sectional Data

  • Objectives: We aimed to estimate the space-time distribution of the risk of suicide mortality in Iran from 2006 to 2016. Methods: In this repeated cross-sectional study, the age-standardized risk of suicide mortality from 2006 to 2016 was determined. To estimate the cumulative and temporal risk, the Besag, York, and Mollié and Bernardinelli models were used. Results: The relative risk of suicide mortality was greater than 1 in 43.0% of Iran's provinces (posterior probability >0.8; range, 0.46 to 3.93). The spatio-temporal model indicated a high risk of suicide in 36.7% of Iran's provinces. In addition, significant upward temporal trends in suicide risk were observed in the provinces of Tehran, Fars, Kermanshah, and Gilan. A significantly decreasing pattern of risk was observed for men (β, -0.013; 95% credible interval [CrI], -0.010 to -0.007), and a stable pattern of risk was observed for women (β, -0.001; 95% CrI, -0.010 to 0.007). A decreasing pattern of suicide risk was observed for those aged 15-29 years (β, -0.006; 95% CrI, -0.010 to -0.0001) and 30-49 years (β, -0.001; 95% CrI, -0.018 to -0.002). The risk was stable for those aged >50 years. Conclusions: The highest risk of suicide mortality was observed in Iran's northwestern provinces and among Kurdish women. Although a low risk of suicide mortality was observed in the provinces of Tehran, Fars, and Gilan, the risk in these provinces is increasing rapidly compared to other regions.

남녀 중.고등학생의 자살시도 예측요인 (Factors on the Suicidal Attempt by Gender of Middle and High School Student)

  • Purpose: The suicide rate of adolescents in Korea is increasing annually. Therefore, this research was done to identify the suicide attempt rate of middle and high school students and to identify factors that influence suicidal attempts. Methods: The Korea Youth Health Risk Behavior Web-based Survey (2007) was used as data. Discriminant analysis and logistic regression were performed to analyze the data depending on gender to consider the gender difference in assessing the influence of each independent variable on suicidal attempts. Results: Discriminant analysis according on gender showed that 13 factors correlated with suicidal attempts for boys, and 20 factors for girls. The most highly correlated factors were smoking, depression and inhalation experience. For inhalation experience, boys had 2.7 times higher possibility of suicide attempts (95% CI 1.8-3.0) and girls, a 2.4 times higher possibility (95% CI 1.7-3.5). Conclusion: The results of the study indicate a need to classify adolescents for expectation of suicide risk and high danger for suicidal attempts through, and introduce suicide prevention programs for these adolescents. In particular, it is necessary to start intervention with students who smoke, have sexual and inhalation experiences and high levels of depression.

Community Case Managers' Experiences of Overcoming Suicide Crises among Late Adolescents in South Korea

  • Purpose: Various intervention strategies are needed to reduce the suicide rate among late adolescents. Therefore, we aimed to investigate the factors that can help overcome suicidal tendencies in late adolescents from the perspective of case managers. Methods: This qualitative study explored the reinforcing factors that can help overcome suicidal behaviors in late adolescents, by conducting focus group interviews with case managers from the local community. Interviews with 10 case managers were recorded and transcribed, and the data were analyzed using the content analysis method. Results: The main theme of this study was "a step in a new direction". Three categories were derived: "inner changes", "another possibility opened up by the interest of significant others", and "connecting with community resources to help overcome". Each category had subcategories that influenced the intention and course of action of behaviors to overcome the suicide crises. Conclusion: Our findings, from the exploration of the experiences of case managers, provide an in-depth understanding of the reinforcing factors for overcoming suicide among late adolescents. They provide useful preliminary data for the development of effective suicide prevention programs for the adolescents.
